Thanks @Altostrata  Yes, I updosed from 1.8 to 2.0 last November 16 with your advice, after admittedly having dropped from 6 way too fast.  Are you suggesting i updose again at this point and by how much? I’m ready to try that. Your guidance would be much appreciated.

@AltostrataFollowing up on yesterday’s exchange, if I updose 2.0 liquid fluoxetine (held at 2.0 mg since last November 16) by 10% that would take me to 2.2 mg or 0.55 ml from current 0.5 ml. Does that sound all right?  Too much?

Updosing from 0.5 to 0.55 ml in my 1 ml syringe amounts to a drop and a half increase

Okay @Altostrata I will give it a try. From 2 mg current dose to l2.5 mg is a 25% increase, if I understand correctly. That would be an updose in ml from 0.5 to .0625

@altostrata, Hypothetically, if I updose from 2.0 to 2.5 mg fluoxetine and find after a month or so that I'm just feeling worse, what then might I do? Start taperig back down to 2.0 or updose even higher? If I weren't feeling so bad much of the time with anxiety and DR, I would be less apprehensive about making changes. I so much appreciate your steadfast support.

This morning I did a six or seven minute EFT (Emotional Freedom Therapy) tapping video with "Brad" on line. There followed about the worst anxiety siege I've had, still nearly incapacitating me at the end of the afternoon. I have to admit the experience with Brad was stressful, not calming. I won't try this anxiety-reducing technique any time soon!

Reinstatements do not always work but it can be an issue of finding the right dose.

Too much is as bad as not enough, which is why we suggest starting out low, so as to lower the chance of an adverse reaction. If more med makes you feel worse you would reduce, if your sure it's the up dose. If you do want to try an up dose it would be best to not make any other changes or add additional treatments, that way you don't have more than one thing that could possibly be too blame for not feeling well. Best thing to do is just keep symptom notes for yourself and check for any long term patterns.

I’ve had helpful advice from Altostrata and Colonial about updosing liquid Prozac. There are so many uncertainties in doing this that I will stay with my current dose of 2.0 mg for now—almost 5 months so far—and expect that I will stabilize in time, feel better, and resume a very slow taper.

Altostrata and Colonial both advised me about a 0.5 updose of my Prozac from 2.0 but also cautioned me that the consequences are unpredictable. So far, I haven’t done it. 
 

Over the past several months the anxiety and DR symptoms have become slowly harder and harder to bear. Windows have been fleeting and infrequent. I was expecting to make slow improvement and have more windows. I don’t know how members who see no improvement, rather deterioration, month after month manage to endure. Thank God for my loving family because at the moment I can barely go anywhere, enter a store, socialize, or do productive work. 
 

But is updosing 0.5 mg worth the risk?

4 days ago I updosed from 2.0 to 2.5 mg liquid fluoxetine. How long  might it take to notice effects of this, for better or worse? Days? Weeks?

Don't worry too much about the half life.  This might help you to understand it better:

 

Most drugs get to full strength in the blood after about 4 days, taking a bit longer for it to register in the brain.  However Prozac is different and can take about 10-14 days.

On Altostrata’s suggestion I updosed from 2.0 to 2.5 mg liquid Prozac 6 days ago. @ChessieCat advised me subsequently about Prozac half-life and the time it might take for me to register the effects of the updose. I’ve had a very hard time with daylong anxiety for some weeks, but today has been so severe I can barely move. Might the updose be responsible for this slide even though it’s only day 6? Should I already consider dropping back down, as Chessie mentioned, to 2.25 mg?

6 days is almost 7 days, so if you think that your symptom has worsened it would probably be a good idea to reduce immediately to 2.25mg.

Thanks to @ChessieCatfor advising me on my updose from 2.0 to 2.5 fluoxetine. This is day 9. On day 7 I had some relief from the worse of the anxiety symptoms, and that gave me hope to continue with the updose. Yesterday was fairly tough again and today nearly as bad as it's been these past weeks. But I can't say I'm getting significantly worse from the updose, and considering this is Prozac, I will continue. Is that a wise decision?

 

One of my symptoms is persistent diarrhea. I've shifted to a more rigorous anti-diarrhea diet. I don't know what else I can do. I know this is a common symptom of WD, and in fact ever since I did the Effexor fast taper in May-June 2019, my stool consistency started to change, though not diarrhea for a long time. This may be common among members as well.

This is day 10 following my Prozac updose. Yesterday my anxiety was very bad all day and got worse than ever in the evening, when I’ve usually had a little easing up before bed. It was bad enough to scare me, and this morning I dropped the Prozac dose from 2.5 mg (.625 ml) back down to 2.25 mg (.5625 ml). Despite some hope on day 7, I see no expectation of improvement as a result of the updose. If I don’t get any better at 2.25 mg or continue to get worse, how long should I hold there before dropping to my former “baseline” of 2.0, where I was holding for more than 5 months? Thanks @ChessieCat

To review, low-to-moderate derealization and anxiety have occurred intermittently since I was in college long ago. But only when I did the quick taper off Effexor and Wellbutrin in summer 2019 and started Prozac taper in January 2020 have I experienced much more intense DR and anxiety, cortisol spikes every morning, fuzzy vision, persistent diarrhea, mental fatigue, low appetite, and 20 lbs. of weight loss.

 

I've been holding at 2.0 mg Prozac for more than 5 months (not counting my recent unsatisfactory updose) waiting for signs of stabilization to resume long taper.

 

You ask about psychotherapy: Yes, I talk regularly to a psychotherapist. But please tell me why you're asking about this in my case.

 

You remarked that ”It could be that Prosac is not the answer.” I’m not understanding what you mean. Do you mean that Prosac is not my problem, it’s something else? Since joining SA I've had no reason to doubt that I’m experiencing protracted withdrawal. So, what might I do, other than what SA consistently recommends: continue to hope for stabilization, then resume careful liquid Prozac taper? Also, in the meantime taking some non-drug measures to alleviate the anxiety?

 

It's hard to get specific direction from your response, and I thank you in advance for clarifying.

I mean your repetitive thoughts, which manifest in questions you post here over and over requiring my attention, may not be controlled by 2mg Prozac and you should work on techniques for self-soothing with a psychotherapist.

 

Whether it's a withdrawal symptom or re-emergence of your original tendency, no one but you can manage it. We are not able to provide treatment or a solution for that habit of mind.

I posed this question a couple of weeks ago. I will limit my questions on the forum, but I would appreciate a response to this one.  I updosed from 2.0 to 2.5 liquid Prozac on SA suggestion on grounds that it might make me feel better, though the opposite could also happen. That was on April 14.  After more than a week I was feeling worse, so, also on SA advice, I dropped back down but only to 2.25 on April 23. I’ve been holding there since then, 17 days. And in the past several days anxiety, etc have been really hard to bear. I know that Prozac changes take longer to register. Should I now assume I should to try to stabilize at 2.25 and hold there until I feel better, which could be more months? Or should I drop back down to 2.0, where I was holding for 5 months prior to April 14? I know no moderator can tell me anything but that I’m rolling the dice. But I would appreciate a comment. And thanks to @Go2zero for earlier clarification.

What do you mean by "feeling worse"?

 

How are you feeling this anxiety? As noted earlier, if you had a tendency to irrational anxiety prior to psychiatric drugs, you may be back in those old habits of mind. This is not a withdrawal symptom. Are you in therapy to learn ways to manage this?

Due to another sudden intolerance had to fast taper venlafaxine to 1,14mg 

Seems like all of this time I was in benzo withdrawal, because when I took it now in desperation to help it made me feel worse, tried reinstatement first 1mg, then 0,05mg both made me feel worse.
