I am really struggling right now and will try to make sense. 

 

I have been suffering with MCAS since the end of last year and was worried I may have been reacting to the fillers in my medications. So I started making the reductions seen in my signature. I didn't understand how stupid this was until finding this forum...I thought it would be a few weeks/a month of suffering after each reduction. 

 

I have also started ketotifen 1mg for mcas on Friday 14th June, a trial period, and it has really complicated things. I am also unsure of interactions, as the site linked here didn't list ketotifen.

 

Currently I am desperate for stability, and am happy to continue with/reinstate the escitalopram until I can find stability and then taper using this forums advice in the future when I am hopefully more stable. 

 

After reading here that reinstatement should be done at a lower amount, I raised my dose by 0.25mg as seen in my signature. I felt I had a few days of some relief, but the last 2 days now my symptoms have been increasing and I'm really scared. Now I keep getting cold body rushes and intense fear, lasting most of the day and increasing. 

 

I have seen that there is a window for reinstatement, and would like to do this & am praying it helps. I am really unsure of how to, and what amounts. Should I raise by another 0.25mg, to 1.5mg for a week and reassess? Or straight to 2 drops? Maybe more?

2017- dropped to escitalopram 10mg

2018- dropped to escitalopram 5mg

7th May 2024- dropped to 3.75mg 

25th May 2024- dropped to 2.5mg & CT'd street amphetamines 

30th May 2024- started 2 drops/mg liquid escitalopram 

8th June 2024- reduced to 1 drop/mg of liquid escitalopram 

18th June 2024- raised to 1.25 drops/mg of escitalopram 

26th June- last ketotifen dose

7th July 2024- increased to 1.5mg escitalopram

It's been getting worse all day....it hasn't let up. I'm close to taking 1ml diazepam. I was prescribed them with ketotifen for mcas a few weeks ago, 2mg tablets. I have taken 1 when needed 2 or 3 times but not for a couple of weeks and after reading on here I'm scared to but I can't cope with this scared anxiety feeling I was close to a panic attack a few hours ago. I don't know whether to reduce ketotifen, increase escitalopram or take diazepam but I can't stand another day of this 😞

I did indeed stop the ketitofen that day, and each day became a little easier. I was in an extremely bad way.

 

I am now suffering some waves of symptoms throughout the day, usually worst in the morning & a few other waves in the afternoon/evening. They seem like quite typical withdrawal symptoms now....can feel quite unbearable at the time but do pass. I understand this can be a long process, and can get worse?

 

I'm unsure if I am stabilising on the 1.25mg, or if increasing to 1.5mg soon would still be wise. I am concerned about adverse reactions too, as my MCAS also makes me react negatively to many things. How do these differ from withdrawal symptoms and how would I identify this? 

 

Any advice is much appreciated, thank you x

Wait a couple more weeks from stopping the ketitofen. If things are improving there is no need to increase.

Hi Erimus,

                   I decided to raise by.25mg on Sunday so am now at 1.5mg. I will try to update my signature after writing this. 

 

Last time after raising to 1.25mg, 9 days later I had that real bad panic episode (27th June) lasting the whole day. I did take 2mg diazepam that evening in desperation as I was experiencing sheer terror & couldn't cope any more after fighting it all day.

 

The following days were bad but much more bearable. Teary, passing milder waves that I could use distraction, breathing, mantras etc to get through. Still hell, but not to my limit & not continuously extreme & building the whole day. They were getting milder. 

 

Thursday 4th I noticed a slight increase in the anxiety/panic/fear and cold rushes in my body. It has increased every day until yesterday was significantly worse and today it has been unbearable. I am currently pretty much sofa bound and trying to make it through moment by moment. It's like an extreme fear that nothing will interrupt or work for, absolutely terrifying. 

 

Please can you let me know if you think this is due to my increased dose....last time it was 8 days after raising until I had this horrendous wave, this time only 3 days but ketotifen may have affected last time. I am so concerned about adverse reactions, or if this is withdrawals. It is honestly unbearable, inhuman. Terror to the extreme and I'm really scared 😞

 

Also after reading here more I think it's relevant to disclose that I CT'd amphetamines on 25th June. Had been taking daily most of the time for about 6 years and several times a week before that for a few years. I think i relied on it to mask the side effects of escitalopram and function. I didn't realise stopping it would complicate this process so much but I'm now incredibly concerned.

I take my escitalopram in the evening around 8pm. I haven't noticed if I feel worse in the following 6 hours yet as it usually starts when I wake to the feeling, I have been blocking morning light to try and help. It then builds through the day...

I think I'm experiencing extreme akathisia. To a completely unbearable level....I had the choice between diazepam & hospital a few hours ago. I'm so scared to go to hospital...please help

What symptoms are you experiencing? How does it feel?

 

Have you made any more changes since your increase to 1.5mg? Taken any other medication, antibiotics, alcohol use, illness, dental work?

Thank you for getting back to me I really appreciate it. No I haven't made any other changes. 

 

I wake and can't sleep with mind spinning, then no matter what I do it progresses. I've been practicing lots of different methods of dealing with it that worked/helped last week but nothing stops this. Baths etc cold shower. It's like an impossibility to stop it, until I was at the point of going to hospital, doing something stupid or taking diazepam. A crazy inner restlessness that I can't seem to interrupt or relieve. Combined with nausea & very thirsty. I'm so scared x

Last night I took my dose at 9pm. Still wasn't great at all but was coping. Then I managed to sleep around 11pm-4am, woke up & then it wouldn't stop. It seems to be worse in the day time but today has been very extreme, so much fear & terror..

And my brain has been feeling hot intermittently. I don't know if I should take the 1.5mg tonight or drop back to 1.25mg. It's building again 😞

I think it would be sensible to drop back down to 1.25mg.

Reducing back to 1.25 seems to have helped...the incessant fear & agitation is milder and more broken up. I hope it was reducing that has eased it, and that it doesn't return soon as a wave. Do you have any thoughts? Thank you. 

 

I'm currently taking Cipralex liquid drops 20mg/ml. I take one drop in a glass of water, and then I add 1 drop in 10 ml of water & take 2.5ml of that. Do you have any advice on how to ensure I'm taking a consistent dose? Or do you think this is acceptable. I have seen that drops aren't very accurate?

The last few days I've been having increasing head pressure whenever I'm upright for more than a few minutes. And also sitting in the sun made me feel incredibly ill....

 

I also have long covid but everything has got so much worse since reducing my meds but obviously I think I'm too sensitive to reinstate any further as that was terrifying. Do you have any insight?

 

Also @Catwoman73 I have read your topic and know you have some experience & insight with long covid, have you noticed anything like the above in the LC community with wd's? Thank you both 🙂 x

I have had the pots hr rises for over a year but rarely symptomatic other than palpitations. But now I feel like my head is so heavy, like an extreme increase in gravity. Hoping it passes with waves & windows but pretty scared I will be stuck like this!

Hi @xTJx- I'm sorry to hear you're struggling with this.  I completely understand... I have nerve pain in my head (likely trigeminal neuralgia), but I also get that pressure feeling from time to time.  For me, it extends through my neck and chest as well.  Almost like my head, neck and chest are a balloon that someone is trying to blow up, and it's just about to burst.  It's a terrible feeling.  Interestingly, it completely goes away when I get in the pool?  No clue why lol- likely something to do with the buoyancy we have while in water.  But it comes back within minutes of getting out.  The whole thing is very strange.  

 

Keep in mind that long covid is an episodic illness.  Just like withdrawal, it comes in windows and waves.  My suspicion is that, with all the changes you've made over the last couple of months with your meds, it's catching up with you now, and you are likely hitting a wave with both conditions simultaneously.  That happened to me a few weeks ago, and I remember saying to my husband that I was actually feeling as bad as when I was acutely ill with covid.  But it passed, and it will pass for you, too.  You aren't stuck like this!

 

In order to ride out these really tough times, I practice mindfulness, guided meditation/progressive relaxation, and engage in very gentle exercise.  Slow, mindful walks, where I pay close attention to what I can see, hear and smell around me are very helpful.  And gentle swimming is my absolute favourite!  I also find it helpful to look at my symptoms through a very clinical lens, rather than attaching any emotion to them.  When I feel truly terrible, I often try to look at my symptoms as 'interesting' rather than scary, unbearable, or any other negative adjective.  As I always say to my daughter- change your attitude, change your life!  I find it much easier to bear if I remove the emotion from the physical sensations.   It takes practice, but it really works! ❤️

1995- 2007- On and off multiple antidepressants (Prozac, Paxil, Effexor, Wellbutrin, escitalopram). Memory poor- can’t remember dates. Always tapered fast or CT.  2007- tapered Wellbutrin, zopiclone and escitalopram over one month to get pregnant.  Withdrawal hell for many years.

2009- Daughter born 🥰 Post partum depression/psychosis- no meds taken.

2016- Back on escitalopram due to job change/anxiety

2022- Severe covid infection- Diagnosed with long covid 08/22.

2023- 01/23- Long term disability approved for long covid.  Started taper under MD advice from 20mg: 11/23- 15mg. 2024- March-10mg. Started low dose naltrexone for long covid-5mg- terrible reaction, reduced to 0.5mg.  April- 10mg escitalopram, 1.0mg LDN. May 1- 9.0mg escitalopram, 1.0mg LDN. May 15- 9.0mg escitalopram, 1.5mg LDN.  June 12- 8.5mg escitalopram, 1.5mg LDN.  July 8- Brassmonkey micro taper started.  8.4mg escitalopram, 1.5mg LDN.  July 15- 8.3mg esc, 1.5mg LDN.  July 18 8.3mg esc, 2.0mg LDN

 

Supplements/other meds: Vitamin D, B12, Claritin
