Good evening all! I know everyone is crazy busy. I just wondered if any of the mods have any suggestions regarding my post 3 days ago.  Instead of improving/moving toward stabilization, I've been battling increased dizziness and visual disturbances over the past few days. Should I continue to hold through this wave (if that's what it is)? If I'm never going to stabilize, should I just get on with a taper? Or do I actually need to updose (I hate to even think about that, but...). Please, any opinions are welcome. I've been holding at 25mg for 9 weeks with only slight improvement. Can anyone provide some perspective? I can't think straight. Thank you so much!

I will summarize to keep people from having to go back in my thread: originally put on 50mg sertraline to "help" me taper klonopin (I was never depressed; put on benzos after one panic attack due to situational issues). 2 months after successful klonopin taper was completed, I did a fast taper off sertraline, since doc said it was gentle. A few w/d symptoms but nothing major. One month later...wham! One minute anxiety was not even a thought in my head; the next minute it's a 10 out of 10! After a 1 week Lexapro experiment (suggestion of a worthless pdoc), I reinstated 25mg sertraline, and of course, in addition to the anxiety, the weakness and tremor began again almost immediately. Only then did I find SA, and learned that I most probably reinstated too high of a dose. But there I was.

 

I've been terribly unstable in the past 2 months, not even remotely functional at first, but very slowly the anxiety, depression, insomnia and inner tremor are improving. People have been telling me I am noticeably better. Today, however, anxiety was much worse than usual. For the past 3 weeks or so, I would rate my anxiety as 3 or 4 out of 10. Today, its been more in the 7 to 8 range. 

 

So here are my questions: since I was on 50mg sertraline for 18months, then stopped and reinstated at 25mg a month later, do my symptoms represent withdrawal from a 50% reduction? Or side effects from sertraline (which I've historically always had)? Or strictly signs of an overly sensitized CNS? This makes a difference in whether I should continue to hold for a while longer, or try a 5% cut.  I was going to cut tonight's dose, but chickened out since anxiety was so high today. I hate to admit it, but I just dosed 45 mins ago and the anxiety seems to have lifted somewhat. Then again, it always gets better at night. 

 

I am so very confused as to how to proceed. I know no one can say with 100% certainty what's going on in my brain, but I really need some advice. If it were you, what would you do? Thank you for reading this long post. I really do appreciate everyone's time. I know there are many other things you could be doing, but choose to help strangers instead.

Are you still taking the probiotic? Stop taking it.

 

I can' take probiotics, I developed a sensitivity to them.

 

Are you still taking Zoloft liquid? Has it expired?

@Altostrata, thank you so much for getting back to me! Ok, no more probiotic. I make my own liquid each day, so no worries about expiration. 

 

I started keeping daily notes on Dec 3 2018. I averaged my symptoms from the first 3 days (Dec 3, 4 and 5, 2018) and the last 3 days (Jan 9, 10 and 11, 2019) and found that: 

 

anxiety has decreased from 7/10 to 4/10

 

depression has decreased slightly from 6.3/10 to 6/10 (my depression has a strong situational component to it; I am in grief counseling currently)

 

Weakness/tremor has decreased from 7/10 to 5.4 (this is a side effect from sertraline and will not entirely go away until I am off)

 

Visual disturbances have increased from 5 to 6

 

Internal tremor has decreased from 5.6 to 4.2

 

Muscle soreness/twitches have increased from 3 to 4.6

 

Dizziness has increased from 3.2 to 5.6

 

Nausea has decreased from 2.3 to 0

 

Palps have decreased from 1.5 to 0

 

I will post daily symptoms in a few minutes.

So your averaging shows you've improved since early December? Does this seem intuitively correct?

 

I can't tell -- are you better or worse since you've been taking the probiotic? It looked before like you were saying you were worse.

 

Do you feel better or worse after you take Zoloft?  
 

Magnesium is more effective in smaller doses throughout the day. When you take too much at once, it can cause loose stools.

Suggest you purchase the most common ingredients, such as theanine, separately, so you can find which ingredient is most effective for you. @Altostrata, the probiotic doesn't seem to be causing me any problems. I have read some of your threads about magnesium and for the past couple of days I've been cutting the 400 mg pill in half and taking it 2x a day (just realized I forgot the 2nd half yesterday).

 

As to whether I'm better or worse after dosing zoloft, I not 100% positive, but I think there is little to no difference. This is one reason I'm so uncertain of what to do at this point. My symptoms wax and wane so often during a day that I believe any worsening or improvement post-dosage is coincidental. I always feel better at night and worse in the morning, but I see that's common for many on here. All I know for certain is that two of my most debilitating symptoms - weakness and tremor - go completely away when I am off. 

 

A prozac bridge is probably not an option for me. My DNA test shows me to be a slow metabolizer of all SSRIs. It also shows that Buspar and Trazodone would work well for me, but I seriously doubt either would cover zoloft w/d. I spent 15 months tapering klonopin in 2016 so benzos are not an option either. I feel well and truly stuck.

 

Alto, I really hope I'm not being a nuisance. I'm just very curious whether you think my symptoms are withdrawal from reinstating too little (at 25mg - half my highest dose), or CNS hypersensitivity/side effects from reinstating too much. I am not yet stable, as evidenced by the fact that some symptoms are worsening (dizziness and visual disturbance) and new ones emerging (burning mouth), and some on this site have advised me to never cut unless I'm stable. However, given that I have so many adverse side effects to being on the drug at all, and that I might have reinstated at too high a dose, is it possible that I might be the exception to that rule? Like most of us, I have no knowledgeable or understanding doctor to whom I can go for guidance - just a clueless but well-intentioned GP who's willing to write scripts. Although I understand you cannot give medical advice, you are far and away the most knowledgeable person on this topic and I would welcome your assessment of my situation. I am so confused, and not knowing what to do makes me feel so hopeless. Please help me find a way out. Thanks so very much!

@Altostrata, yes, my crude averaging shows improvement in several symptoms but an increase in three - dizziness, muscle soreness and twitching, and visual disturbances (hard to explain - things just don't "look right").

@Altostrata, my notes from today are below. All symptoms could either be side effects of a too-high reinstatement, or withdrawal from being on only half of what used to be my normal dose...I just don't know. Do you see anything definitive in my notes that indicate one or the other? My gut instinct is to try a 5% cut and see what happens, but I'm apprehensive since my dizziness has been worse over the past 3 days and the burning tongue is new. Even though both are listed as possible sertraline side effects, they are listed as possible withdrawal symptoms as well. I'm probably overthinking this, but I'm at a loss and unable to make a decision, which is really out of character for me. I am really interested in your and/or others' opinions...please? Thanks - looking forward to your thoughts.

Are you light-sensitive? If so, that may be enhancing that increase in symptoms when you go to work. You may wish to wear dark glasses for a while.

 

What time do you go to bed? When do you stop using the computer?

 

What happens between 10 p.m., when you take Zoloft, and 12 midnight? Do you get sleepy?

@Altostrata, thank you for replying. I don't seem to have any light sensitivity at all. In fact, sun and even artificial light help my mood immensely. So no worries there.

 

I've been staying up too late for the past few months, simply because I feel better at night, but my sleep habits have been the same for the past few months, way before the dizziness started. No computer after 10pm. 

 

The zoloft does not make me drowsy, nor does it seem to be activating. This is soooo confusing to me...side effects or withdrawal? Cut, hold or - God forbid! - updose? 

 

Do you believe its possible that, due to my slow metabolizer status, the sertraline has now, after 9 weeks back on, started to stack up in my blood plasma, causing these new side effects? I'm trying to recall if dizziness was an issue when I was first put on zoloft in 2016. I can't remember being dizzy, but I was also still taking klonopin, which I'm sure covered a lot of symptoms. I did have a bit of transitory dizziness during my 3-4 week zoloft taper last August, but it was nothing this severe.

Also, to clarify, today was the first day I've gone to work outside my home since my crash in October. I white-knuckled it the whole time.

@Altostrata, do you believe I should go ahead and make a small cut tonight? I have had some days better than others, but no real windows to speak of since my reinstatment. Today has not been great so far with lots of muscle twitching and constant dizziness. Have you ever heard of someone actually getting symptom relief from making a cut?

I don't think your "slow metabolizer" status has anything to do with how you're reacting to this small amount of Zoloft. It would be germane if you were taking more than one drug.

 

Since Dec. 11, when you switched to all-liquid, has your daily symptom pattern gotten better or worse?

 

I think working makes you anxious. You need some stress-reduction techniques not to get triggered by it. See

@Altostrata, thank you so much for your response. Since my switch to liquid, my symptoms seemed to be getting progressively, albeit slightly, better until the dizziness and visual disturbances intensified approximately 3 days ago. 

 

I realize my dose is small; that's why I believe its possible my symptoms are largely side effects, rather than all withdrawal.

 

What do you think? Would you hold or make a small reduction if you were me?

No, I make my own liquid from a 25mg pill fresh each day. Liquid zoloft is not covered by my insurance and I can't afford it. I had absolutely no issues switching from pills to liquid. I did a cross-switch over 3 days. The pills dissolve very well and I always rinse the bottle and drink the rinse after dosing.

I suggest that you add in other things.  Not just things to do when you are struggling.  It might not feel that they are helping but if you can take time throughout the day to give your brain some rest it can help and it also becomes a habit.  Set an alarm for several times throughout the day and have some down/quiet time, eg go and sit outside and look at the sky/clouds, listen to the noises around you, check out the leaves/flowers, or if inside look out of the window, or flick through a picture book or some nature videos/YouTube or listen to some nice music.  Like anything the more you practice the more natural it becomes.  It doesn't have to be for long but it can be something you look forward to.

Thanks @ChessieCat, I really appreciate all the great info. I'm working on trying to calm myself naturally. I've always been anxious - my therapist asked me last week to recall my first anxiety attack, and I shocked myself to realize my memory of having anxiety goes all the way back to 4 years old. I've been coping on my own even during a 15 year abusive marriage to an alcoholic narcissist until all these things happened in the span of 4 years: an extremely nasty divorce, my mom became catastrophically ill for 5 solid months (one life-threatening post-surgical complication after another), a very good friend died, another battled breast cancer successfully, my sister died, I faced the nasty surprise of benzo withdrawal, my mother died, my two surviving siblings had emergency open heart surgery one week apart, my godfather having recently been given 6-8 months to live, all the way up until now, with another nasty surprise of AD reinstatement and withdrawal. Now, I'm afraid I've just fallen down under the weight of it all. I think this is what they used to call a "nervous breakdown." I just can't believe I battled through klonopin tapering and full recovery only to have to turn around and do it all over again with zoloft. Sometimes I fear I'm no longer strong enough to go through it again. My worry is that I won't be lucky enough to recover a second time. Intellectually, I realize this is not rational, but emotionally, that's how it feels when the symptoms ramp up.

 

If I could just stabilize, tapering will be very doable for me. But after 9 weeks, still not stable, and feeling like I'll never get there. Seems like I'm stuck in the worst of both worlds - side effects of being on the drug and withdrawal symptoms of coming off. 

 

Sorry - just needed to vent to the only people who truly understand what this is about.

Dejavu,

I hear what you are saying.  You've been through h*ll. And I know 9 weeks is a long time to wait to stabilize, but it is quite common.  Sometimes it can take a lot longer to stabilize. Just continue to hang in there. Continue to hold as suggested by Altrostrata.  There is always that desire to change the medication dosage because we think if we can find just the right dosage we will improve. But the changing of the dosage is at the times the very thing that will set off a new wave of withdrawal symptoms. You might indeed be stuck in the worst of both worlds, but you will make it.  And yes, you have every right to vent, you have been through so much. I feel so bad that after everything that has happened to you that you have to go through this now. It really stinks.  Hang in there my friend. Better days are coming for you.

@RachelSusan, thank you for stopping by! I really needed to hear this, especially today. I know deep down you are right; its just sometimes hard to imagine - or even remember - ever feeling better at this point.

 

Here's something kind of amazing: I've been having really serious dizziness and visual distortion, worsening over the past 4 days to the point that it was severe earlier today. Just got back from my accupuncture appointment, where my ND added a couple of needles for vestibular issues. Immediately afterward, I felt no better. But a half hour later...wow! Dizziness and "funny" vision has decreased by at least 80%! Don't know how long this will last, but I'll take it. Accupuncture might well be a trigger for some folks, but it sure has been working well for me. I'm only doing treatment for relaxation, no detox or stimulation.

Oh my, that's wonderful about the acupuncture. If you can get any relief that's good but 80% is fantastic.  I hope it lasts a while.

I see your trying to stabilise after reinstatement but no joy as yet.

 

I hope you get some relief soon!

 

I’m lucky in that I have had windows of feeling really good and then periods of feeling ‘better’ but still with symptoms amongst the bad periods.

 

I know what you mean about symptoms going and being replaced by others...so frustrating.

Yes, I think I reinstated on too high a dose. Didn't know better and followed the bad advice of a pdoc who didn't believe I could be in withdrawal one month after a too fast taper. I guess it's going to take me a minute for my CNS to settle. Some days are better than others, but I wouldn't call them "windows" yet. Ugh!!!

Thanks....it’s terrible how uninformed doctors are. I was told it was anxiety coming back creating my symptoms (never knew anxiety caused aching bones, flu like symptoms where you can’t get out of bed etc!) and needed to up dose back to a therapeutic level! Just had absolutely no idea about wd. It’s actually embarrassing that they are so incompetent about it all.

 

Im sure in time you will feel better but it seems to be a long process. Never give up. It’s only going to be a small period in our life’s when it’s all done.

My goodness, you have been through a lot.  No wonder you feel like you've "fallen down under the weight of it all".  I think you should give yourself a pat on the back for coping as well as you have. 

 

Please try not to obsess about your reinstatement dose.  I think you did the right thing by reinstating, and it's possible you may have had some side effects even at a lower dose.  It seems to me that you have had some significant improvements since you reinstated.  Nine weeks might sound (and feel) like a very long time, but in terms of stabilising it isn't all that long.  It's possible that you may get more improvement in time, although bear in mind that stabilising doesn't always mean a complete absence of symptoms.  As others have mentioned, there may not be a "perfect" dose where it just all comes right.  There may be other ways to help alleviate some symptoms, and it sounds like acupuncture has already helped with some of your symptoms, which is great.  I would keep up the symptom diary with ratings out of 10 for each symptom, as I think that helps to see symptom patterns more clearly.

Hi @Songbird, thanks for checking in on me. I hope you have been doing well.

 

I know my symptoms won't totally vanish upon stabilization, since I have so many side effects. For instance, the muscle weakness and tremors are definitely a side effect of the drug. These never go completely away until I am off. Neither do the digestive issues.  I'm prepared for that. But I do need to become considerably more functional than I am at the moment. The anxiety and depression have come under control, but the extreme dizziness and inner tremor keep me from working. I often have trouble even sitting up - just feel better lying down. We cannot afford for me to take much more time off. ADWD...the gift that keeps on giving.

Is the tremor a visible shaking, or a feeling of a tremor but not visible?

@Songbird, the tremor is more internal, but visible at times. I can usually control it. My dizziness is the vertigo type with a visual disturbance that's hard to describe. It's almost - but not quite - like a focus issue. It is improved somewhat today.

 

But...I have a confession. A couple days ago, I felt like I needed to know whether my problems were chiefly adverse effects or if they were withdrawal from reinstating at half my previous dose nine weeks ago. I asked for advice, but before anyone had time to weigh in, it was time for me to dose. I made the decision to make a small 2.5% cut. Then I got the advice to hold. But since it was already done, I decided to take the smaller dose last night too. Since 6pm tonight, I've been getting more and more shaky and anxious. So for tonight's dose I returned to 25mg. So I guess I got my answer. I should have known better than to cut before I'm stable, but I couldn't help remembering how much better I felt as I reduced during my fast taper last summer, and I kept thinking "what if...?" Now I feel defeated and helpless, and maybe I'll never stabilize and never get off this rat poison. How badly do you think I've set myself back with this stupid move?

It's very hard to tell what an "ideal" dose would be.  I think your best bet would be to stick where you are for at least a good few months.  Dose changes can further alter the balances in the body causing more instability and confusion in your system.  It's frustrating when symptoms aren't clearing up and we can feel we want to "do something" to "fix" them, but time and again we see that there isn't a quick fix.  I'd be more inclined to think an updose might help if you were only having withdrawal symptoms, but since you also get adverse effects, an updose would likely make those worse.  If you did decide you really wanted to try an updose, I'd make it a very small one.  But as I said I think staying at the same dose for a good long while might be a good idea, to give your system time to sort things out and work towards stability.

 

It's common for people to feel better during a fast taper.  I've found I often feel better soon after a dose drop.  Perhaps the dose change results in some initial changes in neurotransmitter balance that can make us feel good.  Then later on there are downstream effects of the drop which we experience as withdrawal symptoms.  If someone tapers quickly they might begin to feel good, so keep on tapering rapidly, and then the downstream effects catch up with them weeks (or sometimes even months) later.

 

Another thing to keep in mind is that people's systems can become more unstable and sensitive to changes after they've made a lot of medication changes.  I went on and off an SSRI twice with few problems tapering over just a couple of months.  The third time on I got "stuck" and even very slow tapering was difficult.  It looks like you've been on and off Zoloft a few times, as well as a short stint on Lexapro, so it's possible your system is now much more sensitive to changes.

Acupuncture can be quite effective. Sometimes it takes several treatments to re-train your nervous system.

 

Is is possible you made yourself anxious over that tiny dosage decrease?

@Songbird and @Altostrata, thank you both very much for weighing in. Songbird, I think you are right about sitting still for a bit. And Alto, it is entirely possible that you are right also that I was anxious about the decrease. In retrospect, what it tells me is that either way, and as desperately as I want off this rat poison, I am not ready at this time, physically and/or emotionally, to start my taper. 

 

The biggest help for me so far has come from my Naturopath. The accupuncture effects on my vestibular issues only lasted through that day. The next morning, dizziness and visual disturbance was back with a vengeance. I'm hopeful that with more sessions will come longer-lasting effects. He also recommended Lavella (lavender supplements) for anxiety. I haven't taken any until this morning when I awoke from unrestful sleep with higher than usual anxiety. The anxiety doesn't awaken me, but comes on as I become conscious and "remember" the predicament I'm in. This morning it was 6/10, even after my treadmill walk. So I took one and an hour later found it easily cut the anxiety in half. In fact, I was able to take a 1 hr nap that did not go toxic on me. In fact, when I awoke, all my symptoms are greatly lessened (still pretty dizzy, though). I am normally a napper but haven't been able to take a nap for 3 months now so I'm overjoyed. I am inclined, therefore, to believe that I was already in a wave before the cut, made worse by the 2 day dose reduction. I've been back up to 25mg for 2 days now, and today, symptom improvement. Also, when the mods ask to see daily notes, how many days do you generally like to see? I don't want to give you too much to slog through. You guys work hard enough as it is and I want to be mindful of that.

Approx. Nov 2017 - successful taper of klonopin; Approx. Jan. 2018 - rapid taper Zoloft over 2 wks - no w/d symptoms; May 2018 - Reinstate 50 mg Zoloft per doctor; Aug 2018 - Rapid taper Zoloft over 3-4 weeks - no w/d symptoms for 1 mo.; Late Oct 2018 - pdoc rx'd 5mg lexapro -took for 1 wk; Early Nov 2018 - Reinstate 25 mg Zoloft; updose to 37.5 on Nov 28, 2018; Nov 30 2018 - returned to 25mg Zoloft upon mod. advice; Dec 9 - Dec10 2018 - 12.5mg zoloft liquid+12.5mg zoloft pill; Dec 11 2018 - 25mg zoloft all liquid; Feb 14 2019 - updosed to 26.25 mg liquid; Mar 6 2019 - updosed to 26.88 mg liquid - new symptoms; Mar 13 2019 - back down to 26.25 mg per mod suggestion

Dose Changes: Dec 2 2019 - 5% to 25mg; Jan 14 2020 - 10% to 22.5 (increase in sxs all month); Mar 10-15? 2020,  accidental updose to 25mg; Mar 22 2020 - back down to 22.5mg; Apr 12 2020 - 2.5% to 21.94mg; Apr 19 2020 - 2.5% to 21.375mg (symptom increase); May 17 2020 - 2.5% to 20.625mg; May 24 2020 - 2.5% to 20.1mg - Jun 14 2020 - noticed uptick in symptoms settled 2 days later - July 10 2020 - onset of wave

We need to see a sequence of days, to see if there's a pattern. You can post each day at a time or several days at once.
