ReddLedd: Mirtazapine Reinstatement help!

Recommended Posts

Hi @Frogie, just another update after a few days;

Sleep has been okay, about the same as before the RI tbh, getting to sleep okay, getting about 5-6 hours sleep and a little groggy in the morning. Days have been about the same as before also, mostly fine but the odd little flare up.
The one thing I’m noticing, is the panic episodes seem to be my major problem.
Im not sure the Mirtazapine at such a low dose will be able to address this, and it’s something that’s become a major issue that I’d like to sort out.
It’s now been 12 days since I started the RI, and have been on 2mg for 6 days so I’m considering tapering down if that’s safe?

I also realise that nobody here can give medical advice, but my doc wants to get me onto something else (an SSRI potentially) to sort out my panic episodes. I know this is risky but I’m just a bit lost as to how I inform myself in making a choice going forward.

i need to try and get it under control as it’s starting to rule my life

Yes I think at this point it would be safe to go ahead and taper off. Just taper down the same as you reinstated.

I can’t give medical advice but that is up to you and your dr on how you wish to proceed on this.

Keep me posted please.

PREVIOUS medications and discontinuations: Have been on medications since 1996.

Valium, Gabapentin, Lamictal, Prilosec and Zantac from 2000 to 2015 with a fast taper by a psychiatrist.

Liquid Lexapro Nov, 2016 to 31-March, 2019 Lexapro free!!! (total Lexapro taper was 4 years-started with pill form)

---CURRENT MEDICATIONS:Supplements:Milk Thistle, Metamucil, Magnesium Citrate, Vitamin D3, Levothyroxine 25mcg, Vitamin C, Krill oil.

Xanax 1mg 3x day June, 2000 to 19-September, 2020 Went from .150 grams (average weight of 1 Xanax) 3x day to .003 grams 3x day. April 1, 2021 went back on 1mg a day. Started tapering May 19, 2023. July 28, 2023-approximately .87mg. Dr. fast tapered me at the end and realized he messed up. Prescribe it again and I am doing "slower than a turtle" taper.

19-September, 2020 Xanax free!!! (total Xanax taper was 15-1/2 months-1-June, 2019-19-September, 2020)

Thanks @Frogie I certainly will.

Otherwise what is the option with the RI, just keep on updosing until 3mg and if that doesn’t work time to taper down anyway?

im assuming I wouldn’t just keep updosing until I get back to original dose?

You could updose to 3 mg and see if that helps. Sometime you might end up at a higher dose, we always try a much lower dose if possible.

Ill take 2mg tonight which makes 7 nights so far. I’ll have a think about it tomorrow during the day and weigh up whether I continue or not.
It’s so tough, some of the symptoms seem to be slightly better, but the anxiety and panic are extremely difficult to deal with and I need to address them somehow.

My sleep seems to be getting worse again on 2mg. If sleep is not stabilising or slowly improving could that be a sign that it may have pooped out or is it potentially all part of the process?

Each increase seems to yield a good sleep for the first night or two then slowly return me back to early waking after day 3 or 4.

I used to take melatonin before my RI, only 250ug, am I able to start taking that again now or later, or is that going to upset the apple cart?

You can try melatonin. Just try a small amount. I would start at no more that 0.3-0.5 mg.
