@Altostrata, I stopped taking St. john wort on 19th apr and increased my dose of Escitalopram from 1 mg to 1.5 mg. and I am almost in a crisis. My symptoms have gone worst than what they were. I don't know if this is the WD of st, john wort (research indicates that St. John wort may have WD after CT.) or because I up my dose of escitalopram. I should have done one of this first to see body reaction.

Can I go back to 1 mg? or now i did 1.5 mg, I should not confuse my brain and stick to 1.5 mg for now?.

Dec2019 - Started 10 mg Escitalopram for my Anxiety

Aug2020- Did DNA pill check work to find out Escitalopram is not for me.

Dec2020/Mar2021 - Started reducing my dose to 1 mg every week for 10 week.

4th Mar 2021 - Took last dose of Escitalopram 1 mg.

30th Mar 2021 - Reinstate 1 mg Escitalopram. 

19th Apr 2021 - up my dose to 1.5mg.

7th may 2021- hospital staff gave me 10 mg fluoxetine.

17th may stopped taking 10mg fluoxetine. Currently not taking any meds for next 7 days.
Stared taking 0.50 mg Escitalopram on 23rd May 2021

 Vit D3 800 iu once daily ,vit B12 1000mcg once daily, Neurapas 2 tab once daily (added by my Naturopath) stopped taking on 17th Apr, Mg Glycinate 200mg 1 cap a day.

Jan 2023- Started tapering 10% every3months. 

Nov 2023- Jump from 7.5 to 5mg and felt Symptoms in a month. going back to 7.5 mg on 14th Nov.

This gives a SERT of aproximately 52%. As I read your story and see how fast you tapered, I can only advice to stabilize without changing dosage. Stabilizing can take many months. Again, we are all different. But in my experience after every attempt of stopping it took me longer to stabilize after crashing (due to slow, but still too fast tapering).

 

The more often we change dosages up and down and the more attempts we make to stop and reinstate again, the more upset our CNS becomes...

 

The fact that you stopped St. Johns Worth can increase WD effects. Since St Johns Worth can enhance the effects of a SSRI. As if you are taking a second SSRI next to the first. This also has to settle and stabilize.

 

It is not a good combination these two!

1993    Anafranil (Clomipramine) for a few months. Later in 1993 Paxil for a few months 1993- 2006      No medication

2006   Effexor, Cymbalta, some Benzo’s. All for short periods. Later in 2006 Lexapro (escitalopram) 10 mg and shortly after Wellbutrin XR 150mg, against side effects Lexapro 

Since 2006 until end of 2015: Several times on and off Lexapro and Wellbutrin and several slight dosage changes. Mostly taken dosages: 5mg Lexapro and 150mg Wellbutrin

2016  Dosage change Lexapro from 5mg to 2,5 mg. Wellbutrin stayed om 150mg

November 2016 – April 2017 Down from 2,5mg to 0,6mg Lexapro (in steps) without much problems. Wellbutrin down from 150mg to 66mg. Also without much problems.

April 2017 – March 2019       Lexapro 0,6 mg        April 2017 - August 2018       Wellbutrin in small steps down from 66mg in to 37,5 mg . Quite heavy WD after each step.

March 2019 – May 2019 Lexapro down from 0,6 to 0,3mg then Prozac to 0,6 mg switch because severe discontinuation effects (may also have been from Wellbutrin..)    

Wellbutrin down from 37,5mg to 35,3mg 

October 2019        Seroquel 12,5 mg for 4 weeks because of extreme sleeping problems, then weaning off in 2 weeks       Prozac up dosage to 1,2 mg

March 2020     Wellbutrin in 2 steps down from 35,3mg to 33,3mg   Extreme withdrawal effects during 8 months. Stopped tapering Wellbutrin  until total off Prozac. 

February 2020 – November 2020   Prozac down in steps from 1,2mg to 0,57mg. 

Jan 2021  Prozac down to:  0,55> 0,53>0,51mg,   Feb 0,47mg ,  Mar 0,42mg,   Apr 0,37, longer hold because of WD symptoms July 0,36 and hold again, Sept 19 0,35, Sept 26 0,34mg, Oct 3 0,33mg  Long hold of 172 days until March 2022

January 20, 2022:  Wellbutrin from 33,3 to 32,3mg

March 22, 2022 Prozac down from 0,33mg to: 0,30mg, Apr 0,29, May 0,28, 0,27, June 0,26, 0,25, July 0,24, 0,23, 0,22, 0,21, Aug 0,20, 0,19 Sep 0,18, Oct 0,17. 0,16, 0,15, Nov 0,14  Jan 2023 0,13, 0,12, 0,11  Feb 0,10, 0,09 Mar 0,08 ,  June 0,07 , July 0,06,  0,05, Aug 0,04, 0,03, Sept 0,026, 0,024 Nov 0,022, 0,019, 0,016, 0,013 Dec 0,012, 0,011, 0,010, 0,009   Jan 2024 0,008, 0,007,  0,006,  0,005, 0,004, 0,003, 0,002, 0,001, Feb  0,0007.  0,0005,   0,0003, 0,0001,

I stopped taking ST. john wort as It has given me serious eye issues. I went to see ophthalmologist and first thing she mentioned is stop takling St. john wort. it is making your eye go dry.

I can imagine I am going through two possible withdrawal symptoms. 1 of Escitalopram too fast tapering and of St. john wort of CT ( I had no choice as it was damaging my eye).

I have no option to wait and watch I believe as this WDs are serious.

All I have to is calm my self and remind myself of not to focus on this temporary suffering but better future.

 

I will wait for @Altostratato tell me if I stick to 1.5 mg of change back to my original dose of 1 mg. I change my dose only as of last 2 days.

It takes at least 4 days for a change in dosage to fully take effect. However, if you immediately got a bad reaction from increasing from 1mg to 1.5mg, the increase was not a good idea. You can go back to 1mg.

Your other symptoms are mild. Suggest you work on strengthening your sleep via the links I gave you and relax about watching your symptom pattern so closely. By focusing on them so much, you may be convincing yourself they're bigger than they are.

 

Perhaps you might consider non-drug ways to control your symptoms, rather than going up and down in your drug dosage. It does seem you are over-anxious about these changes. Since your withdrawal symptoms are mild, you may be making your own problems.

@AltostrataYou are right. the reason I was on this ADs at the beginning was because of my heath anxiety. I had sever migraine and because I thought I had brain tumor I picked up anxiety. and I have been suffering from that day. It just I am a healthcare worker and knowing about drugs or conditions makes you more nervous than you are.

I agree. however, I do now that symptoms that I feel right now are not of the health anxiety I had and was the reason to start ADs. but purely a WD of escitalopram and now St. John Wort.

I really tankful that I have found SA before it was too late to reinstate drug.

@Go2zero, Thank you so much. I agree that changing dose to much can make it worst and I have to give myself few months before I could get stable on 1 mg dose. So I will stick to 1 mg Escitalopram until I am ready to tapper further.

 

 

My symptoms for half day today are

11:30 pm- Went to bed and fall a sleep in about 15-30 min.

4Am- woke up and thought hard to fall a sleep. in about 45 min fall a sleep again.

7 a.m. Woke with anxiety and sever headache
8 :30 a.m. Took my supplements mentioned in signature.
9 a.m. at work, hard to look at computer.

10 am. felts hard DP/DR symptoms.
12:35 p.m. Ate lunch. feeling same.

1pm. Serious Eye pain and warmness.

I am glad I could help you with some advice. When you start to taper after being stabilized, please do it slow! Listen carefully to your body and feelings. Too fast tapering will take much more time in the end than going slow.... And the last mg is the most difficult one in my experience. No wonder when you see the extreme high SERT level at 1 mg. 

 

There is a lot of support here how you should taper and how to adapt to your personal experiences. When needed I can make a chart for you with the SERT levels in the range of 0,005 and 1 mg escitalopram.

 

About the St John Wort; it is questionable if you are having WD from that itself. But because the enhancing effect on escitalopram, the WD of escitalopram is probably stronger than without taking St John Wort next to it.

That would be much appreciated.  I will contact you when I am ready. right now I am struggling with everyday anxiety and headache. it is very hard to be focused with everything. plus I am suffering with eye issues too.

Make sense. but one thing is sure that My dry eyes and double vision plus difficulty focusing on object and words are because of St. john wort (More) and WD of escitalopram (very less) combine as per my ophthalmologist.  one of the main reason I stopped CT.

6pm - feeling anxious again and my vision issue is getting me more anxious. feels like i am trapped in a dark room with double vision.

8pm- took 1 mg escitalopram. dinner and still feeling anxious and depressed.

@Altostrata, I need your advise.

 

I am on 1 mg reinstate for about a month now and do not see any improvement. also, my eye vision have gone worst ever since I started 1 mg escitalopram again. after 4 days of starting escitalopram, I started suffering from visual snow, double vision has gone worst, difficulty recognizing person or object from a distance, blurred vision, painful eye, dry eye and throat. 

other symptoms are DP/DR, anxiety, headache which stays for most of the day with some windows in between.

 

Do you think that 1 mg reinstate is not working for me?. should I stop taking it together or  I can up dose to 2 mg to see if I can gain any benefited?.

Personally, I reinstated 15mg (was on 10mg before fast taper) and it was hell at first. But it slowly got better and better.

 

So maybe it makes sense raising your dose to 2mg and waiting a few weeks to see if you feel better.

Did you have eye problems or other adverse effects from escitalopram when you were taking 10mg per day? 

 

Do you have light sensitivity? Do you eyes hurt when you use the computer?

 

You may want to take small doses of magnesium glycinate throughout the day, it's more effective that way.

No, I did not had any adverse effect from 10 mg escitalopram.

Yes, I do have light sensitivity and blurred vision. my eye hurts while using computer and are dry according to Ophthalmologist.

I do take 200 mg Mg glycinate everyday for past month now.

 

I have been home last todays as my symptoms have progressively gone worst and keeping me out of job. My symptoms yesterday were,

 

11:30 pm- Went to bed.

3am. my eyes are wide open and it is hard to get a proper sleep.

3:30- slept.

6am- woke up and hard to fall a sleep. 

7 a.m. still in a bed lit anxiety and headache.
10 :30 a.m. Took my supplements mentioned in signature.

10 am. in bed just lying hoping to get some sleep.
12:35 p.m. Ate lunch. feeling same.

1pm. in bed again watching some videos.

5pm.  trying to connect with family. supper hard to do so. but do not want to go to bed again.

6pm - feeling anxious again and my vision issue is getting me more anxious. feels like i am trapped in a dark room with double vision.

8pm- took 1 mg escitalopram. dinner and still feeling anxious and depressed.

10pm in bed hoping to get some sleep today.

4am. no luck with sleep yet. first time experiencing insomnia for a week now. not use to this feeling.

4;15 am- fall a slepp.

6.30am- woke up again of a bed dream.

10am- still lying in bed.

200mg once a day is not sufficient. You might take half a tablet every few hours.

 

If you have light sensitivity and are using the computer, getting eye pain, headache, etc. from it, you will need to reduce light stimulation with tinted glasses or turning down the lights. 

 

Light-sensitive? Try blocking out blue light

 

Increased sensitivity to light, noise, sound, exercise etc

Otherwise, it looks like you tend to make yourself anxious with worry. You will need to be patient. It may very well be that you need to help yourself recover from light sensitivity, etc. rather than seek a pharmaceutical solution.

It may be true that it's not a big worry generally, but when people are light-sensitive from withdrawal syndrome, we find they benefit from reducing light stimulation with tinted lenses. Many of our members like the blue-blocking amber lenses, but anything that reduce light stimulation can work.

I am trying keeping myself as calm as possible. But My WD symptoms are keep getting worst everyday despite of reinstating 1 mg escitalopram.

Update**need urgent help 
I was in emergency hospital 10 days back due to severe WD symptoms. Like inner agitation, migration, Dp/Dr. Psychiatrist put me on Fluoxetine 10 mg for 10 days and when my condition got worst then I went to ER.

so they asked me to come off of Fluoxetine CT and wait for 7 days before starting Escitalopram again.

@Altostrata, @Yesyes123 please help. I am in clear hell. I just want to stabilize and not taking any medication as of one day. My psychiatrist asked me to start Escitalopram 5 mg after 7 days. What should I do.

looking at my history do you think that I can start 1 mg Escitalopram and try and get stabilized??

i have whole body shake, brain shake, vision shake, brain is paining even in bed. Just pure hell.

Hey @Demi918 I'm sorry you are going through this. You will stabilize eventually.

 

I am not a moderator but I do read a lot about these things. We need to confirm this with a moderator but if I were you I would probably reinstate Escitalopram 1.5mg and hold for at least 2 months. 

 

Since you have been on it for many months, your system is familiar with it and may help you stabilize.

i have been taking fish oil regularly. I have decided to stay of off all medication for a week to see. As i am in a serious cocktail right now.

Please post all the drugs you have taken here. It's never good to cold turkey anything, not even fish oil. Fish oil is not a drug, it's a supplement and you should probably keep taking it.

No, I have not. With reinstating Escitalopram it took me to Hospital and there with Fluoxetine for 10 days condition got worst then what it was.

By staying on both drug my condition got from moving guy to bedridden.

How about if I stay completely drug free for now?? @Altostrata

I'm confused about what happened. You were taking 1mg escitalopram, you went to the hospital, you took 10mg fluoxetine for 10 days, and quit. Do you feel better off escitalopram and fluoxetine?

 

It's up to you whether you want to try reinstatement of escitalopram again or not. You will have to learn non-drug techniques to cope with any withdrawal symptoms, which should very slowly and gradually go away. 

 

By the way, if your only withdrawal symptoms are disorientation and depersonalization, you're very lucky. These will also go away in time, but you have to not make yourself anxious or depressed about them.

That is right. I went to emergency as I was feeling awful even after reinstating Escitalopram for about month and half so I went to hospital and they put me on Fluoxetine. Which made my condition even worst. So currently I am off of all medication.

I will see what my withdrawal symptoms are and will decide what I will do. For now I will work around non-drug therapy to stay drug free.

My current symptoms are much more than just disorientation and Dp/Dr.

Gaseous feeling in my chest, headache(migraine), shaky, blurred and double vision. Vision seems darker than it is. But I am taking one day at a time.

i do feel anxious all day long. But i am in a serious cocktail of drug right now.

Escitalopram and Fluoxetine as there was no wash out. From 1.5 mg Escitalopram, CT to fluoxetine and 10days later CT fluoxetine and no drugs there after.

You might have a little fluoxetine in your system now, no escitalopram.

 

When we say it takes a long time for your nervous system to settle down from withdrawal, we mean months, not weeks.

When we say we are in a "cocktail of drugs" we mean taking multiple drugs at once.

 

But you said you're not taking anything currently, correct?

Yes, that’s right. I am not taking any medication at all.

I am having sever morning cortisol spike and Morning Anxiety. Is there any technique to over come this condition??

I used to have a lot of cortisol spikes and I think taking 2000mg Vitamin C Long Release daily helped me a lot with that.
