Aside from the skipped dose of apriprazole, which might account for the dizziness, etc. of the last couple of days, it looks like your last drug change was October 24, when you updosed clonazepam to 0.5mg, is that correct?

 

Your symptom pattern looks stable.

 

Are you inclined to taper apriprazole in a couple of weeks? See Tips for tapering off aripiprazole (Abilify)

 

Reducing apiprazole probably would eliminate your need for metformin.

Yes, that is correct.  October 24, I went up from .47 to .5 mg Clonazepam.

Yes, I am inclined to make my start just after Thanksgiving.  I've checked the Tips for tapering off Abilify page several times now, as well as the page on Pharmaceutical liquids to make suspensions.  I'd really value your feedback on what I am considering given the research I have done:

 

1.  To use the Ora Plus suspension vehicle to prepare a liquid preparation that I will cross over to, computing in a 10% compounding loss, as recommended on the Ora Plus info sheet.

 

2.  Begin making the switch from tablet to liquid (without dose reduction) on November 23.

 

      a.  Because the tiny 2 mg Aripiprazole tablets do not cut easily more than once, I am thinking to take 50% (1 mg) in tablet form and (50%) 1.1 ml liquid (at a 1 mg/ml concentration).  The added .1 gives me the 10% compounding loss on the 1 mg/ml liquid.  Take this for two or four weeks???

 

     b.  Then, switch to 2.2 ml liquid (at a 1 mg/ml concentration, the added .2 figured in for the 10% compounding loss).  Take this for two to four weeks???

 

I made all the question marks to indicate that I am not sure how long of a crossover time from tablet to liquid I should allow before beginning actual titration.  Is 2 weeks enough for each 50% portion of transition?  Or, should I allow 4 weeks for each transition?  Shep indicated 25% graduations from tablet to liquid is recommended, but these tiny tablets crumble terribly beyond the first cut and I just don't think I can achieve that accuracy.

 

     c.  Because I will be changing jobs in the next 3-5 weeks, the crossover period should give me time to grow some roots in my new role before beginning to titrate down.  I can be flexible on when to actually begin the titration, but am feeling all will be well for me to make the crossover from tablet to liquid beginning November 23.

That looks fine. We usually don't do the 10% compensation.

 

Strongly recommend starting crossover on a weekend or holiday.

Thank you, Altostrata.  Then, I shall start on Saturday morning, the 24th.  As I am not looking to make my journey longer, I will not make the 10% compensation.  I confess, I was not liking that idea.

3:45 AM - Wake after ~ 8 hours of sleep

4:00 AM - Begin sipping coffee (total of 2 cups by 5:30 AM).  And, shoot, it is now 3:00 AM again, due end of daylight savings

3:30 AM - Begin working on house refinishing tasks

4:50 Am - Bowel movement

5:30 AM - Breakfast + Supplements + 2mg Aripiprazole + continue working on house

8:00 AM - Feeling a bit tired so take meditative rest for 1.5 hours

10:30 AM - Lunch, then out shopping and return home where I putter around with odd tasks

1:00 PM - Light snack

5:00 PM - Dinner + 500 mg Metformin ER + Calcium + Magnesium + Probiotic

6:30 PM - Yogic stretching

7:00 PM - .5 mg Clonazepam and 15 mg Mirtazapine

7:30 PM - Bed

 

No anxiety, irritability or sensitivity.  And no other symptoms.  It was a routine day.  

 

I got less sleep last night,  however, because my mind/body still thinks were are on daylight savings.  I remembered a dream last night and so have (as always) recorded it.  I count this to be positive.  My dreams have been a very rich and integral part of my healing over the years and it seems that in the past several years, my ability to recall them has not been what it used to be.  That, or I just no longer have as much arising as in times past.  In any event, I am hoping my dream recall might increase once again after I have reduced the Aripiprazole and, later, the Mirtazapine.

I look forward to the day I am off the Aripiprazole and will appreciate the return of the faculties I believe it has taken from me.  The sooner I can start the better for me, so I really hope my new job transition goes smoothly.  I am trusting it will.  I am also hoping I might find myself able to taper faster, using the approach you delineate on the Why taper by 10% of my dosage page.

  • Initially, make a 10% reduction and hold there for a MONTH. It can take several weeks for withdrawal symptoms to emerge. Do that again the second month.

  • "Jump off" at the end when you are taking less than 98% of the original dose and reductions no longer cause any reaction at all.

Under this method, the fastest taper takes about 12 months.

 

If I don't know until I try, it is worth the shot to me.  I would, of course, post good daily notes and take my cues from yours and @Shep's watchful eyes, as well as my own good sense.  I'd sure appreciate garnering the support to make the attempt at it.  If it should not work, I can adjust fire, completely then knowing and accepting that I would need a longer course of tapering.

 

Edited to add:  "If for any reason it is believed it is not wise to attempt a fast taper on the Aripiprazole, I am also fine with that."

 

Hopefully, I will find a get a full night's sleep tonight!

  • Given clonazepam (Rivotril) for Rapid Eye Movement (REM) Sleep Behavior Disorder (RBD). Started with 0.5mg/night as from Nov 2013.
  • First taper initiated Nov 2016 after heart troubles led to ER. Taper dose: 1 drop (0.1 mg)/month. 10 days after last drop, reinstated to 0.5 mg following nocturnal creeping sensations in leg.
  • Second tentative in May 2017. Quickly went down to 0.2 mg in few days with no symptoms. Last 0.1 mg tapered via liquid taper (1+9) and reduction of 1/10 every average 2 weeks. Last dose taken Aug 12, 2017.
  • After 7 months honeymoon, huge wave following massive use of Melatonin (5 mg) asked by sleep doctor.
  • Recovered 65-70% after 14 months, still with lingering eyes floaters.
  • Completely functional in the daily life.

2:30 AM - Wake after ~7 hours of sleep.  My mind/body are still on daylight savings.  I remained resting in bed until 3:15.

3:30 AM - Begin sipping coffee (total of 2 cups by 4:30 AM)

5:00 AM - Bowel movement

6:00 AM - Arrive at work

6:30 AM - Breakfast + Supplements + 2 mg Aripiprazole

11:00 AM - Lunch

2:30 PM - Leave work

5:00 PM - Dinner + 500 mg Metformin ER + Calcium + Magnesium + Probiotic

6:45 PM - .5 mg Clonazepam + 15 mg Mirtazapine

6:45 PM - 30 minutes gentle yogic stretching

7:30 PM - Bed

 

No medication related symptoms.  It was a good day in every way, very positive and upbeat.  Several coworkers affectionately commented on how much I will be missed when depart my present work location.  I will also miss them and the heart warming interactions we have shared for the last few years. 

 

I remain focused in my heart center.  It is a guiding light for me.

 

I was thinking that when I am asymptomatic, as now, I might not need to trouble my readers with seeing my daily habits/routine.  Perhaps I could just make a more condensed daily update?  Please let me know what is preferred...

I am not sure my topic is the best place for Jim's question, but thought to comment with the following:

 

I have researched the internet regarding "what" a compounding loss is.  I believe I asked this question earlier in my topic.  As far as I can tell, a compounding loss does indeed refer to a loss of the Active Ingredient.  I think potential losses of the active ingredient are much more easily mitigated in one's personal kitchen, following careful and clear steps, as opposed to some lab where a multitude of compounded products are being measured and made in a more rapid order.  There is a far greater propensity for error in the latter.  Also, using a smooth glass mortar, as opposed to coarse bottomed porcelain mortar (which will cause the powders to stick) will result in far less loss of the active ingredient.  I really believe that with very careful preparations, I will lose next to none of the active ingredient.  This is why I personally was not liking the idea of up-dosing by 10%.  I am trying to reduce and eliminate the medication, not take more of it.

Yes, you could do that. You may want to keep a more comprehensive journal offline, if you find that helpful.

 

We only need your drug and symptoms journal if you're asking for tapering advice. But if you are going into a long hold, it's not necessary. When you wish to ask for tapering advice, please post a few days of your journal so the moderators can catch up to where you're at.

Jim, I am truly very sorry if my remark came across as unwelcoming.  If it did seem that way to you, please know that was not my intention.  I have been following your conversations over in the Tapering forum and so to suddenly see your question here was curious to me...  That's all.  I probably should have refrained from even commenting this morning as I was short on time before work and I have a tendency to be short and direct when pressed for time.    Perhaps I should not have commented at all, as your question was not even directed at me.  On another note, I'm still learning my way around the forum, too.  Here, I have been boring folks with my daily routine for the past many days when I apparently did not need to be doing that, because I am not actively tapering.  But I am trying to gather information needed for my taper(s) and attempting to demonstrate my stability and sincerity so that folks will have enough knowledge and information about me to be willing to assist me with informed confidence when the time comes.

...

On a final, more general note, as it will be some few to several (more likely several) weeks before I am ready to begin tapering, I am probably going to go mostly quiet on the forum, for now.  I do plan to begin my Aripiprazole crossover to liquid on either the 21st (being I am off both the 21st and 22nd) or on Saturday, the 24th of November.  I'll be sure to reach out if I encounter any troubling symptoms.

I have been feeling under the weather the past few days,  fighting a cold virus 🦠, and cold sore, which strangely turned the corner of my lip numb.  With the hibernation I tend to take to when feeling ill (for healing’s sake), my system is recovering. 

 

I decided I will probably make a post at least once a week, as a way of staying in touch with the forum,  when in a holding pattern, as I am now.  

 

All is well here, symptom-wise.  I feel to have recovered very well from the clonazepam dose change(s)...and am NOT using any alprazolam.

 

The energy in my heart center remains clear and strong.  It became apparent to me in meditation that this was the energy of Tiger 🐅 ... who first came to me in a dream in 2003, presenting her power and essence in proportions larger than life (she was bigger than a house).  Then, as now, I was needing to throw off things that were threatening the sanctity of my being and healing journey.  I woke from that dream wearing the energy of her essence emblazoned in my chest and, because of her, had the courage to make many needed, radical changes in my life. 


Presently, I have undertaken the will to wean myself off my medications, am about to make an important job change, and, in my analytical process, my psyche is continuing to work on withdrawing some very significant projections.

So, it feels quite apropos for me to have changed my Avatar to tiger, because her blessings are pertinently present at this time in my life.

I'm glad you found this site early in your taper, that bodes really well for you successfully coming off the meds long term. Abilify was also one of the ones chucked at me in hospital, although I ditched it after about a year.

 

You seem to have all the right resources in place for a successful taper, not to mention a shed load of transformative life experience behind you. I'll be keeping an eye on your thread as you progress with the reductions, wishing you luck and healing x

2008-2013 - Various meds on and off since age 18 incl. Sertraline, Prozac, Mirtazipine, Abilify. Prescribed for severe OCD.

CT'd several times over these years and reinstated after subsequent psych hospitalisations.

2014-2015 - Clomipramine, quetiapine and Epilum

2015-Jan 2017 - Prozac 40mg (stopped contraceptive pill, most stable period of time)

(Beginning of taper) Jan-October 2017 - Tapered Prozac to zero.

15 Jan 2018 - Reinstated Prozac at 2mg due to acute w/d symptoms

February 2018 - tapered to 1.8mg

May 2018 - reinstated at 5mg due to severe w/d symptoms. 9 month hold, stabilized well at around 6/7 months.

March 2019 - Tapered to 4.9mg

Current supplements: fish oils, probiotic, ashwaganda, colostrum powder, cannabis

Thank you, Abby!  Yes, I am new to the forum and just beginning the tapering of my meds.  I am also glad I was led to this site.  I started over on BenzoBuddies, but when asking about my non-benzo medications, one of the moderators there mentioned Surviving Antidepressants to me, saying another person found help here.  I am grateful that she thought to tell me about this site.

Greetings, everyone...

 

Everything is going very fine in my personal and professional life.  I still wait for the "official" job offer for my pending work change.  This will arrive in a formal letter sent via email, hopefully next week.  My analysis work goes well.  I remain very focused in my heart center, very connected to my inner self/Self.  Some daily short-lived periods of what is similar to hot flashes continues.  This has puzzled me a bit, because the menopausal flashes in my life did seem to have subsided.  I am not sure if some hormonal activity could have amped back up with the 50% clonazepam reduction...  Maybe.  I am inclined to think it might have, as I have also observed that I have been more "wet" than what had become normal for me.  Aside from that, the only other thing that would account for the heat is spiritual energy.  As I do feel like my Self has me in a crucible again, this is also a possibility.

 

My next step is to begin crossing over from tablet to liquid on my 2mg of Aripiprazole.

 

If I have the advice and guidance provided here correct, I should not make another change sooner than 4 weeks.  It is also recommended I begin my crossover from tablet to liquid (on the Aripiprazole) on a weekend or holiday.  

 

I've looked back at my journal and find that the the 25th of October was my last date of any irritability/anxiety, post my clonazepam change(s).

 

The completion of 28 symptom free days will occur with the close of Thanksgiving Day.  Being an Eager Beaver, I was inclined to begin crossing over on the 20th, as I will be off on both the 20th and 21st (Turkey Day).  But, I will exercise patience and begin the crossover on Saturday morning instead, the 24th of November.

 

Here is my recipe for the liquid solution:

 

10 - 2 mg pills, crushed

10 ml Ora Plus (Note:  I have 10 ml syringes)

10 ml water

 

That will give me a 1mg/ml concentration, if I have said that correctly.  I will shake it well (initially and each time I use it) and store it in an amber jar in the refrigerator - though I am not sure refrigeration is needed.

 

My initial dosing will be:

 

1/2 of a 2 mg Aripiprazole tablet

1 ml of the above solution (Note:  I have 1 ml syringes)

 

Reminder:  Instead of doing 25% graduations from tablet to liquid, I am doing two 50% graduations because the tiny Aripiprazole tabs cannot be reliably cut more than once.

 

Question:  How long should I take the 50% liquid and 50% tablet before I jump to 100% liquid?

 

I had another question, but I stopped writing to take a call, and now it has escaped me.  Bugger!

 

Please let me know if there is anything concerning in the above.

Hi, Cleerity. Let's observe how you do with the half and half combination for 10 days. If all is well, you'd do the rest of the switch.

 

I'm wary of keeping the homemade liquid for 10 days. I'd only keep it for 5 days. I would definitely keep it in the refrigerator, and in a dark bottle to protect it from light.

Okay...  I will only make a five day supply at a time.  I do have some squat, wide mouthed, dark amber glass jars (4 oz)  that I found on Amazon.  

 

So, my updated recipe is:  Five 2 mg tablets + 5 ml Ora Plus + 5 ml water, for a 1 mg/ml  concentration.

I will double up, using  a brown paper bag to contain the amber jar, so the solution will be safe from the refrigerator light. Maybe an overkill, but an easy thing to do for safety’s sake.

Hi Cleerity,

 

After this experience, besides a complete healing, another thing is certain. You will become an expert in using syringe!  I imagine myself in your shoes when I have to draw up 1.03 ml, 1.02 ml, 1.01 ml...0.14 ml, 0.13 ml...😅

  • Given clonazepam (Rivotril) for Rapid Eye Movement (REM) Sleep Behavior Disorder (RBD). Started with 0.5mg/night as from Nov 2013.
  • First taper initiated Nov 2016 after heart troubles led to ER. Taper dose: 1 drop (0.1 mg)/month. 10 days after last drop, reinstated to 0.5 mg following nocturnal creeping sensations in leg.
  • Second tentative in May 2017. Quickly went down to 0.2 mg in few days with no symptoms. Last 0.1 mg tapered via liquid taper (1+9) and reduction of 1/10 every average 2 weeks. Last dose taken Aug 12, 2017.
  • After 7 months honeymoon, huge wave following massive use of Melatonin (5 mg) asked by sleep doctor.
  • Recovered 65-70% after 14 months, still with lingering eyes floaters.
  • Completely functional in the daily life.

Actually, I had the updated recipe wrong.  Being I am initially only using 1 ml of the solution each day, I need to make a smaller batch because even the above would give me 10 days worth of solution.

 

So, the recipe will be:  Three 2 mg tabs + 3 ml Ora Plus + 3 ml of water.  That will give me 6 ml of solution at a 1 mg/ml concentration.  I will take 1 ml (along with 1/2 of a 2 mg tab) for 5 days - and discard the remaining 1 ml of solution.  Then, repeat.

Thank you, Jim!  LOL, your message made me reach for my 1 ml syringe.  Reading it will be a bit of a challenge for my presbyopic eyes.  Yes, as it is going to take me some time to accomplish this, I will get lots of practice!  I will go slowly and double check my self and ask questions if needed, so as to not make errors.

Hello:  I just finished making my first little batch of liquid Aripiprazole to begin my crossover to liquid tomorrow:  Three 2 mg pills + 3 ml Ora Plus + 3 ml distilled water.  I placed it in a dark amber jar, which I wrapped in blue painters tape.  The tape (my overkill effort to protect it from refrigerator light) fit the jar perfectly and will stand up to some washings.  Having crushed the three tiny pills, I was sure they were all "powder" and so I expected not to see any particles once mixed in the liquid.  However, I did see some very small particles, which I attempted to reduce using the end of a plastic handled artist's paint brush.  I was able to further reduce those I could land the brush handle on.  Perhaps I did not need to worry about the particles.  Were they just fillers?  As I will be making my own liquid for quite some time, I went ahead and purchased some micro spatulas/spoons, to help me further reduce any bits I find as I go. One thing that dawned on me as I was mixing the liquid preparation is that I will not be able to dose at what was my normal time of 0630 hours, because I cannot take the little bit of liquid to work with me.  In changing/adjusting my dosing time, I think the best thing for me to do is to take the Aripiprazole at 0500 hours every day.  I am thinking this may not be of great consequence, but wanted to mention it in case it does have any bearing?  Oh, because it is such a small amount of liquid, I will not be shaking it, as I am too concerned it will stick to the sides of the jar.  But I do understand that I need to stir it before dosing each morning.

Given the long half life of Aripiprazole, I think it is probably way too soon to have noticed a difference with my beginning the 50% liquid + 50% tablet yesterday, but last night was different enough from my norm that I want to record it.  I went to bed around 7:30 PM last night, but woke at 8:15 PM with a profound dream memory.  Then, back to sleep in short order.  I woke again at 2:30 AM (my body is STILL on daylight savings, most days ... ugh ... 3:30 AM is my normal weekday wake time) and again returned to sleep in short order.  Next, I woke again at 4:30 with a dream memory and again returned to sleep in short order.  Finally, I woke again at 5:30 with another dream memory and remained awake.  While the morning sleep/wake pattern is normal for my weekends, when I try to remain in bed to get a couple extra hours of sleep, the amount of dream recall is not what has become ordinary for me.  I really like to have a close relationship with my unconscious and, in years past, I had myself well enough trained to wake up up to 3-5 times a night to record my dreams ... but this changed when I began taking the Aripiprazole.  Will have to wait and see how things do/don't develop.

Hi RubyJ...and "thank you."  I was just wondering earlier today how you are doing with your taper?  I have not started to taper yet, just now crossing over from tablet to liquid.  But the crossover is a change and because my psyche "may" be responding to that change, whether anticipatorily, substantially - or not at all - I wanted to record (for my own sake) what may (or may not be) a correlated manifestation.  Cleerity 

4/2001 - Clonazepam, .5mg (at bed); 5/2010: 1 mg; 9/2018: .5 mg; 10/20/2018: .47 mg; 10/24/2018: back up to .5 mg.  Began daily micro taper by liquid prep on 3/12/2021 (avg. 10% redux of last dose every 28 days).  At .17 mg/ml as of 12/24/2021.

4/2002 - Alprazolam, .25 mg (PRN), up to 2x/day.  DISCONTINUED 10/21/2018
5/2010 - Mirtazapine - 15 mg (at bed)
3/2012 - Aripiprazole - 2 mg (in A.M.) - Began reducing Dec. 30, 2018.  Daily micro-taper by liquid preparation.  DISCONTINUED 1/14/2021.

6/2012 - 500 mg  Metformin ER, 2 tabs, 2x/day.  DISCONTINUED April 2020.

Ah, it's true that the crossover isn't the taper itself but it's a step in that direction!  I understand wanted to record anything different.  It especially helps when looking back later, I find.  I'm nearing the end of the third week and I'm doing well.  I think I'll be fine to taper again at the four week mark-ish.  The symptoms lasted about a week, although they were worse at the beginning instead of the middle, which was different.  I need to update my post but have been quite busy.

Dextroamphetamine 30 mg, started 2011 -- still taking

Paxil 2004-2005

Wellbutrin/Bupropion 300 mg, started 2005 -- still taking

Prozac/Fluoxetine 40 mg, started 2006 -- still taking

Abilify/Aripiprazole 5 mg, started 2014 -- started taper in 2017, finished 2020

This morning at 0500, I took my 6th dose of 1/2 a 2 mg tab and 1 mg/ml liquid Aripirprazole.

I am definitely feeling different since beginning the crossover to liquid.  

 

For one, my dream recall is pronounced, nearer to what was normal to me before I began taking the Abilify.  If this holds out to be true, I do not consider this a symptom, but a benefit.  What else I am experiencing is that my consciousness feels different.  Yesterday morning (so, Day 5), at about 0800, I had an unmistakable sensation that was very similar to something I experienced as teenager, as though my consciousness was teetering on the brink of another realm.  I had these experiences of being between realms both during an NDE and also when coming to conscious awareness when re-entering my body after astral traveling.  I did not then know "that" is what I was doing ... it was only in later years that I learned/understood that it was how my Self protected me from the abuse of my childhood environment.  I hope this does not sound woo-woo to those reading and that some here may understand what I describe.  When I had the sensation yesterday, I was sitting at my desk at work, but feeling I was in some liminal space.  This is not normal for me.  Having become aware of it, I worried some over it and that depersonalization may set in.  It was somewhat like the world felt strange to me...or I felt strange to myself.  So, I just took notice and carried on with work tasks, but it did not and has not entirely left me, though it seems not as acute right now.  There was/is also a weird feeling of depth, as though I was/am in touch with some deep aspect of myself that I was not accustomed to.  While I generally have very good eye contact with others, I found that my gaze was ever more steady and unflinching.  Oh, I also feel more aware of my subtle body.  I am not yet sure what is going on, but thought I would at least share this.  I think it is going to be important for me to revisit what I was experiencing when I began taking the aripiprazole... 

 

Question:  When folks do crossover to liquid from tablet, what is the most experienced effect?  Does it more often seem the medication is too strong?  Or, diminished?

I forgot to say that my concentration and focus has not been as good these past two days and I have also been more tired than normal the past two days.
