da2021: Surviving medications in lancashire

Hello,

 

I'm a mum to my 13 year old son and have gradually come to the conclusion, that most of my health complications are due to psychiatric drugs, most recently sertraline.

 

I've been prescribed a few in the last 10 years and no one warned me of dependency issues. I found out by researching with other patients.

 

 

I would like to be able to give up work but can't afford to. I have been gradually reducing since 2020 and have been having terrible low mood including hormonal issues.  Sadly I feel the medication has caused fertility issues now and I'm now 44 so it's too late.

 

I hope this group helps me to keep going as the last few days have been awful.

 

I have watched and researched into psychiatric withdrawal and how to taper and really want to come off this drug. I've had lots of therapy which helped but the medications caused far too many side effects.

Before we can answer your questions, please give us more information first. 

 

Can you please give us specific information about your drug history for all drugs you are on and have been on, especially for the past 18-24 months?  It would be especially helpful to have the details of your drugs in a concise list (no symptoms), only drug names, specific dates (as best you can say for example early March if you don't recall the day) and dosages of each medication decrease or increase.  Please read the link below for instructions.  This will allow us to give you the best guidance.

1993    Anafranil (Clomipramine) for a few months. Later in 1993 Paxil for a few months 1993- 2006      No medication

2006   Effexor, Cymbalta, some Benzo’s. All for short periods. Later in 2006 Lexapro (escitalopram) 10 mg and shortly after Wellbutrin XR 150mg, against side effects Lexapro 

Since 2006 until end of 2015: Several times on and off Lexapro and Wellbutrin and several slight dosage changes. Mostly taken dosages: 5mg Lexapro and 150mg Wellbutrin

2016  Dosage change Lexapro from 5mg to 2,5 mg. Wellbutrin stayed om 150mg

November 2016 – April 2017 Down from 2,5mg to 0,6mg Lexapro (in steps) without much problems. Wellbutrin down from 150mg to 66mg. Also without much problems.

April 2017 – March 2019       Lexapro 0,6 mg        April 2017 - August 2018       Wellbutrin in small steps down from 66mg in to 37,5 mg . Quite heavy WD after each step.

March 2019 – May 2019 Lexapro down from 0,6 to 0,3mg then Prozac to 0,6 mg switch because severe discontinuation effects (may also have been from Wellbutrin..)    

Wellbutrin down from 37,5mg to 35,3mg 

October 2019        Seroquel 12,5 mg for 4 weeks because of extreme sleeping problems, then weaning off in 2 weeks       Prozac up dosage to 1,2 mg

March 2020     Wellbutrin in 2 steps down from 35,3mg to 33,3mg   Extreme withdrawal affects during 8 months. Stopped tapering Wellbutrin  until total off Prozac. February 2020 – November 2020   Prozac down in steps from 1,2mg to 0,57mg. 

Jan 2021 Prozac down to:  0,55> 0,53>0,51mg,   Feb 0,47mg ,  Mar 0,42mg,   Apr 0,37, longer hold because of WD symptoms July 0,36 and hold again

 

Supplements: Fish Oil (3000mg), Magnesium 100 mg, 2 drops of Lavender Oil when feeling anxiety. 50mg of L-Theanine when severe discontinuation effects caused by Wellbutrin

I have completed my prescribing history in the signiture.

 

I have been reducing my 10% every 4-6 weeks. I have been gradulating and mixing with water with a syringe to achieve the reduction.

 

I was prescribed Sertraline in 2019 after experiencing side effects from Venlafaxine 225mg. I remember a fast taper off Venlaxine and introduction to Sertraline which was an error by the GP I'd say. I can't remember how fast but I'd say about 4-6 weeks swap over.

 

 

I have become very depressed again now I've got down to 100mg but I do not want to stay on this medication. It's caused IBS, menstruation issues, frequent erratic periods during withdrawal, general anxiety, sleep disturbance, restless legs, cognitive impairment, loss of libido, emotional numbing. A long list. The last GP I saw told me Sertraline is not available as liquid from my CCG and said I can be off them in 6 months by alternating one lower one day and one higher the next.......hopeless, hence why I've done it via disolving in water. The pharmacy refused to help and said I can reduce as long as possible in tablet.

You might have benefit from a period of stabilizing on the current dosage. Sometimes our body and mind need a pause/break to settle down from all changes and tapering on meds. 

 

I do understand that you want to quit. But quitting on this dosage or a very fast taper would most probably make everything worse.

 

So I would stabilize until feeling better and then continue tapering. 10 % pr month can be too fast sometimes. We need to listen to our body. When WD symptoms get too extreme we need to hold and/or go slower.

I have paused my taper after a rough patch.

 

I wanted to ask how people knew when to continue the 10% reduction every 4-6 weeks as I feel I'm stuck on 100mg of sertraline. I want to be off it or on a minimal dose. If I'm always going to feel like this surely I may aswell continue reducing....??

Have you noticed any improvements in how you felt last month? Please also mention when it were just moments.

 

Healing comes in the pattern of waves and windows as you can read in a link in an earlier post. 

 

When withdrawal symptoms are too extreme it is best to make a hold in tapering until symptoms go or at least become bearable.

The symptoms have become bearable. I was bed bound for a few days last month but I've been able to go back to work.

 

 

I wanted to try going down to 90mg next and to see it helps making reduction  every 8 weeks??

I've not reintroduced magnesium as yet

If you are feeling better, you could try to go down max 10%. You could also go down 2,5 % every week (during 4 weeks) and make a hold then of 2 weeks. As in the Brasmonkey Slide Method I informed you about before. 

 

This is a even more gentle method of tapering. And when symptoms become too much, make a longer hold.

I divide a 50mg in half to get 25 however to get lower I grind and use water to get my own liquid.

 

 

Would 1000mg omega be ok for can I increase it? I have a pretty good diet however this medication makes you feel like it's striipping your body of whatever you put in. It's like having a car with a fuel leak.....

1993    Anafranil (Clomipramine) for a few months. Later in 1993 Paxil for a few months 1993- 2006      No medication

2006   Effexor, Cymbalta, some Benzo’s. All for short periods. Later in 2006 Lexapro (escitalopram) 10 mg and shortly after Wellbutrin XR 150mg, against side effects Lexapro 

Since 2006 until end of 2015: Several times on and off Lexapro and Wellbutrin and several slight dosage changes. Mostly taken dosages: 5mg Lexapro and 150mg Wellbutrin

2016  Dosage change Lexapro from 5mg to 2,5 mg. Wellbutrin stayed om 150mg

November 2016 – April 2017 Down from 2,5mg to 0,6mg Lexapro (in steps) without much problems. Wellbutrin down from 150mg to 66mg. Also without much problems.

April 2017 – March 2019       Lexapro 0,6 mg        April 2017 - August 2018       Wellbutrin in small steps down from 66mg in to 37,5 mg . Quite heavy WD after each step.

March 2019 – May 2019 Lexapro down from 0,6 to 0,3mg then Prozac to 0,6 mg switch because severe discontinuation effects (may also have been from Wellbutrin..)    

Wellbutrin down from 37,5mg to 35,3mg 

October 2019        Seroquel 12,5 mg for 4 weeks because of extreme sleeping problems, then weaning off in 2 weeks       Prozac up dosage to 1,2 mg

March 2020     Wellbutrin in 2 steps down from 35,3mg to 33,3mg   Extreme withdrawal affects during 8 months. Stopped tapering Wellbutrin  until total off Prozac. February 2020 – November 2020   Prozac down in steps from 1,2mg to 0,57mg. 

Jan 2021 Prozac down to:  0,55> 0,53>0,51mg,   Feb 0,47mg ,  Mar 0,42mg,   Apr 0,37, longer hold because of WD symptoms July 0,36 and hold again

Hi Da,

 

Can you update your signature line with your dosing history this past year for the Sertraline?

I'm trying to see the relationship between how your dropping both overall and in particular.

What were-are the symptoms currently on the Sertraline that You want to reduce?

You won't have to feel poorly all the time.

I think you may have just gone too fast over all the past year and-or maybe 10% is too much for you at any given drop.

 

The 10% every 4-6 weeks is best case scenario, and a maximum.

It is not something to aim for repeatedly all year long, every drop.

I would wait at least 2 months from the time you were well enough to be up and out of bed again, and then start with a maximum of 5% to test.

 

Rushing will only take longer in the end to get off the med, and you will just be sicker than whatever side effects are making you want to go off.

 

But filling out your WD history a bit, the problems from each drug, and also a little more detail on the symptoms you had that made you bed bound will help formulate a good plan moving forward.

I have updated my signiture, check my medical records however I cant remember what advise GP gave from swapping drugs. I came off Venlafaxine because I was experiencing brain zaps / jerks while I was sleeping. Horrendous side effects.....

Yes, your Venlafaxine at 225mgs was a high dose, it's no wonder you had side effects climbing up to that dose.

 

I see you already made a drop this month?

 Can you please put the date of that drop in your signature line as well?

It makes a big difference going forward watching and helping your taper in real time to know how many weeks your looking at in between drops.

 

Is this your first drop since May?

This is my first drop since May. I got really ill in August and had to take time off work.

 

 

The reason I was put on that drug was I'd tried ssri drugs and I've always been in some kind of talking therapy

 

 

My brother ended his life in January 2020. I blame that on poor medication prescribing and there is no wonder I am scared of reducing lower. Now I'm back on an SSRI but haven't been able to tolerate numbing, restless legs, no libido, the list goes on, apathy is the worst just not caring or having desire in life. Anyone relate?

 

More significant weird stuff like cognitive functioning, slow to process into and very very easily overwhelmed with information, sensor stimulus, lighting, sounds, high anxiety etc. Very odd feeling and stuff I've never had. Maybe I have just haven't been tuned in to my body. I feel bad because perhaps I should of addressed this sooner. I've tried to challenge GPs but it's made me more anxious 

 

One of the biggest challenges has been maintaining my job, I've had to reduce my job. I don't feel it's the right job for me in my present circumstances but I can't not work. Id be even more distressed.

 

 

The problems in August were very low mood and chronic fatigue where I couldn't get out of bed much..... but I think some of my symptoms are perimenopausal given my age. Just to the complicate the matter.

 

I feel I'm out of control of my own body. Am I the only one?

Your symptoms can be either too high a dose or adverse reaction to the med altogether.

Either way it's good that you have begun the taper.

 

If this was your first drop since May, and it was 5% from 100 to 95, hopefully it will be ok.

Please put in your signature line the ACTUAL DAY of this month you began the drop.

This will make it easier as time goes by to know exactly how long in between drops.

 

Then, you may want to keep a notebook of differing symptoms that may come since this drop over the next few weeks to a month.

That will help gauge how your doing.

 

As GZero said, the little slump in August could be the cumulative total catching you with you as everything else was going on as well.

Add that to the hormones beginning to flux.

 

If the symptoms are adverse reaction, they may show more right after dosing.

So a notebook  recording of the time you take the med and when what symptoms start may help going forward to know how to proceed.

 

Usually we ask people to post daily notes in their thread with the times they take the med.

Some members are on more than one med and it can get tricky figuring out what symptom is what

However you may be able to see a pattern yourself, if you are worse with certain issues an hour or 2 right after dosing.

 

If you keep a daily log for the next month, you may pick up on something useful going forward.

You may also piece together what it is your doing at the time symptoms spike, that may or may not be related.

I had most of these, as you say it was like, a sensory over load, as if I had become autistic.

I found it was worse for me under fluorescent lighting as they turn on and off a hundred times a minute.

My sensory overload was also around too many people talking, and it was worse in the afternoons.

By later in the evening I was fine again.

I take 50mg AM and 45mg dissolved in water at bedtime.

 

I will update 95mg current dose as 17 Sept.

 

Since reducing to 95mg, I no longer have restless legs or jerks.

Well that is good news the leg jerks are starting to let up.

If you are split dosing 2 times a day, also put the dose amounts and times of day in your signature

This will help going forward connecting side effects with dose amount and timing.

 

I am not sure but it seems are taking a full 50 mg tablet in the am and liquid partial dose in the pm?

At some point going forward you will want to keep those dosages as close to equal as possible to avoid interdose wd.

Meaning for your next drop it is better to go to 45mgs twice a day and not 50 am-40pm.

Symptoms can be as much a result of inconsistent dosing as dropping too much too fast.

Yes I wondered if I can use cordial to drink the dissolved tablet. It tastes absolutely foul in water.  I direct the syringe to back of my throat but it takes disgusting!

 

 

Will cordial change the formula of the drug much?

 

I use a syringe to take 45ml at bedtime and a 50mg tablet in the morning.

 

Are you saying it's best to take all of the 95ml at bedtime?

No, I meant that it can be important to take near the same about if the reason you are split dosing is to avoid interdose wd or your plasma levels fluctuating. This med however, has a half life estimate from anywhere from 22 to 36 hours.

Most seem to fall in the 26 hour range so you MAY be ok in taking it all once a day.

I suggest checking with the pharmacist to find out what the half life estimate is for your particular brand.

 

Obviously, for ease of dosing it would be nice to only have to do it once a day but there may be some symptoms from interdose wd if you are a fast metabolizer and it is all out of your system in only 22 hours. If you do decide to switch, I would move the dose one hour a day either forward or back to the other dose. So if you want to take it at night, move the morning pill one hour later until it matches the night dose.  If you notice a symptom spike, than stop and let your system settle before moving the dose and then move it only one half hour later in the day again.

 

I don't know what your specifically referring to by "cordial" but I would not recommend anything but distilled water.

You don't know what effect other ingredients are having on the active ingredient if your saving solution from dose to dose. day to day.

Firstly what is plasma levels?

 

I don't mind taking entire dose at night. To make 95mg it would be a 50mg and 45 as liquid unless I can get a box of 25mgs and liquid for the rest

 

I meant a fruit juice to sweeten the dissolved tablet in tap water 

 

Why is tap water not good enough as you'd be taking that anyway with a tablet form?

 

Understandably I'd like to make the process as straight forward as possible to get down to a comfortable dose.

 

The pharmacy have stock issues now with 25mg tablets

This needs to be clarified as I don't understand?

If your med has a short half life it leaves your system faster and can cause symptoms until your next dose.

As long as your med has a long half life you can take it once a day.

 

However, it's still recommended to move the one dose slowly over time to avoid any symptoms you may have adjusting to the new time schedule.

So if your taking the whole pill at 8 am, take it later an hour a day each day until your arrive at the same time as the night dose.

If anywhere along the way  your symptoms get a little worse, hold the times where you are taking both until you feel better and then move forward again at a slower rate of either half hour a day or 1 hour every 2 to 3 days.

 

Getting some of the 25mgs is great, since if you only have 50s you will have to make liquid dosages all of the time unless you are at 50mgs.

If you can get some 25's as well then once you have dosed down to 75, 50, and 25mgs you can remain at those levels for a longer hold with ease.

 

Cordial in my area implies alcohol, that is why I asked.

Distilled water usually has a constant ph and mineral level.

Some use tap water but you want to try to avoid anything that might interfere with your liquids stability over time.

Each med is different so what may affect one may not effect another and its generally best to avoid any reason for doing poorly.

I don't want to keep badgering about something you may not understand the importance of right yet, but having your med schedules with times and dosages and dates of drops is important going forward for many reasons. If you can please do this, it would be appreciated. It's much easier to ascertain why someone is not doing well going forward if we are not guessing as to how the new symptoms revolve around the med schedule.
