JessB: feeling stuck tapering Effexor

Hello! 
I have been taking Effexor for quite a few years. I wanted to start to try to get pregnant with my husband and would like off the medication before trying to conceive. So I began my own research. I came across shocking information about withdrawals! I landed upon a Facebook group for tapering off Effexor and began my journey tapering (see signature) 5% drops every 4 weeks. 
after reinstating a small amount after a regular 5% decrease (counting beads). I experienced extreme symptoms. Usually I would deal with some nausea and dizziness for a few days after a drop, but this was much more. Trembling, crying, depressed, extreme anxiety and fear, unable to eat. It was so traumatic! Lasting several weeks gradually getting better. So I reinstated 10 beads and leveled out. And experienced a few bad waves monthly until finally levelling out. I have since been good. Functional. 
I have been able to drop a bead at a time. But at this rate I’ll never come off. I feel stuck. I feel like I won’t be able to ever get off this medication. I don’t know what to do or where to turn. Do I attempt larger drops? Do I give up? I feel so stuck! Each bead drop, I’m ok, generally some brain fog, maybe some heightened anxiety. But I fear going to fast and getting sick again. Any help or guidance would be so appreciated! 
I take 200mg magnesium, 1000mg omega 3 and tumeric.

Can you please give us specific information about your drug history for all drugs you are on and have been on, especially for the past 18-24 months?  It would be especially helpful to have the details of your drugs in a concise list (no symptoms), only drug names, specific dates (as best you can say for example early March if you don't recall the day) and dosages of each medication decrease or increase.  Please read the link below for instructions.  This will allow us to give you the best guidance.

So sorry to read what you are going through. You have been doing a very well job in tapering so far! Gladly enough it seems that you did everything OK and did not make things worse by changing meds or going up and down in dosages often.

 

You should know that Effexor is a SSRI that becomes more and more difficult to taper when we reach certain dosage levels. Most often it becomes more difficult when we reach 35mg and go further down in dosage.

 

Before we can give you a good advise how to continue, we need to know what dosage you are taking now. When you know or count the total number of beads in 1 pill and you know how many beads you are taking daily, you can calculate the current daily dosage. Of course you/we also need to know the dosage of the capsules your are using.

1993    Anafranil (Clomipramine) for a few months. Later in 1993 Paxil for a few months 1993- 2006      No medication

2006   Effexor, Cymbalta, some Benzo’s. All for short periods. Later in 2006 Lexapro (escitalopram) 10 mg and shortly after Wellbutrin XR 150mg, against side effects Lexapro 

Since 2006 until end of 2015: Several times on and off Lexapro and Wellbutrin and several slight dosage changes. Mostly taken dosages: 5mg Lexapro and 150mg Wellbutrin

2016  Dosage change Lexapro from 5mg to 2,5 mg. Wellbutrin stayed om 150mg

November 2016 – April 2017 Down from 2,5mg to 0,6mg Lexapro (in steps) without much problems. Wellbutrin down from 150mg to 66mg. Also without much problems.

April 2017 – March 2019       Lexapro 0,6 mg        April 2017 - August 2018       Wellbutrin in small steps down from 66mg in to 37,5 mg . Quite heavy WD after each step.

March 2019 – May 2019 Lexapro down from 0,6 to 0,3mg then Prozac to 0,6 mg switch because severe discontinuation effects (may also have been from Wellbutrin..)    

Wellbutrin down from 37,5mg to 35,3mg 

October 2019        Seroquel 12,5 mg for 4 weeks because of extreme sleeping problems, then weaning off in 2 weeks       Prozac up dosage to 1,2 mg

March 2020     Wellbutrin in 2 steps down from 35,3mg to 33,3mg   Extreme withdrawal effects during 8 months. Stopped tapering Wellbutrin  until total off Prozac. 

February 2020 – November 2020   Prozac down in steps from 1,2mg to 0,57mg. 

Jan 2021  Prozac down to:  0,55> 0,53>0,51mg,   Feb 0,47mg ,  Mar 0,42mg,   Apr 0,37, longer hold because of WD symptoms July 0,36 and hold again, Sept 19 0,35, Sept 26 0,34mg, Oct 3 0,33mg  Long hold of 172 days until March 2022

January 20, 2022:  Wellbutrin from 33,3 to 32,3mg

Strange! I thought I did this lol! I will go back and redo. 
*effexor since 2013- 75 mg (260 beads)

*march -December 2020 decrease 5% by counting beads and  holding exactly 4 weeks in between. Only experienced moderate nausea and dizziness for a few days. 

*December 2020 decreased 145 beads -140 beads. 
*experienced severe symptoms, nausea, diahreah, weeping, depression, anxiety, fear, trembles, a buzzing sensation. 

*3 days of symptoms I increased  5 beads - some relief

*2 days later I increased 5 more beads  some more relief

*Held here for 5 months. (150 beads 

one bead decrease every 4-5 weeks since may 2021. Currently at 145 beads. 
😭 I so want to go quicker I so want to get off faster. I feel so stuck and damaged and I have no idea where to go. 
I should repeat, I have been feeling good recently, probably about 4-5 months of hard waves and around may 2021 I finally stabilized and have been doing good. When I decrease a bead I do get brain fog, headaches and anxiety. But I use all my coping mechanisms! I just want to go quicker this rate is driving me nuts!

I understand that you want to go faster. A lot of members (including me) know this frustration. But the faster we go, the more WD symptoms we may get and the longer the whole process may take.

 

In the end is going slow the fastest way of getting of these meds. And unfortunately this can take years for many people. Others can be lucky an get away quite easily from these meds. 

 

It is good what you did. To hold for 4-5 months to stabilize. Actually you have been doing very very good until here! 

 

When you go 1 bead down, how many does it take before WD symptoms start? And how long do they last then?

I know all have this frustration, I’m feeling like “one bead really?!” One little bead! I’m just too nervous to do more than one and ruin what I’ve accomplished and have to heal all over  again! 
so usually, the day of I’m wreaked with anxiety, a feeling of waiting for withdrawal symptoms. I’m always waiting for them to come back. Physical w/d symptoms will start days 2/3. And it’s usually heightened anxiety, ruminating thoughts, some headaches, and some brain fog. For me it’s the anxiety that is the worst. CBD helps a lot but I don’t want to be dependent on it.

Don't be frustrated, don't blame yourself, it's not you! Your nervous system just needs more time before you can embark on the next stage of your tapering. Quite often, people need to take a break from tapering for a while.

Thank you altostrata,

hopeful I just need this break, and will be able to continue at a better rate soon! I just feel like maybe I damaged myself, played with fire if you will, and got burned and now this is how it will be for the rest of the taper. Which is infuriating, I would take the moderate nausea I was getting at 5% decreases over this.. I guess I’m reaching out to you all to see where can I go from here? Do I have any other options? Can I do anything more to heal? Heal my CNS? Do I try more of a decrease? What did I do wrong and how can I prevent this in the future? Am I further hurting myself doing 1 bead tapers? I just feel so confused. I feel like I failed. And like I’ll never have baby at this rate. (as I will need fertility drugs to get pregnant) 

 

 

My last taper is July 2nd. And was going to start another tomorrow.

If I were you, I would extend your taper interval to 6 weeks, and see if that's enough.

Hi again! I tried a 1 bead taper and it didn’t go well. Day 1 I had a debilitating headache and was on the couch for hours with ice packs. I also felt off, just strange. Due to work commitments I went back to 146 beads the very next day. I obviously need to hold longer. Maybe not the best decision but along with the anxiety I was having it made sense at the time. I definitely need to hold longer. I’ve had on and off headaches since. I’m not sure if this is a wave. 
my plan is to hold a few more weeks, and try again. However, I feel pretty defeated.

Hi JessB,

 

Hold on. Lots of us come to a point where withdrawal is that heavy that it seems that we cannot go further down in dosage. As Altostrata suggested it is a very good thing then to make a LONGER hold. 

 

I am doing the same at the moment. I am on a hold for 5 months already. I noticed WD symptoms became worse. And my red flag was upcoming insomnia. I knew that continue tapering would make it worse. Brassmonkey gave me the advise for a serious longer hold and I am very grateful for that. Now after 5 months my sleep is getting better. The burning feeling in my legs and arms is slowly getting less, headaches are less frequent, my eyesight has improved.

 

These are all signs for healing. I will continue at least one more month with this hold. When WD symptoms are still there, I will even give it more time. 

 

Please understand that fast tapering takes more time most often to get of these meds, than slow and careful tapering.....

 

If I were you I would not change my dosage for several months.... See what that brings and when you start feeling better you can give it a try again for further tapering.

So happy to share my thoughts... Also understand that stress is very bad for people in withdrawal. This can give firm setbacks. So good to hold, relax and get a different mindset than only tapering....

I didn't see any of the other mods mention this yet and it's missing from your signature but CBD can act paradoxical for some people and actually make you worse after the short amount of time you think it's given you some relief. That's not to say the wd isn't your most significant problem, but once we are destabilized anything such as caffeine or alcohol can throw us for a loop and make our symptoms more dire.  Even over the counter meds like an anti histamine. 

 

Was there anything else going on at the time of December 2020 when you seemed to get really bad?

A lot of excessive alcohol consumption at the holidays?

Were you on a course of anti-biotics for an illness or something?

Did you start any new prescription medications or over the counter ones of any category?

It’s interesting that you mention this, because I never put the 2 together. I was not drinking, however started to dabble with THC and CBD at that time. I never noticed anything but relief from CBD, however I was starting to try some THC to help with the moderate nausea feeling I always had for a few days after. That day specifically I went for a hike and smoked some. It was like night and day, I was totally fine in the morning and when I got home from the hike over the course of hours it started to show. And progressed for days after until I reinstated. Since reinstating the 10% I have only used CBD in small amounts by smoking low - 0 % THC and high CBD for anxiety. Which helps tremendously and makes me sleepy. 
I also take rupall seasonally and inhalers. I have to take the rupall (antihistamine) as my severe seasonal allergies trigger my asthma and wind me up in the hospital needing prednisone annually. (Taking this around may-sept)
I did not think something natural would tip me over so severely. It’s been 8 months and I can’t taper 1 bead. 
Also recently in July I was on a round of antibiotics and then a round of antivirals for seperate issues and now I’m wondering if this contributes to my most recent failed taper. 
Alot to think about now,  Thank you! I just wish I knew where to go from here.

Also to add - I was not taking any supplements until that time because of the symptoms everyone on the Facebook page encouraged me to start supplements. Now I cannot stop taking them or all of the symptoms come back. I tried with the magnesium, tapering off of it by dumping some out and led to crying spells and anxiety.

I’m so sorry to do this, but 2ndly to add again more

(See, you all have given me a lot to think about already 😛)  

At the beginning of these symptoms January / February I could not have caffeine, I tried , but it made me shaky and super anxious. I cut caffeine completely out. And could not for months. Only recently (June -ish) have I been able to tolerate small amounts of caffeine (and boy did I miss it 😢). I have always just chalked this up to healing, I have healed more since January to be able to enjoy my tea again, but now realizing this may be setting me back. However, I do not notice any side effects from it anymore. Thoughts? 
I just want to also express my total gratitude to you all for replying to me, in desperate circumstances I have been feeling, stuck, defeated, desperate for answers, you make me feel less alone and heard! So thank you!

Wow, there's a lot there to think about!

 

Obviously, you need to breathe for the asthma.

Did you take this, rupall, and the inhalers last year during the taper from March to December without any trouble?

 

We support magnesium for anxiety help if your able to tolerate it, so if that was helping your anxiety, continue to take what you were.

Is this when the anxiety started, the end of 2020, and is that the reason you tried these 2?

I'm trying to see where the anxiety came into the picture, and you didn't explicitly say WHY you started to "dabble", what were you treating?

If it did, it may have been early signs of your taper being too fast and needing a hold for a few months.

Oh, dear, possibly, for a few different reasons.

First, What drugs did you take? Some are more problematic than others.

 

Also, trying to taper while sick is a No-No.

Your body is already "fighting" something and a drop in med is another major body stressor, so the symptoms are your bodies was of politely telling you that it's already busy trying to heal, please hold up a bit on any more brain chemistry changes.

 

If each new drop of one bead is fighting you, than the caffeine probably has to go until your symptom stable and recovered from all these illnesses and the TBC and CBC issue is sorted out. Depending on why you were taking them, they may need to go as well, I'm still trying to get a fuller picture on why you were adding things when.

yes I took both rupall and inhalers with no issues last year. I was starting to smoke THC and CBD as it was helping a lot with the nausea.  I would say I started around October, and fell ill december 1. 

The antivirals were valtrex, and the antibiotic was ammoxicilian. I took the antibiotics around tge 18th-24th July and the antivirals around the 8-14th of august and tried to taper the 21st. 
 

the anxiety started the day I got sick, december 1, and got incredibly worse for days until I reinstated a few beads. And lasted about a month +/-, and then comes in waves now. 

You could say I had a pretty flawless taper from March-December, only having moderate nausea for the first 4-7 days after a taper.

(Ok, a few possibilities, nausea may just be a symptom you will have to deal with each drop, or your taper may have not had enough longer holds in between drops and over time  the nausea was a possible symptom of you needing either a longer break or more weeks inbetween drops or that even the 5% all at once was too much. At the dose you were at, i think it was probably one of the first 2.

Then you tried to treat with other items that can make you worse. 

Now, nausea can come at any drop, but it can be a sign too much is being dropped all at once or the % is ok but a taper is too fast over time.)

Ok, yes, tapering while fighting 2 different back to back monthly infections would definitely hit you hard.

Tapering, even done properly so symptoms are slight, is still a major function change in your body, it's still working harder than you realize

Wether either of these drugs are know to be problematic, maybe one of the mods who know more about the valtex may be able to chime in.

 

The good news from my thinking is there isn't anything that time can rest can't smooth over.

You haven't made either of the 2 big mistakes of over reinstating at too high a dose or starting a 2nd drug, so that's good.

 

I think it was just the usual thing I see alot of, that people start making drops every month, and somewhere around the 6-10 month mark they start showing signs its catching up to them and they try to self medicate the symptom in others ways, not realizing that is what can make them worse.

 

There seems to be a cumulative number of monthly drops many people can only tolerate both  per 6 months, and yearly.

People can tolerate a certain % drop for just so many months and then they need to stop and rest.

Sometimes it's an issue of they are dropping too much, sometimes the brain just need you to STOP adjusting it every month and give it a 2-3 month hold.

Add illness and or injury and it's a recipe for severe symptomatic issues.

 

Each persons taper rate is different, I found my "sweet spot" was 5% every TWO months.  My brain just needed more time to settle.

Once your ready to begin tapering again, you may want to try 5% every 6 weeks and see how that goes.

 

If the nausea is really too much, then you may want to do a smaller drop of 2.5% every 3 weeks, that may be enough.

But I would still recommend a few longer holds periodically.

Constant dose tampering past the 6-8 month period is problematic for some.

Think ahead and you can see when it would be a good time for an extended break: vacations, holiday, etc..

 

I would not recommend continuing the to smoke THC and CBD.

It's a Russian roulette and ignoring the nausea was a sign you may have needed a 2-3 month hold when October-November came or you needed to drop the taper rate.

Stopping a taper is the safer, healthier and quickest way to resolve symptoms and get you "healed" enough to begin it at a later date.

 

I would not recommend any new drops until 30-60 days after recovering from an illness or massive injury, like a broken leg, etc, Dependent on severity.

At this point, you just may need an extended hold for 3-6 months and remove the other items out.

The magnesium and omega 3's are all that we recommend.

 

We have a thread on anti biotics you will want to look at for future reference, the Fluoroquinolones are very problematic for many in WD.

Wow Colonial! I have learned so much! Thank you! I feel like a lot of my questions have been answered, and have been given alot of insight to what is going on. 
I do believe you are right I tried to self medicate and it went sideways for me. At the time I was doing research into cancer patients who were using CBD and THC to medicate their nausea  (obviously completely different situations) and thought to give it a go. 
I am going to hold alot longer, this isn’t a race. As though sometimes I feel it is, and Feel like I’m failing at it, I would much rather be able to continue my life and enjoy day to day, than suffer again now that I am functioning and having big long windows of being symptom free. 
I am definitely now chalking this most recent failed taper to the 2 medications I was on and then tried to taper a week later. 
It is so nice to be able to reach out and have someone understand me. 
I am very interested in how people usually go 6-10 months and it catches up to them. That is actually quite fascinating I haven’t heard that before. Also I likely contributing factors in how I feel. Stuck is the best way I can describe how I feel sitting at 145 beads (55% of my original dose) but I am willing to take this stuck feeling and change my mindset in the mean time and hold longer.  Tapering is all I think about and research about, it’s always on my mind and I stress myself out consistently thinking about it. Maybe I need another break!

For certain people there is only a maximum amount we can drop during certain extended intervals along the way regardless of what our maximum for any one drop can be and if we rush that, we have trouble.

 

For example, there was a maximum amount I could drop any specific time, a total amount I could drop in any given 6 month period (which was about equal to my individual max times 5), and a total per year, which was about 9.

 

So if my total per drop for any 1 time was 10% of my current dose my total per 6 months would be equal to 5 ten percent drops, and over a year it may be about 9 ten percent drops. I found it was not just about what is 10% of my current amount, but an over all percentage drop over time had to be respected as well.

 

It's sort of related to how much of a med you CAN take over extended periods. My bottle of Tylenol says I can take a dose every 4 hours, but NOT more than 4 dosages a day, and while I can take 4 dosages on any given day, not to do that for more than a total of 4 days a week. Over time, you cannot keep taking the same amount of Tylenol every 4 hours every day or you end up in liver failure.

 

I've noticed making a WD taper plan is a bit like that, but in reverse.

Over extended periods of time a lesser cumulative amount needs to be respected or it catches up with us.

 

When we make a drop, it takes a certain amount of time to recover 100%.

This depends on numerous variables, % drooped, time, nutrition, stress, genetics, illness, etc.

 

What can happen, is we can make a drop again before we have recovered 100%, even though we may not even yet me symptomatic.

This is key: NOT HAVING SYMPTOMS DOES NOT MEAN THE BRAIN HAS RECOVERED 100% IN THE MONTH UNTIL YOU DROP AGAIN.

It may have recovered 85%

 

Over time, all those drops and variables add up, we cross a point and we become symptomatic.

IF we continue to push through, we eventually cross to a point of no return and crash to an extent that is horrific.

 

People often don't realize how a stressful day or life prolongs the amount of time needed for the brain to recover to that 100%

Even "happy stress" like getting a new puppy or a vacation.

Anything that takes your brain away from the work or healing delays the time, since it has to stop and deal with the anti biotic or caffeine, etc.

Fellow bead counter here!   I'm off of it now for several years, but I have a soft spot for anybody who has had to taper off of Effexor.  I thought I'd never be able to do it when I got near the end.  I talked to my Dr. and he got me smaller capsules so when I reduced a bead, it was a smaller bead than before and tolerable, which is certainly not the same as easy!   I took a lot of Omega 3s, which seemed to help.   Wishing you all the best!!!

Hi everyone! And thank you again colonel! Sorry for the delayed response- I have been taking a social media / internet break for my mental health! Everything makes so much sense! Thank you for explaining everything! I have still been holding. Currently been holding for 8 weeks and have been feeling really good! I plan to hold longer still. I had some medical tests done for hormones and everything is coming back really good! I haven’t had a female cycle since august 2020, and they are doing some exams to figure out why. I have been given a clean bill of health- and hopes for my future that getting pregnant will be relatively easy!  Woo! I just need to tackle the Effexor now! I have short term and long term goals! Firstly to get down to 37.5 to be able to get the next dose in pill form and hold for a long time! And of course long term to eventually come off completely safely and slowly. Counting beads weekly will not be feesable with a new born so I need to get down to a “clinical dose” or off completely- ideally off!

Hi everyone

i am now going on 12 weeks since my last drop  of 1 bead. I feel good for the most part. I get random headaches which can be explained by the weather -I get storm headaches- and it’s been particularly rainy the last little while here. When working or distracted I am well, when I have a minute to think about Effexor I can spiral myself into anxiety and depression. How I’ll always be this way and never heal or get passed this 1 bead at a time crap -lol.

I'm not against it but just remember we are going into Autumn and in Ontario you are loosing a lot of sunlight and your brain is making chemical changes to hibernate for the winter, so it's already making those adjustments.  I still get Wellbutrin symptoms every Autumn for a few weeks and its been years since I was off it. If you want to make a modest drop go for it but don't force too high of a percentage or you might just tip yourself off into symptoms from the "Autumn blues" synergistic effect.

I didn’t think of it that way! Holding these last 3 months have been great to my body and mine! I feel better every day! I wouldn’t try anything dirasric maybe 2 beads !

Sometime this week or weekend Can you add to your signature line the amount of beads you have been at for 3 months, the day that started and then what you will be taking now and when?   There's really no definite history there of how you've been dosing the past year.

It may not seem important now but going forward as your dose gets lover over time it will be beneficial to have that there.

I tried to stick with just the dose number and very little words.

 

It's one of the most precise and detailed taper registry's I've ever seen if I do say so myself.

At least I got one thing right in life.

 

Yes, if you've been at that modest of a taper 2 beads should not be a problem.

Even if you have symptoms I'd say their more seasonal than anything.

ive only ever done 1 beads since my crash last fall / winter.. I’m soooo nervous I will be taking 3 steps back! And go back to needing 8 months of healing. But of course feeling impatient and hard on myself to just jump and get on with it! 2 beads would be about 2% taper 2 % would be 2.9 beads! But I’ve been feeling so good I feel I could do this. I’m so torn! I hate this drug! 😩

@Colonial well I tried one bead and at the same time - I got my woman cycle, first time in 14 months… obviously a bad time to try a taper. 😩 just as the title I am so stuck !

The hormones can set off symptoms as well.

14 months, I don't know your age but did you have any blood work to see if you are you perimenopausal?

That alone making tapering and wd very difficult.

Are you only taking 1 bead a day now?
