rubywednesday: Olanzapine withdrawal

rubywednesday

Recommended Posts

@JohnBanesthanks John, I seem to have more problems coming off antipsychotics (Olanzapine)  than the duloxetine.  Had a good day again today hope it continues.

I read somewhere that some people don't experience withdrawal symptoms for months or more after reducing the psychiatric drug dose.  Does anyone have any experience of this as I find it hard to believe but scary if it does happen.

Day 9

2003 - 2018 Started/stopped lithium, sodium valproate, fluoxetine, venlafaxine, seroxat, citalopram and many more that I can't remember. 2018 - 120 mg Duloxetine, 20 mg Olanzapine  2019  - 60 mg Duloxetine, 20 mg Olanzapine  Feb 2020 - Quetiapine - up to 200 mg slow release, 60 mg Duloxetine, Olanzapine to 10 mgu Aug 2020 - Tapered Quetiapine far too fast under GPs advice -  withdrawal symptoms included mood swings, insomnia, anxiety, dizziness (misdiagnosed BPPV), low blood pressure - 60 mg Duloxetine, 10 mg Olanzapine

4 Mar 2021 - failed taper to 5 mg Olanzapine 20 Mar 2021 -  reinstated 10 mg Olanzapine

19 Apr 2021 - 60 mg Duloxetine, 7.5 mg Olanzapine

Hi Ruby,

I was started on olanzapine in 2003, switched to risperidone a couple of years later, then back on olanzapine in 2015.

I was put on 10mg, but found my way down to 5 pretty quickly in 2015.  When I tried to cut to 3 1/3 from 5mg, it was too much to bear and I just figured I would have to keep taking it.  Then a friend gave me Breggin to read and I found SA along the way.  I've been in therapy and tapering down from 5mg since 2018, now down to a very low dosage and thinking about how/when I can drop it altogether, but that's how long it's taken me to taper off without major disruptions and still manage housing, relationships, work and all that TCOB.

Some people seem to manage a bit quicker but usually they've not been on it as long.

Regarding how long till I notice side effects - if I skipped a 10mg dose, I'd spend the next day basically staring into the void and feeling the edge of a huge tide of emotions.  Tapering gently has let them emerge at a healthy pace, and since around 2.5mg I've felt 'back in the world'.

Cheers

I also read on WEBMD that personality disorder can be a side effect of Olanzapine and wondered if anybody can shed any light on that?

No. This should more accurately be stated as drug induced behavior problems if that's what they mean, and it should be even more specific than that as far as what may be observed with this person. Personality disorders are thought of as enduring types of dysfunctional patterns over time that cause problems for the person's life (or those around them). This is not caused by drugs. Pretty much everyone has some dysfunctional patterns, so it's not a very valid diagnostic category anyway.

The drug company is probably trying to avoid being more specific, ie that the drug might cause you to go off your rocker (self harm, paranoia, isolation, who knows what "personality disorder" means to them). It's like saying "dental disorder" when a drug makes your teeth rot out of your head.

Thanks for the information @ShiningLight I've certainly changed a lot in the last 15 odd years I've been on olanzapine and I've not seen a psychiatrist since my dose was halved last year.

I've always disputed the diagnosis and I'm feeling much more like myself on the 6 25 mg olanzapine. Personally I think psychiatry is a load of rubbish, given the chance I'd pump them all full of drugs and see how they like it.

thanks again.

@haydukewow that sounds like hard work and a hell of a long time.  I'm hoping I'm not as sensitive as it takes about a week for the olanzapine withdrawal to kick in with me, I think?

Day 12

I'm prioritising the duloxetine taper at the moment and staying on 6.25 mg olanzapine until I come off the duloxetine.  My GP has prescribed a 2 month taper but luckily I have some 30mg tablets that I can reduce slower, approximately 17%.  It's going quite well, sleep isn't perfect but it never is and my mood is fine.  Feeling a bit stressed but I've got a lot going on in my life at the moment.

@Greatful thanks for stopping by, positive comments really help the motivation thanks.

Day 13 - another good day, feeling ready to drop to 40mg Duloxetine on Friday if things go well (from 50mg - 20% reduction).

I take it when I get down to weighing beads, I need to take them out of the capsule and weigh them and then scoop them back up again?  Looking forward to that day!

I wanted to pitch in with my thoughts on this earlier question:

Quote

I read somewhere that some people don't experience withdrawal symptoms for months or more after reducing the psychiatric drug dose.  Does anyone have any experience of this as I find it hard to believe but scary if it does happen.

The longer it takes for symptoms to develop after discontinuation, the higher the likelihood it is not withdrawal but an actual mental health issue.

Previously: Lithium (incorrectly diagnosed bipolar although I never had a manic or hypomanic episode)(taken during summer/fall 2020); Olanzapine (taken from late spring 2020 until late December 2020); Abilify (briefly for a couple of weeks in 2016, caused akethesia); Risperidone (2014-2016); Fluoxetine (two weeks in 2020); various sleep aids, e.g., Ativan, trazadone, mirtazapine and other antihistaminics (winter through summer of 2021).  I don't remember most of the dosages and am leaving them blank.

Currently: Latuda (40mg) (hoping to taper).

Other: I do not smoke, do not drink, have only one cup of coffee in the morning, and don't use anything else.  I exercise 3-4 times per week and watch my diet.

Day 2 Duloxetine taper (from 50 mg to 40 mg)

Wrote a long post yesterday but don't quite know what happened to it!  Thanks to everyone who's taken the time to read/reply to my post.

Feeling a little low/anxious still but it was quite a stressful day.  Sleep was quite a bit better, only woke once.  Had a lovely cycle and did my meditation (I'd not done it properly for quite a few days).  Still a little worried about PAWS and w/d in general but not too bad a day.

Ruby

Sorry I've been AWOL for a while, been away on holiday, thanks for all your comments and I like the idea of psychiatrists practising on each other lol.

@JohnBanes sounds reasonable but I hear some people get PAWS, sounds pretty scary.

Day 1 on 40 mg of duloxetine, mood a little low, probably too soon for it to be the drop in meds and my sister is pissing me off!  Sleep a little restless but not too tired as I can sleep late.

Will try and update my signature if I can figure it out DOH.

Personally I think psychiatry is a load of rubbish, given the chance I'd pump them all full of drugs and see how they like it.

I quoted this once earlier but saw it again and it made my day,  I even got a chuckle out of it.   I will hold them down and you can pump them in😁

You got away for a little bit, nice to take a brake from everything.

Tell your sister to leave you alone, you need your space🙃

Hope you feel better soon

@Greatful I'm so pleased it lifted your mood even if just for a little while.  I'm looking forward to the day that we reverse the doctor/patient relationship.  I wanna be the quack in the psyche ward, you can be the matron, we'll knock those b'stard psychiatrists into shape.  By the time we're finished with them they'll be needing all the effing drugs! lol.

Day 4  Slept a bit better last night but mood has been a little low today.  Yeah staying away from one of my sister's aint a bad idea, all her and her husband think about is money (and they've got pots of it) and I can't do anything right in their eyes.  Still keeping up the exercise and keeping busy, the meditation helps.  Least my friend is back from her holidays, I've been missing her and she bought me a lovely necklace for looking after her cat.

@rubywednesday  I think it is a good idea to limit your time with people who irritate you, they will only add to your already morphed neuro emotions we tend to get when in WD.  Besides why would we want to feel worse then we already do.  Remember to limit your stress.

Spend time with the people that will enhance your recovery, or at least make it more tolerable.

Hang in there😊

@Greatfulgood advice to keep my sister(s) at bay, I imagine they have a good b**** about me but (can't remember where I read it) what other people think about me is none of my business!  I hope you have a window soon, hugs x

@Escitalopram21 thank you for the encouragement, I'm really just taking each day as it comes, so far touch wood, it's been ok.  How's your taper going?

Day 6 sleep not bad, a bit stressed/ low mood first thing but soon wears off.  Spent the day with a good friend shopping, even managed some housework (maybe doesn't sound much but it's close to a miracle lol).

@haydukeyeah I rejected my whole family when I was a teenager, it's only since my parents died that I've tried to cultivate a relationship with my sister's.  One is ok most of the time but I don't plan on seeing the other one much, if at all and don't get me started on her husband lol.

Day 7 sleeping pretty good (for me), quite anxious in the mornings mostly about my voluntary work but sometimes I'm just anxious about the taper and the future/ withdrawal.  I try not to dwell on things and my life is pretty busy so it passes quite quickly.  Had a good day, met a friend's new puppy and had a coffee with my befriender.  Forgot to do the housework, what a shame lol.

Sounds like you've got good boundaries, and glad you get on well with at least one sis.

Do you have any practices around anxiety?  Meditation, yoga, exercise, time in nature etc?

Thanks @hayduke, yeah I meditate and practice mindfulness daily and exercise most days a week either going to the gym or cycling.  I used to do yoga but haven't done any for months, hopefully I'll get back to it soon.  Good luck with your own taper.

Day 9, slept well the last couple of nights, my volunteering is causing me some anxiety but I've done my homework and I'll just have to hope for the best (I'm seeing my first client tomorrow).

A bit stressed as I only got half my prescription today but made an appointment with a GP to hopefully get the rest.  I've felt dizzy a couple of times but can't be sure what's causing it, it may just be low blood sugar.  Mood is better than before I started tapering, so I'm happy.

Thanks @Greatful, I felt very numb and angry at times before tapering, duloxetine and olanzapine, plus I've been doing a lot of self care since I attended a resilience group back in the spring.  Hope you had a good day

Day 11, my volunteering went well yesterday and I'm feeling much more positive about it.  Had a good day today, lunch with a friend and another friend popped in.  Going for a day trip tomorrow on the train, looking forward to it.

Day 13 a bit bored today, slept late, did some cleaning, a friend helped and we pigged out on caramel lattes and scones.  It was pretty miserable out so didn't get any exercise and spent most of the day being a couch potato.

@Greatfulit was good but it pissed down (rained) all afternoon.  I went with someone I met on a dating site but I've decided I'm just looking for friendship.

I weighed out my first 40mg tablet tonight, I was worried I didn't have enough poison (meds) but weighing it was easier than I thought, hopefully I haven't messed up the maths DOH

Day 17 duloxetine taper, didn't sleep well last night as my volunteering was causing me quite a bit of stress but overall I've had a really good day and thinking of making another drop soon.

I read that it's better to drop activating drugs first ie the duloxetine but so far I've found the olanzapine taper harder and wonder whether I should tackle it first? Any advice would be appreciated, thanks.

Day 18 - Had a good day, enjoyed my cycling and cappachinos, not to mention the muffin!

Decided to continue with the duloxetine taper, going down to 35 mg tomorrow.  I've been doing fairly large drops up until now but I'm only going down 12.5 % tomorrow as I believe it gets harder the closer you get to zero.

Thanks @Greatful for your concern.  I, like everyone else, want to come off these horrible drugs as quickly as possible but I am trying to do it quickly but (hopefully) as safely as I can.  I guess I think if things go pear shaped, I can reinstate within the 2 week window.   Much as I've been quite grumpy/had off days when tapering, I'm also feeling so much more like my old self.  I'm more patient, empathetic and generally less self absorbed than when I was doped up to the eyeballs on drugs.  And although I'm sad I've wasted 18 years on them, I just want to look to the future and regain my personality and a new life (and sue the arse off them lol!)

Day 1 on 35 mg Duloxetine, didn't sleep well but didn't get up til late, so it was fine.  Had a good day, a friend helped me do some housework and I cooked her her dinner.  It was lovely and sunny so we sat on the patio drinking juice/lattes.  I've just realised why I struggled with the Olanzapine taper, I dropped about 25% from 10 mg to 7.5 mg which is obviously why I found it so difficult.  So far, touch wood, the duloxetine taper hasn't been too bad (hope I'm not tempting fate).

Yeah, you'll notice a drop like that.  I was ok with it, but I'd been steady on 10mg for some time and had no other prescriptions.  You have another taper in the picture as well.  Why not try a 10% cut of it once you're ready steady again, and see if that feels like an easier step down.
