DaveB: trying to stop a roller coaster year

Well Dave, you've made three extremely large changes in just a few months.  It's not quite a CT but pretty darn close. Any large udose is just going to cause a lot more trouble.  It may be possible to take the edge off of the symptoms with a very small updose like in the 1 to 2 mgai range.  Read through the, thread on updosing and see what you think.  You'll have to ride this one out for a long time, hopefully the updose will take the edge off.

I know that paxil causes a lot of problems with hormones, but I really don't know how it will react with the T shots.  My guess is not good.  Tapering shots can be a really messy process, but again I don't know what just stopping them will do.

Thanks @brassmonkey, appreciate you weighing in with your insights. I really wish I hadn’t tried the T shots, but not sure if staying on them or discontinuing them is best at this point. No question I have been worse since I started them. In the spring I was having whole week windows, that simply isn’t happening anymore. I know my tapering has been much too aggressive, I just am fed up with taking a medication that doesn’t help anything.

Anyone experience PE when coming down or off an SSRI? Does it go away?

Of topic: has anyone experienced REALLY premature ejaculation when tapering down or off Paxil or any other SSRI?

If it happened after the T shots, they might be the cause.

I understand, thanks for the tip! Is it the consensus that I should quit the testosterone? Or is that changing two things at once with my recent dose reduction? Are there going to be withdrawals related to stopping the T shots? Also they give me an estrogen blocker called anastrazole, anyone know anything about that drug?

Also big thanks to @RusTW who has been helping me through DM work through some of this testosterone stuff.

As previously stated, SA's focus is on psychiatric drug tapering.  I suggest that you find sites which deal specifically with testosterone issues which will be better able to offer suggestions about whether you should get off them and how to get off them.  I'm sure that they would exist.

In general, I would be careful with any steroid therapy during withdrawal (or any other time). Bumping up one hormone tends to imbalance everything else, because the body depends on self-regulation.

There seems to be a lot of overselling of testosterone treatment right now, with much near-quackery, playing on male insecurities. Be very cautious.

It may be best to wait until you are stable after the recent reduction.  You made 3 huge reductions to Paxil so it may be several months before you are stable.  If you are going to reduce your testosterone you will need to decide when it will be best to do this.  Keep it Simple, Slow and Stable

You will need to keep daily symptom notes so you can be objective about how things are going.

Great advice, I really appreciate it. I think I will slowly make some reductions in the testosterone to see if it may help.

Happy Thanksgiving to all you brave amazing people. Still a struggle for me (probably worse on 10 mgs than I was on 20) but I guess I a thankful that I am on less of this garbage medication than I was a year ago, 30 meg less to be precise.

I'm sorry you feel so bad.  All I can tell you is that you have to savor those moments when you feel better, make gratitude lists, and remember that if you are sleeping you are healing.  It is so slow.  I'm sick of it, too.  I can't believe how long this takes.

I'm still still a newbie at this but, from what I've read the brain needs time to get used to dosages. From what I gather 'stabilisation' doesn't necessarily mean you feel 'ok' it just means you've reached a point where your mood, even if its a low anxious one, is stable.  If you feel anxious all day in waves and consistantly better by evening in a regular cycle with no new or dramaticly worsening symptoms, then surely, that's stable?  Get your self to point where the days are fairly consistent, even if they're horrible, and taper.  I hope that helps. Like you, almost all of my day is awful, the only repreave comes at around 6pm. When I still don't feel 'normal', just a lot less in pain. I wish I'd have tapered Sertraline properly the first time around rather than trusting the advice of those who said 8 weeks would be enough.  One day when we're better we'll sue. We'll march, and protest and hold those who have done this to us to account.  I want to believe we will anyway.  You've just got to stay strong there is no other way.  I saw a vid of a guy who'd had extremely bad akathisia from ssris for four years today. If he can go on living, we all can.

Citalopram. Briefly early twenties, no ill effects seemingly. Don't remember dose.

Sertraline on and off for ten years.  I was ignorant and started and stopped frequently. Doses of 50, 75 and 100. I can not recall/did not record dates prior to 2018.

JANUARY 2018: Last period of use was was 150mg of sertraline on 14/01/18  (which triggered extreme depression and anxiety that never settled, amongst other symptoms). I then tapered to 100mg in March (15/03/18), then down to 75mg in April(01/04/18) , 50mg later (11/04/18) 25mg in May  (16/05/18),  and was at 0mg in June (02/06/18).

Mirtazapine 15mg 01/08/18- 02/10/18, 15mg (6 weeks at 15mg the two week taper). - caused deeply unpleasant waves of extreme anxiety, depression, zombie state and mania.

Still on 40mg of Propranolol twice a day since April 2018. Supplements: Fish oil, Magnesium, Vit B6

Interesting thought, maybe that is stable? To be fair I have reduced my Paxil load a great deal in the last few months. Hopefully the mornings can become a little more manageable soon.

Does it matter much when I take Paxil as far as morning or night? Right now I take it before bed with the Zyprexa, is that ok or would morning be better?

Just wanted to document I have decided to stop the testosterone injections. I feel the up and down nature of getting a weekly shot has only added to my instability and anxiety. I was actually starting to do much better in May before I started the injections. Hoping I haven’t done too much damage by fast tapering Paxil down to 10mgs. My brain should sort it all out over time, that is the idea right, our bodies eventually figure this out and stabilize?

I suggest you make yourself aware of the possible withdrawal symptoms:  https://mentalhealthdaily.com/2014/08/06/testosterone-withdrawal-symptoms-what-you-may-notice/

From the list on that website, it would be very easy to attribute testosterone withdrawal to AD WD.

You will need to keep daily symptom notes so that you can be objective about your symptoms.  It might also be helpful to rate them.   Rate Symptoms Daily to Check Patterns and Progress

Patience is going to be necessary, and you will need to try not to panic.  When we panic we can make bad decisions.  If you did decide to updose the Paxil, my suggestion would be to not increase your dose to more than 15mgs.

On, great info and thank you. I am not going to lie, it scared me a little bit and has me doubting whether to quit the injections. My body would go back to producing the testosterone on its own, correct?

Hi Dave,

As I have just read in the link provided by ChessieCat, there are many factors that could contribute to Testosterone withdrawal.  From what I can understand it appears to be a type of a steroid.  I know that when people go off an oral steroid too fast they can have a heck of a problem. Also people in AD WD can be beaten up with steroids - on or off - I know, I'm one.  Should you perhaps talk to you Testosterone provider and tell them you have problems when decreasing certain types of meds? You don't have to discuss AD WD if you don't want, because it may complicate the conversation, but it might be worth getting their input as to the safest way to go off the shots. You are already suffering from a rapid change in your Paxil, don't add to your suffering by reacting to suddenly.  Perhaps do some investigating, as well as talk to your provider?  It might in indeed be right for you to go off, just be cautious as to HOW you go off.

Respecfully

RS

Today is an exceptionally bad morning. Was hoping not getting the testosterone shot this week would improve things.

I did get a numbing shot shot in my nose yesterday by the dermatologist before he cut something off...any way that could be affecting me today...or am I grasping at straws?

I am guess no one has had a similar experience with a numbing shot by a dermatologist? Thought it may be grasping at straws...not sure why today was so bad though.

Yeah, I'm better.  I know this because I can do more.  My sleep is worse, but I am activated all day long.  So, I'll be glad when sleep improves.  Still having a lot of cortisol issues through the night and in the morning.  They are probably the worst part right now.  Like you, anxiety is a big problem.  After it wears off, I am so exhausted, but I can't nap.  Fortunately, I can still see the long term improvement, and I have faith all will return to normal in time.  It will for you, too, but I know how hard it is to live through this, Dave.

Thanks Rosetta, another really tough day today. I quit the testosterone injections as I think they have only hurt the situation. I did take some Ashwaganda last night, and I do think I slept better, but it certainly hasn’t helped me any today. Ashwaganda is supposed to reduce cortisol, anyone had any experience with this and found it helpful?

There are many existing topics on this site.  You can use the site search or use google and add survivingantidepressants.org to the search term.

So google to find SA's discussion:  survivingantidepressants.org ashwagandha

There is also a topic about cortisol, so do a separate search for that as well.

Hi DaveB,

I know you are going through some tough times lately.  You will get through it. Yes it is nasty and painful, but you will make it.  At this point you might be reacting to a drop from a month ago, or from a drop three months ago, or all together. Yes, this drug is very insidious.  Right now time is your friend. Just hold on and know that you will get relief. I also want to say how very sorry I am that you are suffering so.

Thanks guys. Thanks @ChessieCat for more words of wisdom. @RachelSusan you are always so helpful and supportive I really appreciate it more than you know. @Rabe I am sorry you are going through the thick of it as well. It has to get better sometime, right? I just don’t know what to do, actually had a pretty good day yesterday, was hoping I was in to something with the Ashwaganda...today I woke up into a nightmare of anxiety that won’t let up. So who knows if I will ever make real progress.
