Schmiddens Celexa issues after switch to Lexapro

I was on Paxil 10 mg for a decade, then Celexa 10 mg for 13 years.  On September 1st of the year, I was switched to Lexapro 5 mg.  It made me suicidal, panicky, nauseated and caused depersonalization.  So after 7 days, I was told to direct switch back to Celexa 7.5 mg.. This is where my problems began.  I could no longer tolerate the Celexa at that dosage.  After extreme heart palpitations, 2 EKGs, an ER visit, and complete loss of appetite,  my care team told me I need more---to get up to 10 mg.  I did that for 4 days before telling them, not working.  Then I was given a 5 mg Celexa and 5 mg Paxil cross taper with the thought that I needed to end up on 10 mg Paxil eventually.  That lasted 3 days because I vomited all day. Told my care team today I am not trying any more of their suggestions. I kept telling them the Celexa -->Lexapro--> Celexa switch changed something and I need LESS of a dosage right now, not more.   As of today, I am now remaining on 5 mg Celexa to stabilize for a month and may work to taper from there.  My current issue is, each morning I wake up so anxious.  I take the 5 mg and feel very ill for about 4 hours.  Then I feel ok the rest of the day.  I assume this is withdrawal symptoms.

welcome to SA. I am so sorry you are suffering so much. What you are going through sounds horrendous. We can try to help you come off of these drugs if that is what you want. But I have a few questions. Please answer as succinctly as possible with the necessary detail to help us understand your situation. 

 

1) Is your goal to come off of these drugs or is your goal to find the right cocktail for you? Why?

2) Why were you switched to escitalopram in September? Was citalopram not working or giving you side effects? How was it not working? What were the side effects?

3) During this time, did you have any other issues - such as Covid, other medications or supplements (steroids etc.), other emotional upheavals. 

4) Why were you switched back to citalopram 7.5mg rather than 10mg 

 

Once you answer these questions we can help better.

Hi, thank you so much.

1.  My goal short term is to get  stabile, long term to wean off all together.  Because I do not think these drugs are helping me at this stage in life.

 

2. I was having occasional anxiety breakthrough on the Celexa, which I'd been on for 13 years.  So my Psych switched me to Lexapro.  The side effects of Lexapro were immediate--panic, depersonalization, a feeling of being a bit "high and manic"

 

3.  No issues during this time--no Covid, no steroids, no upheaval, no supplements, no alcohol (sober 21 years), no caffeine.  I have had 4 COVID vaccines over the past 18 months, however.

 

4. Because the week of Lexapro was so bad, my Psych told me to re-start the Celexa lower,  at 7.5 mg for awhile.  It still felt, from day one, like too much.  Each day, I would take the med about 8 AM.  By noon, I'd be in great anxiety and panic, with severe heart palpitations. Then around 3 PM, it would subside and I'd feel more normal.    My 2 EKGs were normal as was my blood and thyroid.  During the ensuing weeks, my team advised me to go up to 10 mg.  I tried for a few days and could not tolerate it.  So the suggestion last Friday was 5 mg Celexa and 5 mg Paxil cross taper, with the end goal of 10 mg Paxil, discontinuing the Celexa at that point.  I lasted three days on this--vomited all of the days and fainted on Sunday.

I keep saying--these dosages, though small, seem too much for me.  I feel poisoned.

 

5.  So I am on day 2 of 5 mg Celexa only.  I could not tolerate the Celexa and Paxil cross taper.  I know this 5 mg Celexa is a big change from the 5 mg Celexa and 5 mg Paxil combination of the past 3 days, and almost 3 weeks of 7.5 mg Celexa.  But my gut feeling is, I need less of this drug, not more.

 

Both 5 mg Celexa days have gone like this.  Take 5 mg Celexa 8 AM, do ok until noon when the anxiety and palpitations become severe.  Lasts a few hours then I am ok, mostly.  I do feel flu-ish today, maybe due to the Paxil being discontinued.  Since it's only been 2 days since I went down to 5 mg Celexa from a total of 10 mg Paxil/Celexa, maybe I am in withdrawal.

@Schmiddens thank you for answering my questions. 

It seems like your symptoms get worse when citalopram achieves peak concentration in your blood and then get better.  This to me looks like an adverse drug reaction.  I think your days of SSRIs and psych drugs may be over. 

 

Withdrawal usually, (not always), has a different "signature." People tend to have extreme agitation in the morning and then symptoms ease. 

 

The way to ease ADRs is to stop the drug but with psych drugs that's complicated because immediate stopping causes withdrawal so you need to weigh your options - do you want withdrawal or ADR, what we call a rock and a hard place.  

 

It is also possible that you are having both right now but that one is stronger. 

 

I think we need to observe how you do on the 5mg for the next few days and make a choice and stick to that choice until you stabilise somewhat and then proceed. Please keep a detailed symptom checklist with time of day for the next few days and post them together with activity.

Yesterday, I woke up extremely agitated, anxious, and nausated.  I took 5 mg Celexa at 8 Am and an hour later, the symptoms were gone.  At about noon, they returned for about 2 hours.  I had a bit of anxiety throughout the day, but nothing severe.  I've also entirely lost my appetite.

 

I agree with seeing how the 5 mg goes in the next few days.  This is a mess

Update on day 3 post paxil/celexa dosage reduction.

Woke up extrememly agitated, anxious, nauseated, heart palpitations.  Took the 5 mg Celexa, no resolve.  Symptoms persisted until about 4 hours later, when they lessened.  Still no appetite at all

Hello, @Schmiddens. It appears you are now used to 5mg citalopram but also experiencing intermittent withdrawal symptoms. This is typical of The Windows and Waves Pattern of Stabilization , "normal" for withdrawal effects.

 

It may take weeks for the withdrawal symptoms to stabilize into a predictable pattern -- you might not feel them at all anymore. After they completely go away, which is probably some months out, you might consider tapering more gradually, see Tips for tapering off citalopram (Celexa)

TY.  It is so frustrating.  My Psychiatrist says I am not in withdrawal.  I am on day 4 of down to 5 mg Celexa.  I was taking a cross taper of 5 mg Paxil and 5 mg Celexa combination for 3 days, and before that, 4 days of 10 mg Celexa.  Before that 3 weeks of 7.5 mg Celexa.

 

Today I am nauseated, anxious, heart palpitations, GI issues.  Took the 5 mg Celexa at 7:45 AM, symptoms resolved a bit for a few hours,then worse again unit about 2 PM .

 

I feel like this is a combination of the Celexa dose reduction, plus the Paxil leaving my system.  Withdrawal

It would be unusual to find a psychiatrist who can identify withdrawal syndrome.

Hi @Schmiddens

you wouldn't really have Paxil withdrawals if you took it only for 3 days. Paxil is for sure a nasty drug to get off of but other drugs give it a run for its money as well. It's more likely citalopram withdrawals combined with the ADRs from escitalopram that left a trail as well. 

 

But if you only have serious symptoms for a short part of the day, this bodes well.

Get up, anxious, heart palpitations, take 5 mg Celexa with food.  Next 3 or 4 hours feel anxious, nauseated, ill, heart palpitations.

Usually resolves by 1 or 2 PM or so.  No appetite all day, feel flu-ish and tired all day.  Night time less symptoms, sleeping ok.

Thank you, I feel all alone.

On Friday when I talked to my Psych, I told her I thought this was withdrawal, or at the very least, adjusting to the lower dosage.  She would not say anything other than, "Thanks for letting me know.  I hope it subsides soon".

 

I do have some waves in the afternoon, but the mornings are just the worst.

Also, seems like all of this is a very extreme reaction to a drug (Celexa) I tolderated for over a decade.  It's scary.

When you switched to Lexapro, it seems you either had an adverse reaction to 5mg escitalopram (2-3 times stronger than citalopram) or got a withdrawal reaction from going off Celexa. Hypothetically, this sensitized you to even a customary dose of Celexa, a fairly clear example of a kindling reaction, which occurs when a drug is reintroduced.

 

Since you had been accustomed to 10mg Celexa and now are taking 5mg, you may also have withdrawal symptoms from the difference in dosage. But you probably don't want to risk kindling again. 

 

Your system should gradually settle down from withdrawal and the adverse drug reactions. In the meantime, you may find you are sensitive to many things that hadn't bothered you before. Be patient.

TY, this means so much.  It makes sense that this is how I got here.

 

After a few days of being as ill as I've ever been, I now am in the rhythm of waking up with bad symptoms that usually resolve around 4 hours later.  My afternoons and evenings are almost normal, with a few waves here and there.  I hope this is a good sign.

I have a question.  I am not puling out of the heart palpitations, chest tight feeling, anxiety, nausea, etc today, as usual.

I keep thinking I should go to the hospital due to the intense palpitations and chest tightness.

I'm afraid they'll just give me meds.

Any thoughts?

You may want to go to the hospital to make sure nothing is wrong with your heart (chances are that it is not) but if they offer psych drugs you can say refuse to take them or pick up the prescription and not fil it.

When I was on the higher dosages, I would wake up ok, take the drugs, then feel the symptoms build for about 4-5 hours until I felt like I needed to go to the ER.

 

I am on day 10 of the titrate down to 5 mg.  The pattern seems to be that I wake up with symptoms (chest tight, palpitations, nausea, panic, shaky, sweaty), take the 5 mg at 8 AM, then things settle down about an hour later.  But they build again and usually settle down around 2 PM or so.

 

I have the least amount of symptoms at late afternoon and night.  The nausea is constant. however.  I've lost a lot of weight.

I've been keeping notes and it's been a lot of back and forth.  TY so much for the help

 

Day 1-4 very severe

Day 5-6 less severe

Day 7 very severe then flu-ish all day

Day 8 very severe, resolved, was able to eat a meal at night

Day 9 very severe, waves all day, and a few at night that woke me

Day 10 less severe, seems to be resolving 4 and a half hours in today

Thank you so much.  My psych just suggested that I add a 2.5 mg dose of Celexa at night because she thinks the levels drop off at night and it would make my mornings better.  She refuses to believe I am in withdrawal.  I know you cannot tell me what to do here, but I am going to continue on with the 5 mg.  When I was at the higher dose, it was just too much.  It landed me in the ER and EKG place twice. 

 

As additional information, I am a healthy 65 year old woman.

This may be so but only 10 days of observation is too short. In my opinion, let's see what happens over the next week. Your nervous system has gotten a beating and I would not jump up in any dosing that might stress it further.

I so agree.  Thanks Alto.  I am really grateful.  This has been a terrifying experience.  I intend, unless impossible, to stick to my short term plan of stabilizing on 5 mg for some months, and then doing a long slow taper off this med.  I had no idea this drug could do such harm to me.  I am so angry at my medical providers.  I know that does not help, and I'm working to overcome that anger.  One day at a time here!

Hi, Day 11 here.  Took the 5 mg Celexa at 7:30 AM and the morning palpitations subsided at about 10.  Good news there.  Had a few waves yesterday, as expected.  I am overall a bit flu-ish, off balance, nauseated, but hanging on.  My Psych continues to push taking a 2.5 mg dose at night.  She admits I have discontinuation syndrome, but seems to forget that titrating up landed me in terrible condition in the ER.  So, I will give this a bit more time with the 5 mg and continue on.  TY, as always

Given that your symptoms are tolerable, I would not rock the boat and instead wait until the change settles. I think in a few months you will be much improved.

Day 12 of Celexa titrate from 10 mg to 5 mg.  No palpitations or panic this AM, which was good.  Felt flu-ish, but ok.  Took 5 mg at 8 AM.  Did ok until a big wave of panic, palpitations, and nausea at 1 PM.  I understand this is how it goes as things settle.  Thanks for all!

can you track to see if you get your worst symptoms about 4-5 hrs after you take the medicine? Or does it vary? That would indicate adverse drug effects as that is when the medicine reaches peak. If so  you may get relief once you start tapering.

Hi, thank you for responding.  

As a brief reminder--I went from 10 mg Celexa (on this for a decade) to Lexapro for a week and had an extreme reaction.  So I direct switched back to Celexa 10 mg. 

 

At that time of the Celexa 10 mg reinstatement, the worst symptoms would hit 4-5 hours after I took the meds.  This landed me in the ER and with geting 2 EKGs.  All normal.  The medical people kept saying to keep titrating up.

 

I titrated myself down to 5 mg 12 days ago.  For the first 3 days, the worst symptoms came about 4-5 hours in.  Now, they come at different times during the day.  So I don't see a pattern currently that the worst symptoms are at med's peak.  TY

HI, I have a new question today.

As history, I am on day 14 of Celexa taper from 10 mg to 5mg.

Waves come at different times, but today I am especially anxious and nauseated.

 

When this mess began 30 days ago, my Psychiatrist gave me 100 mg Gabapentin to take as needed for anxiety attacks.  She said it was not addicting. I just realized that although I did not take these on a scheduled dose (ie once per day)-- for most of these 30 days, I took gabapentin.  Some days none, some days one, some days two.

 

I read up about gabapentin and realized it can cause dependency.  So I quit it three days ago.  Could gabapentin withdrawal  be a part of my current issue with anxiety and nausea today?  I felt pretty good yesterday.

Hi @Schmiddens

that seems like a reasonable explanation. I usually get my worst symptoms from celexa dose reduction around day 4. It is generally bad for a week and then slowly gets better. If you took gabapentin every day you may need to reinstate and taper it. When did you start taking it regularly (we usually see people get dependent in a month). Can you please put that drug in the signature? That's why signatures are important so we can give you proper advice.

I did not take it every day.  Some days none, some days 1, some days 2, 100 mg, from September 10-October 12 2023.

Lexapro 5 mg 7 days Terrible reaction, depersonalization and panic

Celexa 7.5 mg 15 days, heart palpitations, anxiety, told to increase dose

Celexa 10 mg 4 days, landed in ER with extreme heart palpitations and chest tightening, panic

Prescribed Celexa 5 mg and Paxil 5 mg to cross taper up to 10 mg Paxil 3 days, extreme vomiting and anxiety

Prescribed 100 mg gabapentin for panic, took sporadically, maybe 1 per day Sept 10-Oct 12 2023. 

October 2,2023.  Now on 5 mg Celexa and feel in withdrawal

October 17,2003 Felt what might be gabapentin withdrawal

October 30, 2023.  Updosed Celexa 1 mg by mistake (meant to do .5), not tolerated

November 7, 2023 Back to 5 mg Celexa

Ok, maybe the best thing is to wait it out then but be prepared for a few rocky days.

Aug  2000 - July 2003 (ct, 4-6 wk wd) , citalopram 20 mg,  xanax prn, wellbutrin for a few months, trazodone prn 

Dec 2004 - July 2018 citalopram 20 mg, xanax prn (rarely used)

Aug 2018 - citalopram 40 mg (self titrated up)

September 2018 - January 2019 tapered citalopram - 40/30/20/10/5 no issues until a week after reaching 0

Feb 2019 0.25 xanax - 0.5/day (3 weeks) over to klonopin 0.25 once a day to manage severe wd

March 6, reinstated citalopram 2.5 mg (liquid), klonopin 0.25 mg for sleep 2-3 times a week

Apr 1st citalopram 2.0 mg (liquid), klonopin 0.25 once a week (off by 4/14/19- no tapering)

citalopram (liquid) 4/14/19 -1.8 mg, 5/8/19 - 1.6 mg,  7/27/19 -1.5 mg,  8/15/19 - 1.35, 2/21/21 - 1.1 (smaller drops in between), 6/20/21 - 1.03 mg, 8/7/21- 1.025, 8/11/21 - 1.02, 8/15/21 - 1.015, 9/3/21 - 0.925 (fingers crossed!), 10/8/21 - 0.9, 10/18/21 - 0.875, 12/31/21 - 0.85, 1/7/22 - 0.825, 1/14/22 - 0.8, 1/22/22 - 0.785, 8/18/22 - 0.59, 12/15/2022 - 0.48, 2/15/22 - 0.43, 25/07/23 - 0.25 (mistake), 6/08/23 - 0.33mg

 

Supplements: magnesium citrate and bi-glycinate
