@AltostrataAfter my last reduction in Dec 17, I have been getting a large increase in anxiety, which was not a problem last year. My principal withdrawal symptom has always been internal tremors, which were not previously not linked to anxiety, but this year is worse because of the increase in anxiety. I don’t want to take a benzo, so I was wondering about a beta blocker. How often can you take it without it becoming a problem?

@AltostrataThanks for replying Altostrata. I have not had another episode like the one I did in Jan this year.

 

Also I have had VitB12 and folate levels done and they showed no problems, but I understand the VitB12 blood test is not accurate. I have not been game to take the tablet because everything seems to rev up the tremors. As I take methotrexate, I also have to take Folic acid 5mg three times a week. I know, update my signature! Sorry!

 

My rheumatologist knows I take Prozac, but these days I wonder if they keep up to date with interactions. I don’t feel confident with doctors now. She also knows about my withdrawal, and tremors, but I don’t think she believes in it. As for my GP, forget it!

Reduction of what drug? Prozac? How much are you taking now?

 

When you say "not a problem last year," what time period are you talking about?

If you take propanolol every day, it may have systemic effects and require tapering.

I am reducing Prozac. I have been taking 14.5 mg since my last reduction in Dec 2017.

 

I can’t take fish oil because my haemorrhoids bleed. I did try flaxseed oil, but it revved  up the tremors. I do take a magnesium supplement, but just realised it has VitB6 in it and that can be a problem. What is the best form of magnesium? How much do you’d take?

 

After being brought down from 40-10mg Prozac too fast in 2015, I have had the temors, but the anxiety was not really an unmanageable problem until 3 months after the last reduction in Dec2017.

 

What do you mean by systemic affects? Is it easier to reduce a beta blocker than a SSRI? Would reduction increase anxiety? Wouldn’t it be great if you could ask a doctor this!

I get mixed up between relapse and withdrawal. My anxiety and ocd has been ok until my last reduction late last year. Now it is really bad and I wondered which one it is. I know my basic problem has always been ocd, but I have managed it until now. I use breathing and relaxation techniques and am trying meditation, but they don’t seem to help. I just feel the waves of anxiety. I take magnesium too.

Is there anyone who had ocd before going on antidepressants and is now suffering with symptoms that are much worse than they ever had before. I think mine are made worse by massive anxiety. I have never had this amount of anxiety and sticky thoughts before. The thought of having to get over this again is very daunting. Does the ocd of withdrawal settle down to a normal level so I can deal with it again?

Hi Bunyapine, 

 

Just popping in to see how you’re doing. I’m sorry that your anxiety has been so high. Do you have some good distractions in place? I find distracting myself through the waves is the best thing to do. Do you like art? Or crafts? Crosswords? Going out in nature? Movies n TV shows? There are many things one can do to distract oneself.

 

I know when you have severe anxiety from withdrawals you may not find joy in doing anything n everything seems extremely overwhelming but I think it’s important to keep finding things to distract yourself.

@Songbird I swapped in Jan this year because the generic compounded liquid had larger bits floating in it and I was not happy with that. The pharmacist suggested pure Prozac. At the time I did’t realise that wasn’t a good idea.

 

i think I am in trouble here. My anxiety and ocd are much worse and the internal tremors which have not been related to anxiety in the past are now worse. I suspect the increased tremors are due to the excessive anxiety. 

 

I am seeing a psychologist and doing relaxation, breathing and meditation, but nothing makes a difference. In desperation I went to see my GP who is clueless. I asked her for a beta blocker hoping it might help the anxiety surges and tremors. I don’t think it is making much of a difference. I haven’t wanted to take anything, but I am desperate! I have a referral to a new psychiatrist, because I can’t go back to the last one,as he didn’t understand withdrawal and couldn’t understand why I had increasing tremors when he brought me off Prozac too fast. I felt he gave up on me. I am worried the new one will suggest increases or changes from Prozac to another drug or a benzo. I don’t want to make anything worse! What do I do!

 

Does anybody know of a doctor in Australia who understands SSRI withdrawal?

I was wondering would a benzo help this extreme agitation or make it worse. I am getting desperate.

What time of day do you take Prozac? What are your symptoms before and after you take it?

 

Do your symptoms follow any daily pattern?  Please keep daily notes on paper about your symptoms, when you take your drugs, and their dosages. Use a simple list format with time of day on the left and notation (symptom, drug and dosage) on the right.

I normally have low to mid level internal tremors everyday. This started after coming down too fast off original doseage in 2015. They improved towards the end of last year, but then I reduced the dose and changed from generic to Prozac powder. This year they are now made worse by increasing anxiety and ocd, in spite of relaxation and breathing exercises. As anxiety and ocd were not a real problem during the last 2 years, I could handle waiting out the symptoms. Now the anxiety goes through me in waves every so often during the day and I am having trouble coping with the ocd.

 

5am.     Wake up with minor tremors. Anxiety increases while I am lying in bed. 

6am.     Get up. Nervous diarrhoea. Anxiety.

7am.     Breakfast with magnesium and calcium supplements. Anxiety. Tremors increase after eating anything!

8am.      Prozac 14.5mg  Inderal 10mg  VitD 3. Anxiety.

9am-12md      Work outside. Woking physically outside seems to lessen anxiety.

1pm.       Lunch. Back inside I notice the physical aspects of anxiety again. More tremors after eating again.

1pm-5pm.        Anxiety goes through me I’m waves every so often and  ocd makes it worse.

5pm-7pm.          Outside again. Again I feel better outside.

8pm.         Night meal. Usual increase of tremors after eating. Anxiety is not so bad as long as ocd does not aggravate it.     

Magnesium and Inderal 10mg.

11pm.       Bed. Have no problem getting to sleep. I just wake up really early in the morning. 

 

Anxiety is increased by ocd every so often during the day and with this level of anxiety I find it hard to work with ocd. I have not had this level of anxiety with ocd since 1987. Every so often during the day the anxiety comes out of nowhere.

 

i don’t notice a difference after taking Prozac.

You might find wearing sleep mask helpful.  I wasn't using one during winter but started using it again when it started getting lighter in the morning.  I think I used mine all through last summer even when it was hot.

Please add all your drugs to your daily notes. While you're looking at drugs.com, strongly recommend you look at side effects for each of your drugs. Your symptoms may be from Humira or Methotrexate rather than Prozac or Inderal.

 

Are there any foods you eat frequently? You may have developed a food sensitivity. Do you drink caffeine drinks throughout the day?

1pm-5pm.        Anxiety goes through me I’m waves every so often and  ocd makes it worse.

5pm-7pm.          Outside again. Again I feel better outside.

8pm.         Night meal. Usual increase of tremors after eating. Anxiety is not so bad as long as ocd does not aggravate it.     

Magnesium and Inderal 10mg.

11pm.       Bed. Have no problem getting to sleep. I just wake up really early in the morning. 

 

Anxiety is increased by ocd every so often during the day and with this level of anxiety I find it hard to work with ocd. I have not had this level of anxiety with ocd since 1987. Every so often during the day the anxiety comes out of nowhere.

 

i don’t notice a difference after taking Prozac.

@Songbird I switched in January this year as the generic had large floating bits which would not dissolve. The pharmacist suggested it. Now I know it was the wrong thing to do. Would I have troubles switching back?

I'm not saying it was the wrong thing to do, it's more that some people are very sensitive to differences in formulations, so it could be a factor.  I wouldn't change anything just yet - Alto wants to look at your daily notes and drug interactinos.

If you get a liquid compounded using a tablet, would you have withdrawal problems at all from switching from tablet to liquid?

When changing the form of a drug we suggest doing a cross over which makes it gentler on your system.

 

You would do each of these combos for 3 to 7 days:  3/4 tablet + 1/4 liquid, 1/2 + 1/2, 1/4 tablet + 3/4 liquid

 

Also do not make at a reduction at the same time.

@Carmie My psychiatrist wants me to jump off Quetiapine 12.5mg. I have refused because of the Prozac withdrawal problems I had last year, when I ended up in hospital.I had extreme anxiety and OCD, which just got worse and worse! I have never had anxiety like that! Of course, the doctor is not convinced about withdrawal and it’s a battle every time I see her. She also wants me off one of the antidepressants at the same time. Again I said I was only doing one at a time. I had such a bad year with anxiety and OCD in 2018, I don’t want to get that bad again.

  

I have got a pharmacist who will compound a liquid Quetiapine for me. I am worried about the switch from tablet to liquid, as this contributed to my awful year in 2018. I was going along alright until then with mostly tremors and normal anxiety and OCD for 2 years. 

 

I would appreciate your advice on the switch and how much to taper by as I am very sensitive to adjustments. I have been on Quetiapine for 5 months.

Hi Bunyapine, 

 

Yes, changing formula can put you into waves, I found out about that the hard way last year when I knew nothing about crossovers. I changed from water titrating my original tablet to water titrating a compounded capsule, and ended up in a three month wave or so. I decided to go back to the the tablet, because it’s more accurate, and did a crossover this time. It went well. 

 

This is the formula for the crossover, as ChessieCat mentioned above. 

 

3/4 of the old formula.                    1/4 of the liquid. 

 

1/2 of the old formula.                    1/2 of the liquid. 

 

1/4 of the old formula.                     3/4 of the liquid. 

 

You do every stage for 3 days to a week, though I did each stage for more than a week to be safe. 

 

I just saw in your signature that you jumped from 125mg to 25mg in January, and then from 25mg to 12.5mg in February. Is that correct? They are huge drops if that’s the case. I wouldn’t be doing any tapering as yet until you are stable. I would wait a number of months before I changed anything. One should never taper by more than 10% a month, I’m tapering just over 3% at the moment. 

 

Whatever you do DO NOT jump off at 12.5mg like the doctor suggested, and DO NOT do two changes at the same time. 

 

If your doctor doesn’t cooperate with you just try another doctor. Any doctor can give you a script, it doesn’t have to be a psychiatrist.

 

Please be careful, and hold for a while longer. You’ve tapered too quickly, and your brain needs time to stabilise before you change anything else.

@Carmie Thanks Carmie. I might have done my signature incorrectly, as increases and reductions were done quickly in 25 or 50mgs instalments. I know not to do more than a 10% reduction, but at that time I had to do what the doctor said.  Now I don’t have to, as I am in a much better state. I was really bad before. She wants me off Quetiapine and one antidepressant, because of my age. I want to be off them too, but I couldn’t stand another year like last year. Of course she doesn’t agree with my slow tapers, as this probably means I won’t be off these drugs before I am a very old woman! I have had a bad time with withdrawal for 3 years, so a slow taper is important.

 

I am halving the tiny Quetiapine tablet now and can’t do much more. How do I do a cross taper? I’m afraid I don’t feel very brave about tapering after having such a bad time. I don’t want to be suffering for my remaining years seeing I am in my middle 60s.

Hi again Bunyapine, 

 

You’ve made too many changes, I wouldn’t do a crossover yet, your drops have been way too big. You don’t need to listen to the doctor, they make you taper too quickly and that’s why you’ve been having problems. All you need from the doctor is a prescription, and you can go to any doctor for that. If your doctor isn’t cooperative find one who is. 

 

Please don’t make any more changes for a while. Stay on the 12.5mg for now and let your brain stabilise a bit. And please don’t jump off the Seroquel, as the doctor said, or you may end up suffering for quite a while. It’s too high a dose to jump off. I won’t be jumping off it until I’m down to 0.0something. Please stay safe.
