At present, I am looking for stability. That cross-taper was brutal. He wants me to increase Anafranil while at the same time decrease Clonazepam (benzo) which I have slipped down to 2.5mg tonight. Either way will not be increasing Anafranil will decreasing benzo. Enables me to see which drug is affecting me, even though, I really do want to come off the Benzo badly as well. 

 

The lowest effective dose I would have no idea 💡. There isn’t a great deal of information for me of patients taking less than 150mg Anafranil.

 

Also I just started Anafra, so I’m guessing I would need to stabilise for 6-12 months  first before thinking about tapering down?

I'm sorry, Ryder, your doctor's responses made me laugh. They are standard from doctors who have no real understanding of the drugs or what they do. He probably never pays attention to his patients' adverse reactions to psychiatric drugs. It's always "drink more water" or "get more exercise."

 

He still seems unconcerned about your quality of life.

 

You did not cross-taper, you went off sertraline quickly and then got put on clomipramine.

Going off sertraline and the benzo too quickly is what's brutal. Now you have side effects from the clomimpramine.

 

It doesn't matter what the "normal" dose is, what's important is how it affects you. "Therapeutic" means whatever makes you shut up about your problems. A highly sedative drug is ideal for this purpose.

 

If you want to go off drugs, you don't need a "therapeutic" dose, you need a dose that stops withdrawal symptoms. If you want to live with whatever side effects you're getting, that's your choice. Side effects generally reduce when the dosage is reduced.

 

If what you want is "therapeutic" drug treatment, we can't advise you on that, you'll have to work with doctors. We're about going off the drugs, not getting comfortable on a "therapeutic" dose. We're interested in stopping your withdrawal symptoms, not your OCD -- that part is up to you.

 

"Half-life" means the amount of time a drug stays in your body. A drug with a half-life of several days does not need to be dosed more than once a day.

 

Please correct the dates in your signature. Also, please include sertraline.

Dear Alto,

 

Im still unsure as to what it is you feel is in my best interests. I’ve been in this place for almost 8 weeks now. The doctor or nurses don’t budge on the doses that they give to you - it’s all tightly controlled. 

 

I find excercise helpful, including mostly swimming when my parents come visit weekends. I also do some yoga when I can. I have also bought many self help books on anxiety bc it’s killing my life. I’ve also traveled to India and London to listen to Guru by name of Mooji, which some of your users like Shep are familiar with - seeing him in person lying at his feet. The current anxiety book 📖 I read is called Dare which I use to help me to diffuse from my anxious thoughts. I also have a psychologist luckily I can afford to see. I am writing this to you as I came here to hospital feeling suicidal. I have had these thoughts in the past but they didn’t bother me, and I’m sure it was from the Sertraline aiding the intensity and frequency. 

 

I am am not a +1 for Pharmaceutical drugs. I friggin hate it and the baggage side affects. I’m always ill in someway or another physically or mentally. Even excercise I have stopped, which I enjoy, as my rest is not adequate and I’m lying down a lot not activating my muscles. They are pretty sore and I’m pretty tired with all the information I have, I wish I knew less. I am already tapering down Clonazepam from 3 > 2.5 for the next few weeks. The Sertraline stopped. And Anafranil you want me to decrease from 150 to 140mg. 

 

I know the drugs don’t work. The nurse who came into my room stated that her son had a similar condition to me - himself meditated and made himself a good routine to allow his mind to give himself something to focus on during day. 

 

Other than that im not sure what it is you wish me to do. Even my previous psychiatrist said that the new drugs won’t work by themselves, I will have to try tackle my OCD with coping mechanisms such as meditation. I also read in your link that one of your users (BTDT) stopped cold turkey taking Effexor in 2007 using non-drug coping mechanisms. 

 

That having just got to 150mg going up by 25mg since the start of March. That was freakin brutal. I don’t enjoy the side effects but at least I still feel like living. Other than that, and the half life of the amount of time it lasts in the system, I’m unsure what it is you’re asking of me. As you can appreciate I’m pretty exhausted not just by typing away here, but from having to take medicines for a good 10 years now.

Yeah. I’m getting some blood tests done to see how much the body absorbs - it’s dependent on metabolism the new drug is, which is worrying.

 

They do care here, it’s been carried out by a public hospital which process the findings. So I’m not on the side of the drug companies, nor am I on the side of SA. I believe the drugs do have a benefit if you have a long history of being on them for a while and only should be used as last resort. I don’t believe doctors are actively trying to get everybody doped up. I don’t think they would be able to carry their heads walking into work each day. When I’m ready I guess I’ll be able to let go of everything.

Are you in a psychiatric hospital?

 

You need to decide what's in your best interest. It sounds like you are re-medicated and not in a position to consider tapering.

 

Please let us know when we might be of assistance in reducing your drugs.

In an ideal world I would taper. But I am adjusting to this new medication.

Alto, I’ll see you in 3 months. From what I’ve read, I should be in a stable state of mind before even thinking to taper. So as re-medicating takes time. 3-6 months should be the sweet spot. 

 

Unless I am missing something? I just want to give my body the best chance.

In my diary, I reached a dose of 150mg Clomipromine on the 23rd May. I started an initial dose of 25mg from beginning 1st May as I’ve put it down in my Signature. 

 

The drug is broken up into being taken day / night. 

 

Current: 50mg day.

               100mg night time before sleep. 

 

That is just the the way it’s taken and been prescribed. 

 

Also I am going down on the Benzo Clonazepam. Also I won’t taper two drugs at same time this could take some time. 

 

I will continue taper of the Benzo and I think that will give me enough time on new drug to see predictable symptoms and for it to stabilise in the body.

 

Have I missed anything Alto?

Dear @Altostrata @ChessieCat ,

 

I have been discharged from hospital as of two weeks now which is good news. (11th June)🤘

 

I take the Clomipromine as follows per day:

 

50mg Morning Dose.

100mg Night time Dose.

 

If I wanted to make an adjustment to 140mg, would I shave it off the morning dose??

SA,

 

Wanted to provide an update with regards to tapering off Clomipromine.

  • I came out of Hospital on 150mg divided into 50/100mg morning night Anafranil (Clomip).
  • 23rd June I reduced one tablet 25mg. Did well.
  • 6th July I reduced a further 25mg though it may be a week too soon.

The tablets are tiny. Not cuttable with a cutter as they scatter like egg shells so I have used Sandpaper instead incase I need to bumper by 12.5mg. That is the plan for the rest of my Anafranil taper 25/12.5 reductions and see how I go every two-three weeks. Couldn't eat properly nor go to bathroom on such a high dose but have seen good improvements and am excercising.

 

For the Benzo Clonazepam. Reducing 0.5 off every 3-4 weeks and have done ok.

 

Also, ceased taking Melatonin (Circadin 2mg) which is a positive as my sleep is less forced. Continuing with Magnesium before bed.

 

Symptoms:

- Still having trouble sleeping during night hours but when I do its more restful.

- Rash on side of face has gone down and reduced in swelling.

- Ability to excercise again as muscles don't feel so week + having gone to the bathroom before hand (thank god) has turned me into a lightweight running in the breeze.

- Less angry (probably from Zoloft reduction) but more tired during day as Anafranil is a sedating drug.

- Memory is slowly returning and I'm trying to keep my mind busy in addition to meditation.

- In the morning I have noticed I won't get out of bed for a bit as i'll feel like hell if theres no sleep, but all seems to subside around lunchtime like it was a storm in a teacup.

As of 5th August 2018, its been a hard slog and a lot of times I have felt like giving up but I'm pleased to say I've come a long way :excl: I'm doing alot better ... and have cut down to 50mg Anafranil (Clomipromine), 1.25mg Clonazepam. 😀

 

Sometimes it takes writing it all down again as a reminder to how far I have actually come from hospital just a few months back.

Hi @ChessieCat@Altostrata@mammaP,

 

I hit 50mg on 5th August 2018. Then I lowered my dose to 37.5 Mid-late August and then > 25mg. I noted persistant stomach problems when going to the bathroom etc.

 

This seemed to ease on 25mg except at this dose I started experiencing much anxiety, eye pain, eye strain, jitters and even panic attacks. , but my stomach problems improved on 25mg Anafranil.

 

I know you have to taper slowly, and I recenty got offered a job too working for family in a warehouse and I was just too stressed too accept and go.

My reductions in Anafranil were on average, but I dont want to start living my life once I have tapered off all medicines so taking both SA guidelines and my bodies reactions together I have re-instated to 50mg.

 

Was this the right thing to do ??

Titled:  Compounded doses versus home made brews

 

Hi guys,

 

I had been tapering using my gem20 scale and a nail file to get from 100 > 90mg of my SSRI.

My doctor has supported me and allowed me to fill a compounded script measured at the pharmacy to equal the same dose = 90mg total.

 

I feel a bit funny taking the compounded formula even though they are 'caculated' to be at the same weight. Should i go back to my homemade brew? Has anyone else encountered this problem ?

Are you referring to your Anafranil above, as to what you have had compounded?

When did you switch from weighing to the compounded formula?

This may allow for more accuracy as you are making reductions.

Anafranil is a trycyclic AD, not an SSRI.

I wouldn't go back to your "homemade brew" just yet.  You haven't changed your formula very much, as you were shaving and weighing before.  This compounded formula may be more accurate, going forward.  Was it expensive, or too pricey?  If not......it sounds like a good plan, for better, improved consistency of your dose.

 

When did you last taper the Anafranil?

Main difference was irritability, headaches and a rough sleep!

 

Usually when taking Clomipromine, you feel a good degree happier and get a lift when first taken. I think you guys call it Serotonin Syndrome. As the drug wears off you start to feel fatigued at around lunchtime for me. When taking the compounded tablet version, i felt ok for the first hour and a huge dip in my mood for the rest of the day.  Perhaps the release is quicker in a compound rather than prolonged release Just my guess?

Please consider that you just may have been making too many changes at once.  It appears that you tapered, as well as then tried the compounded formulation.  Each of those, can be considered what we refer to as a dose change event.  We like to caution members to just do one dose change event at a time.  Your overlap with trying the compounded clomipramine/Anafranil, AND a taper makes it unclear what caused what.  You might have been experiencing some WD, after your taper, rather than a problem with the compounded formula.

Do they use your tablets when compounding?  And if so, then the only difference is that they would be using crushed material, rather than a shaved pill.  So yes, it might have absorbed a bit quicker, the compounded version, than what you are used to.  Was it a liquid or tablet, your compounded clomipramine?

 

I don't think it would be serotonin syndrome, that feeling happier and getting a lift.  And again, you are not on an SSRI(see my previous post).

Hard to know, but the symptoms could have been from your taper.

 

So....Yes, there are some, who have not felt comfortable with compounded formulas of their medication.  And I don't know much about the regulatory system for pharmacies Down Under.  In the States here, the pharmacists do need to stick with certain guidelines, which is reassuring.

I'm just adding in here, editing, that others have done really well with using compounding to taper.  I think both Karma, and possibly Rhiannon too, may have.  You can do member searches by just putting the member name, in the search box, at the top right, in this forum......Introductions.  And then you could just put in compounding too there, to see if some more members experiences come up.  Maybe some will come by here too, to support and share with you.

 

I think if you do decide you are going to stick with the "home brew" you might want to get a bit more consistent and comfortable with using your Gemini 20 scale, for the long haul.  Nail filing down, may not be the most accurate going forward.  You could consider making your own liquid too.

^ has all the information that you need, and then further links to topics on weighing doses, or making a liquid preparation.  Usually, members, crush tablets, and then weigh, and then put the dosage contents into a capsule.  Take a look, and then look at some of the links there too.  Then ask, as you are deciding, just how to go forward, if you have questions.  Okay?

 

Personally, I think, if the compounded form, is affordable, that might work out just fine too, for the long haul.  There might just be a minor adjustment, or bumps, as you adjust to it. 

 

And you are familiar with the Why taper by 10% of your previous dosage topic now, aren't you?

I went back through some of your history since you've been here, and just don't want you going too fast with anything.  As it sounds like the last year or so, has been pretty tough to manage.  Look at this now, as a marathon of sorts......rather than a race to the finish.  And hopefully you can function really well, through a nice, slow, taper.  And some good HOLDS, when needed.

 

I think that, if you don't mind, just HOLDING right now, that might be a good idea.  And then decide, just how you'd like to proceed with the clomipramine taper, weighing, liquid, or compound.  Don't do any immediate dose changes.  Stay put at the 90 mg dose for at least a few more weeks.  I hope your symptoms have improved.

When do you take your drugs, Ryder?

Would you give a try of daily notes, please.  Look at the link, and example.  Just keep it simple.  Time on the left.  Drug(s) name and dose on the right.  And then symptoms too, as they occur throughout the day.  Include any supplements and sleep too.

This sounds pretty reasonable to me. Just going to stick where i am at the moment am comfortable with the Nail file. "Home Brew" is not a perfect science with the Gem20 but it's what my body feels most comfortable with at present. I also have some joy at doing it i don't know why it gives my mind something to do when i wake up in the a.m.

HI Shep, as it says it my signature i started the Anafranil with 25mg morning / 75mg night. Also i take the 19mg of Valium at night time before bed. Since the reduction i have reduced the morning dose to 15mg Anafranil morning and continued the 75mg at night.

 

The main reason i take them at night is so i am not so sedated during the day time and can get out of bed. Is there anything wrong with how i have made my reductions or when i take the meds?

You may want to space the nighttime Valium and Anafranil out by two hours in the evening to help prevent any drug interactions.

I think you're right to reduce the morning Anafranil since it's so sedating. BUT, only if you gave yourself time to stabilize since the hospitalization. Otherwise, you may want to go into a long hold and build a solid foundation before making any more changes.

Shep,

 

I have manged to stablise on my meds for the past 6-months, To the point where i feel able enough to do things and want to get out again. I know i have a long taper journey ahead of me(Valium), part of me wishes i would go for my dreams.

 

Maybe it is wishful thinking, but do i wait until the taper is over before i commit to something or i just go ahead and do it anyway ?

@RyderI moved your last post out of my success story and over to your own thread so we can keep all of your information about your taper here. 

 

You're asking good questions, but it really depends on what your goals and priorities are. 

 

If coming off these drugs is your number one priority in life, than you'll want to only take on responsibilities that will allow you to  manage your symptoms and not stress out your nervous system. If coming off these drugs is secondary, and you're happy with where you're at now with your current drug load, than you might be able to take on more. Of course, please be mindful of the unpredictability of psychiatric drugs - they do tend to turn on us even after being stablized for long periods of time. 

 

But first, figure out what your priorities are and then set up a game plan accordingly.

Hi Shep, 

 

Getting off these drugs is my priority. When I made that post I was feeling well , and felt able to take on more responsibilities in my life. During that time, I managed to enroll in a University Course online to give myself something to do. It was something I had to try.

As I have reduced my Valium, I have noticed that I have not been sleeping at all well. To the point where it has affected the way, I attend and manage my responsibilities day to day. Going from sleep to a lack of sleep has been hard to attend online classes and go over course material.

Ryder: Journey Reducing Diazepam and Anafranil.

 

Hi Guys,

 

The majority of my posts are spread across two seperate threads. I wanted to post updates to track my own progress here if i may. This is primarily for myself, but also for others who are going through a similar journey.

 

- 14th November 2021: Tapering for two years off Diaz. Recently went down to 14.5mg. 

                                        Also went down from 80mg > 75mg in November for two weeks. This anti-depressant is the one that helps me sleep and cope with WD symptoms from the Diazepam. I have not been sleeping
                                        at all well for the past two weeks as once i made the adjustment on my anti-depressant.

- 15th November 2021 - I will be increasing the dose of my Clomipromine to 80mg. This is my previous dose at of this medicine. I had been relying on sleep for this.

 

At 75mg, I am really not sleeping at all and quite restless. 

 

As for my mood. Sometimes I am sleeping during the day. This is just my coping mechanism really for when I am experiencing pain. I just like to lie down until it settles down. I was using gaming as an asset to spend my time, I do realise however that this is quite mindless and I could be doing useful things. This change in how I spend my time, will take time.

Q:  Why not try a small updose instead of going all the way back up?

I am not coping would be the reason. 5mg is also the smallest compounded tablet I have.

I have increased my anti-deppressant by 5mg for sleep (75>80mg).

 

I haven't been sleeping so well as of late and these are my sleeping pills.

Is it advised to sleep during the day ? I do this as i don't sleep during the night time.

2008-2012: Cymbalta, Zyprexa, Valium (5 days supply),

2012 - Seroquel x 4 weeks C/T. 

2014 - Seroquel x 2 Weeks C/T. Crossed to Risperidone 3mg for 6months until December.

2014 - Stopped Risperidone. Xfer > Anti-Depressant 200mg Zoloft and 6mg Clonazepam. 

2018 - 150mg Clomipromine changed Anti-depressant. Tapered Benzo to 1mg Clonazepam. 2019 - xfer to 20mg Diazepam. 

 

Currently:

Anafranil: 75mg. 17th Dec 2022 70mg. 27th Dec 22: 75mg, 14 January 23': 70mg. 16-26th January: 50mg (too fast drop no sleep). Jan 28th 2023: 70mg. 20 Feb 2023: 65mg. 11/06: 60mg 9/08: 55mg 15/08/23 : 50mg
3/03/2024: 60mg (Updose)

 

        Diazepam (V): 25th Oct 2019' 20mg. 22 Dec 19' 19mg. 04 Apr 2020' 18mg,  30 September 20' 17.5mg , 13 Nov 2020' 17mg. 01 January 2021: 16mg, 13th Aug 21' 15mg. 1st Nov' 2021 14.5mg. 1st Dec' 2021 14mg. 13 January 2022: 13.5mg, 11 Feb: 13mg.  11 April 22' 12.5mg, 12 May 22': 12mg, 6th September 2022: 11mg Valium. 9th October: 10.5mg, 25th Oct 10mg. 12 March 23: 9.5mg 2 April: 9.25mg 23 April: 9mg 12/05: 8.75 26/05: 8.5 12/09: 8.25 21/09: 8.5. 3/10: 8.25 17/10: 8mg 20th Nov Brassmonkey: (7.9.,7.8, 7.75) 5 Feb: 7.25mg. 23 Feb: 7mg
