Where to start...firstly hi! I am a 33 year old female currently at her wits end. I was prescribed anti depressants for severe depression around 13 years ago, I was then put on sertraline 10 years ago as the first 2 I tried were not working as they should. I was on doses as high as 150mg and as low as 50mg. I stayed on 50mg for around 3 years until last year when I started to taper as I was experiencing no libido which was affecting my marriage and I also started suffering from minor anxiety in August 2023 due to 3 bereavements in our family. The GP was convinced the sertraline was causing my symptoms so said to start taking 25mg for 4 weeks and then 25mg every other day for 4 weeks to then stop taking them all together. December 2023 I was completely off sertraline. February 2024, I suffered from a 2 hour panic attack, I had never experienced this before and the paramedics were called as I genuinely thought I was dying. I then had a follow up with my GP a week later and was diagnosed with crippling anxiety and was put back on 50mg sertraline. After 4 weeks I was still bed ridden, shaking 24/7, not eating or sleeping and having 3 or 4 panic attacks a day, some lasting more than an hour. My GP then increased my dose to 100mg. 18 weeks back on sertraline and 13 weeks on the increase to 100mg I have been in a state, don't get me wrong there have been some days in the last 3 weeks where I've not had one symptom but more often than not I wake up with jitters in my legs and feet to the point where I can't stop shaking them, shakes in my hands, on edge and irritated by everything. I have finally gone back to work (home working) which I have been able to do but yesterday I woke up in a state, had a bad panic attack and couldn't log on. I'm still scared to go out, I do it but I struggle and can't wait to get back home. After reading Dr Mark Horowitz's publications I finally feel less crazy but I have no clue what to do. My GP thinks I should increase, the mental health pharmacist thinks I should change my anti depressants completely and the psychiatrist I saw once didn't even believe in anti depressants and just said try CBT. I've had 2 sessions of CBT so far. I am trying everything recommended but my body seems to do it's own thing. I am stuck in a rut and have no idea what to do for the best

On anti depressants since May 2011 years for depression.

Started 20mg citalopram May 2011, increased to 40mg citalopram April 2012, decreased to 20mg citalopram May 2012, increased to 30mg citalopram March 2014, decreased to 20mg citalopram Sept 2015, decreased to 10mg citalopram Oct 2015.

Switched to 100mg sertraline Nov 2015, increased to 150mg sertraline Feb 2016, decreased to 100mg sertraline Oct 2017, stopped sertraline 7/6/2018 after weaning from 150mg to 50mg over 5 months.

Put back on 50mg sertraline 25/6/2019, increased to 100mg 27/6/2019, decreased to 50mg sertraline April 2022.

Weaned off 50mg sertraline from Oct 2023 - Dec 2023 due to minor anxiety symptoms surfacing and no libido (GP was convinced it was the anti depressants).

I was then diagnosed with crippling anxiety Feb 2024, after a 2 hour panic attack and weeks of horrendous symptoms.

I was put back on 50mg sertraline 19/02/2024, increased to 100mg on 22/03/2024 (as advised by GP).

I am still on 100mg and nearly everyday I still suffer from symptoms.

Hello, and welcome to Surviving Antidepressants. We are a peer support forum to assist in tapering off psychiatric drugs safely, or recovering from psychiatric drug withdrawal.

 

It sounds like you are experiencing withdrawal from tapering sertraline too fast, and then reinstating a large amount of the drug. When you have taken an antidepressant for more than 10 years your brain cannot function without it, this is called dependency. You are now in The Windows and Waves Pattern of Stabilization. None of us can tell you how long it will last, all we know is the best thing you can do is hold on the same dose for the coming months. Things will improve with time and patience, you can still work on your anxiety through non-drug coping techniques like CBT. I would highly advise you do not further increase your dose, nor that you try another drug. Your brain is adapted to sertraline now, whether you like it or not.

Hi, thank you for your reply. Yes, I realised after seeing information on other websites about tapering and reinstating that all what I had done through the guidance of my GP was wrong. I am going to try and hold out on 100mg for a bit longer and I have started CBT, had 2 sessions so far. I just struggle everyday and try my best to get through it but it's so hard, it's also so hard not knowing when I will be back to my normal self. I'm currently on a different manufacturer as well which I've been told doesn't help but my GP said they can't ask for one specific manufacturer. Just a really awful 4 months and I wish I found this group and Mark Horowitz sooner

You absolutely can ask for the same brand if you push for it. The NHS like to penny pinch but I requested to have my prescription changed to Lustral, which is the name brand. It was a pain but after some persistence I got it sorted. You can also speak to the pharmacy and get them to add a note to your prescription with a brand preference.

 

When you are more stable you will find that brand changes don't impact you as much.

I've heard a lot of people say Lustral is the best, I've never had it. I was on teva for 12 weeks straight then it was milphram which I've been taking for the last week and a half. I dunno whether to now stick on milpharm or go back to teva. Either way I always have the jitters etc so don't think it's the manufacturer

Once you've taken a brand for a week or so you will adapt to it, so eventually any changes will have minimal impact. The only difference between brands is the fillers used to bulk the tablets, and the coating.

Yeah I must admit the teva ones are huge and have a horrible coating on so if you don't swallow them quick enough they taste vile. I might ask to stick to milpharm, I dunno anymore haha. I picked up another prescription as I'm due to go on a cruise this weekend (all being well) so I asked for back ups just incase and I've got teva again so I dunno what to do.

Yeah I think I've had them before. You can break them in half to take. You can even dissolve them in water and drink them, but it won't taste pleasant.

Ew I definitely won't dissolve them in a drink haha. I dunno whether to just stick with milpharm now, God knows. I'm just getting so frustrated with it all

After a horrendous 18 weeks back on sertraline following a 2 hour panic attack which came out of the blue in February 2024 after weaning off over 2 months Oct 2023-Sep 2023 (following being on them for nearly 10 years) I am still struggling with restlessness in my legs. As soon as I wake up in the morning I can't stop shaking either one or both feet and legs to the point where my feet start to hurt. I move around and it stops but as soon as I sit back down it goes. I find it's intermittent during the day and worse in the mornings and at evenings. As soon as I stop doing anything it starts but I can't just never sit down because of it. I am trying to hold off on making any changes to my dose as it seems my GP put me back on too high a dose which has caused all of my ongoing issues (was back on 50mg for 4 weeks, increased to 100mg and been on this for 13 weeks). I am trying my best to get through each day but it's so hard. I have no motivation or enthusiasm I just don't want to do anything each day and these jitters take control. The mental health team think it's my anxiety rather than the meds and discharged me as I now am under CBT therapy.

Hi,

 

I am really struggling again. I was on sertraline for nearly 10 years for depression until last December 2023 when I was weaned off over 2 months following guidance from my GP. They thought the sertraline was causing me to experience very mild anxiety symptoms which started in August 2023. Following a horrific panic attack and being diagnosed with crippling anxiety in February 2024 I was put back on 50mg sertraline, increased to 100mg 4 weeks later, again following guidance from my GP. 19 1/2 weeks back on sertraline, 15 weeks on 100mg and I still have days where I am constantly on edge, feel panicky, experience shakes in legs and feet, sweats and feeling spaced out. I don't know how much longer to give this before thinking about either increasing or decreasing. This week I have had 3 good days, 1 really bad day and 3 average days. Don't get me wrong I'm nowhere near as bad as I was but I still don't feel right or back to my normal self and not sure I can carry on like this for any longer

This sounds very encouraging to me.

 

I've read accounts of others that have taken 6 months plus to settle after reinstating. People can feel terrible from the CT, then feel terrible from reinstating too high.

Try to remember these good moments when you feel yourself struggling. Over time the windows will become better and longer. Increasing your dose will not fix any of your problems, considering you have not been above 100mg for 6 years.

Yeah I think I have to be more paitent, it's just easier said than done. It doesn't help I've tested positive for covid today so I'm feeling really rough with that and it's heightened my anxiety too

We’re in similar boats here, my friend. Sert and stabilizing. 
 

Im currently in a gnarly wave that’s lasted a few days. I also have very good days and very bad days and just average days. It’s seemingly getting better over time (Im at the 13 week mark), I still have REALLY awful days that make me question everything. 
 

Anyway, today is a bad bad day for me. I see you’re at that point also. Let’s pray for each other. Feel free to vent to me! 

Zoloft 25 mg (Jan 2016)

50 mg (Feb 2016)

Off Zoloft for second pregnancy (2019)
Back to Zoloft 75 mg (Sept 2019)

Down to 25 mg for third pregnancy (2020) 50-> 75 mg ->100 mg (Jan 2023)

Felt good for long while so reduced to 75 (Nov 2023). Back up to 100 after some symptoms of anxiety (Dec 2023)

125 (Dec 2023) didn’t help so->150 (Feb 2024) Horrible reaction (almost psychosis) 
Doc suggested lowering Z and adding buspar. Started 2.5 mg 3x per day same day at going down to 125 mg Z.  then to 100 a week later. 
Found SA, stopped the madness.
(April 2024)87.5 mg Z and holding 

(May 22) Natural Calm: 200 mg per day in water

Thank you! I'm 15 weeks on 100mg today although nearly 20 weeks from being back on sert in general. It's knowing how long to give it to say right I need to change something here or just allow more time. I'm really poorly with covid the last 3 days so feeling very sorry for myself

Unfortunately I have been struggling with my anxiety because my covid has been so bad. How are you getting on?

On anti depressants since May 2011 years for depression.

Started 20mg citalopram May 2011, increased to 40mg citalopram April 2012, decreased to 20mg citalopram May 2012, increased to 30mg citalopram March 2014, decreased to 20mg citalopram Sept 2015, decreased to 10mg citalopram Oct 2015.

Switched to 100mg sertraline Nov 2015, increased to 150mg sertraline Feb 2016, decreased to 100mg sertraline Oct 2017, stopped sertraline 7/6/2018 after weaning from 150mg to 50mg over 5 months.

Put back on 50mg sertraline 25/6/2019, increased to 100mg 27/6/2019, decreased to 50mg sertraline April 2022.

Weaned off 50mg sertraline from Oct 2023 - Dec 2023 due to minor anxiety symptoms surfacing and no libido (GP was convinced it was the anti depressants).

I was then diagnosed with crippling anxiety Feb 2024, after a 2 hour panic attack and weeks of horrendous symptoms.

I was put back on 50mg sertraline 19/02/2024, increased to 100mg on 22/03/2024 (as advised by GP).

I am still on 100mg and nearly everyday I still suffer from symptoms.

@Shelley123 I’m sorry you’re struggling. I hope the illness passes soon. Im still in a bad wave 🙁

Zoloft 25 mg (Jan 2016)

50 mg (Feb 2016)

Off Zoloft for second pregnancy (2019)
Back to Zoloft 75 mg (Sept 2019)

Down to 25 mg for third pregnancy (2020) 50-> 75 mg ->100 mg (Jan 2023)

Felt good for long while so reduced to 75 (Nov 2023). Back up to 100 after some symptoms of anxiety (Dec 2023)

125 (Dec 2023) didn’t help so->150 (Feb 2024) Horrible reaction (almost psychosis) 
Doc suggested lowering Z and adding buspar. Started 2.5 mg 3x per day same day at going down to 125 mg Z.  then to 100 a week later. 
Found SA, stopped the madness.

Oh no, I hope your bad wave passes! I had a panic attack earlier because I had a coughing fit and couldn't breathe. I just want everything to be back to normal
