AgentofKarma Posted December 21, 2023 Share Posted December 21, 2023 (edited) Greetings, I’m seeking some information on how easy/difficult it may be to switch from a compounded dose of Paxil IR (15mg) to a tablet dose (10mg tablet plus 1/2). My wife has tapered down from 60mg to 15mg over the course of a few years and has been holding on 15mg for a couple years since. She has expressed perhaps wanting to switch from having her paxil compounded to the tablets and cutting/shaving pills & using a scale to start tapering again. I’m curious as to what people’s experiences may have been making this switch. Any and all info is welcomed and much appreciated. You need to be cautious when making changes to your AD, such as tablet to liquid, different brands of the same AD, etc. I'm sure another member with experience with Paxil/Paroxetine and compounded doses will drop in shortly to add their thoughts. Never experienced any issues. I always did a ‘one on one’ switch. I know some people do have a reaction, maybe because of their nervous systems being sensitive already. So maybe do a slow switch. Like week 1: 75/25%, week 2: 50/50% etc. And then wait a month before starting the taper. Paroxetine: 1995: seroxat at 11 yo for three months (?!). 2008: Jan: started 10mg, was hell (incl acathisia, dyskinesia). 2009: Feb: started taper, May: 0mg, July: went downhill fast (WD?), Dec: started 10mg again. 2015 + 2017: up to 20mg and down to 10mg, no WD. 2021: June: started taper, aug: 2,8mg, short WD, hold on 3mg, more emotional ever since and more frequent panic attacks. 2023: Jan 26: started taper, March 9: 2,2mg, got WD, hold, WD got worse, March 25: 2,5mg, April 4: missed a dose, 2x suicidal crisis April 11 and 23, severe WD ever since. Reinstatement 0,1mg microsteps after six months: Sept 5: 2,6mg to Dec 25: 3,0mg and hold. I think almost 90% recovered after reinstating. Benzo’s: 2002-2021: using appr. 5-10x 10mg oxa a year. 2008 + 2009: 3x 10mg oxa daily for two weeks when starting Prxt, no WD. 2008: used 10-20mg Dia for a few days for severe orofacial dyskinesia and akathisia (when starting Prxt + high dose Metoclopramide and severe underweight), WD? 2022: 15x 10mg oxa. 2023: March 1-24: around 60mg Oxa total. After March 25: oxa daily around 5-30mg, April 12: switched to 1mg Lora daily (crisis), April 23: 3mg Lora daily (2nd crisis), April 27: 2,25mg Lora daily, May 12: tapered further. June 20: reached 1mg Lora daily, got WD, July 01: 1,25mg Lora daily, no relief. Aug 25: 1,5mg Lora daily, got better after 5 days. Dec 5: 1,7mg Lora daily. 2024, started taper: Feb 01: 1,63mg. March 03: 1,599mg. March 24: 1,566mg. Apr 26: 1,557mg, May 1: 1,548mg —-—> Oct 9: 1,41mg (liquid microtaper: 4 steps of 0,5% every 5 days, followed by 10 days stabilization. So minus -2% each month). Intermediate metaboliser (‘bit more slow’) on CYP2D6 (*1/*4) and CYP2C19 (*1/*2). Extensive (‘normal’) on CYP3A4. Link to comment

Having doses compounded seems to simplify the process for many people. 2003-2009 on and off various SSRI's for short periods 2010-2011 Ativan 2013-2021 ativan 1-1.5mg 10-12x/month, daily starting Oct 21 to help with buspar WD 2016 - Effexor 75mg, short-term 2021 Mar -Jun Buspar ADR at high dose, tapered 3 months Oct 22/21 - Direct switch ativan to clonazepam (don't do this) Tapered clonaz Oct/21 - Apr/23 - 0mg! Is the pharmacy making a liquid compound or a capsule? Either way they can tell you who the manufacturer of the drug is. I would stick to the same manufacturer. Firstly, thank you all for taking the time to reply, I’ve been “lurking” on these forums for several years now and I’ve always been struck by the kindness of its members. @Emonda I believe she does have an account, but can find people trials, struggles etc that are written here a triggering thing to read, so I tend to do digging etc for her. Thank you as well for the helpful links, I’ll make sure I read in detail. @MarliesDutch I am glad to hear that you experienced no difficulties making the switch. My wife had a similar experience when switching from tablet to compounded. I will keep your tip in my back pocket, it’s much appreciated. I’m curious if/how you may ensure an accurate dose when “splitting” a compounded capsule. Do you happen to have some experience that you could share? @LotusRising the thought process behind the switch back to tablets revolves around 2 things, travel and control. We have had largely good experiences with the compounding pharmacies that we have worked with, but travel with a compounded medication can be a bit precarious (temperature, loss of luggage etc) as travelling from Canada to the US is something we are looking to do more post-COVID. Control wise, our theory here is that we can taper very slowly by listening to how her body responds, hold as long as needed, and continue when feeling ready, rather than being “stuck” with a costly supply of doses of Xmg. @LostInCanada We have compounded capsules made using a generic Paxil IR and a filler (the name escapes me currently). Switching within the manufacturer makes sense to me. I am curious if you have any insight on if there is any significance to the colouring used on various doses of Paxil. If memory serves, our pharmacy uses 20mg (pink) tablets to make the compounded capsule and we would be moving her to 10mg tablets (yellow if I’m not mistaken?). Your thoughts on that would be much appreciated. Once again, thank you all who have taken the time thus far. Your kindness and thoughtfulness won’t be forgotten. I have been able to use pink and yellow interchangeably without issue. I would not try to split the compounded pill but just move directly to the same manufacturer's tablet. The percentage transition was for liquid to tablet. There are a few generic manufacturers in Canada: Apotex, Jamp and Teva are the ones that come to mind. Please find out which company and go from there. Always request the same manufacturer. I had issues myself with having the drugs compounded as it is not as reliable as the tablet. Also the tablets vary a bit in weight and therefore dosage. It took my pharmacist calling the manufacturer to get the exact weight. So for Apotex, a 10 mg pill weighs 118.4 mg. The other manufacturers will be different. I also weighed a bunch and averaged them and got the same figure of 118.4 mg. My system is highly sensitized so accuracy is very important for me. Definitely still wait one month after transitioning before starting to taper. 👍 I am not a medical professional. LostInCanada Posted December 27, 2023 Share Posted December 27, 2023 @AgentofKarma I bought a pretty decent scale. So my drug plan only pays for 20 mg paroxetine which weighs 236.8 mg. I use a pill cutter to make 4 pieces, then I use a one sided razor blade to shave off some till I reach my dosage. Since 59.2 mg is 5 mg of actual drug, I use that to figure out the drug amount. At present the pill weight is 53.8 mg(mgpw) which is 4.54 mg of actual drug (mgai). The drug is evenly distributed throughout the pill so no need to worry about what part of the pill you use. I don't know if you have a drug plan that covers 10 mg pills but they are alot more expensive than the 20 mg size. It might be easier to use the 20 mg and cut it down to 11 mg. Just remember to get an average pill weight first and then always use that as your standard. Each manufacturer has a different average pill weight as they use various fillers, etc. Also I save all the off cuts because sometimes there are drug shortages and it is better to have some extra if necessary. My doctor gives me a 10 mg prescription so that helps with extra. I am not a medical professional. Unfortunately, both my wife and I are self employed, so we are without a plan. I feel like pill cutting can be easily over thought, I know my head was throughly spun when learning of the pills being greater than the dose weight. I like your method, really simplifies things for me. Do you have a scale recommendation? We had a decent one years ago, but I don’t recall make & model. thank you once again Link to comment

It takes some practice and gentleness but you will be fine. How does the doctor prescribing feel about the taper? Will he be willing to prescribe 20 mg pills? or is it preferable for him to just think your wife wants to go back to 20 mg? 1 I am not a medical professional. We are fortunate to have a supportive GP who is willing to work with us and what’s comfortable for my wife. She has been on Paxil for 20 something years and all of the medical professionals that we have had dealings with in the recent past have shook their heads at the doses she was prescribed (as high as 60mg at one point) and the length of time she was kept on. In regards to pill shaving and shaved down pills, do you take them “as is” or do you put them in an empty capsule for consumption? This is one thing I’ve never actually read anywhere. I cut quite a few ahead of time and have different containers for different sizes. (when I mess up the cut,etc) I have weighed using bits and pieces and stored them in capsules but I open them and dump them on my tongue to take. Just my preference. Some people grind the pill to powder and weigh it in a capsule adding the capsule weight. Definitely and especially if you are very sensitive or when you get to lower numbers. I found the same with my pills. I took 20 pills and put them all on the scale together. Took the reading and divided it by 20. I got the number 236.8 mg. I got the pharmacy to call the manufacturer to verify and that was the exact weight each pill should be. Are you using Apotex 20 mg? If so 236.8 is your measurement. So moving forward use that. So once you start the taper for example if you are going to do a 10% cut- it will be 236.8-23.68. So each pill will be shaved down to 213.12 mg. On the scale that will look like 0.2132. Always round up. 236.8 mgpw=20 mgai (pw=pill weight/ si=actual ingredient) 213.12 mgpw=18 mgai I am not a medical professional. Make sure the table is solid and stable. Try to limit air movement, thankfully this scale has a shield. Remember we are using the weight that includes filler, not mgai. 236.8 mg looks like 0.2368 on the scale/ which is 0.0200 mg of actual ingredient/drug. Hope that answered the question. Then I weigh a piece. (I use tweezers to transfer on and off the scale) On a small plate, I just hold the piece with my finger and very carefully take the razor and slowly shave off bits of powder from a cut side. If you have any extra pills, you could practice. I even practiced in some vitamins, just to figure out how to use the razor and see how much to remove. There are some YouTube videos on it as well. So on the scale 1 mg looks like 0.0010 therefore 0.18944 mg would be 0.0002 mg. The scale I use only measures in 0.0002 mg increments. Maybe that answers your 2 nd question better.
