When you say if the aka is already there, you mean that I won´t be able to keep tapering? I have to keep tapering my risperidone.Maybe 2 percent or 5 percent.

No, if the akathisia was there before you started tapering the drugs, it's a drug or drugs causing the akathisia. We don't know which drug. The akathisia probably won't go away until you lower all the drugs substantially. The akathisia is not a withdrawal symptom, it's an adverse effect of one or more of your drugs.

Is it worse before you take the combination of Abilify, Cymbalta, and biperiden in the morning, or after you take them? After I take (abilify, cymbalta and biperiden).

Is it worse before you take the combination of risperidone and Klonopin at night, or afterwards? Before I take the risperidone and klonopin.

If the akathisia is worse AFTER you take Abilify, Cymbalta, and biperiden, one or more of those drugs is most likely causing it.

If the akathisia is less after you take risperidone and Klonopin, one or both of them is helping it (most likely Klonopin).

Have you tried tapering Abilify? When did you start taking Abilify, Cymbalta, Klonopin, biperiden, and risperidone? Please add those dates (month and year) to your signature.

Can the biperiden 4mg extended release broken in half?

It is not a good idea to break the tablets. Even though they have a score line they are not designed to be broken. Breaking them changes then from extended release to instant release which is very dif

How do I taper them biperiden? Do I take 2 pills of biperiden 2mg non extended release?

Thank you Altostrata. I don't have to many lines left to add to my signature. How can I create a nice long list of the drugs I tapered? As far as I know the signature can contain only 12 lines.

The history of the tapering is not as valuable as the history of when you started the drugs you're currently taking and the dosages you're taking now.

Details of your risperidone taper prior to March is not very useful to use, though you might want to save it for your own reference.

Hey, hope I can give my 2 cents here. I agree with Altostrata. I know this is personal, but at least for me the akathisia came because of the drugs and goes away (bear in mind - slowly!) as I taper off of them. And I've only ever taken AP, not an AD, so that leads me to...

It's a really wild guess, but Abilify is a partial agonist of dopamine (unlike other APs, which are solely antagonists) and it's widely known to cause restlessness. I've tried taking it before bed time while experiencing insomnia and that made it awful, I was so unrest in bed I could barely fit in my skin. OTOH I've taken Risperidone (never the two simultaneously) and it always made me sleepy, super calm and unwilling to move in general. Again, I know this is personal, but if I were you, my main suspect would be the Abilify. Try monitoring your symptoms during the day and you'll definitely find the reason! Good luck 🤞

Thanks, Nocturna. Abilify has always been a prime suspect.

Manny, gradually tapering off Abilify makes the most sense. It may very well be causing your akathisia, it's duplicated by the risperidone, and it may be conflicting with other drugs.

Sorry Altostrata. I have read the tips to taper off abilify before. My question is how about combining pills with abilify liquid. Will it be suitable to take abilify 5mg with abilify 5ml?

This information is in Post #1 of the Tips for Tapering Abilify topic:

Using a combination of tablets or capsules and liquid
Rather than switch directly to an all-liquid dose, you may wish to take part of your dose in liquid and part in lower-dose tablets or capsules, gradually converting to all liquid as you get to lower dosages. This can be very convenient and reduce any problems switching from one form of the drug to another.

If your doctor prescribes liquid and tablets or capsules at the same time, most likely he or she will have to indicate "divided doses" in the prescriptions to get the drugs covered by insurance.

Hi,I spoke with my pharmacist. She told me they only have abilify 1mg/mg oral solution in 150 ml. To measure liquid there is a small plastic vase that measures 2,5ml and makes decrements of 0,5ml. What do I do? My psychiatrist thinks I am taking abilify 20mg when I am taking abilify 10mg.

Thank you,

Manny

I am reducing 2%of abilify 10 Mg.  My psychiatrist thinks that I was taking 20 mgs. He dropped 5mg of those 20mgs,so they are 15mg. I don't know what to tell him. That I take 19,8mg (Abilify 20mg),or 14,8mg (abilify 15mg) when in reality I am taking 9,8ml,but these I cannot tell him. I want to make sure he gets that I cannot taper more than 2 percent. my options are telling him I was on abilify 15 mg but I felt bad dropping from abilify 20mg to 15mg and I had to go back to 19,8mg.Second choice, I could handle a 5mg reduction from 20mg to 15mg and I want to taper the  abilify 15mg(abilify 10mg  plus 5ml oral solution) to 14,8ml.
I won't tell him I am in 9,8ml,No way. My taper would be like this 9,8ml,9,6ml,9,4ml,9,2 ml, 9ml each month. My last taper of abilify took me two years to get rid off of abilify 10mg.

How do I talk to my psychiatrist about tapering? He just don't get it. He wants me to taper the abilify making reductions of 1mg each time (15mg,14mg, 13mg,and so on). I am reducing 2 percent and I have problems managing my Akathisia.

I spoke with my psychiatrist today. He wants me to take more biperiden for the akathisia. He wants me to lower down the dose of abilify to 14mg. He doesn't want me to take below 12mg. I know biperiden causes cognitive problems. He said biperiden 4 mg extended release cannot be broken. How I am going to taper it? He told me that biperiden causes constipation and dry mouth.

This is what he wants me to take:

Abilify 14mg - abilify 10 mg and Abilify 4ml

Risperdal 0, 86ml at night

Cymbalta 60 mg-1,0,0

Klonopin 0,25mg-0,0,1

Manny, we can't talk to your doctor for you. You need to tell him you want to be taking less drugs, not more.

You may need to find someone who can go to the doctor with you to help emphasize your wishes about your care. You may need to find another doctor.

These are problems we cannot help you solve.

+1 for this idea, I once brought my man with me to the psychiatrist and it had a positive impact. Think of someone who is really close to you and has a good overview of your situation (how stable you are, etc.).

Hi Altostrata. I told him I want to reduce slowly but he doesn't understand. I don't have anyone to take with me to the psychiatrist. I had a phone appointment yesterday. I feel rage, anger towards him. I am afraid I can end up in a psy ward if not compliant to his way of thinking. Eventhough he agreed to take me off of all psychotropic drugs with my GP, and he doesn't want to me to take off of abilify because I might relapse.

I don't have anyone with power authority to come with me. This psychiatrist is playing with me with respect to Psychiatric drugs. I am in danger with him. Either I speak up my mind with all what I know with respect to his tapers or I let him to do what he wants with me because of fear.

Manny, you need to speak calmly and frankly with him. You need to explain that you need to reduce more gradually.

If he won't help you taper Abilify, ask him to help you very gradually taper the other drugs, such as the Risperidal first. You may need compounded liquid preparations for some of them.

You should also point out the biperiden may be causing your symptoms.

We've reviewed this so many times, I don't know what else to suggest. You need to find a way to gradually go off your drugs.

This is not medical advice. Discuss any decisions about your medical care with a knowledgeable medical practitioner.

I feel rage, anger towards him.

I get it, Manny, I would totally feel angry if I were you. It's quite normal that a doctor doesn't offer you understanding - most of them are simply not trained to recognise withdrawal as a real issue.

From my experience the doctors are advising you to stay on drugs because they believe otherwise you'll get into a relapse, which might be dangerous for various reasons. They wish you best, but sometimes their best might not be your best. For instance your dose of different medications might be too high. In this case, they'll eventually agree that a lower dose or no medication might be better for you, but you need patience to prove to them that you won't get sick again. When you asked him to taper to e.g. 10 mg Abilify, what does he reply? Why exactly 12mg? I know that even 5 mg might be an efficient support dose for this, so why not 10 or 7.5?

Abilify

2016 - 7.5 mg

2017 - 2018 - 2 mg

April 2019 - May 2020 - 5 mg

June 2020 - 4.5 mg

July 2020 - 4 mg

August 2020 - 3.5 mg

September 2020 - 3 mg

October 2020 - 2.7 mg

November 2020 - 2.5 mg

December 2020 - 2.3 mg

You don't need someone with power or authority to go with you, just someone who will repeat your wishes to the psychiatrist. The two of you need to calmly repeat your wishes to the psychiatrist.

Can you find a free lawyer or patient advocate? Even a neighbor who is sympathetic will do. In Spain, do not doctors have to listen to the wishes of their patients?

Manny, you need to speak calmly and frankly with him. You need to explain that you need to reduce more Gradually. He doesn't understand tapering.  He has never done. He is a fraud and a fool who doesn't listen to his patients. His tapering is cutting medication to 50% or 25%. I had to taper on my own without him knowing. I tapered klonopin, abilify and risperdal. I never finished my tapers because when I tapered klonopin(1,25mg to 0,25mg) I came to surviving antidepressants and I started  to taper the antipsichotic first (abilify 20mg to 10mg).Then I changed my GP, and was ordered to start tapering risperdal (2mg down to 0,86ml now).

What pisses me of is that he agreed with my GP to taper me off of all drugs I take.Maybe because I mentioned I have akathisia he is afraid I could do something to harm myself. I told him I need to move from one place to another, his brilliant idea is to have me taking 6mg of biperiden when I am taking 4mg. I can't taper the abilify 4mg extended release.

He usually is very collaborative reducing medication but making very stupid mistakes, making large reductions such as reduce klonopin from 1mg to 0,5mg, abilify 20mg to 15mg, Zyprexa in the case of my mom from 15mg to 7,5mg,and so on.

You should also point out the biperiden may be causing your symptoms.

He was so stupid that he mentioned to me the constipation and dry mouth side effects. Also, the cognition disfunction we discussed it before. I don't know what he thinks akathisia is. His fix is to add more biperiden and then not knowing how to taper it. How do I taper biperiden 4mg extended release if it needs to be tapered 10 percent too?

Only way is to lie him and taper on my own. Can  I speak why I was put on these drugs? Is it forgiven to talk about religion in the group? I was put on psy drugs because of my religious beliefs, and my family told them I had paranoid schizophrenia who was my supposedly diagnosis in the USA. I was more obsessed with heaven and hell than to hearing voices.I couldn't sleep that was true. I never heard voices that time. I was just afraid. I think psychotherapy would be a better option for me. I really believed that was I believed it was true. Now I know that was foolish for me to believe that.

Maybe I need to pay a lawyer to respect my wishes to go off medication. It is very hard to have last wishes if you are a person who has a mental diagnosis.

Thank you Nocturna for being there. I don't know why he said 12mg. He said he doesn't worry about the risperidone because it is less dose and it is easily to wd but the abilify it very hard to come off. He wants me in abilify 14mg and biperiden in 4mgs extended release and 2mg normal. He wanted me in 8mg when he knows of the cognitive dysfunction. I already have constipation and dry mouth in less degrees. He is a drug dealer because he said don't worry increasing biperiden, side effects are constipation and dry mouth. He did not provide me a way to taper for the biperiden. His ways of tapering are foolish like 50  and 25 percent reductions.

I have tapered on my own and I have not relapse, but I understand their side. I don't agree with my diagnosis with me (schizophrenia paranoid). I had OCD but not schizophrenia.

We've discussed your drug cocktail many times. This psychiatrist also agreed it it excessive. You experience adverse effects. Your doctor needs to consider your quality of life.

You are taking 5 drugs. If he insists Abilify is necessary, you have a choice of 4 other drugs to taper. Risperidone duplicates Abilify action. Your dose of Cymbalta is high and may be contributing to your adverse effects. You also need to discuss adverse effects of biperiden with him. I would save tapering Abilify and Klonopin for last.

We've been talking about this so long, I don't feel there's anything else I can add. If you want to minimize your drugs, you need to find a way to gradually lower the dosages of one at a time. If this doctor won't help, perhaps another will. If you want to titrate your drugs yourself, we have given you the tips for tapering topics over and over.

You need to make the decisions for yourself. I honestly don't think there's anything else I can say.

This is not medical advice. Discuss any decisions about your medical care with a knowledgeable medical practitioner.

Hi Manny!

Sorry your having so much trouble with medication side effects and the doctor. Is there a chance you can find another doctor? I realize that might be hard right now with the lockdowns but I think the stress of the relationship can only be making the overall situation worse, especially that you have to be lying to him about what your actually taking. The anger and rage and lack of trust can't be helping and I wouldn't be surprised if He isn't picking up on that subliminally and it think that it could only be worse for your relationship in the long run if He finds out how long You've been lying to Him.  It may also be hard for Him to understand why your having the symptoms you are if He thinks your on one dose when your actually taking another. You've received a lot of great advice here on which drugs are the most likely culprit of the akathisia and which meds are repeats. I hope your able to find someone your more comfortable with, as I feel that is just as much your main complaint as the symptoms.

The good news is many people find they do not have to be completely off a med or cocktail to see improvements. Wishing you all the best as you move forward! 🙂
