Im sorry you're having a tough time. I hope the estrogen updosing helps your anxiety. I wish I had something more helpful to add.

Pristiq tapered over 8 months ending Spring 2011 after 18 years of polydrugging that began w/Zoloft for fatigue/general malaise (not mood). CURRENT: 1mg Klonopin qhs (SSRI bruxism), 75mg trazodone qhs, various hormonesLitigation for 11 years for Work-related injury, settled 2004. Involuntary medical retirement in 2001 (age 39). 2012 - brain MRI showing diffuse, chronic cerebrovascular damage/demyelination possibly vasculitis/cerebritis. Dx w/autoimmune polyendocrine failure.<p>2013 - Dx w/CNS Sjogren's Lupus (FANA antibodies first appeared in 1997 but missed by doc).

Man, I did not enjoy that estrogen trip! Finally got my estrogen back up to where my system is happy and the world doesn't seem as threatening or doomed. I had also tried to reduce the amount of HC I take upon the recommendation of someone I trust, but ended up with brutal evening cortisol spikes that caused anxiety and insomnia. I switched back to the previous dose yesterday and everything went back to normal. Guess it isn't time to reduce that.

On a happy note, I did some more research on the alkaline balance and I have been right where I need to be ... hopefully this major adjustment in diet is helping to stop my system from robbing my bones of calcium ... only time will tell. But I tell you, the bunnies in the neighborhood are jealous of my green salads ... leaf and romaine lettuce, dandelion greens, spinach, alfalfa sprouts, cucumbers ... a little feta cheese and some kalamata olives for flavor ... always mix those dandelion greens with other greens ... they are BITTER.

I actually did apply for another job at work ... it is a job that I think I would enjoy more than the work that I currently do ... but it means I would take a bit of a demotion ... salary would be the same just a title change ... I earned the first and second interview, but now I'm wondering if I'm in the right frame of mind to make this kind of decision.

I really won't miss the self-doubt when it completely disappears :blink:

I've been balanced on estrogen for a couple of weeks so went ahead and dropped 3 mg off of my Effexor dosage. I had been at 35 mg for 6 weeks.

 

The last time I had significant anxiety was last Monday leading up to my interview for the other job at work. When I woke on Tuesday I was calm and didn't have any self-doubt - the interview was Tuesday. I've generally felt good all week and actually felt joyful today. I think the stress leading up to the interview was contributing to the anxiety and once I had "performed" the interview the stress fell away - although if they offer me the job I have to make a decision, but I'm not thinking about that now because it doesn't exist now (and that is positive behavior in and of itself).

 

It amazes me how confident and hopeful I can feel at times, looking forward to all kinds of amazing possibilities, and then at other times be filled with self-doubt and cowering under the anxiety. I wonder if some of that is normal and I am just hyper-tuned to my shifting moods :unsure: ?

 

I might have made a mistake decrementing the Effexor at this point, but I have a feeling that it is going to be fine. Of course, I'll keep you posted.

Wishing you well. I understand how torturous this polydrugging thing can be. Especially long term. Just read your intro and it sounds like you are able to get stable. I hope I can become stable enough for a micro taper someday. Take care...

Klonopin 21 years (did an 8 month taper off of 2mg's then had to reinstate @ 7 months out)Current Dose is 1.5mg daily.

Dilantin 19 years @ 300mg (did a 6 month taper...had a few doses since then but off it now)Off Since 1.5.2012

Remeron Started 3/30/2012. Helped me for 2 months, not so sure what it is doing to me now. Still take 15mg @ night

 

**Currently not stable and trying to "stabilize".

You are an amazing person and wonderful support to me. I am so thankful for you! I'm glad you got the estrogen worked out. I am very responsive to estrogen, also. If im just 1 day late on my patch, I feel my mood going very dark. It colors the way I think about everything.

Hang in Karma , you are doing so well, pure progress on your part ! you can do this ...you are quite strong, Effexor will not take us !!

2007-2012started Effexor xr 225mg -150mg- Varied2months of Taper,(March 22/2012 Off Med/in W/D)Currently No medicationMarch 22/2012- August 16/2012 - Totally Off Effexor( Rough Rough time) Reinstatement for a week(didn't work)****** New Doctor******* Very nice/helpful and in touch.Tried Zoloft- No successAUGUST 17/2012- STARTED 37.5 EFFEXOR FOR ONE WEEK - NOT GOOD < STOPPED.Started Buspar Oct 11th 2012----10 mg for the first 7 days and then 15mg a day---- Taking a new route----Racing thoughts - Gone.Oct 9th ( Done Zoloft,wellbutrin week project trial to feel better) - OFFICIALLY OFF ANTI DEPRESSANT !Oct 26 - Raised Buspar to 20mg a day- Tolerating wellJan 2nd 2013-

Reduce Buspar until full off march 20th 2015 Off all medication !

UPDATE: One week into the 3 mg Effexor decrement and everything is fine. I've had absolutely no symptoms in the last week. :) I've felt joyful and had no unnecessary anxiety. I've had a total of four interviews for this other job at work over the last two and a half weeks ... the last on Friday ... and not even that has thrown me off ... I think they will be making a decision next week. I think it would be a positive change back to something I have passion for. If it is meant to be it will be and if it is not my divine right job, then it wouldn't have made me happy anyway and I happily release it. Went to the PCP (primary care physician) for an annual physical on Friday. He is a D.O. Very supportive about my choice to adjust my diet, begin a walking program with weights and take supplements to build bone. He said that I'm doing all the right things. He was very impressed with my Effexor taper ... wanted to know how I was able to dose 16 mg AM and PM ... I told him about compounding the liquid and which compounding pharmacy I used. He has been bridging people with Prozac, but I don't know how successful he has been. He did acknowledge that there is the possibility of terrible withdrawal from Effexor ... and that in and of itself is an amazing admission from a doctor. So, I did survive the awful anxiety from jacking around with the estrogen patch. When my Nurse Practitioner is back in town I will attempt switching to the patch with her guidance in another week. If that goes well then after a week or two of that I will decrement Effexor again. All is well with Karma :D Karma

that is an awesome update Karma!

Started in 2000 - On 150mg most of the time, (but up to 225mg at highest dose for 6 months in the beginning)
Reduced off easily first time - but got depressed (not too much anxiety) 6 months later
Back on effexor for another 9 months.
Reduced off again with no immediate w/d - suddenly got depressed and anxious ++ again 3 or 4 months later.
Back on effexor - this time for 3 years
Reduced off over a month - 6 weeks later terrible anxiety - back on.
Rinse and repeat 4 more times - each time the period before the anxiety comes back got shorter and shorter
Jan - July 2012 75mg down to 37.5mg;, 8/3/12 - 35mg. 8/25/12 - 32mg. 9/11- 28mg, 10/2 - 25mg, 10/29 - 22mg, 11/19 - 19.8mg; 12/11 - 17m,
1/1- 15.5mg; 1/22 -14mg, 2/7 14.9mg, 2/18 - 17.8mg - crashed big time: back to 75mg where i sat for 2 years....

4th  March 2015 - 67.5mg;   31st March - 60mg;  24th April - 53mg; 13th May - 48mg; 26th May - 45mg;  9th June - 41mg; 1 July- 37.5mg; 20 July - 34mg; 11 August - 31mg; 1st Sept - 28mg;  1st Dec - 25.8mg;  28th Dec - 23.2mg; 23rd Jan-21.9mg; Feb 7th- 21mg; March 1st - 20.1mg, March 30th - 18mg

Update:

 

On Friday I dropped another 2 mg Effexor ... I'm down 8 mg since May, so averaging about 2 mg per month. and down to 30 mg total. I had to hold for a month again this last time because I made changes in other treatments.

 

The last two decrements didn't result in any discernible symptoms. But I might be having symptoms with this last drop. I've felt a little irritable since Friday, but I've also been under a lot of stress. I was out of the office in training for two days and came back to a pile of work and a customer who didn't accept our polite, "no." I am also very disappointed in the person who is supposed to be helping me at work. Still, it could be withdrawal symptoms - but I don't have any other symptoms ... so I'll track it and watch it.

 

I've also had horrible ringing in my left ear for about two weeks and had a doctor's appointment on Friday to check into it. (The ringing can get really extreme and that makes me irritable, too). Turns out that my eardrum is contracted! As I understand it, it is a pressure thing ... the Eustachian tube is potentially partially blocked causing negative pressure which is contracting my ear drum. So we are trying a nasal spray with a bit of steroid in it to see if it will help - only need to give it a week and if that doesn't do the trick we'll look at something else. At least there is a cause instead of being told there is nothing they can do for me. I do think that the ringing is improving.

 

Overall Karma is doing well.

How does one contract a contracted eardrum? That's a new one to me but, as you said, glad they found a root cause that can be addressed.

 

I hope things settle with work. It's difficult to imagine you being irritable. :)

Started in 2000 - On 150mg most of the time, (but up to 225mg at highest dose for 6 months in the beginning)
Reduced off easily first time - but got depressed (not too much anxiety) 6 months later
Back on effexor for another 9 months.
Reduced off again with no immediate w/d - suddenly got depressed and anxious ++ again 3 or 4 months later.
Back on effexor - this time for 3 years
Reduced off over a month - 6 weeks later terrible anxiety - back on.
Rinse and repeat 4 more times - each time the period before the anxiety comes back got shorter and shorter
Jan - July 2012 75mg down to 37.5mg;, 8/3/12 - 35mg. 8/25/12 - 32mg. 9/11- 28mg, 10/2 - 25mg, 10/29 - 22mg, 11/19 - 19.8mg; 12/11 - 17m,
1/1- 15.5mg; 1/22 -14mg, 2/7 14.9mg, 2/18 - 17.8mg - crashed big time: back to 75mg where i sat for 2 years....

4th  March 2015 - 67.5mg;   31st March - 60mg;  24th April - 53mg; 13th May - 48mg; 26th May - 45mg;  9th June - 41mg; 1 July- 37.5mg; 20 July - 34mg; 11 August - 31mg; 1st Sept - 28mg;  1st Dec - 25.8mg;  28th Dec - 23.2mg; 23rd Jan-21.9mg; Feb 7th- 21mg; March 1st - 20.1mg, March 30th - 18mg

Update: I found out what was causing the fatigue and irritability ... low estrogen ... it seems that the troche is not allowing me to keep my estrogen consistent and when I lowered my dose about six weeks ago it dropped too far. I ended up with a list of low estrogen symptoms including joint pain ... this is what I mean about logging your symptoms daily so you can see subtle changes ... when I started noticing joint pain in my fingers and my knee I was able to trace it to the change in my estrogen dose. We tested and sure enough my estrogen is in the basement. (Note, I was already using hormone replacement therapy before I started tapering off of ADs ... I am not recommending adjusting sex hormones while trying to come off of ADs, I'm just logging my experience). So, I took the plunge and switched to the estrogen patch last Friday. This time I'm working with a Nurse Practitioner so that I don't end up with terrible anxiety like I did last time. I'm trying a dose three times what I tried last time and I'm watching my symptoms carefully. The joint pain has nearly disappeared, irritability has lifted and the fatigue is lifting. So far, so good ... even my husband said he noticed an improvement in my mood. The only down side is that I have to hold my Effexor dose longer ... because I've found it is best to only change one thing at a time. If all goes well, I can decrement Effexor again in two weeks ... and it also means that I haven't experienced ANY noticeable withdrawal symptoms from my recent Effexor decrements. That is good news. Thanks Tezza, Barb and Jemima :) . Thanks for thinking of me :rolleyes: . I'm doing pretty well.

 

The estrogen patch seems to be working out just fine. We started out at 0.075 mg and dropped to 0.050 mg and that seems to have gotten the estrogen back up and now balanced out. I had a rough patch in there where I went down to 0.0375 mg and man, did I get irritable and quick to anger - had to go back up to 0.50 mg. I am just so amazed by how these hormones can effect me.

 

I did make an Effexor decrement in there and I plan to decrement again tomorrow ... that will be at 3 weeks. I'm toying with beginning to drop gabapentin in between Effexor drops, but I'm not committed to it. I don't want to rock the boat, but I do want off of all of these drugs. I've been at 0.75 mg Xanax for over 200 days, so I'd say that my taper from 1 mg to 0.75 mg was successful :rolleyes: .

 

I do have a bit of additional positive news, according to lab work I am reducing my bone turnover ... and I'm doing it by eating a highly alkaline diet, not by taking drugs. The doctor didn't think it would work, but I've dropped 579 pg/mL on the CTX test which is an indicator of bone turnover. I'm still over the top of the normal range, but only by 106 points. Two months after I started eating a highly alkaline diet I had dropped by 173 points and after a full six months on the diet I've dropped by a total of 579 points.

 

The estrogen is supposed to help, too, but I've only been on it for just over 3 weeks, so it is not likely a big factor at this point. I had hoped to have been further along with reversing my bone loss by now, but I can't argue with the trend - it is going the right way. I should also be building more bone because I am actively doing weight-bearing exercises while wearing a 4 pound weighted vest.

 

Oh, one more thing ... I'm doing a 21-day meditation challenge with Deepak Chopra. He uses a mantra and I find that it is much easier for me to meditate if I can use a mantra. I've literally been getting up 15 minutes earlier every day so that I can make time for the meditation ... after the second day I felt so centered and all the colors of the world were brighter - that's better than drugs :D . Of course, the idea behind doing it for 21 days is to make it a habit ... I think it will be a good habit.

I'm curious as to how the excess cortisol manifests with adrenal insufficiency and while taking hydrocortisone? (I'm not considering adding phosphatidylserine)

Hey Barb,

 

In my experience adrenal insufficiency manifests as high cortisol when one would expect low cortisol. The pattern I experienced was high first thing in the morning (higher than the top of the normal range), then too low about 4 hours later, then too low about 4 hours later and then high in the evening when I expected my cortisol to be at its lowest. As it was explained to me, my adrenals were struggling to produce sufficient cortisol for bodily functions. The pattern I measured has been described to me as a classic example of early adrenal insufficiency.

 

I know that this site does not support the concept of saliva cortisol testing, but in my case we were looking for the pattern and I believe that it was meaningful in my case. The normal diurnal pattern is high (within range) first thing in the morning, then lower by noon, then a little lower by early evening and then at its lowest in the evening when your body is preparing for sleep. If the adrenals are struggling, patterns other than this will show up indicating a problem.

 

You asked how it manifests while still taking hydrocortisone ... again, in my experience, if one is not mimicking the normal diurnal pattern accurately with multi-dosing hydrocortisone the adrenals may over produce cortisol, or the supplementation of hydrocortisone may build up in the system by the evening, thus causing symptoms of excessive cortisol, which can throw off sleep. This is what I experienced ... I believe that at the time I was dosing too heavily in the morning and the hydrocortisone built up in my system causing excessive evening cortisol. I do recall that once the problem was identified that we adjusted my pattern of dosing and the problem resolved within a few days.

 

Note to anyone else reading this ... I work with specialists who manage my condition with hydrocortisone ... I do not recommend this treatment unless you are under the care of a competent medical professional who understands the protocol.

Karma Update: I had a FANTASTIC Thanksgiving holiday! I had distant family in town and we had the BEST time! I experienced joy for 5 days straight. I love them so much and they were so excited about everything ... like little kids at Christmas ... they are in their 30's B) . No anxiety, no depression, nothing uncomfortable ... just joy and fun and enjoyment and love ...

 

Ok, there was one thing, the ringing in my ears really kicked up. But that is a small price to pay for so much activity and fun. The ringing in my ears is still what I would characterize as intense, but it has been worse and it is bearable.

 

I decrement my Effexor dosage again this Friday at the 3 week point. We're making progress over here ... just wanted to share :rolleyes: Karma

If you held for prolonged times on occasion, would your tinnitus go away? I'm asking because I'm on a 2 to 3 mo. hold. The tinnitus is getting better, and may go away by the time I've decided to start tapering again, but ARGH!.. tapering is soooo slow.

Involuntary medical retirement in 2001 (age 39). 2012 - brain MRI showing diffuse, chronic cerebrovascular damage/demyelination possibly vasculitis/cerebritis. Dx w/autoimmune polyendocrine failure.<p>2013 - Dx w/CNS Sjogren's Lupus (FANA antibodies first appeared in 1997 but missed by doc).

Update:

 

Should have listened to Alto :rolleyes: . I decremented on 30 November and had a week of being really off without my "joy" :( .

 

I haven't really noticed whether the ringing in my ears gets much better or worse by holding effexor. But I have great respect for the research and knowledge of Alto and think that the intense ringing in my ears from having fun might have been an indication that my CNS needed a little more time. I've had moments this week where the ringing was so mild I had to listen for it, but for example, it is moderate to intense this morning.

 

Suffering great stress at work - grateful to have a good paying job with a stable company, but I really want to decorate my Christmas tree and make Christmas cookies this weekend. I've got some things I have to get done for work due to lead times and Christmas breaks prior to the start of the new year and there just doesn't seem to be enough time. I'm really tired of the stress.

 

My plan is to hold my current dose of effexor through the new year and then assess my state and move forward.

My guess is you are having difficulty nailing down the effect of Effexor on tinnitus because of the windows and waves pattern. I've been holding for the last month, and it takes that long for me to see a definitively positive effect. Argh.. tinnitus is the pits.

Thanks, Schuyler. I've never really recognized windows or waves. I've just had tinnitus since about two and half years ago and I never really understood where it came from. I didn't start to experience recognizable withdrawals until this time last year. At any rate I am SO GRATEFUL that I overcame the withdrawals I experienced last year :D . I am not experiencing any of the anxiety and irrational fears that I had last year, so my conclusion is that if you go slow enough you can recover.

 

At least the shifts I'm feeling this year are something I recognize and they pass rather rapidly. The fact that I can make the connection between the decrement and the symptoms and then knowing that within two weeks I should be back to normal is really quite comforting.

 

So, Ok ... I got the tree decorated ... made the cookies ... and finished all the work I had scheduled for this weekend. Now I need to get my butt in gear and go workout ...

That's wonderful! I'm starting to get there. I'm fair to partly cloudy, lol.

 

I really don't know why I have tinnitus, either. It could be from taking meds or not taking meds. I have it on the same level as you right now too.

 

It's great to know you're feeling much better! The thought gives others much needed hope in addition to making us happy for you!
