Hi Ripley. Thanks for coming by. It's frustrating for sure when all was going well to have this med change. I slept decently last night with starting the crossover. I had a bit of a headache though, it could be other things. I sure hope this will work. It will save me time and aggravation. It's also very convenient to have it made up for me.

Your decent sleep last night bodes well I think for your crossover, fingers crossed. 

 

I've been holding for 16 months and due to start tapering again soon, so not much to report atm! I'm just hoping it goes a lot better than last time! I'm certainly way more attuned and wary than I was which should help.

looks like you’ve hit a bit of a rough patch, I hope things settle for you soon.  I have found every time I think I’ve got this withdrawal figured out, I don’t.  Seems every cut is similar yet different from the others.  My cut to 1.5 was a particularly nasty one, but to 1.2 I have barely felt anything.  Again, I hope you find stability real soon.

Dec 2020, health issues, severe insomnia started

Jan 2020 Doc thought anxiety and started duloxetine 20mg

Feb 9 2021, dose increased to 30mg

Feb 16, 2021 due to side effects, told to stop, opened

capsule and dropped a few beads at a time while starting 7.5 mirtazapine

March 15, finished duloxetine taper, continued on mirtazapine

June, tapered 1/4 pill mirtazapine every 2 weeks under doctors orders, insomnia came back so back on 7.5 mirtazapine

august started taper again after being back on 7.5 for few weeks

8/27/21 - 5.7, 9/17 - 5.0, 10/1 - 4.3, 10/11 - 4.0, 10/21 - 3.7, 10/31 - 3.2, 11/7-2.9, 11/16 - 2.4, 11/26 - 2.0, 12/4 - 1.8, 12/11 - 1.5, 12/29 - 1.2, 1/12 - 1.0, 1/22 - .8, 1/29 - .6, 2/9 - .5, 3/5 - .3,

Hello everyone. I continue to do the 75% new, 25% old Mirt liquid. I am getting sleep but it's very choppy. I wake up every couple hours with the last wakeup around 3:30 AM. It's a cortisol surge I believe. I've had them at different times during the different withdrawals. Two nights ago I woke about 3:30 AM but could not get back to sleep. This morning I got back to sleep some. It helped but it was with some vivid Mirt dreams. This is funny. In my dream I was good friends with Kim Kardashian and hanging out with her. Gotta love those Mirt dreams. I'm tired more in the AM.

 

I know one thing for sure with insomnia patterns.They always change. Surely the stress I am feeling about the change in med is leaking into my sleep. I'm able to contain it mostly so that's a good thing. I am curious what happens when I go 50/50 with new and old. Late afternoons and evenings are mostly windows. So I am hanging in there. Be well. Hello. I slept really well last night (over 7 hours) with only one wakeup. There was no jolting cortisol wakeups which I hate. I hope this bodes well for the crossover. I think I really won't know until it's at least half and half. What I find interesting is that I am in good windows these days. I've been holding the dose since the middle of November. Mornings are pretty good too. So now I know if I want to, I can hold two months to be stabilized and windowy.

 

I'm still waiting for my pcr covid test result. It's now a joke in my house.

Hello everyone. I just wanted to update on how the crossover is going. It's been okay so far. I had one great night of sleep, a couple pretty good, and a few days waking up too early. I think the sleep stuff will change up and improve at some point after the crossover. I am finished with the 75% old, 25% new. Tonight I'll change to 50%/50% and do this for a week. I'm just a little tired, but no other symptoms.

 

I hate this whole business of tapering but it will be so very worth it once I'm done. I'll keep going. Be well. Marie.

Hello. This is just an update for my records. The first night of 50%/50% split of the new and old liquid Mirt I got lousy sleep of about 4 hours. It's amazing that I don't feel that badly when I get a bad night. I guess I am used to it. Last night though I got 10 hours, a luxurious amount. Even though I am switching liquids I do not have any other w/d besides a bad night of sleep here and there. I'm sure it's because I have been holding the dose since November. I can't wait to get back to tapering.

 

The lab that was supposed to give me the covid pcr test now says they never got it. I wanted to know if I had antibodies from an infection to give me better protection. I'm a little pissed about that. Stay safe. Marie.

Yes, you can use Propylene Glycol.

 

The ratio is 1mg : 2ml PG : 8ml Water

 

You can scale that ratio to make up to 10 days.

Current status: Venlafaxine XR 29mg, 10mg Prozac, 10mg Valium

  • Polymedicated. Clonzepam and various antidepressants for approximately 17 years - 20 years.

Hello. This crossover to the new liquid Mirt does not seem to be working. I just don't understand why because it's the same brand of Mirt. I have had three straight days of practically zero sleep, maybe 1 hour or so of micro-sleeps. I am at 50% old liquid and 50% new liquid. I'm 12 days into this crossover.  I am not sure how long I should keep at it. I would have to return to the Mirt pills and cutting and weighing. I am going to look for my scale today. This insomnia is for the birds. I think I'll order some Bachs Rescue Sleep and some Charolotte's Web CBD gummies. The reviews are good for sleep. My only problem is very bad insomnia.

 

@ChessieCat What do you think about holding some more or should I just go back to the pills? Thanks. Marie.

I spoke to the pharmacy again and explained that this new mixture is giving me insomnia. She said that they can't get the old one anymore. She asked if I cut the dose and maybe that is why I have insomnia. I did not cut the dose. In the back of my mind I was thinking that one the pharmacists might have made the compound differently so there was a deficit of Mirt. I was speaking to my spouse about and he said, "Maybe the pharmacist effed up". He said maybe you should try to take a little more. This did cross my mind and I've decided tonight I would updose just a bit to see if it helps. I will take it for 4 days to see if it makes a difference. If not I'm going to go back to pills. I don't want to, but I think I have too. Now I just have to think of the magic number. Marie.

Hi Bestill. Thanks for coming by. I slept 8 plus hours. It was great and luxurious. It seems to be a pattern though now with bad sleep for a few days then I crash and get a great night. I changed a couple things too so maybe it will work out.

 

Last night I added a small amount of .03 mg Mirt. I also realized that lately I ran out of my half caff coffee in the morning and was drinking a cup of regular caff coffee for a week. I'm back to the half caff. I'll wait a few days and see how the updose works and if my sleep levels off a bit. I have enough of the old med to last a few weeks, but it's old and getting older. When I crossed over from pills to liquid originally I took my time. It was 3 months until I made the complete switch. I can't take my time now. I hate being so sensitive but it is what it is.

Hi Marie, sorry to hear about this latest glitch but glad you got some sleep last night. I’ve been sleeping better than at any time since I started taking mirt 13 months ago. I drank a cup of caffeinated coffee last Saturday afternoon and only slept 3 hours that night. Won’t do that again! Hope you can manage the switch. I’ve been making liquid since the beginning of my taper and really like being in control. It is a (minor) hassle to make it every night but I’m down low enough I can get two doses from one 7.5mg pill now. Take care!

Hello Gigi68. I like being in control of tapering too. I let the compound pharmacy do it because when it was time to taper Mirt my brain was still fried from my last two tapers. I did not think I could do it at the time. I like the convenience of having someone else do it though. It was going great and the taper got easy. I might add another week or more to the crossover, we'll see. The crossover may still work if I stretch out the crossover. Running out of the old med is an issue. I think the old med could last a couple weeks though. I'll know if it works if the sleep evens out. I am going to try my best. Be well. Marie.

Hi Marie,

I have been reading about this latest challenge you are having. I am sorry. I know you will come out of it as you do every other hurdle. When I read you were drinking regular coffee by mistake, at the same time the insomnia set it, I had some real hope for you. The same CYP450 enzyme that metabolizes mirtazapine, metabolizes caffeine. So, just maybe this has slowed down your mirtazapine metabolism and caused the insomnia. If I drink any caffeine I get no sleep. I have been genetically tested and I am a slow metabolizer of 2D6, and caffeine metabolizes it. MAYBE just cutting out your (small) amount of caffeine, coupled with a longer hold will do the trick. Your body/brain are way more amenable to change than mine. Keep trying. You are doing great.

  • Current dose of diazepam is 8.8 and valium is 5.7.  I had to up-dose the total valium/diazepam from 12.74 to 14.5 where I have stayed since June 2023.  I am crossing over to generic at a somewhat tolerable rate of .3mg/month after about 2 months of trial/error w/ updosing.  I am not currently tapering; will continue to cross over.

Coffee is considered a high histamine food by some as well.  I CRAVE coffee, but I totally abstained for 5-6 years.  I am back to drinking about 4-6 oz. of organic decaf/day which has a little bit of caffeine in it but not enough to change my sleep pattern.

 

2D6 is one of 5 major CYP450 enzymes that metabolize almost all psychiatric drugs.  Psych drugs are often metabolized by more than one of these enzymes making it all much more complicated.  I have seen mirtazapine listed as a substrate (affected by) 2D6 on many chart and not on others.  Mirtazapine seems to be a very poorly understood drug as far as mechanism of action.  SIGH!

 

I feel for you with this cross-over.  When I was still on generic valium (diazepam), I tried having my pills made into a liquid by a compounding pharmacy.  They kind of treated me like a criminal, but they did it.  I had to bring the pills as filled from the regular pharmacy to them (stapled tightly and not tampered with!) and they made a liquid solution.   The first night I felt like I was in complete withdrawal and I didn't taper anything from my dose!  Of course, no one believed me.  I think my body just absorbed the medicine as such a different rate.  THAT is when I threw up my hands and went to brand-name only, which costs a fortune.  Remeron (mirtazapine) is getting harder and harder to get in brand-name.  It worries me.   I try not to think about it. 

 

I would be tapering remeron now but I had a huge setback, nutritonally, and am now trying to adjust to B12 injections and also supplementing with folate.  It has been super stimulating but ignoring my essential nutrients wasn't working for me any more.

 

The point is,  you have come against so many things that would have felt insurmountable to me...all your dental work, traveling, COVID , etc. and you keep recovering and moving on.  I think you are a normal metabolizer, likely, but just can't take too many challenges to the same enzyme.

 

Crossing my fingers for you.  Yes, genetically the CYP450 system has been, and will continue to be, my Achilles' heel.  Once it's broken, you can't fix it.  You can just try to control the challenges it offers.

Hi Marie,

I am sorry to see what you have been dealing with recently trying to crossover to the new liquid.  I hope things are settling down for you and the rest of the crossover goes smoothly.  Strange how our bodies react to the slightest of changes with these tapers.  I am finding these lower doses most difficult, I’m starting to get a lot of body aches and pains, especially back and hips, hoping they go away soon but we never know do we.  It’s been making sleep a bit difficult and now it seems like every morning, very early like 4am, my body feels like it is vibrating internally, again mostly back and hips, sometimes ribcage, not painful but very annoying, can’t sleep through it.  I hope the suffering lightens up for all of us here on this forum. Hello. It's still not going well. I am in a pattern of 3 days of very little sleep following by 1 day of great sleep (8 hours). I'm fine with five hours at least. I seem to be crashing after 3 days of little sleep to get one great day. I can't keep doing this so it's looking I might have to abandon liquid for now and go back to pill form.

I believe there is something in the new mix that is getting to me. I'll hold on for now see how the updose works and possibly updose another .03.  It's been 16 days like this.

 

I'll regroup on the pills, stabilize and try making me own. I will use maple syrup. I know my system likes maple syrup lol. Cutting/Weighing could be hard getting the weight right due to the fluctuation of the scale. Well one way or the other I will leave Mirt behind.

 

I wonder if anyone has gone back to taking the pills instead of the liquid. I've been using the old liquid successfully for over a year and a half. Will my nervous system take it well? I guess I will find out. Be well everyone.

Hi Marie, 

 

I have not heard of using maple syrup It seems like that would make a suspension and not a solution which would be problematic. Perhaps this is a new liquid taper I have not heard of. 
 

I am very familiar with the sleep pattern you have. It is really hard to live life normally. The night I crash is wonderful though. 
 

You will get through this. 
 

Hmmm…the same “old” liquid for a year and a half? I wonder if the strength weakened and the new liquid with new pills are just stronger and gave you an up dose?

Oh, Marie, I just read about use my maple syrup to taper. I guess you shake it a lot and work fast. I LOVE maple syrup. But the good stuff!

Hi Grace. I have a concern about going back to the pill form of Mirtazapine. It will be the same dose I am taking now but I  have not taken the tablet form in a year and a half. I was stable on the liquid before I got the new pharma liquid. Essentially I would do a direct switch from liquid to tablet. Do you think it would feel the same?

 

Mirt liquid works better in a suspension. I feel perfect when I crash and get the 8 hours. Marie.

I am just wondering if I understood you right...the liquid you are using is a year and a half old?  If so, I would be concerned that the new liquid and the old liquid could not possibly be the same potency;  I guess that is my opinion, and I obviously could be wrong, but I don't know about using liquid that is that old.  I know you have been using it, and that is what your body/brain are used to, so this new liquid just might be weaker or stronger?  I cut up pills and use them but I discard the pieces pretty quickly as I am just not sure what "exposure" does to them.

 

I think it may be hard to dry taper with the small doses you are taking and the small cuts on the inexpensive scale when you get way way down.  We bought a (very expensive) scale to get more accurate, but I have been, and will be tapering forever.  I wish I had a better answer for you.  We found the Gemini (or like scale) too inaccurate for the low doses.  I measure 2.74 mg. valium and add it to a 10 mg. pill.  The pill did not weigh enough for us to get to 2.74 mg. (dose not weight)  accurately.  Without an expensive scale, liquid tapering would be necessary, I think.  The other thing I did long ago was switch to brand-name...expensive but too important to cut corners.  That said, switching from generic to name-brand is a guessing game as well.

 

Wish I could be more help.  We have gone down a lot of different paths and I am barely tapering.  I have to be THAT careful.  I know you feel awful but the only way is going to be trying something different.

 

I hope this didn't confuse you or upset you.  This tapering business is so hard and so complex when we have to go so slow and make such small cuts.  Too many variables.  The people who are able to do it seamlessly are so lucky.

Hi Grace. When I say old liquid it's not that old. I get monthly liquid med from the compound pharmacy and was using it monthly for a year and a half. I have a little bit leftover from this past month. I refilled the liquid two weeks ago and they gave me a different concoction. The new concoction is not working for me. Monthly I go with my 30 Mirt pills, give it to them, and they make my monthly liquid.

 

I'm think I'm ready to abandon the new batch of liquid. I have some of last month's batch (the good stuff). I think I will just use that until it runs outs and then switch to the pill form of Mirt.

 

I see you tapered mirtazapine down to 4mg. Did you use your expensive scale for this or liquid? I would pay more if I knew it could be accurate. Was it really much more accurate? Thanks.

Yes, we were fine with the old scale with 4 mg. mirtazapine although we can't be sure how fine as I am never really stable. The inexpensive scale is accurate to .001g  (pill weight; not dose) My husband had a chemistry major in college (and taught chemistry).   He does/did the math on all this.  With the valium, I needed to make really teeny cuts and we just could not make them accurately on the Gemini scale so we transitioned to the scale here.

Because we already had the scale, we use it for all my meds now.  It measures out to .0001g. and allows smaller cuts. (some teeny margin of error with any scale)   If I EVER get well enough to taper remeron again, I am counting on this to make the cuts more accurate as we find that remeron is not easy to cut without it breaking up.  Of course you can crush the pills and weigh that way too,(people do this and put it in capsules; there is more waste, I suppose)  but so far we are sticking with whole pills and a razor blade.  It is tedious, but since you know just how lousy being in withdrawal can make you feel, it is worth it.  (My husband cuts three meds for me....you only have to cut one) 

 

If you need/want help with the math, on your old scale, we still have ours.  Do you use 15 mg. pills?  If not, we would need to know the weight of the pill you use, and if you need help with the math with your cuts, we would be happy to help you.  We do this whenever I cut trileptal.  I am only making 5% cuts once in a while but the cuts are about as accurate as you can get.  As you can see, the scale is pricey, but the peace of mind is worth it.  Liquid tapering was/is not going to work for me.  I am not sure what this looks when your dose gets really small...you can use a counter-weight to help with that but that is sometime down the road.

 

Let me know if you want/need any more help.  I don't want to make you worse by dizzying you with too much information.  I can't help myself too much but maybe i/we can help you?

Hello. Oh I see you tapered the Mirt to 4 on your scale. That scale is pricey but worth it if it gets you off these meds. Someone recommended the Gempro 250 and said it was accurate too. Someone was selling it on ebay for 150. I was practically an expert on the Gemini after that trazodone taper. I'd like to stick with it if I could. We'll see. I remember weighing Mirt and getting the average. The scale showed .750 I believe. The manufacturer does that I believe on purpose.

 

Yes it was the same brand of Mirtazapine mixed with a different liquid. The liquid they used I can't handle.

I hope you get whatever is happening straightened out soon, how aggravating it must be.  I just wanted to share with you that I have a little cheapy scale (think is was $25) I bought on Amazon for this mirtazapine taper and I have been dry cutting and using it all the way down to my current dose of .8 without issue.  I have found that it is sometimes easier to weigh the smaller doses by putting one of the weights on the scale and figuring the increase from there. For example, this morning I sat down to cut a few weeks of doses at .5 so I calibrated the scale following the scales instructions and then I put one of the 10 gram weights on the scale and then added the little crumb of mirtazapine until the scale read 10.05 with the mirtazapine so the increase was the weight of my mirtazapine. I start with about 1/6 of a 7.5 pill and file it down, then weigh and file and reweigh until I get the dose I’m looking for.  It is probably not nearly as easy as having the liquid compounded for you but it has worked for me.  I try to find a time when nothing is going on so I can sit and make up a few weeks worth of doses at a time.  I hope I explained this clearly, if not feel free to ask any questions and whichever route you decide to take I wish you much luck.

Hello Mimi65. The way you are tapering with the Gemini scale is exactly how I tapered trazodone. I used the little weight too to get a better reading for the smaller doses. It's good to know that the smaller doses can be weighed with Mirt. Does the smaller dose feel accurate though? When I got extremely low on the taper I ground up the Traz and separated it into equal tiny piles for doses and split the piles for smaller doses.

 

I'm stopping the new Mirt mix today. I cannot tolerate it. Besides the terrible sleep it's making me feel nervous. I don't need that. I hear some compound pharmacies use alcohol as a base. Maybe that's what's bothering me and revving me up. So tonite I'll dose with the good old Mirt only and do that for this week. One dose in the middle of the week will be in the pill form. I'll cross over to the pills within a few weeks. I wonder if the 3.87 dose in pill form will match up with the 3.87 in liquid form. There could be a difference.

I'll stay on the pills for at least a month then reevaluate. I might make my own liquid or weigh it out. I like the maple syrup idea because you can see it in the syringe, it suspends, it's natural, and tastes  good on pancakes lol. Marie.

Hey Marie,

checking to see how you’re doing.  What form of mirtazapine are you using now and how’s it going for you.

I am on day 9 of a cut to.8 and things are really rough right now, sleep is a big issue for me as well right now.  I take the mirtazapine about 930 pm and generally am asleep about 10 but wake up several times and usually up for good at 3.  Then about 7am nausea and nervousness set in and I try to stay busy and distract but it’s not easy on so little sleep and with it being so cold outside.  Usually by mid afternoon things settle down and I feel completely normal until it all starts again that night.  I’m going to hold on here for at least a few more days to see if things can stabilize but I sometimes wonder if it’s the mirtazapine causing the issues and not just withdrawal.  Husband and I are supposed to go away for a couple weeks later in February so I’m hoping I can find some kind of stability by then or I don’t know how I’ll be able to handle it.  Take care, stay safe.

Jan 2020 Doc thought anxiety and started duloxetine 20mg

Feb 9 2021, dose increased to 30mg

Feb 16, 2021 due to side effects, told to stop, opened

capsule and dropped a few beads at a time while starting 7.5 mirtazapine

March 15, finished duloxetine taper, continued on mirtazapine

June, tapered 1/4 pill mirtazapine every 2 weeks under doctors orders, insomnia came back so back on 7.5 mirtazapine

august started taper again after being back on 7.5 for few weeks

8/27/21 - 5.7, 9/17 - 5.0, 10/1 - 4.3, 10/11 - 4.0, 10/21 - 3.7, 10/31 - 3.2, 11/7-2.9, 11/16 - 2.4, 11/26 - 2.0, 12/4 - 1.8, 12/11 - 1.5, 12/29 - 1.2, 1/12 - 1.0, 1/22 - .8, 1/29 - .6, 2/9 - .5, 3/5 - .3,

Hello Mimi. I went back to the good liquid for a couple days and I'm stable. Sleep is good, some wakeups, but good. I'm going to start weighing out a 3.87 of pill. I'll take one tonight and crossover the next couple weeks to all weighed pills. I have enough liquid for a couple weeks or more. I may go to liquid (my own) a bit later down the road. I will not give up control again.

 

It's withdrawal Mimi. This is exactly what happened when I tried the new liquid. It felt like a big cut. If I were you I would slow down. The cuts catch up. I hope you feel better quickly. Be well. Marie.

Hello all. I am trying to figure out the best way to weigh the Mirt. The 7.5 mg pill weighs on my Gemini scale .075 (average). It's set to grams. Now my dose is 3.87 mg. I can only weigh it to the second number .038

 

Any ideas? I don't want to use my own liquid yet if I don't have to. Be well. Marie.

When we were still using the Gemini scale, and wanted a  4 mg. dose, we used a counter-weight, of, I think, 10 g with a piece on top of the counter-weight and shave it off from there.  There was considerable was if trying to get as close to the 4 mg. as possible.

  • 2002-2010 Klonopin 1-2 mg., ambien 10--20, mg, remeron 4 mg. and  trileptal 300 mg
  • 2011 Stopped ambien and crossed over to valium 17.5 mg. (updosing 2.5 mg. to cover ambien C/T )
  • tapered valium w/ long holds to 12.74 mg. from a high of approximately 20-30 mg/day
  • 2015-present tapered trileptal aggressively for a year; now intermittently; interacts w/ other drugs
  • currently 2024 still on 96 mg. trileptal and 4 mg. remeron
  •  Currently on benzo hold as I have to cross-over from brand-name valium to generic diazepam.  
  • Current dose of diazepam is 8.8 and valium is 5.7.  I had to up-dose the total valium/diazepam from 12.74 to 14.5 where I have stayed since June 2023.  I am crossing over to generic at a somewhat tolerable rate of .3mg/month after about 2 months of trial/error w/ updosing.  I am not currently tapering; will continue to cross over.

Hello Grace/Eileen? Yes, that is what I'm doing. I just can't weigh my current dose of 3.87 mg exactly. Scale reads .038 or I can use .039. With the weight it's .048 or .049.  It does not weigh into the next column to the right. I guess my cuts will be a bit bigger so I guess longer holds then. I only want to cut 5% per month. I really don't want to only cut .1 mg dose per month, so I may go .1 mg dose every three weeks. Maybe at 2.00 mg dose I'll just cut .1 mg dose per month and call it a day.

 

We are friends with a very nice couple. They love cruising and keep booking cruises and cancelling them due to well you know. The husband has been calling us for a few days and really, really wants us to go with them.

They are booking a Mediterranean cruise 10 days for 2023. I said to them you don't know what it will be like then and you still might have to cancel. They finally convinced us to book with a very small deposit and all cancellable. This is Italy, France and the Greek islands. It's crazy since I hardly ever leave my house now.

 

At least I have time to get to a really low dose. Be well. Marie.

Well, that is something actually wonderful to look forward to.  We haven't vacationed in 10 years due to my health issues.  We have been to Italy.  Greece would be the first place on my wish list.

 

Without getting a more accurate scale, I think all you can do is what you are doing, and take longer holds.  Most people would not understand what .001 means as far as a cut goes but it can be significant.  That said, there is always a margin of error, no matter how good your scale is.  You can figure out what the .001g weight difference is in dose and know what you are dealing with OR you can just proceed this way, knowing you are doing the best you can.  Good luck!  Even with our more accurate scale, we can't always get the weight exactly right.

 

We await a blizzard.  I am kind of looking forward to it.  Hope it happens.
