Tanha: never stabilize under diazepam but want to taper

I read that quetiapine/ seroquel is more sedating the lower the dose?

 

i am at 250 mg cutting my slow way down and since a week I experience daytime sedation quite badly. Doesn't go well with the depressions and two more sedating drugs. 

Any hope that my slow taper won't be all in all oversedating for two years?

how does one manage?

I am not making this sedation with mirtazapin quetiapine and diazepam for a longer time.

 

any ideas what to do?

 

is it advisable to have almost the same dose of quetiapine during the day

or is it possible to take a huge evening dose and less during the day? Are there interdose withdrawls with quetiapine?

 

 

Are both these versions possible:

m 50mg

n 50mg

e 62mg

n 67 mg

Too sedating during the day

 

 

or 

m 25 mg

n 25 mg

e  87 mg

n 92 mg

Less sedating during the day but blood level won't be stable with this version?

Daytime sedation is killing me after daily noon-flue.

 

can I do anything about it? It is no good for depressive people.

 

 

I take 250 mg per day quetiapine, 5 mg diazepam and 16 mg mirtazapin in the evening.

is it advisable e.g. to take

if one is sensitive to variations in the overall bloodlevel?

As you can see, your doses of quetiapine and diazepam are not reducing your symptoms. On the contrary, my guess is that you are getting a paradoxical reaction from them by taking too much of both too often and this causes your symptoms all day long.

Now since a few days (and one month after dropping mirtazapin from 20 to 15 mg) 

i start to have severe daytime sedation specially in the afternoon. (Before, around noon, i have the painful flue). 

 

How can quetiapine be sedating and paradoxical at the same time 

or one day sedating and the other giving me terrible hyperarousel 1,5 hours after taking it?

I am afraid the old pattern is back:

Worst hyperarousel everyday around 4 p for two hours.

 

(seconde pattern is bad flue from 9am to 2 pm)

 

and this old pattern does not fit with the idea of quetiapine being paradoxical to me. This pattern was very stable from April to august and only stopped or was covered by an updose from 200 to 400 mg. 

I am scared that it comes back and grows stronger everyday although I reduce quetiapine. 

 

That means there is another cause for my hyperarousel, one which we cannot influence. 

It is there now absolutely every day around 4 pm (since I am lower than 260 mg of quetiapine). 

 

Could be my underlying sickness, could be the diazepam or mirtazapin. I am so debilitated by it because it is strong and persistent.

We need to find out what causes this pattern that followed me through almost the whole year except under heavy updose of quetiapine, levomepromazine and diazepam (3,5 to 5 mg).

It is terrifying.

a terrible old and enduring symptom pattern is back

and it doesn't fit with the idea of quetiapine being paradoxical (although this is the case, too)).

 

hyperarousel everyday 4-6 pm

I could climb up walls everyone it happens.

This pattern followed me everyday since April and only stopped for a while from august to November after we updosed quetiapine from 200 to 400 mg and added more diazepam (3,5 to 5 mg) and levomepromazine).

 

We need to find the cause of this, too, 

because it seems to show up only after I reduced quetiapine to less than 260 mg daily. 

 

I also have the painful flue from 9 am - 2 pm and after that heavy sedation until 4 pm.

Maybe that is the time of day when the Hormons switch to "sleepingmode"? 

And thus the body reacting to this with adrenaline because I get so many sedative drugs.

but this pattern persisted even when I was on 45 mg mirtazapin, which is definitely activating. 

 

I need an idea what causes this terrible hyperarousel everyday around 4 pm. It gives me really a hard time building a perspective living without drugs. 

I initially got most of them due to this 4 pm hyperarousel.

No, if something occurs at the same time each day, it's because of something earlier in your drug schedule.

 

I really do not have time to debate paradoxical reactions with you any more. You have designed your own drug regimen, it looks to me like you're overdosing yourself. If you want to keep doing this, go ahead.

Please listen to Alto’s advice. You have been all over the place and as she said you are overdosing yourself. Taking more and more of the meds is never a solution. 

 

As regards tapering Alto already suggested to taper the Seroquel first. I’ve tapered from 300mg do Seroquel down to 7.5mg but it has taken many, many years. I have got many more years to go too. Seroquel is an awful med and being on the high doses you are on can cause all sorts of health problems, but you have to taper off it slowly.

 

Only ever taper by 10% a month maximum. I can only taper around 4% to 5% now. Listen to your body though, and don’t go by a calendar. You need to be stable, or withdrawal normal between tapers. That doesn’t mean you will be symptom free but you will have similar bearable symptoms on a daily basis for a while with no major waves. 

 

You can do this. Try and just take a step at a time and reduce being so flustered. Make wise decisions. Take a deep breath and try and get focused. I know it’s hard and overwhelming, but if you are willing to centre yourself a bit you can do this. 

 

Are you still in the psychiatric hospital? I don’t have the brain power to read through your whole thread.

I never updosed until two weeks ago.

Instead I tapered three drugs parallel and a bit too fast. 

Levomepromazine i tapered since august until six weeks ago from 50 to 0 mg

Mirtazapine from 20 to 15 mg a month ago, 

Quetiapine i tapered all the time from 400 to 250 mg. A bit too fast and parallel to the other tapers

(after an emergency updose by the doctors in July from 200 to 400 mg).

I only updose after depression and flu got too severe (I have this flu and depression all the time anyway): 23.11. from 262 to 230 mg and after 10 days only once up 

To 250. 

10 days later (yesterday) I reduced again to 235 mg because I followed altostratas idea to taper quetiapine immediately due to the paradoxical reaction. 

Brainfog and now insomnia make things even more complicated. 

 

I didn't mean to overwhelm anybody with my need for help.

the doctors here in hospital think I still take 400 quetiapine and 30 mirtazapine. They refuse slow tapers.

i was and am a bit confused what you ment by "starting tapering quetiapine immediately".

i have been tapering it too fast for the last 8 months and tapered other meds parallel (mirtazapin, levomepromazine). 

When I read "taper quetiapine immediately" i wasn't sure if I had to stick to the rule of waiting longer or really starting immediately since I had only tapered 32 mg (of 250 mg total) three weeks ago an did my first emergency updose to 250 mg 11 days ago.

of course I have to stick to the 10% rule and at least holding four weeks. 

But since you said I will never stabilize on this high dose due to the paradoxical reaction (which is all very true) 

i wonder now if you think this is kind of an emergency and cutting faster is adviseable. 

 

And did get you right that we still have to find out wether the quetiapine or the diazepam or both are giving me paradoxical reactions?

today anxiety and anhedonia are bad. 

 

I want to do things right from now on.

If I taper quetiapine 10% a month and wait with the diazepamtaper until quetiapine is being finished (in three years) I would not be able to reduce the paradoxical reaction soon. Is that the way I have to accept?

I have cut quetiapine two days ago by 15 mg from 250 to 235 mg

Please do not make any more drug changes for several weeks. Take all your drugs at the same times and dosages each day. Keep notes every day so you can identify significant changes in symptom pattern.

As usual the first two days after a cut my feelings come back, a bit more hyperarousel and anxiety all day, anhedonia really worse but mood is good

also for two days the ever persistent painful midday flue with nausea is almost gone, usually it comes back on the third day after a cut and then persists. 

 

The later afternoon hyperarousel persists.

I am so exhausted and sick. And now I have this new condition on top, this feeling of not being able to properly move legs ands arms. Like lead and disconnected and nausea until 1 pm 

i am in utter despair.

made mistakes all the time messing up my body but all I ever want and wanted was taper taper taper (too much too fast too many drugs too many sideeffects and wdsymptoms every day all day except at most nights so far). 

I am not capable of enduring any taper in this severe condition. I am bedridden again and cannot stay in bed due to akathisia. Fatigue feels deadly if it's there.

how are you doing with your taper. 

I also have "old" problems from Ct cymbalta in march. 

Interesting that you can see your akathisia worsening after three week cuts. 

I am afraid of this akathisia. Have it all the time but worse during cuts.

i am very afraid of it because when I cut Lorazepam from 0,5 mg to 0 in may I got such an incredible akathisia I thought I'd die. Reinstated but didn't help. 

doctors upped all my meds and that seems to help.

 

do you know what actually causes your akathisia? Is it only withdrawl or also the sideeffects of a drug?

I can say I totally crashed today after crashing slowly the last months

Forgot to mention night medication:

67,5 mg quetiapine 

1 mg diazepam

 

knew it better

reduced too fast

total crash, bedridden, 

new syptoms (shaking, lightsensitive and worsening eyesight, nonstophyperarousel and weak muscles and feeling not to be able to control too fast). 

 

It is too difficult to really understand that taking a few mg more or less is having such an impact on health. 

I know this from many taperingexperiment over the last months but I am still too blind to see the obvious again and again. These medications are just so unbelievebale in their effects and symptoms vary so greatly. 

I just feel stupidities by them on another level besides the constant brainfog.

 

And i am in hospital but cannot talk about this to the doctors because i tried so often and they are so ignorant. 

They either make me take another pill (of course an ad!) against nausea and brainfog and as I refused they told me I had to attend all therapies from nw on or go home.

i cannot do any of these therapies because I am in pain but I do go there and am always stunned that I never faint because it feels like that all the time. I wonder how long I can pretend to function in order to avoid this new pill which I am not allowed to know the name. They say I have too much control over my pills and now they want one that they an control.

it is the therapies that make me stay because at home I would stay in bed all day no eating no drinking as I did before March.

My wife confronted my psychiatrist at home with my debilitating situation (being so sick and in psychiatry for eight months now) and she replied why I am staying on that medication if I feel bad with it?

I’m very sorry to hear of your situation Tanha. It’s very difficult to read of all the ignorance of medical professionals on this forum. There is still so much knowledge which is yet to become mainstream.

 

Stay strong and I hope things improve.

we all know that Psychiatrists must know about sideeffects and withdrawl from their clinical experience. It is impossible that they don‘t know. Yet I believe that the power of this medication is by far not percievable for them as it is for anybody except the sufferers. That is the face of the pills just being so small, easy to apply and after all they are ‚just pills‘. We seem to not have being made by evolution to understand the power of ad‘s benzod and neuroleptics and else.

i am very afraid of it because when I cut Lorazepam from 0,5 mg to 0 in may I got such an incredible akathisia I thought I'd die. Reinstated but didn't help. 

doctors upped all my meds and that seems to help.

 

do you know what actually causes your akathisia? Is it only withdrawl or also the sideeffects of a drug?

I have been off Zyprexa(Olanzapine) since spring. (Getting off got me a big part of my life back. I am able to work now, go outside etc).

I am currently tapering Diazepam and holding Valdoxan.

 

My akathisia was from Cymbalta. If you read my  thread, you will find out how I almost died :)

The strong akathisia came in bouts about 3 months after the CT and a slightly milder version persisted in the mornings for... a long time. But it was not the same intensity than it was right after CT from Cymbalta. I also do believe olanzapine added sooo many symptoms to my list and being off it has made such a difference.
