Hello @bubble

Thanks for visiting, nice to hear from you.

It will really be a challenge to go into the office - my legs and anxiety aren't getting much better (in fact are worse).  I will see how I go.

As for thoughts of the years ahead, I mostly try to not think about them and just get on with what I can do in the day.  On the worse days, I do get dispirited and wonder if there will be an improvement, or just years of WD symptoms.  Then I look at someone else's signature and realise what they are going through and know that I am not alone in this mess of drugs and disabling effects.

That's why it's so good of you to visit - support.

Many thanks and the best of wishes

Hello @wantrelief

Thank you for your encouraging words - they really help.

I do so hope holding will bring further stabilization for you.  You have had some better days as of late so I am hopeful this will lead to more.

That would be lovely.  And I hope you get some relief at times as well.

Today I'm fatigued but have been to a quilting group to drop off some of my baby knitting (a friend took me), I could feel an anxiety spike in that group setting.  This afternoon I'm supposed to be going for a walk - I think it will be a short one (half-hour max.) if I do manage to get out there (have to drive myself which is more anxiety-provoking).

The sun is out, though.

I'm so grateful for your friendship  - walking the path side-by-side.

I was just about to update my thread, Neroli, and you remind me to be grateful while I'm doing so!

Currently taking Ramapril (blood pressure) 5 mg twice a day

Omeprazole 10 mg AM and 20 mg PM  (the taper has gone nowhere after the first cut)

Famotidine   once a day (and I still needs tums sometimes)

magnesium 200 mg at night

as of yesterday 2 fish oil capsules "EPA-DHA 1000"

off Lexapro as of 5/2018  - last dose had been 5 mg every other day for a couple years

highest dose had been 20 mg at which point I was diagnosed with Bipolar II, which went away when I cut the lexapro down to 15 mg.

I spent years on Paxil before Lexapro (can't remember dose), briefly on Effexor and Abilify and others I have forgotten. in fact, when I was diagnoses with BPII I was put on all kinds of things which made me feel so bad I stopped them cold turkey within maybe 3 or 4 weeks, thank goodness. since then I've known these pills were terrible and I weaned down the Lexapro with zero help or support over I'm not sure how many years.

Hello @Rosetta

Thank you for dropping by - oh, I do hope I can be of support, it helps to give me some purpose in all this suffering.

I just posted to Shep, so I'll copy it here as an update:

"I have been in to the office today and it was kind of ok but having managed to get back home after 4 1/2 hours at the office (taxis and train for the commute, so add on about 2 1/2 hours for travel)  I can barely walk my legs are so stiff and wobbly, I am shaking with underlying anxiety and absolutely wiped out - all of them at the 8-9 level.  Plus lower back ache and an irritated bladder at about level 5."

Can't say I'm feeling too hopeful at the moment, or particularly convinced that this is going to get any better.

I hope your days have been better.  I can't remember what I last wrote on your thread - forgive me, I'm still trying to recuperate from the day.

Beat of the best to you, Rosetta

Time to write some gratefulness down:

I slept pretty well last night.

Took today off sick to recuperate from the exhaustion of yesterday

Felt calm most of the day.

Fatigue wore off during the day.

Managed a 40 min. walk and coffee with a walk-mate

Made contact with people I haven't seen in a long time.

Felt pretty good, when I thought I'd be totally wiped out.

Enjoyed the sunshine on the beach walk.

Popped into a charity shop on the spur of the moment ("popping' anywhere these days tends to be a planned exercise so I don't use too much energy) and got a nice cardigan for $10 (also made mental note for when I'm retired that charity shops can be a good source of clothing etc.)

Able to do my hobbies.

In bed at last - thank goodness!

Tough day.  Didn't sleep so well last night.  Fatigue/tiredness has been up around 7 all day, legs started off at 7-8 and have eased to around 6-7 now.  Lots of weeing.  Apathy, couldn't get myself to go out for a walk.  Took a big effort to get into colouring but once I did that is what has taken me through the hours of today.  A finger-tip-hanging day is how I experience it.

Frustrated with a really good, supportive friend who started to suggest CFS as the reason for the tiredness and then kind if insisted that there must be someone, somewhere who knows what is going on and be able to help.  Yee gads, I know the words are coming out of her mouth because she just wants me to be better, to find some way to get me back to "normal" but it feels like an onslaught being on the end of this sort of talk - responding that there is no one anywhere who knows how to do that, feeling like I have to defend myself against the charge of just not looking hard enough for the right Dr. or whomever.  And really?  What would they do?  Push more pills at me?  I cannot fault the utter support this friend has given me but stuff like that leaves me drained and feeling isolated again.  I need to learn to ask her not to go down that route, find some way of saying it kindly and thanking her for her concern.

Shep reminds me that  "battle fatigue" will be in play as well as the symptoms.  Yep, think I just about met my Waterloo.  I'd just like to REST, properly, sleep, be relaxed, go out for a coffee without it being a big deal.....

Whining.

Tonight, sleep could be better, tomorrow may be a different kettle of fish, I have been surprised how the next day can be one of more hope and optimism and energy after a tough one.  So will hold on to that.

in the spirit of looking for the positives, three things I enjoyed today (maybe I'd better say "appreciated", as "enjoy" is a bit over the top!):

Looking at the monarch chrysalis on my lavender plant - green body with gold dots round the top rim

Eating dinner

Video calling a woman in the UK who is having protracted AD CT injury symptoms

ok and a fourth: support from wonderful people on SA

After a mega-wobbly start, the day got better - the fatigue lifted and my legs got less stiff and achy. I worked at home and felt motivated and engaged, with a reasonably calm nervous system.  Very different to yesterday.  Legs at the end of the day at about 6. Fatigue 0.

Three things I enjoyed:

Autumn/WInter golden sun lighting up my living room in the afternoon

Feeling calmer and not utterly fatigued

Very low hungers and wees - so nice to be not aggravated by this for one day

From reading your posts, Neroli, I am reminded how important it is to seek out opportunities to feel grateful all day, every day. 😍

another thing that is important is believing things are getting better, or are going to get better.

Can't say I'm feeling too hopeful at the moment, or particularly convinced that this is going to get any better.

over-all, so you usually feel things are going to get better?  when I don't, I try to practice it anyway. not saying it necessarily helps because at my worst I can't tell, but my bottom-line under-lying belief is I am getting better.  I think and hope it is the same for you!  (from your posts, I'm sure it is)

 🤗

Hello @JackieDecides

Thank you for popping in.

over-all, I would say I am in land of "I don't know if things are going to get better" - there is absolutely no way to tell.  What I can say is that my legs have got worse over the past two years since I had a breakdown caused by the damaging effects of these drugs.  My walking capacity has decreased, the muscles are getting achier, they feel heavier and more weak, as if they are going to buckle under me.  I have been tested neurologically and the diagnosis has been that I have Functional Neurological Disorder, which has been explained to me as the musculo-skeletal system is not diseased or damaged, like in some process such as MS, but the brain is sending the wrong signals to my muscles.  Basically "they" don't know what it is caused by or how it is treated.  The neurologist said it would take about a year to resolve but it hasn't.  My own thoughts consider things like the mitochondria being damaged - but I don't think there's a test for that over here.

I am now on three psych. drugs (I was med-free when the breakdown happened) and have no idea if they are having an adverse effect - and of course am faced with slow tapering.

On a good day, I have hope that reducing the drug burden will help the legs.  On a bad day, when WD effects such as anxiety is prodding at me, when fatigue is pulling me down, when depression soaks into me, I see how long that could take and meanwhile the legs seem to get worse.  My practice then is to try to incorporate the "I don't know" aspect into my awareness and come to some greater acknowledgement of the not-knowing. (I'm not going to say acceptance because that is too facile a word for me at the moment).

I'm glad you have an underpinning belief things will get better for you, and why not?  Your experience may be showing you that there is improvement and many, many people do get better.

Wishing you all the best, keep on keeping on, we're all here with you

Hi Neroli, so understand this and am with you.  How do we know?  For me all this started in 2014 with a breakdown as well with some wanting to reduce meds so I rapidly tapered, then had other meds introduced that reacted to and stopped.  But I do feel I would have been better in the long run had I done nothing except rest and reset in 2014.  The constant jars to the system, for me, just keep things stirred up, reactive, tired, anxious, etc etc.

I have people as well who say things that I honestly dont know how to respond to.  Why I have isolated so I think.  A once dear friend said that "everyone our age has breaking body. Yes, you have some drug rescue issues, but we are all going through this crap.  You are not the only one!!'

I really have nothing to say to anyone pretty much anymore...just an instinct to help with survival guess.  It is hard Neroli...just wanted you to know I understand that they dont understand. But how could they?  I would want as few people to understand this as possible...there are so many here already.  So I say nothing because it is something that, unless experience, cannot possibly be phathomed.

Thinking about you so and hoping today has been kind to my PF! Love and hugs!!💜

Hello dear Rabe

Thank you for your understanding.  You and I (and many others here) are so wonky from the drug damage that we cannot know what will turn out.

I thank you for your understanding especially of the (what I think) is realistic view and that you get it about other people saying (probably unintentional) ignorant or potentially hurtful things.  We have to keep ourselves safe.

I hope you have had a reasonable day.  Mine was a little tumultuous but I am in bed now and able to rest.

much love to you, PF

Tried a 0.1mg (1%) reduction in Escitalopram starting last Saturday - felt optimistic.  By Wednesday extreme fatigue and depression, so updosed to 10mg again (seems this one is one of the hardest for me, or it just isn't the right time).  Wednesday and Thursday gruesome but today a fair bit better, mood lifted and calmer in the afternoon. Have recorded details in my Symptom Diary to keep track of how things go so I can keep learning about tapering.

Roller coaster week but things to be grateful for:

Still able to work from home - got a great team I work with

Friend-in-withdrawal who phoned me when I was feeling particularly desperate and talked me down off the ceiling (that's generosity and kindness)

Able to drive the car today

Wonderful people on SA who send encouragement and support

Calm moments

Hobbies that relax and distract

Can still fit clothes even though I've put on weight

Good, plain, food

Things I enjoyed today:

Work

Taking the afternoon off

Going out in the car

Treating myself to some flowers, nice soap and hand cream and a scented votive which I burned

Visit from a young woman I used to walk with and her 6 month-old baby boy

Calm afternoon

YouTube videos of Fred Astaire dancing

Note:

Symptoms appear to have worsened - legs a lot weaker and now the muscles have started aching more, aches in lower back and shoulders becoming more pronounced.  Fatigue high (it's only 8.00am) and shakes more pronounced.

Not sure which way to turn re. reductions.

Thinking I may need to see my GP to see if I can get help with looking after myself  - neighbour took me shopping yesterday but legs weren't good and it made me ache.

neroli, how much of each drug are you taking now, at what times of day?

Here is your drug interactions report

Do you see where it says escilatopram and nortriptyline can cause "muscle spasm or stiffness, tremor, incoordination"? Does that sound like your symptoms?

Hello Alto

Thanks for dropping in.

Diazepam 6.00am - 4.00mg;  12.00pm - 4mg;  6.00pm - 4mg

Escitalopram 9.30am - 10mg

Nortriptyline 8.40pm - 85mg

Yes, I see the interactions checker lists:

Using escitalopram together with nortriptyline can increase the risk of a rare but serious condition called the serotonin syndrome, which may include symptoms such as confusion, hallucination, seizure, extreme changes in blood pressure, increased heart rate, fever, excessive sweating, shivering or shaking, blurred vision, muscle spasm or stiffness, tremor, incoordination, stomach cramp, nausea, vomiting, and diarrhea.

All my research around serotonin syndrome seemed to suggest that this would be apparent at the beginning of ingesting the drugs - I haven't been able to find anything that talks about "creeping onset".  These latest symptoms, which are increased muscle stiffness, and lingering "inner tremor/shakes" (ie. my fingers don't visibly shake so much) seem to have crept up over the past two years of being on these drugs, more noticeably since early March this year (I did my last Diaz reduction on January 12).  So, I'd not thought to attribute symptoms to serotonin syndrome.

I'm also getting more wooziness, which I'd also wondered whether the benzo might be causing, along with increased fatigue.

Last week I tried a 0.1mg reduction (1%) in Escitalopram in case that was causing the leg and fatigue issues, as when I was on Citalopram, that caused me some leg weakness and strong fatigue.  I got to day 5 and felt so fatigued and depressed I went back up to 10mg.  It is a bit frightening that the Esci. seems to be so difficult to taper, so I'd dismissed doing an Esci. taper for now.

Taking out the Nortrip. scares me in case I get insomnia (the Nortrip does at least black me out for a reasonable portion of the night).

Then again, taking out any of the "brake-like" drugs (Diaz and Nortrip) scares me too as it has been suggested that the Esci could "break through" and be more activating (which I take to mean more anxiety and inner shakes and possibly getting a bit manic(!?)).

neroli, you do not have serotonin syndrome. However, escilatopram and nortriptyline can interact in a bad way that's short of serotonin syndrome. It's called serotonin toxicity.

As you get older and add other drugs or make other changes, the effects of a drug-drug interaction can become more pronounced.

But whatever you want to call it, that's my best guess about where your symptoms are coming from.

The wooziness is probably another drug-drug interaction, with the benzo. If you look at the interactions report, you can see that among the adverse effects as well.

Since escilatopram is citalopram's chemical sibling, you may very well have similar adverse effects from both.

Is it possible that when you reduced escilatopram, you scared yourself or you mistook your usual symptoms to be a result of the reduction? Are you sure the symptoms you felt were from the escilatopram reduction?

If you don't want to reduce any of your drugs, you don't have to. It's up to you.

From this I would infer that reducing either of the two drugs could improve symptoms - particularly leg muscule stiffness.

Moot point.  I wrote in my other thread '

"On Saturday I made a 1% cut in the Escitalopram  - and was feeling quite enthusiastic that such a small cut wouldn't rattle me this time (I'd tried 1% before).  That's 0.1mg.  I wanted to try that one to see if I could get down on the dosage and possibly that would help sort out some leg symptoms.

By Wednesday I was so fatigued and depressed and my legs were more wobbly (more than usual I would say) and it was "real", I'm sure, not a nocebo response [Emphasis added].  I could stand it no longer, nor risk getting worse, so I updosed back to 10mg on Wednesday.  I'm still dealing with the fall out - extreme emotion on Wednesday, bad sleep last night, legs so tight and heavy/weak feeling and fatigue at top level"

Thank you Alto

I'll take your perceptions into consideration.

I’m not sure it’s possible to tell whether or not your response was ‘nocebo’ or not.

 = medication taken now

2007 quetiapine to March 2019 200mg

2019 quetiapine March to present 225mg

2007 citalopram to present 40mg
2018 March Abilify 5mg
2019 Abilify February rapid taper over 3 weeks from 5mg to off

2019 March Clonazepam as required, taken very occasionally, then taken 0.5mg for 2 days 28th and 29th March, now phased out

2019 1st April reinstated Abilify 0.5mg / day

2018 to 2020 Liquid B12 2g twice daily (diagnosed B12 deficiency)

2020 July reduced quetiapine to 200mg

2022 October began taper of Abilify

Hi, Neroli,

I am very familiar with the muscle stiffness.  I had it from both Celexa and Zoloft.  I'm certain that I had serotonin toxicity.  I also have been struggling with muscle stiffness from the CT as a WD affect, I suppose.  So, that's why I always cringe when someone says "I know I should go slower, but the side effects made me decide to drop 20 mg.". I believe that severe and life threatening side effects justify quicker/larger decreases, but I hope you will try to reduce slowly unless you think your life is in jeopardy.

I'm sorry to hear you had to updose.  I'm not sure either way, Neroli.  I don't feel I know enough to make that call, but please know that I am thinking of you.

All my love,  Rosetta

2012-2016 - Doctors raised dose of Zoloft up to 150 mg

2016 - Xanax prescribed - as needed - 0.5 mg about every 3 days (bad reaction)

Really feel for you! Very glad Alto chimed in and drew attention to drug interactions. You are in control.

Just wanted to briefly share my experience of starting Lexapro taper from a position of instability and lots of symptoms. There was definitely great fear of things getting even worse and spiralling out of control. Incredible fatigue was also a major symptom: I couldn't get out of bed for 2 days. It felt as if I had influenza. I somehow manage to ride it out and subsequent cuts were not so bad.

It's hard to tell what the best course of action would be... but you have the information, you know yourself and you will make the best decision when the time comes. Now I just hope you return to your WD normal asap.

Current: 9/2022 Xanax 0.08, Lexapro 2

neroli, if you cannot taper escilatopram, you might taper nortriptyline. One way or the other, if your symptoms are from a drug-drug interaction (which seems likely), you will need to reduce a drug.

Hello Mr McVities Biscuit (don't leave it dunked in the tea too long or half of it will drop off to the bottom of the cup - ex-pat Brit here)

Thank you for popping in.

Ha! Well, there's the rub, isn't it.  I can only say that I had some major melt-downs and tremendous fatigue which seemed to be greater than "normal".  Maybe my next trial will be when I don't have to work (say take some leave) and see what pans out over a whole week.  Would need to have supports in place as the depression was rather deep.

I'm hoping you are going ok with your taper - you've done very well on the Abilify and Clonazepam.

Best of the best to you

I now realise that Citalopram caused leg weakness and fatigue (but I didn't know that was the cause, I kind of thought it might be ageing but it didn't seem right to me), then I I had the muscle weakness start in the latter part of the prolonged WD syndrome from CTin,g just before I collapsed and since then the legs maintained their heavy, leaden feeling.  Over the past few months they've got more stiff and achy, as have other parts of my body, so something's going on.

Don't worry, I won't be hacking chunks off!  I don't feel my life is in jeopardy and I certainly know I'd get into dire straits if I made big reductions.  I'm also trying to balance the tapering so that I can remain functional, look after myself and keep up working.  I think I need to give up expectations re. the outcomes from tapering as well (in my mind, I always see me tapering without distress and getting my running shoes back on, not that there's anything wrong with this but it sets me up for frustration when the taper doesn't go to my mind's plan).
