Bee5: reinstated paroxetine, dose too high?

Thanks @Altostrata. I had a look on the Symptoms and Self-Care forum, and did a search of the site, but have not been able to find a specific thread related to it. I think the issue is that what I am looking for is a group of symptoms of a viral infection (brain zaps, low mood, anxiety etc.), that I experience, that are also a select group of withdrawal symptoms. I did, however find some posts from the mods replying to users saying that withdrawal symptoms ramp up during infections, so I think this confirms it for me. I just think it would be an interesting topic to investigate, because our brain and CNS seem to be affected by viral infections too.

I had the viral infection last week. It coincided with the symptoms. I am better now though. I have not had covid-19, but I am aware that it does cause psychiatric symptoms. These symptoms of mine occur every time I get any virus (respiratory or gastro).

Yes. Brain zaps are my go-to reliable sign that I am getting sick. The only other time I get them is when I step down a dose of my paroxetine or quetiapine.

The primary cause of brain zaps is tapering too quickly. A quick look at your signature says it all. You are tapering way to fast for your body to adjust. The viral infection is just a catalyst that is reducing your bodies ability to stabilize and prevent the zaps. I would suggest an extended hold for many months to let your body stabilize on your current doses and then continue your taper at the recommended 10% rate.

20 years on Paxil starting at 20mg and working up to 40mg. Sept 2011 started 10% every 6 weeks taper (2.5% every week for 4 weeks then hold for 2 additional weeks), currently at 7.9mg. Oct 2011 CTed 15oz vodka a night, to only drinking 2 beers most nights, totally sober Feb 2013.

Since I wrote this I have continued to decrease my dose by 10% every 6 weeks (2.5% every week for 4 weeks and then hold for an additional 2 weeks). I added in an extra 6 week hold when I hit 10mg to let things settle out even more. When I hit 3mgpw it became hard to split the drop into 4 parts so I switched to dropping 1mgpw (pill weight) every week for 3 weeks and then holding for another 3 weeks.  The 3 + 3 schedule turned out to be too harsh so I cut back to dropping 1mgpw every 4 weeks which is working better.

Final Dose 0.016mg.     Current dose 0.000mg 04-15-2017

Hi @brassmonkey. I appreciate your input. I always listen to my body when tapering. And I never drop dose when I have brain zaps. After every dose drop, I do get brain zaps, and I hold as long as I need to until I feel stable physically and emotionally to drop again. The only other time I get brain zaps is just before I get sick with a viral infection. I have noticed the brain zaps are a 'pre-warning' for colds, flu and gastro viruses since 2016. I have done a lot of reading on it, and there are some other people who get it too.

Hi @GVR91

Just a clarification on your post: I took Vitamin D3, Folate (which is Vitamin B9) and Vitamin B6 for my allodynia. None of those is  niacin (niacin is Vitamin B3).

The three I took did help with my allodynia pretty quickly, but I think it was the Vit B6 and B9 that mostly helped, because I usually take Vitamin D3, and I just added the two B-vitamins.

I am lucky in that the allodynia was just a transient, short-lived symptom, and I hope it passes for you too.

Yes, the B-vitamins can be stimulating, that is why I would suggest trying one at a time, and not starting with a B-complex. I worked my way up to B-complex by taking the vitamins one at a time and testing if any of them were stimulating for me.

Hi @Carmie,

As you can see from my latest signature, I decided to move from 9.375 mg to 0 mg quetiapine over the month of June. The morning drowsiness and brain fog from the quetiapine was getting too much for me. I did a lot of my own research on it, and at those doses, the main action is antihistaminic and anticholinergic, so I decided to get off. As expected, I am experiencing a bit of anxiety, and I don't expect my CNS to stabilise for a while, but at least I am able to sleep, which is the most important thing for me at this stage.

How are you doing with your taper?

Bee

Just journaling an update. I tried to finish my quetiapine taper in June (tried to go from 9.375 mg to 0 mg). Unfortunately, I was not successful, and if I'm being honest with myself, I probably should not have tried this taper because I had not yet stabilised on the 9.375 mg either. Hoping the reinstatement helps. Will watch my body over the next few weeks. Trying herbal teas (chamomile and lemon balm to help with this wave).

Hi @Bee5

I see your taking the same drugs I am. How is the Seroquel taper going for you ? I see your a mother as well,so that makes things really hard.  I have two beautiful boys, so having to go through these difficult times can be extra challenging 😰 you seem to have a good outlook  in coping , that’s so important . How did your last drop of Seroquel go ?

1995 started Paxil 20mg slowly increasing to 50mg until 2014

-2014 I decided to tapper myself not knowing how too and crashed , DR added 50mg Seraquel

-2015 tried tapering again and crashed

Started Tapering Both drugs at the same time 6% per month doing daily micro-taper

Guided by Mark Horowitz

24/09/23  14.47mg Seroquel.  16.19mg Paxil

27/11/23.  12.13mg. Seroquel.   13.85mg Paxil

Hi @Cheeky,

So the rushed taper from 9.375 to 0 was a fail, so I decided to reinstate, but I only reinstated 3.125 mg. It has been going much better since the reinstatement; I am able to fall asleep on my own (without herbal teas or valerian or listening to audio books). So I am relieved, and I will be staying on this dose for at least two months, before doing the next drop.

I have one daughter, and the postpartum depression was so bad that another child was not an option for me.

I am sorry that you are going through such a hard time, and I hope you are on your way out of your wave.

That’s great you feel better, what a relief that must me. I had postpartum depression with both my kids and it was awful. I think it’s wise to stay at your current dose for awhile. I will be doing the same. If we didn’t have to work full time and  be a mother it would be so much easier. I have a business and could not afford to stop working. I know why I’m like this, I dropped too much and other stupid things, I’m paying the price now. Wouldn’t be a dream not to have to go through this , it’s made me appreciate my stability.

@Cheeky, definitely, withdrawal has made me quite envious of the people who are stable. But one day, we will be off the meds AND stable.

Hi Bee,

Hope you’re doing okay. Yes, that was definitely too quick a taper, I can’t even taper over five percent every few months. If I jumped to zero from 4.30mg, which I’m at now, I would definitely get severe Akathisia.

I’m glad the reinstatement worked and you’re feeling a bit better.

I’m off to have some dinner😄

Hi @Carmie,

I have had to switch to a liquid taper, because my scale cannot weigh anything below 20 mg of weight, and the 3.125 mg quetiapine weighs about 10 mg. I asked the compounding pharmacies in my city if they can compound it for me, and unfortunately they both refused because they can't get the active ingredient here in South Africa, and they feel it's too risky in terms of dose accuracy to compound it any other way. So I made a simple sugar syrup (based on some pharmacy guidelines I found) and am going to stabilise on the sugar syrup at 3.125 mg for at least a month.

How are you doin? I wanted to ask you: does the brain fog get better as you reduce the quetiapine dose?

Bee

@Bee5 I’m glad you’ve managed to put together your own mixture. I’ve been water titrating my meds for years, just crushing the tablet and adding water, it’s worked really well.

As regards your question, my brain fog is extremely severe all the time, but it’s due to my illness, CFS, which I’ve had it for 26 years.

Take care🧡

Just journalling an update. I am holding tight on the reinstated dose of 3.9 mg quetiapine. And I have told myself that I will wait for one full month of WD normal before contemplating a slight drop again. I have been on 3.9 mg since beginning August, so I am into the third month of this dose. I am also trying to manage stress better so that I don't aggravate the stabilisation process. I am feeling much better though overall, but still have an anxiety blip around once a week.

Update. Have decided to drop my liquid from 2.5 ml to 2 ml, which is a drop from 3.9 mg quetiapine to 3.1 mg. As expected, WD symptoms have ramped up - a spike in anxiety being the main one.

20.5128% decrease

A 10% reduction is 3.51mg.

You might consider updosing to 3.5mg.

Thanks @ChessieCat, I think that's a better way to go. The syringe is graduated adequately so I will adjust the dose to 2.2 ml, which is 3.4 mg, which is closer to 10%.
