@JanCarol @brassmonkeyI am about to make the cut from 0.011 to 0.010. I believe it is the last cut I can make on the Saphris without somehow dissolving and holding the dose in my mouth. I know there was a plan. Just checking in.

That is exciting! I find it hard to know what "feeling normal" is until it happens. Like others here, I fear that the WD me is the "real me" and I'll be like this forever. So that is great you are feeling that!

Is it possible that the release of the studio from your stressors has helped?

 

These will be coming more and more frequently.  Keep in mind, you are still tapering, and have a lot more to go through (sorry), but this means that you are healing, you are able to heal, and will have more windows coming!

Next time you have a window, I suggest a creative project.  A collage, or knit a special bracelet - to remind yourself - there are windows, and they do come.  Make something tangible to remind you when you are having waves!

They say, "Fine, quit the cogentin."  I have told you, no, you need a taper, and we recommend 10% tapering.  Brassmonkey and MMT agree.

OK!  So in a month's time, you need to start making liquid from your Saphris.  You will want to use as little liquid as possible, and make up only 4 days of liquid at a time.  So you might want to weigh your tablets and only use 1/2 at a time to make up your liquid.

I have two dosage weights because the mail pharmacy accidentally sent me the wrong ones! 

I have 2.5mg, which is what I am currently working with and am on 0.10 on scale. I guess that's the weight it just shows up as .10

I have also 5mg.

Now for the next question:  how much do your tablets weigh?

 

The 2.5mg tablet weighs (???) in mg

and the 5.0mg weighs (???) in mg.

 

I'm going to teach you two new abbreviations (thanks Brassmonkey) 

mgtw = milligram TOTAL weight (that's the number I'm asking for)

mgai = milligrams of active ingredient (that's the 2.5 mg tablet and 5.0 mg tablet)

 

So you will tell us:

 

2.5 mgai = (???) mgtw

5.0 mgai = (???) mgtw

 

learning the different between total weight (mgtw) and active ingredient (mgai) will help you keep things clear.

 

Nice and easy, we have a month to get this down.

 

So - which tablets were given you by mistake?  The 2.5?  Or the 5.0?  I reckon the 2.5 are going to be easier to work with in liquid suspension, it would be nice to use that for the rest of the taper.

 Thank you so much. I’m actually not at home right now to check it on the scale but the 2.5 weighs 25. So I guess there’s not really an active ingredient wait in there. The 5 mg tablets were given to me by accident. The 2.5 tablets are the pediatric dosage. And that’s scary Pediatric dosage. I am dictating so please excuse errors. When I get home I will weigh the tablet just to doublecheck and also to look at where the decimal points are. I can tell you definitely points really messed me up. So I’m just going to have to be careful with that.

OK I really don’t understand how this is happening but when I weigh the 2.5 mg tablet and the 5 mg tablet I get the same weight on the scale which is 0.025

 I got the 10 mL syringe is and I like them better than the 5 mL syringe is actually. I have some amber glass jars but they might be too big. Remembering somewhere along the line I think it was you who suggested a certain kind of container or maybe something else I’m going to need. I do have a cylinder for measuring but it is not glass is plastic. Actually somewhere I do have a glass one from many years ago when I tapered Clonopin. I could look for that if you think it would be better.I’m driving right now and dictating like I usually am in between Teaching so I hope this comes out understandable.

Hi DMV,

Your signature notes your Saphris cuts in mg(milligrams).  July 1 cut Saphris to 1.4mg. July 29 1.3. Aug 29 1.2 Sept 29 1.1

So.....which Saphris wafer have you been using to get to these doses?

The 5 mg or the 2.5 mg?

And then how did you use your scale to measure out the previous dosing?

Please wait to respond when you are not driving.  Just a safety precaution really.  I know you are dictating, so at least there is that.  Be safe DMV.  No reason I can see why you would be reading and then responding here while driving.  If your next drop in tapering is a few days off, it is just not going to be the end of the world.  I know you are anxiously awaiting a change but be careful.  Okay?

It's possible.  Are you weighing several of each?   And then taking the average weight?  Weigh them separately though, not all at once.  Which tablet are you going to stick with for your final part of your Saphris taper?

So, for example: from the quotes above you might consider answering like this:

My 2.5 mg tablet = 0.025 mgtw  Therefore,  you have 2.5 mgai in your 0.025mgtw tablet(sublingual wafer in your case)

My 5.0 mg tablet= 0.025 mgtw 

 

I hope this helps.  See if you can do what I did with the 2.5 mg tablet with your 5.0 mg tablet in the example in green above.  That might help you.  Just complete that 2nd line.  How much mgai in mgtw of your 5.0 mg tablet?

5/28/2015-off Adderal salts 2.5mg. (I had been on that since hospital 10/2014)

12/2015---just holding, holding, holding, with trileptal/oxcarb at 75 mg. 1/2 tab at hs.  My last psycho med ever!  Tapered @ 10% every 4 weeks, sometimes 2 weeks to

2016 Dec 16 medication free!!

I'm concerned about your readings on your scale.

 

0.025 = 25 mg?

 

so 2.5 mgai = 25 mgtw?

 

this means that you need 10 mgtw for 1.0 mgai.

Are you weighing 0.011 mg total weight (mgtw) on your scale?

 

If the total weight is 0.011, then 0.0011 is what you are taking, and you may not need to taper anymore.

No.

 

When you weigh a 5mg tablet the scales read 0.025.  This would be called 25mgpw (no decimal point)

If there were no fillers in the tablet and all you had was 5mg active ingredient the scales would read 0.005

 

If we divide the pw by the ai we get 25/5 = 5  which tells us that for every 5mgpw (0.005 on the scale) we get 1mgai (0.001 on the scale).  This is referred to as the AIC (Active Ingredient Concentration)

 

To figure your dose multiply the AIC by the desired dose  5 x 1.1 = 5.5.  So to get the dose listed in your signature you would weigh out 5.5mgpw (0.0055 on the scale) which will be hard to do because the scale doesn't measure that small.

 

Now if we used the 2.5mgai tablets the AIC is 25/2.5= 10 so every 10mgpw (0.010) gives you 1mgai (0.001) This assumes that the 2.5mgai tablet actually weighs 25mgpw (0.025)

To get the weight of the dose in your signature  you multiply 10 X 1.1 = 11 (0.011mgpw) which your scale should easily measure.

20 years on Paxil starting at 20mg and working up to 40mg. Sept 2011 started 10% every 6 weeks taper (2.5% every week for 4 weeks then hold for 2 additional weeks), currently at 7.9mg. Oct 2011 CTed 15oz vodka a night, to only drinking 2 beers most nights, totally sober Feb 2013.

Since I wrote this I have continued to decrease my dose by 10% every 6 weeks (2.5% every week for 4 weeks and then hold for an additional 2 weeks). I added in an extra 6 week hold when I hit 10mg to let things settle out even more. When I hit 3mgpw it became hard to split the drop into 4 parts so I switched to dropping 1mgpw (pill weight) every week for 3 weeks and then holding for another 3 weeks.  The 3 + 3 schedule turned out to be too harsh so I cut back to dropping 1mgpw every 4 weeks which is working better.

Final Dose 0.016mg.     Current dose 0.000mg 04-15-2017

@brassmonkey @JanCarol @manymoretodays Thank you all. Brass monkey I am afraid I am having trouble following. I plan on using the 2.5 mg wafers. They weigh 0.025 on the Gemini 20. Currently I break them into bits to get my dose. Which is as of today 0.010 

can I just keep dropping like this? To 0.009 next for example? I am afraid I am not going to be able to figure it out otherwise. I am sorry but my math disability makes tracking and comprehending numbers very very difficult.

OK my maths were confirmed by BrassMonkey - (whew!)

 

What we are seeing on the scale when she gives 3 digits past the decimal point = are milligrams (mg).

 

0.025 = 25 milligrams.

 

My question to DMV is this:

 

Are you weighing 0.010 on your scale?  That's the total amount of Saphris you are taking?

 

Because that's only 0.001 active ingredient.  1 milligram.  The perfect dropping off point.

 

Wouldn't that be good news?  We may not have to do the liquid thing after all.

Hey D - 

 

In mod discussions, I do have a maths error, that I don't quite understand.

 

It seems you are taking less active ingredient than you thought - but I'm still confused by the scale readings.  Other mods have a better handle on this.

 

They say you are taking 0.1 mgai.

 

Which for many people is a good jumping off point - but Saphris hits on so many receptors, we'd like to see you continue tapering down to half of what you are on now.

 

So you will need to do the liquid.

 

Your equipment list seems fine - if your bottles are too big, you might want smaller ones.  

We want to use as little liquid as possible to get maximum active ingredient - that you will hold under your tongue for at least 10 minutes!

 

I know it's possible, because I used to do "oil pulling," where you hold coconut oil in your mouth for 10 minutes (great for oral health, and supposedly digestive health - but it was hard to keep up every day forever).

 

I'm terribly sorry if I got your hopes up that you were done with Saphris - but we're not doctors, and we want to see you stay safe and functional as much as possible.  If something goes awry, we will be unable to do more than offer you internet {{{{HUGS}}}}}} - which we will do - but that doesn't help much if you go into serious symptoms.

 

So - the bottom line - please taper until you get down around half of what you are on now. 

 

We will help with the maths!

 

As it stands now, you will dissolve 2.5 mg tablet in 5 ml of water (about a teaspoon).  Shake thoroughly, and use a syringe to extract your dose.  Do not keep solution for more than 4 days!

 

To get 0.9 mgai, you will need to extract 2 ml of water.  This is a tiny, tiny amount.  MODS please check my maths?

 

You will need to hold 2 ml of shaken extract under your tongue for 10 minutes.  (learn to fast from words for 10 minutes a day!)

First I’m just wondering if it’s possible for me to do one more dry drop? I’m asking because I’m traveling to Florida in December and doing the liquid is going to be really difficult. I’m not sure what the percentages between my current dose in the neck drop if I could get away with it. And then start the liquid after that. The other question I have is how am I going to not swallow for 10 minutes. Just her being at the dentist I know that’s not possible for me. Is there somewhere you swallow without swallowing the liquid?

 Off-topic, I have been asked to teach a class and include a meditation at the end. Normally this would feel like a great opportunity with the really highly respected teacher. But in withdrawAl It seems like more than I can commit to right now especially with the studio closing down. I’m going to go to the class today to see what it’s like, I know her style of teaching and lineage is totally different than mine and so that is a big concern. I think it wouldn’t feel so scary like I said if I wasn’t in the place i am in. Anyway wondering what thoughts people might have about taking on the challenge like this.

Yes it's possible to hold it under your tongue without swallowing, or rather you can swallow around it.  Try practicing with a bit or water, you'll get the hang of it.

20 years on Paxil starting at 20mg and working up to 40mg. Sept 2011 started 10% every 6 weeks taper (2.5% every week for 4 weeks then hold for 2 additional weeks), currently at 7.9mg. Oct 2011 CTed 15oz vodka a night, to only drinking 2 beers most nights, totally sober Feb 2013.

Since I wrote this I have continued to decrease my dose by 10% every 6 weeks (2.5% every week for 4 weeks and then hold for an additional 2 weeks). I added in an extra 6 week hold when I hit 10mg to let things settle out even more. When I hit 3mgpw it became hard to split the drop into 4 parts so I switched to dropping 1mgpw (pill weight) every week for 3 weeks and then holding for another 3 weeks.  The 3 + 3 schedule turned out to be too harsh so I cut back to dropping 1mgpw every 4 weeks which is working better.

Final Dose 0.016mg.     Current dose 0.000mg 04-15-2017

What do you think about making one more dry cut because of the travelling? See last message. Not sure what the percentage cut is but if its not too bad it would be a lot more manageable.

I don't see a problem.  I always made up all my doses before we left on any trip so it was just another pill to take and I didn't have to think about it.

If I were you......I would do the travel on your present dose of 0.1 mgai of Saphris.  Get those doses weighed out and ready to go.

.....and then when you come back do your next drop, using the liquid.

Which is going to be 10% of your present dose. 

(also......my calculation on Monday was off.....as brassmonkey pointed out, when I was trying to get you to do the calculations along with us)

 

Is that a possibility you could consider? Waiting until you have finished traveling to make your next change?

Hey hi again DMV64,  I am seeing where brassmonkey already answered your question asked on the previous page.  And then you asked again on this page.  Did you see the reply?  Top of this page.       On another somewhat related question about WHEN you go to liquid.....have you tried to see how long it takes for, say 2 ml of water, to absorb under your tongue.  Just to see with plain water.  It may be quicker than 10 minutes.  How long do you think it takes to melt and absorb....at least get absorbed into your body (out of your mouth, from under your tongue) with your partial wafer presently?  I would love to see you participate a bit more in your tapering.  Just meaning that you share what you think and have found too.  That you consider and participate in finding answers too.  Lord knows....the math iS tough and thanks to much input to check and recheck, eh?    And good on new opportunities presenting themselves for you....referring to the Yoga and/or meditation instruction you mentioned you might do.  Okie doke.  Love, peace, healing, and growth.  mmt
