How to make a liquid from tablets or capsules

Just like to add here, I made 3-4 days worth of a liquid suspension with disastrous results. This was with filtered/boiled water.  

 

I'm now making the solution daily with much better results. It seems the suspension loses it's potency somehow after 24 hours. 

 

I'm hoping this wasn't done intentionally by big pharma after discovering sites like this. 

2011-2012: Fluoxetine
2012-2014: Citalopram
2015-2016: St John's Wort (Perika 300mg)
2016: 50mg Sertraline daily

2023: 25mg Sertraline - beginning of liquid taper

 

Daily Supplements:

  • 200mg Magnesium Bisglycinate
  • B-Complex
  • Probiotics

Looking for input from anyone with direct experience switching from brand name beaded Effexor XR to liquid venlafaxine? Any issues with the transition from a once a day dosing to twice a day?

History of alcoholism, used benzo’s on and off in the past (acute benzo withdrawal in 2012; no PAWS)

 

21 years on Effexor @ doses between 75mg and 150mg. 2021 Effexor 75mg: 3/8/21 -> 67.5mg; 3/22/21 -> 56.7mg ; 5/12/21 -> 48.6mg; 6/8/21 -> 42.3mg; 7/1/22 -> 37.5mg; 7/15/21 -> 33.5mg; 8/20/21 -> 27.5mg; 1/5/22 -> 25mg; 1/13/22 holding - aggravating factors lead to delayed W/D /destabilization 3/4/22 -> anxiety, panic, dry-retching, diarrhea, weight loss; 3/8/22 1mg Ativan added 3/22/22 self-admit to hospital; updosed to 37.5mg E - switched to clonazapam 3/25/22 updose to 75mg3/30/22; fast taper C after 24 days of intermittent use. [2 years pass] Currently still at 75mg

I don't have direct experience so this is a more general comment. Not an expert but feel compelled to point out that what you describe, "switching from brand name beaded Effexor XR to liquid venlafaxine? Any issues with the transition from a once a day dosing to twice a day?" involves multiple changes at once.

 

Maybe you know this already, but just in case -- and for posterity -- SA recommends making only one change at time, and then holding until things stabilize.

 

- switching brands is 1 change

- switching from beads to liquid is 1 change

- switching from once a day dosing to twice a day dosing is 1 change

 

Perhaps that's obvious and you simply phrased your question in a condensed manner for the sake of posting, I don't know. 

Erring on the side of caution here, help topic below.

@Arielwow! What an intelligent, thoughtful reply. Yes I am aware that it is a few changes although I hadn’t thought of it like that… in those steps. You have given me much food for thought. It’s not as simple as it sounds is it?

 

The compounding pharmacy said they use extended release bead-version of venlafaxine, pulverize it, and make a suspension; but it begs the question, do they use the brand name stuff which I’m taking? Probably not, as it is more expensive. I will inquire/specify. 
 

I insisted on them using immediate IR because in the Maudsley guide it is listed as available in Canada. And I’m thoroughly sick of dealing with the extended release medication and its different sizes of beads and dubious dose. But I maybe need to rethink that in regards to it being a change. 
 

I am on 75mg. I also thought about introducing an intermediate step in the change over to liquid which involves taking a a regular 37.5mg capsule and two doses of liquid to make up the difference. Maybe going even slower is warranted, although then I’m forced to open the caps and deal with the inaccuracies involved with bead counting or weighing. 
 

Thank you for your reply, any additional input would be appreciated. What would you do?

Doesn`t that prove the fact that the liquid is losing it`s medical effect very fast? So even if make only 3 days worth of medicine, every dose is less effective that previous?

 

I`m noticing a pattern that I feel better when I take my 1st dose. Days 2 and 3 are downhill and things get a little better when I make a new patch. I can not be sure about this yet but I`m very scared to experiment any further.

I'd like to add - don't make a liquid for more than 24H. I've tried tapering with a few days before with bad results. I'm mixing 50mg Sertraline with 50ml water on the day. I throw out the bit I don't want and consume the rest (all the debris stuck to the side of the jar). If you make a batch for a few days and try to take a sample, it's highly inaccurate. Throw away the excess and consume the rest when fresh. Good luck.
