FindRest: 30+ Years on AD’s. Effexor to Pristiq. Now what?

With that severe adverse reaction, if I were you, I would not continue on 25mg Pristiq.

 

You should have reinstated maybe 10 beads of Effexor after you cold-turkeyed off it in July, rather than all those other drugs and then too much Pristiq.

 

Prozac is an SSRI not related to Pristiq or Effexor.

Since you seem to be having such a bad reaction to Pristiq, I would not cut Pristiq tablets. That would release the drug very quickly into your bloodstream, as would a liquid made from immediate-release Effexor. This might make the high blood pressure reaction a lot worse.

 

Other than splitting a Pristiq tablet, there really is no way for you to take a very small amount of Pristiq. Effexor is an SNRI like Pristiq. Effexor, the older drug, is venlafaxine and Pristiq is deslavenfaxine.

 

Switching directly from 25mg Pristiq to 10 beads of Effexor XR may be bumpy but hopefully will relieve the high blood pressure. As it's extended-release, Effexor XR is a better substitute for a Pristiq tablet than a liquid made from immediate-release Effexor.

@Altostrata That makes so much sense. Thank you. Calling my dr tomorrow asking to be switched back to Effexor XR. I know it could very well be a bumpy ride and am scared to death. But, I really can’t continue on the Pristiq. BP is continuing to go up even higher.

What a morning! The short story is that I pick up my Effexor 37.5 tomorrow. Scared to death, but I and my God will do this! 
 

Without going into elaborate detail, the dr’s nurse called me after my initial call and said the dr was prescribing 37.5mg for one week and then to start taking 75mg for the next month. Wow. I said I wanted to try the 37.5 for longer since I’d been switched to so many different ones the past year. That’s when she started treating me like a “mental patient,” telling me what’s best for me, saying that now we’d do one week at 37.5 with a dr’s appt for mid next week to decide what’s next. I told her that a month’s supply would probably cost about the same as a 7 day supply. She said, “No, they cost the same per pill.”

 

An hour later or so, I called the pharmacy to approve the name-brand Effexor. They told me that after insurance, my co-pay for 7 pills would be $86 and said that in the 7 years this pharmacist has been there, they have never dispensed the name-brand. So, I signed up for the savings card  on Effexor’s website (https://www.effexorxr.com/savings-card), but the pharmacy said it doesn’t cover a 7 day supply and it would still be $86. I asked them to type in a 30 day supply and check and they said, “Wow! Only $4!”


I then called the dr’s office back and informed them of the price differentiation. The nurse said, “But, we only do it this way whenever we start someone out...7-days at 37.5, then 75 after that”, but that she would ask the doctor anyway. So much for individual tailoring of meds! It didn’t matter what my body has been through these past few months, or years. I was not an individual. I was a standard dose, one size fits all. I was a mental patient. I’m so upset. 
 

But, I did what I had to do. I took it and didn’t create waves in order to get my prescription.  They called back to tell me the dr approved the 30 days at 37.5. Yes, I now know I need to change doctors. Been going to him, my PCP, since 1989. It’s so sad. I will tackle this part after I become stable again.  I shouldn’t have to go through this. None of us should. Rant over.

Oh my goodness! Is this the same Dr and Nurse that were responsible for all those changes in medication and the CT? 

 

Good on you for sticking to your guns and getting what you need. It's so unfortunate but it seem so many of us have to play this game with our ' health care professionals'. 

 

You sound like a very intelligent and resourceful person. I really admire your attitude and approach.  You can do this!

Would it be wise to order the Gemini scale off Amazon for future use, or would it be useless since I’ll be dealing with only 10 beads or so (wishful thinking)? Would 10 beads even register on the .001g scale accurately enough?

Mission accomplished, eh?  That's excellent.  Sounds like quite the adventure in negotiations though.......ugh.  So sorry.

 

I think if I were you, I would probably want to put the 10 beads in a capsule......some do, some don't.  And I think you could get some capsules pretty quick.  The OO, a lot of health food stores have them now or Amazon.  It's not neccessary, but for the longer haul, it may be more convenient.

And then make sure you have a good surface or material to do your counting beads on too.  I read of black velvet or black paper.

Then a container for extra beads too, marked well......so you don't waste any beads or drug itself now.

 

I finally got a Gem20 myself......just to help others with and use for stuff like melatonin tapering.  LOL.....I'm just into it all sometimes.  I don't know that you'll need one immediately. 

Do find out how many beads in each 37.5 mg capsule there are in this prescription batch.  Your 30 days worth may last much longer than 30 days......and that will be good.  You may have to convince your prescriber that you are doing really great on 37.5 mg and just intend to stay put.  Not all of them appreciate a good discussion around tapering and WD......I mean some are just rude about it, as yours was......or the office staff was.

 

And then notes please, again.  As soon as you begin or sooner.  Add in your BP's.  You'd be surprised.  Even though you think symptoms are the same and it's repetitive, sometimes differences do show up, and then in relations to your drugs.........it's just a easy method to use.

 

  I've got my fingers crossed and wand waving that this is going to go really smooth for you......the transition.  Hopefully BP back to normal and some other symptoms may also improve.

I was just worried about that today, lol!  My appointment with my GP is 4 days after I drop the Pristiq and start the Effexor.  This may be hard to pull off. 🥴 But, I guess, it would be close enough to the actual ct of Pristiq that he could believe it’s withdrawals. This whole game we have to play is just really ticking me off.

Thank you. I too hope and pray it goes as smoothly as possible. The scariest part is knowing that figuring out the correct dosage in the beginning could be tricky. I’m so thankful I have you and everyone else here to walk through this with me. I think once that stabilizing dose is figured out, it will get easier. 
 

I’m going to keep reminding myself of two things I’ve learned here:

1)  Don’t panic and updose or reduce without checking here first. Well, I won’t guarantee the panic part (my weakness...hate it), but I can’t go changing the dose. 
2)   Once stabilized (yes, it WILL happen), I plan to hold for a long time.

 

So, hold me to it, mmt!  I will post my daily notes as well. I’ve even told my husband how to post here in my thread if things go south and he needs some encouragement.  Here goes!  ❤️

Ha, I’ve got to get my act together!! Here I haven’t even started with the taper yet and have myself all worked up. Get your head on, girl! Woke up with lots of anxiety and now have visited the bathroom twice in 30 mins with the runs. Now that that’s out of my system, literally, I can regroup and get on with my day. Might try an Epsom salt bath later (can’t tolerate magnesium supplements well).

 

Off to sit in the sun for awhile.

Counted my beads tonight out of three Effexor brand name capsules:

 

Capsule 1 = 128 beads (22 small, 63 med, 43 large)

Capsule 2 = 129 beads (22 small, 85 med, 22 large)

Capsule 3 = 143 beads (42 small, 84 med, 17 large)

 

I think Wyeth intentionally does it like this so it’s harder for us to taper off this crap. Of course, all of the sizing is subject to my aging eyes (although, I did use a magnifying glass). 
 

Average number of beads per capsule = 400/3=133 (rounded)

Avg Mg per bead (which really doesn’t mean much with such a huge variety in sizes) = 37.5/133=.2819

 

First dose tomorrow will be: 10 large beads, roughly 2.819 mg (10x.2819)

 

Yikes!!! That’s not much at all!! 😳

Day 1 of the switch to Effexor has been pretty good so far! PTL! Some mild nausea, head feels a tiny bit off-centered like the beginnings of vertigo, but not bad at all. I’ll post my daily symptom log tomorrow once 24hrs has passed. 

 

The biggest change?? My blood pressure was 109/66 this afternoon!!!! Amazing what skipping one dose of Pristiq did!! It had been up in the 150’s-160’s / 80’s-90’s. Amazing!! The tinnitus has been better today as well. No wonder!!

Diary for Feb 1, 2020, day 1 of cold switch from Pristiq 25mg to Effexor XR (10 large beads, roughly 2.189mg):


It was a good day! As seen below, I had some minor symptoms, but they weren’t very bad at all. I’ve been feeling like crud ON Pristiq, so most of these symptoms weren’t new (except for the nausea, dizziness, and muscle jerks)

 

As stated in my post from yesterday, “The biggest change?? My blood pressure was 109/66 this afternoon!!!! Amazing what skipping one dose of Pristiq did!! It had been up in the 150’s-160’s / 80’s-90’s.“  

 

Regardless of the hell that may follow in the next days/weeks/months, I at least know that making the cold change was necessary, for my heart’s sake. When things get nasty, I can look back to Day 1 and remind myself that it is all worth it.


Feb 1, 2020 (Day 1):

  • 9:45a.   Synthroid 125mcg. Woke up this morning w very low anxiety, low level tinnitus
  • 10:30a. Effexor XR (10 large beads, roughly 2.189mg), Premarin .625mg
  • 11:00a. BP:112/76 HR:97
  • 11:30a. Probiotic, D3 25mcg, fluticasone nasal spray. Tinnitus.
  • 12:15p. Mild nausea, bit of anxiety, mild upper abdominal pain
  • 1:15p.   Mild anxiety, nausea gone, head a little “wooshy” like I’m about to get vertigo. Tinnitus.
  • 1:30p.   BP:109/66  HR:88
  • 2:00p    Ears ringing more, bit of nausea and anxiety
  • 2:30p.   More nausea, bit dizzy
  • 3:15p.   Backache in middle of back
  • 5:00p.   Famotidine 20mg. Tinnitus, no nausea or dizziness, less anxiety
  • 6:00p.   Light tingling and pins/needles in left hand, mild tinnitus 
  • 6:53p.   BP:128/74  HR:81
  • 7:30p.   Mid back pain continues. Took 1 Tylenol. A little jumpy, mild tinnitus.
  • 9:00p.    Feeling good. Occasional tinnitus
  • 10:00p.  Tinnitus increasing. Had a couple of muscle jerks in upper left arm. BP:130/81  HR:87
  • 10:15p.  Left arm weakness (been a problem off/on since starting Pristiq). Palpitations in throat.
  • 12.15a.  Low tinnitus, mild neck pain, acid reflux.  Ambien 10mg, clonazepam .125mg, cetirizine 10mg, capful Miralax. Bed.

You may not have to replace your GP, just don't take any more advice about psychiatric drugs from him or her or that awful nurse.

In the future, you'll want to be definite about what you want in drug prescriptions. Just speak in a firm, steady voice. Don't ask them what they think you should do, they don't know anything.

 

It may take a day or two for the Pristiq to be totally out of your system. Hopefully, the tinnitus will fade away.

 

As the Pristiq goes out, you may or may not feel you need to take more Effexor. Let us know how you're doing.

I’ve been on Ambien for years and know I am dependent on it, sadly. Was prescribed 3x.5mg/day clonazepam last May when the nightmare began w Latuda and severe panic attacks began. Was told by my psych NP (who prescribed it) to take one each night along with the Ambien. Only took a .5 of the C at night a few times and limited myself to .125mg instead. I had seen on TV about the benzo problem. Now, I’m afraid to drop that evening dose at this time for fear of those withdrawals on top of the ADWDs. I very rarely take .125mg during the rest of the day, maybe 1x .125mg every week or two (divide the .5 into fourths). I hope to taper off both of these once the Pristiq/Effexor taper is done. Should I do it sooner?

 

Ive been reading here about the antihistamines. I’ve been taking the cetirizine since 2018 after my rhinitis got real bad years before that and would never get better. Kept getting ear infections. ENT prescribed this along with the fluticasone spray and a netipot rinse. It has helped. After reading the info here and in the interaction checker, I would like to eliminate this one too. Just not sure if doing it now will make the ADWD worse. Your thoughts?

Fluticasone is a steroid spray, which can influence glaucoma. You may wish to switch to a non-steroid, such as azelastine.

 

You may wish to move the cetirizine and Miralax earlier in the evening, to separate reactions to it from the other drugs.

 

Symptoms you have during the day may be rebound from the Ambien and clonazepam at night. Please take your drugs at consistent times and continue to post your daily notes, att. @Shep

Today is day 3 on 10 beads of Effexor (ct off Pristiq). I have a feeling this is going to be a bumpy ride. Each morning wake up is a bit worse than the previous day, and the anxiety mid-day is worsening. This morning, I awoke with loud tinnitus, mild anxiety, bruxism, and a lot of ruminating.

 

Yesterday’s timeline follows:

 

Day 2, Feb 2, 2020

  • 4:15am  Woke up due to red, hot and burning, itchy feet along w left arm very itchy. Medium tinnitus.
  • 8:30am. Synthroid 125mcg.  BP:119/72  HR:96
  • 10:30a.  Effexor XR (10 large beads, roughly 2.189mg)
  • 11:45a.  Premarin .625mg, Probiotic, D3 25mcg, fluticasone nasal spray. Mild tinnitus. Bit jittery.
  • 12:30p. Little wobbly and off-balanced. Mild tinnitus. BP:123/71. HR:83
  • 2:45p.  Tinnitus continues. Mild anxiety. 
  • 3:00p.   Left leg buzzing from knee to foot in short, rapid, zaps. Feels like electricity running through. Comes on suddenly then goes away. Just a few times. Omeprazole 20mg.
  • 4:00p.   Anxiety increasing. Shakiness. Mild upper-body tremors.
  • 5:15p.   BP:119/79. HR:81
  • 5:45p.   More electric zaps in left leg. Pins/needles/tingling in left arm pretty bad.  Anxiety continues 5/10.
  • 7:00p.   Left arm weakness, also w pins/needles/tingling. A bit of pins/needles in other parts of body as well, but lots worse in left arm. Tinnitus. Feeling sad and worried. BP:138/73. HR:75
  • 9:00p.    Mild anxiety. Left arm better but still weak. Mild tinnitus. Moved Cetirizine 10mg and capful Miralax from bedtime to here, per Alto’s suggestion to take it earlier to help identify reactions. 
  • 9:15p.   Head tremor, like my head is too heavy for my neck.
  • 10:00p.  Netipot. Mild tinnitus (overall, has been better today)
  • 10:30p.  Pounding heartbeat in head. Tinnitus. BP:137/75  HR:73
  • 11:00p.  Tinnitus increasing. 
  • 1:00a.    Med/high tinnitus. mild neck pain.  Ambien 10mg, clonazepam .125mg. BP:139/80  HR:78. Bed.

This suggests to me that 10 beads is not enough. How about taking 14 beads? Please let us know how you're doing, with the daily notes.

FR, you're doing an excellent job with your symptoms notes. Please continue just like you're doing. Alto mentioned you may be getting some rebound symptoms from the Ambien and Clonazepam, so we'll look for symptoms such as interdose withdrawal and rebound that could be helped with spacing out these drugs if necessary.

 

It will be good to get the Effexor sorted out first.

Want to make sure I’m counting the beads in the best way. I’m opening up 3 Effexor brand capsules (each with varying numbers of beads and sizes) and pouring them together into a folded colored piece of paper. I then eyeball them and separate them, placing them on folded pieces of colored paper labeled “small”, “medium”,  and “large”. I then use a magnifying glass to quickly look at the divided piles and double-check them. I don’t spend a lot of time with the magnifying glass. Just glance. Amazing how long it takes to do just 3 capsules! 

 

I then pour each pile respectively into amber-colored pill bottles I’ve labeled “small”, “medium”, and “large”. 
 

I then open up 3 more capsules and repeat the above process, pouring them into the same bottles that already contain the previously sorted beads.  


Since my dose is 14 large beads, I take them out of the bottle labeled “large”. 
 

Am I doing it ok? I thought combining 3 capsules would be more accurate than just taking the largest 14 beads from one capsule, then opening a second capsule and taking the 14 largest ones from that one, etc..  (Note: When I first opened this prescription, I counted how many beads were in 3 separate capsules to get an avg number of beads per capsule and to calculate a rough mg per bead.  It really doesn’t mean much, however, as there was such a big variance in number of beads and sizes. I did NOT count the total number of beads in any of the other capsules I opened/sorted... just those very first three capsules.)  


Ha, I’m now brain-dead just writing that!

 

BTW, today has been a better day!

That sounds like a  good way to do it. I found counting the beads on a piece of black cloth inside a shallow tray with sides stopped them from rolling about so much. 

 

Counting the beads became a bit of a chore after the first few days-  so went with the Gemini 20g scale. I weighed my minisule amount of pellets with the 10g calibrating weight in the pan so I could get a reading in the middle of the range. I kept all my beads in one jar and then picked up what I thought I needed with a pair of tweezers.

You’re right, the bead counting is tedious! My husband said he’d help me do it. I don’t want to spend a lot of time obsessing over bead counting in the future. I plan to take him up on his offer. He doesn’t know what he’s in for, lol!

Yesterday afternoon and evening were better after adding in 4 more beads for a total of 14 Effexor. The numbness/tingling and pins/needles were almost non-existent and no more body zaps/jerks! I also had more energy and actually did some very light exercises (trying to stretch/relieve the back and neck pain).

 

Day 3, Feb 3, 2020:

  • 8:00am  Loud tinnitus, neck pain (stiff neck), bruxism, anxiety, ruminating.  Synthroid 125mcg.  BP:118/73  HR:98
  • 9:15am  Above symptoms continue. Scared.
  • 10:30a.  Effexor XR (10 large beads, roughly 2.189mg). BP:122/80. HR:87
  • 11:45a.  Premarin .625mg, Probiotic, D3 25mcg, fluticasone nasal spray. tinnitus, neck pain.
  • 1:15pm. Took 4 more large beads of Effexor XR. Tinnitus continues.
  • 2:00p.   Tinnitus a little better. Stiff neck/pain remains. Took 1 Tylenol
  • 3:00p.   Upper-abdomen tightness/discomfort. Mild tinnitus. Famotodine 20mg. BP:128/81  HR:79
  • 4:00p.   Pretty good afternoon so far! Mild tinnitus, ears feel full/stuffed.
  • 4:20p.    Spoke too soon...sudden, short wave of anxiety, heart palpitations in throat, bit shaky, head feels a bit too heavy for neck.
  • 5:30p.    Anxiety better, ears popping, mild tinnitus continues
  • 6:30p.    Stiff neck/ache continues, but better. Been using a heat wrap on it. Also feels a bit like my neck can’t support my head very well. Now have medium level of pain in the middle of my back. BP:127/78 HR:91
  • 7:00p.    Acid reflux.
  • 9:00p.    Feel pretty good except for neck and back pain. Little to no anxiety. Tinnitus much better but still there. Cetirizine 10mg and capful Miralax.  BP:135/79  HR:80
  • 1:00a.    Been a better day today! Neck is a bit better now, but back is worse. Tinnitus increasing.  Ambien 10mg, clonazepam .125mg. BP:135/88  HR:78. Bed.

Day 4, yesterday, was great until about 7pm.  I think the increase of 4 beads on Day 3 helped tremendously, as the morning/afternoon of Day 4 was the best day I’ve had in a couple of months! Then, things went a bit south with an onset of wd symptoms around 7pm, including muscle jerks, tightness around chest, back of head being pulled and jerking my jaw at the same time (it was weird), and of course, this all led to anxiety. 
 

Day 4, Feb 4, 2020:

  • 9:30am  Didn’t notice any tinnitus when I woke up earlier this am! Mild ruminating, neck and back are better but still ache.  Synthroid 125mcg. 
  • 10:30a.  Effexor XR (14 large beads).  Mild tinnitus.  BP:109/75 HR:105
  • 11:45a.  Premarin .625mg, Probiotic, D3 25mcg, fluticasone nasal spray. tinnitus, mid-back pain.
  • 1:30pm. Tinnitus continues.
  • 3:00p.   Mild tinnitus. Omeprozole 20mg. BP:122/77 HR:80
  • 4:00p.   Very mild tinnitus. Doing so well today!! Weakness in left arm even much better!!
  • 6:00p.    Right arm biceps have started aching. Tinnitus.  
  • 7:00p.    Weakness in left arm reappears. Numbness/tingling/pins/needles in left hand. Tightness around chest. Chills, tinnitus, with small wave of anxiety.  BP:127/79 HR:76
  • 8:00p.    Left arm weak w mild tremors and pins/needles. Tinnitus.
  • 8:30p.    Muscle tremors in both arms, very tight chest, anxiety 5/10
  • 9:00p.    Took shower. Feel better but both arms still shaky.  Cetirizine 10mg and capful Miralax. 
  • 9:15p.     Muscle in back of head on right side “pulled” my jaw to the right suddenly.
  • 9:30p.     Mild jerking movements in head
  • 10:00p.   Buzzing down left hand. Netipot. Mild tinnitus. BP:131/84  HR:82
  • 11:00p.  Arm tremors in both arms, muscle weakness in left arm, anxiety, tinnitus 
  • 11:30p.  Right earlobe burning, tinnitus 
  • 1:00a.     Most of the nasty symptoms of the past few hours have improved. Post nasal drip. Tinnitus.  Ambien 10mg, clonazepam .125mg. BP:134/83  HR:77. Bed.

@alto What do you think? I know it’s recommended to wait 4 days on a new dose and today, Feb 5, is day 3 of 14 beads.  Or, would you add a bead or more now?  Also, I woke this a.m. with a mild zap down my leg, neck ache, and stuffed nose/sinuses.   I really don’t want to take Mucinex (know not to take the DM) as I don’t want it to cause any other symptoms.

Oops. Today, Feb 5th, is day 3 of 14 beads.

That's good. It will take at least 4 days for the dosage increase to fully register. Let's observe for the next week and see if 14 beads does the trick.

Yesterday, day 5, was pretty good! Much better than the evening before. Mid-afternoon was the hardest time, but very manageable. It’s hard to be too miserable when the KC Chief’s victory parade is on television. Go Chief’s!!!
 

Had a dr’s appointment re my pre-existing stomach/esophagus issues. He wants me to stop alternating days between the omeprozole 20mg  and famotidine (Pepcid) and to take the omeprozole at all times. In fact, he’d prefer I go back to the 40mg dose of omeprozole. I had previously read here about the PPI’s, so we discussed the risks vs benefits. Prior egd results + family history of Barrett’s disease + some reflux recently made him concerned. He even admitted that stopping an AD and/or switching could wreak havoc on stomach acids/reflux. He also acknowledged that stopping ADs could make you more sensitive to drugs. Anyway, I will be going back to taking omeprozole 20mg every day and then, if things get a lot worse, I might consider going back on the 40mg.

 

Finished my afternoon at the chiropractor’s office. Hadn’t been there since my open-heart surgery in mid-2018 (have a wired sternum now). Ha, I think he was more afraid to adjust me than I was! He was almost too gentle. Not much “popped” or moved that I heard/felt, but he insisted that he felt things move. I did feel a tiny bit better afterwards.

 

Praying today is even better than yesterday!

 

Day 5: Feb 5,2020:

  • 9:30am  Sinuses clogged, mild tinnitus, mild electrical zapping in left leg, low anxiety, neck ache. Synthroid 125mcg. 
  • 10:30a.  Effexor XR (14 large beads).  Mild tinnitus.  BP:118/73  HR:94 
  • 12:00p.  Premarin .625mg, Probiotic, D3 25mcg, fluticasone nasal spray.  Tinnitus, bit more anxious, buzzing/zapping from left knee to foot.  BP:132/76. HR:92
  • 1:00p.    Arms a bit shaky. Mild anxiety. Tinnitus continues. 
  • 3:45p.    Mild tinnitus. Shakiness in arms. Mild brain fog off/on. Sadness, with brief thoughts of wishing God would take me now. Back/neck ache. 1 Tylenol, Omeprozole 20mg. BP:137/82  HR:86
  • 5:15p.    Loud tinnitus.
  • 6:00p.    Loud tinnitus continues. Brief palpitations, numbing/tingling in left hand (but better since chiropractor!). 
  • 7:00p.    Mild, brief buzzing/zapping in left hand. Tinnitus loud. BP:135/82  HR:91
  • 9:30p.    Tinnitus, mid-back ache. Neck better. Brief wave of chills.  Cetirizine 10mg and capful Miralax. 
  • 10:00p.  Tonight has been lots better than this afternoon and much better than yesterday evening! PTL! Mild tinnitus.
  • 11:00p.  Arm tremors in both arms upon exertion. Mild neck and back ache. Mild weakness in left arm. Tinnitus.
  • 12:30a.  Loud tinnitus. Mild/brief zapping in left leg.
  • 1:30a.    Almost fainted after stretching to touch my toes. Had to go down to floor w head between legs and wait awhile. This is new. Will get up slower next time! Palpitations. Brief zap in left foot. Tinnitus. Also wondering if I have a UTI coming on.  Ambien 10mg, clonazepam .125mg. BP:138/83  HR:75  Bed.

Do you actually have Barrett's disease or is this only family history?

 

Are your digestion troubles associated with any particular foods? Dairy products are a common culprit, also spicy foods and food additives.
