Matt7777: Seroquel / quetiapine

I am new to this forum. (Moved from Tips for tapering Seroquel topic)

I have been on Seroquel since Aug 1, 2018, 4 months at 25mg. I tried to come off of it cold turkey after 6 weeks, and my dosage ended up getting doubled to 50mg. I have since tapered down to 25mg. I notice every time I taper successfully, my sleep is initially fragmented for a few days progressively getting better.

I just had a unsuccessful taper attempt from 25mg to 22mg last week. I went four days without improved fragmented sleep. So then I went back up to 25mg again. Is this a case of perhaps simply finding a taper amount that I can tolerate easily enough? I am wondering what will happen if I had just stuck it out when I came down from 25-22mg. Would my body eventually have adjusted to that level? The uncomfortable sleep deprivation would be the obvious consequence.

(Hey Songbird,) - post moved from Songbird's Intro.

I am at 25mg Seroquel currently. It has been four months on the drug. I was successful coming down from 50mg to 25mg. I am having difficulty breaking below 25mg having tried twice. The latest was to try coming off 3mg. I had four days of fragmented sleep with increasing overall sleep deprivation. Usually, I will have initially a bad night with increasing improvement. So I bumped back up to 25mg. Any words of advice other than trying to come off at a smaller amount?

Thanks!

Matt

(Hey Ruby,) moved from Ruby's intro topic

I am using the Smart micro-scale to come off Seroquel. I am currently at 25mg and have been on doses up to 50mg. I am struggling to come off 25mg after two failed attempts. The latest attempt was to go from 25 to 22mg. I had four days of fragmented sleep with increasing sleep deprivation tiredness. I went back up to 25mg. It seems like the keys to tapering are as follows: 1) the amount of the drug I am coming off; and, 2) the period of time between tapering. Are there any of factors you can think of?

Another approach someone told me they used coming off 25mg for three months, their doctor had them take 25mg every other day for two weeks, then every third day for two weeks, and then off. Have you heard anyone find value to training your body to tolerate the drop by this approach?

Thanks!

Matt

Hi,

I am interested in people's experiences getting off Seroquel. I have been on the drug since August 1, 2018. The doctor took me off it cold turkey Sept 20, 2018. Since I couldn't sleep, the doctor prescribed several benzo combination of drugs. Finally, another doctor put me back on Seroquel at 50mg October 3, 2018. I have since tapered down to 25mg.

I understand that the two most important variables that dictate coming off are the following: a) the amount of the taper; and b) the time in between tapers. Are there any other important factors for consideration?

I spoke with a guy who was on Seroquel 25mg for three months or so, and his doctor told him to come off of it by taking 25mg every other day for two weeks, then 25mg every third day for another two weeks.

Any advice is welcome!

Thanks!

Thank you for completing your drug signature.  Please include ALL the drug dose changes with their dates in your drug signature.  It is helpful for the mods to be able to see your history at a glance and not have to read back through your posts to get the information.  Thank you.

SA recommends that the same dose is taken daily at about the same time.  The brain likes consistency.  Do not skip days.  Skipping Days vs Every Day Dosing Graph

I will ask the other mods for their assistance with your drugs, because some of the other mods are more knowledgeable about benzos.

So that the mods can assess your situation please keep daily symptoms notes on paper and post them in here in your intro topic.

An example of what is needed:

6 a.m. Woke with anxiety
8 a.m. Took 2.5mg Lexapro
10 a.m. Stomach is upset
10:30 a.m. Ate breakfast
11:35 a.m. Got a headache, lasted one hour
12:35 p.m. Ate lunch

This explains why we need what has been asked for:

On 9/28/2016 at 4:49 AM, Altostrata said:

A symptom pattern that occurs regularly over several days could mean the symptoms are from withdrawal, other adverse effects of drugs, or something else you do on a daily schedule.

In the course of discussion in your Introductions forum topic, you may be asked to keep notes on paper of your daily symptom pattern, including when you take your drugs, their dosages, and any symptoms.

What we need to see for every single day over several days is what symptoms you get before and after you take your drugs.

Where would a person keep this daily journal online here?

By the way, the benzos listed were prescribed to get me to go to sleep after the doc took me off Seroquel cold turkey. Come Oct 3rd, I was put on a 10 day valium taper to get me off. So I am no longer on benzos.

Songbird,

I measured the dosage with American Weigh Scales GEMINI-20 Portable MilliGram Scale, 20 by 0.001 G . I take the Seroquel between 1030-1130pm at night. My sleep has been much better since returning to 25mg.

According to your signature you started Ambien, Ativan, and Seroquel all on 1 August 2018, is this correct?  Was this all aimed at treating insomnia?  I'm wondering why the doctor felt that three meds were needed, that's pretty heavy-handed.  Any meds prior to August 1?

2001–2002 paroxetine

2003  citalopram

2004-2008  paroxetine (various failed tapers)

The Ambien was started by an Urgent Care doc a week prior to getting into to see my Primary Care doc. I told my Primary Care doc I wasn't a fan of the Ambien and she put me on Seroquel as a "longer term" solution to Ambien that she termed a "short term" solution. She thought the Ativan would be just "nice to have around" as needed. All three drugs were aimed at treating insomnia. The underlying issue I was having was frequency issues related to overactive bladder/nocturia. I had a prostate procedure August 1 to deal with the overactive bladder/nocturia that was keeping me up at night. I think the combination of the overactive bladder/nocturia combined with anxiety of having the procedure led to the insomnia kicking in a week before the procedure. Plus, the post-op was miserable with increased frequency issues, having a catheter in for three weeks and a UTI as a consequence. So I used the drugs to get through the first six weeks after the procedure.

Thanks for updating your signature, but I'm a bit confused by the Ambien starting Dec 26, 2018 - did you mean July 26?

Did you have any side effects from the cipro?

So currently you are taking only Synthroid 150mg and Seroquel 25mg?  Are you sleeping well currently?

I didn't have any side effects with the Cipro. But I had bad diarrhea from the Amoxicilan.

I am currently on Synthroid but haven't taken it since August 1, 2018.

I am only on the Seroquel 25mg. I am sleeping relatively fine currently. The constant leg muscle twitches are really annoying though.

I stopped taking it and need to start taking it again. I stopped taking it around the operation. It has to do with being on a catheter and being male.

You've made a lot of med changes in recent months.  If you need to start taking Synthroid again, it would be best not to make any other changes for a while.  One change at a time.  I believe you will be able to taper the Seroquel eventually, but it might be a good idea to let things settle for a good while first.

Thanks Songbird. I appreciate you sending the attachment. Do you know if the leg muscle chirping or twitching to be a pretty normal side effect to Seroquel?

Hi, Matt. Good notes!

Is this a case of perhaps simply finding a taper amount that I can tolerate easily enough?

Pretty close. See Tips for tapering off Seroquel (quetiapine)

Since a 3mg reduction seems too much, you might want to stay well within the 10% reduction guideline and maybe taper by 2mg or even 1mg at a time.

The twitching could be a withdrawal symptom or it could be a very concerning side effect of Seroquel. Side effects are dosage-related; also, with the anti-psychotics, after a while they may start to have a bad effect on the nervous system. If I were you, I'd taper by 1mg every week until you get down to 15mg Seroquel. Please track your symptoms daily to see how well you tolerate this reduction schedule.

Why were you prescribed Synthroid? Was it related to "psychiatric" symptoms? Thyroid hormone has a half-life of weeks, so not much likelihood of withdrawal symptoms, but it can cause insomnia when you start up again.

This is not medical advice. Discuss any decisions about your medical care with a knowledgeable medical practitioner.

I got muscle twitching on Seroquel, severity was dose-related, and eventually it went away once I was off.

Dec 19: 21mg                           6/10                                Awoken several times, fell back to sleep

Dec 20: 20mg                           2/10                                Slept for 2-3 hours, then couldn't fall back to sleep.

Dec 21: 21mg                          0/10                                 Couldn't sleep all night. Even tried NightQuil. Feels like Insomnia carried over from 20th.

Any advice what to do??? I tried tapering to 20mg on Dec 20th. Slept really bad. So I went back up to 21mg last night. I didn't sleep at all!

10mg Melatonin is a very high dose of Melatonin for someone in WD.  At this high dose it can have a paradoxical effect and cause you to stay awake.  We recommend starting with a dose of 0.25mg (1/4mg).  I started with 0.5 and slept better when I went down to 0.25.

Gridley Introduction

Lexapro 20 mg since 2004.  Begin Brassmonkey Slide Taper Jan. 2017.

End 2017 year 1 of taper at 9.25mg

End 2018 year 2 of taper at 4.1mg

End 2019 year 3 of taper at 1.0mg

Oct. 30, 2020  Jump to zero from 0.025mg.  Current dose: 0.000mg

3 year, 10 month taper is 100% complete.

Ativan 1 mg to 1.875mg 1986-2020, two CT's and reinstatements

Nov. 2020, 7-week Ativan-Valium crossover to 18.75mg Valium

Feb. 2021, begin 10%/4 week taper of 18.75mg Valium

End 2021  year 1 of Valium taper at 6mg

End 2022 year 2 of Valium taper at 2.75mg

End 2023 year 3 of Valium taper at 1mg

Jan. 24, 2024: Hold at 1mg and shift to Imipramine taper.

Taper is 95% complete.

Imipramine 75 mg daily since 1986.  Jan.-Sept. 2016 tapered to 14.4mg

March 22, 2022: Begin 10%/4 week taper

Aug. 5, 2022: hold at 9.5mg and shift to Valium taper

Jan. 24, 2024: Resume Imipramine taper.  Current dose as of Feb. 22: 7.6mg

Taper is 90% complete.

Supplements: multiple, quercetin, omega-3, vitamins C, E and D3, magnesium glycinate, probiotics, zinc, melatonin .3mg, anti-candida, iron, serrapeptase, nattokinase

Have you tried reducing the Melatonin as Gridley suggested?  It think it would be better to try this first before fiddling with your Seroquel dose.

I tried CBD oil for the first time last night. I was dealing with a two-day insomnia bout related to my Seroquel taper. The CBD oil is awesome! It is nice to know there are adjunctive medicines out there to help!  See tip #6 in link

Hi, Matt. Good notes

@Altostrata,

In one of your posts you mentioned you have a Sleep Medicine doctor. I just started working with one in my home state in the Pacific Northwest. I was thinking of seeing one down at Stanford Sleep Medicine. Do you think qualitatively there is much difference between a Sleep doc at Stanford vs that of a Sleep doc out of a top hospital in Seattle or Portland?

My reason for working with a sleep doc has to do with getting off Seroquel and treating any underlying conditions that may exist.
