Hi i'm italian 33 years old, sorry for my english. I'm new and i'm in this hell for one month and one week. I take paroxetine for one year and stop it in past without any problem. Then after one year without antidepressive i take another year paroxetine at different dose from 10 mg to 5 mg. When i stopped I think i create an imbalance i stopped at 10 mg suddenly after my marriage and always was ok for ten days during my honeymoon, then i decide because i was stupid to reinseriment after 10 days at 5 mg because my fear and problem at work. After three weeks i experienced muted orgasm and so stop suddenly. I don't know about pssd i read it in past but stupjdly i don't trust at the article. I have also terrible insomnia intially i managed with benzo beacuse i don't able to fall sleep all night. Now from 10 days i sleep without benzodiazepine i take chamomile but tonight i was able to sleep without chamomile. I wake up every 1 or two hours but i'm able to fall sleep again. I sleep like 5 or 6 hours before med i slept for 9 hours. I also don't fell adrenaline and anxiety, i take med for anxiety wich was my main problem. I want only a little bit of encouragement because intially i was very depress with suicidal thoughts. Now i'm trying to be positive but it's difficult, beacuse i have to change my mind and personality wich is always be a little pessimistic. I have a fantastic husband wich support me. I try initally only support aid with melatonin and magnesium but when i take only one pill of fish oil my sleep became vary bad. So i don't take anything anymore only chamomile and somitimes melatonin. Please give me hope i'm very scared of all this darkness. Thanks

Paola

Hi @Paola90,

Welcome to this support site. Good that you came to this website. There is a lot of knowledge to be found and shared here. Also the members can support each other here.

Before we can answer your questions, please create your signature as asked for in the e-mail you received when registrating for this website..

Can you please give us detailed information about your drug history for all drugs you are on and have been on, especially for the past 18-24 months?  It would be especially helpful to have the details of your drugs in a concise list (no symptoms), only drug names, specific dates (as best you can say for example early March if you don't recall the day) and dosages of each medication decrease or increase.  Please read the link below for instructions.  This will allow us to give you the best guidance.

https://www.survivingantidepressants.org/topic/28240-how-to-summarize-your-drug-history-in-your-signature/

My first impression is that you are suffering from withdrawal. Paraxetine is a very strong drug and should be tapered very slowly and careful before stopping.

When you have made your signature, we will be able to come with a more detailed advise what you can do.

Here is already some important and useful info you can read:

Which date in June did you stop the 10mg paroxetine cold turkey?

Do you still take the 5mg Paroxetine as reinstatement?

If not, what was the last day you took Paroxetine?

Please put all these dates in your signature (as detailed as possible, this is important)

Please do not take any benzodiazepines. What is Minias?    (Lormetazapam, or other benzo). Please also put in your signature.

If you do not react well on melatonin, you better stop it. We can all react different to melatonin. If it does not help you or gives extra symptoms, better stop it.

Are you having other symptoms than insomnia and sexual numbness? If so, which?

You most probably suffer from withdrawal from because of stopping Paroxetine cold turkey. Paroxetine is a very strong SSRI, that should be tapered very slowly and careful before stopping.

Usually withdrawal symptoms start after 4-14 days. They often become worse in time. Insomnia is a very common withdrawal symptom.

Since you have not been so long off the paroxetine it is possible to do a small reinstatement (like 1mg).

This may help to lessen the withdrawal symptoms. If you do this, it can take up to 2 weeks until you may feel some relief or at least some stability in the symptoms. There is no guarantee that it will work, but it is the best remedy to try in a case like yours. Also to prevent that symptoms become worse.

When you start to feel better or more stable after reinstatement, you should wait until you feel stable for some time (some weeks). Then you can start to taper off the 1 mg paroxetine very slowly. Please understand that even 1 mg is still very strong. 1 mg still blocks around 29% of the serotonin receptors! Therefor it is important to taper it down to at least 0,02 mg before you can stop this medication.

See this graph:

This is how it works very often. Lots of people even seem to do very well the first 3-5 months after stopping the medication (cold turkey or at a too high dose). And then suddenly the Big Bang comes.

The slower and further (to a very low dose) we taper before stopping will prevent this.

I cannot say for sure if it will get worse in your case. But you stopped and started 2 times in the last years. And very often this sensitises the nervous system. Often resulting in worse symptoms every time we stop again.

You went off paroxetine at a very high dose (5 mg). The fact that you suffer from insomnia and sexual numbness are signs that you are having withdrawal symptoms.

If you would notice clear improvement and you would not develop other withdrawal symptoms you could be lucky. In that case the problems could go alway by itself.

But if you are having withdrawal syndrome (and you show signs in that direction) you NOW have the chance to deal with that is a safe way.

You stopped quite recently (6 weeks ago) cold turkey with the paroxetine. Now is still a good moment to reinstate a very low dosage (like 1 mg). The longer you wait the less chance that reinstatement will work.

There are (normally) no risks to reinstate such a low dose when one stopped recently. On the contrary, it is expected to lessen the withdrawal symptoms. After being stable at that dose, you can slowly taper to zero. Never stop cold turkey.

Do you have other symptoms than insomnia and sexual numbness?

Let me know if you want to reinstate. If so, we can advise you how to take dosages of 1 mg and lower.

Yes, some people heal just by time. Others may keep having symptoms or get worse. If I were you, I would start a small reinstatement and see if my symptoms stabilise or lessen.

In the end it is up to you whether you would want to reinstate a small dosage.

I guess you mean "reinstate" not "resenerate"?

Reinstatement may help to stabilise. In that way you make the withdrawal symptoms less worse. After some time you may become stable and after that you can taper the small reinstatement dose down to zero.

If you don not reinstate there is a chance that the withdrawal symptoms become worse than now. Since you stopped cold turkey recently, you now still have the chance to reinstate.

When you wait longer without reinstatement, reinstatement may not work anymore. And withdrawal symptoms may get worse then.

Do not expect a quick fix. Stopping paroxetine is often difficult. Withdrawal symptoms need time to go away. There is no faster way.

@Paola90 how much sleep are you getting now? How has your sleep pattern changed in the last 2 weeks?

Do you currently have any withdrawal symptoms other than sleep disruption and PSSD?

Antidepressants such as paroxetine and benzodiazepines such as lormetazepam can also dull sexual sensation, so if withdrawal symptoms are mild, we generally do not recommend reinstatement, to allow the nervous system to recover naturally.

Hi @Paola90  I took paroxetine for several years and also felt numb and sadness during withdrawal and recovery.  I can assure you that your emotions and feelings will return.  Give yourself time. As the others have said, paroxetine is a very powerful medication and can cause extreme withdrawal symptoms and needs to be reduced slowly.  I know you're afraid it will last forever, but it won't.  The fears and negative thoughts you are experiencing are side effects of withdrawal but they have no power over you and can't stop you from recovering. They will begin to slowly fade with time. Are you trying to practice any coping skills like CBT?

Thank you very much @mstimci need these words because people in internet are so pessimistic for recovery. I understand that recover is slow but this is not a problem for me only have faith and belive that recover is possible. I start last week a journey with new psychologicist. The problem is in italy but maybe in all the part of the word, psicologhist and psichiatric don't belive in withdrawal and pssd so now i try another psychologicist beacuse the last didn't help me unfortunatelly. He thinks that my symptoms were psychological problem only and force me to do things. I want to overcame the trauma and the difficult situation with a good psychologicist

We have the same problem here in the US; many psychologists don't believe in SSRI withdrawal or think it only lasts a few weeks.  Besides being wrong, it allows a lot of misinformation to be spread, like the effects of the medication causes lasting damage to your nervous system.  It doesn't.  But it does take time and new ways of managing your thoughts.  It took me some time, but I was able to find a psychologist who understands medication withdrawal, and he was able to help me tremendously.

Hi

I would say it took a little more than a year after getting to a very low dose for me to start fully feeling emptions again.  Its not something that happens all at once.  Its gradual but you begin noticing you are reacting to things that you weren't before.  Also, you should be prepared to feel emotions very strongly at first.  Your nervous system becomes accustomed to having numbed emotions, so it can overwhelming when you start to feel things again.  I remember crying listening to songs I heard many times before.  But those feelings are a sign you're getting your emotions back!

Hi

One year after I went to a very low dose.  Remember, we are all different so you can't really judge by somebody else's experience.  When I was in withdrawal, I remember thinking it seemed most people were making progress much after than I was. It took me a while, but I finally began to recover too.

@Paola90  In all my years of mentoring people coming off meds, I have never met anyone who did not recover if they were willing to work for it.  I know its hard and it seems like it will last forever, but it won't. Try changing your thinking to concentrate on how you want to recover, instead of what you're feeling right now.  The feelings won't last, but recovery will last forever!
