It could just be a wave that settles down in time. And it could be an uptick in symptoms due to battle fatigue. Feeling sick and rundown can make what we used to be able to handle much more difficult, even if the symptom itself really hasn't changed. It's like carrying a 10-pound weight at the beginning of a marathon. It's not that heavy the first few miles but by the 15th or the 20th mile, it feels like a ton. But it's still a 10-pound weight. 

 

Your sleep seems to be mostly in the 7 or 8 hour range, which is excellent. Your use of non-drug coping skills is also impressive. 

 

I would continue holding a bit longer and do as much self-care as you can and see if the symptoms settle down some.

I did the math, Shep, and since I went up to 18.5 mg of trazodone 11 days ago, I've been averaging a bit over 6.5 hours a night, with a 4 hour night thrown in. It's been deteriorating over the past few days though. So not terrible, but not my norm, that's for sure.

 

I'm applying the coping skills as best I can. It's really all I have to lean on right now.

@TreeFrogI'm not sure your sleep is getting worse. You're getting some 7 and 8 hour nights. It looks like you're stuck in a pattern of good night / bad night, which over time, can definitely make you feel like it's worse, especially if the sleep quality isn't good.

 

Please note that the good night / bad night pattern is extremely common during withdrawal. It's exhausting, but nothing out of the ordinary.

1/8 of 50 mg is 6.25 mg and you're also taking 6 mg in liquid, which totals 12.25 mg. But your signature and daily notes say you're taking 18.5 mg. Sorry, I may not be reading this right. Please clarify the total amount you're taking.

Sorry for the confusion...I am taking 1/8 of a 50 mg tablet, which is 6.25 mg, and about 6 ml of liquid. I made up the liquid in a 2:1 ratio, so there are 2 mg of trazodone in each ml. (A 50 mg tablet dissolved in 25 ml of water).

Thanks, TreeFrog. So 6.25 mg in tablet form and 6 mL liquid with 2 mg in each mL, which is 12 mg. So the total dose is 18.25 mg.

 

But you have 18.5 mg in your signature and daily notes (not 18.25 mg).

 

If you're really at 18.25 mg, please adjust your signature and going forward, please add this amount to your daily notes. This is important because your next reduction is a percentage drop off of what you're currently taking.

 

From your earlier post here, you were taking 1/4 the 50 mg tablet for 12.5 mg and 6 mg in liquid (3 mL), which did give you 18.5 mg in total. If you can remember when you switched to cutting your pill into eight pieces (6.25 mg) and increasing your liquid to 6 mL (12 mg), you'll find which date you inadvertently made a reduction from 18.5 mg to 18.25 mg.

 

It's only a 1.36% reduction, but it could explain some of the upticks in symptoms that you mention.

Hi TreeFrog,

 

I had been taking duloxetine for 10 years, and I stopped taking gradually (too fast though), and completely stopping in October 2021. There was nausea for a few days. Brain zaps lasted for a long time, but it does not disturb me much.

 

It was well until I got triggered by an insensitive remark that caused me to feel angry for a few weeks. I also had trouble sleeping, because of the anger. I was also sensitive to mild unfriendly comments. I had some nightmares too.

 

Due to sleep deprivation, anger and anxiety, it affected my work. When work issues arose because of my low performance, it compounded my emotional issues (anxiety, anger, etc.).

 

I talked to my therapist, and she suspected it was due to me stopping the drugs. I took it end of January again. The anxiety, anger subsided the following day; however, it caused insomnia, worse sleep problem than before. After being without the 2 drugs (duloxetine, olanzapine) for a few months, there was a personal satisfaction, so I was reluctant to take olanzapine to aid my sleep like before as I wanted to keep it at 1 drug (duloxetine instead of duloxetine and olanzapine). I continued with just duloxetine, which helped to stabilize my mood, but it caused too much sleep deprivation/insomnia, which after a long would make me moody. End of February, I decided to take olanzapine but a very small dose--it did not work, unfortunately. I increased it to the dose I had before--it did not help me anymore with my sleep.

 

After going through so much insomnia, I worried I would deteriorate my health of having about an average of 4 hours sleep; I decided to stop taking all drugs completely end of Februaury; however, to my shock, I could not sleep still--"What is happening to me? Will I never be able to sleep? If I continue like this, I may die of exhausation." For 2 days without drugs, it was like this until I got another drug, mirtazapine. How I got it though was by complaining that my GP suspected it is causing my high blood pressure. The original advice was initially to increase duloxetine dosage to 60 mg from 30 mg. The doctor somehow believes it should aid sleep.

 

For the first time in a long while, I could finally have the typical 7 hours sleep. However, after a long while, I realise I am generally anxious, suspecting to be the absence of duloxetine and olanzapine drugs. A few times the feelings were so intense and lasted for a long while, like how I felt before. I seem to be sensitive to minor issues.

 

Often I am tempted to experiment to reinstate the old drugs, but after reflecting it, since it caused insomnia, and a long time had passed since I last took it now, I concluded I cannot afford to be sick or experience new withdrawal symptoms because of my difficult work. I decided to manage my feelings or withdrawal effects instead.

 

The benefits I gain though, are lower blood pressure and no weight gain. However, I would prefer to have my old self when I was on these two drugs. I regret I did not know about withdrawal symptoms at that time.

Sep-Dec '12: Cymbalta (60mg), Zyprexa (1.25 mg), Lexappo (5 mg), Wellbutrin SR (150mg); Dec '12-Apr '13: Lithium Carbonate Tab (150mg), Wellbutrin SR (150mg), Rivotril (1mg); May '13-Feb '15: no record; Mar '15-Aug '21: Cymbalta (60mg), Seroquel XR (50mg), Rivotril Tab (0.5mg); Jul-Sep '21: switched from private to government psychiatrist who changed my drugs to Cymbalta (60mg), Quentiapine (50mg)(IR); however, Quentiapine (50mg)(IR) led me to poor sleep; Oct '21: Due to poor sleep, I requested Cymbalta (60mg), Olanzapine (2.5mg), Rivotril Tab (0.5mg, when needed)

Critial Stage: Nov '21-Jan '22: ceased drugs voluntarily in Nov '21. No major issue an offensive remark triggered my anger. Anger was intense and lasted unusually long. End of Jan-Feb '22: Doctor reinstated me with Cymbalta (30mg) only on 29 Jan '22. However; without Olanzapine (2.5mg), it caused poor sleep; End of Feb-Jun '22: after too much poor sleep, doctor replaced Cymbalta (30 mg) with Remeron (15 mg); Jul '22: attempted to tapper off slowly by 10% of last dose. It resulted in weird dreams and poor sleep; Aug-17 Nov '22: Resumed exactly 15 mg. My sleep restorerd and no more weird dreams; 17 Nov-11 Dec '22: Started to wake up after 2-4 hours of sleep for unknown reason; 12 Dec '22-May 23: 3-6 hous of sleep, partly due to work stress; Jun-Aug '23: 3.5 to 4.5 hours hours of sleep; Sep '23-present: take mirtazapine in morning and it resolves my insomnia.

This is one thing I don't have good notes for, but here's what I did, as best I can recall, with reference to the instruction to switch from tablets to liquid gradually:

 

April 29 to May 1: 1/4 tablet (12.5 mg) and 3 ml liquid (6 mg)

May 2 to May 4: 1/8 tablet (6.25 mg) and about 6 ml liquid (12 to 12.25 mg) (I try and add a bit more than 6 ml to the syringe)

May 5 to May 7: 9.25 ml liquid (18.5 mg)

May 8, 9 and 10: 1/8 tablet (6.25 mg) and about 6 ml liquid (12 to 12.25 mg)

 

It's not possible to achieve 1/2 tablet (~9 mg) and 1/2 liquid, so I went from 2/3 tablet and 1/3 liquid directly to 1/3 tablet and 2/3 liquid. I went back to 1/8 tablet and 6 ml liquid on May 8? because it felt like I wasn't getting as much of a dose, in terms of the sedating effect I would typically feel (and because I had a 4 hour night the night before).

 

In any case, it's not possible to cut the tablet into 1/8s very accurately, so I'm not really confident of the precision of this dosing. I'm also not totally confident in how much trazodone I'm actually getting via the liquid, even though it's easier to measure with the syringe. Bottom line, I'm not sure exactly how much I'm taking; though it's close to 18.5 mg, I'm not sure what it actually is.

 

What would you suggest, Shep?

You may want to go back to the way you started out - you were taking 1/4 the 50 mg tablet for 12.5 mg and 6 mg in liquid (3 mL), which did give you 18.5 mg in total. That was only about 10 or 11 days ago. You were getting a more reliable dose that way.

 

Or you may want to slowly transition all the way over to liquid. If you wish to do this, please post and I can walk you through that.  I would go back to the 1/4 pill method and stabilize a bit first (which you'll want to do either way).

The physical sensations don't really vary, just their intensity. So whenever my notes say "anxiety" at a specific level of intensity, I was feeling the following physical sensations: buzzing/quivering in my chest, heat and clenching in my gut, gripping in my throat, weakness in my legs and arms and hands (with or without an accompanying reaction of fear and aversion). So perhaps in my notes I can restate the physical sensations up front for reference, but when the notes say "anxiety", it means I felt those sensations, unless stated otherwise?

Having a big wave of anxiety the past 3-4 days, and today has been brutal...just wondering if the tylenol and/or ibuprofen could be contributing to it?

Just had a look at the topic on painkillers in withdrawal. It seemed a bit inconclusive but I noticed that since I added in ibuprofen symptoms have gotten worse. I'm going to try a couple days without either Tylenol or Ibuprofen and see if anything else changes.

Yes, the over-the-counter (OTC) meds are a hit or a miss with folks going through withdrawal. If you're able to cope with heat or cold packs and distraction, it will give you a chance to see if you're better with or without adding in pain meds.

I went for physiotherapy today and the physio thinks my low back pain is due to an inflamed sacroiliac joint. I have an appointment with my family doctor tomorrow and I think it's possible that he will want to prescribe a stronger NSAID than ibuprofen (probably celecoxib/Celebrex) to try and bring down the inflammation (and therefore the pain).

 

I did a drug interactions check on celecoxib and was concerned by what I read for both citalopram and trazodone:

 

  • Coadministration with celecoxib may increase the plasma concentrations of drugs that are substrates of the CYP450 2D6 isoenzyme. The mechanism is decreased clearance due to inhibition of CYP450 2D6 activity by celecoxib.
  • Caution is advised if celecoxib must be used concurrently with medications that undergo metabolism by CYP450 2D6, particularly those with a narrow therapeutic range. Dosage adjustments as well as clinical and laboratory monitoring may be appropriate for some drugs whenever celecoxib is added to or withdrawn from therapy.

 

That doesn't sound like something I want to add to the mix right now. As far as I can tell, the other NSAID options are naproxen and ketorolac; neither have that type of drug interaction with citalopram or trazodone (though both naproxen and ketorolac plus citalopram increases the risk of bleeding).

 

I searched the site for references to celecoxib, naproxen and ketorolac. I saw a comment about celecoxib affecting AD plasma levels, and that some members have had bad experiences with naproxen, but nothing much about ketorolac. Maybe the best option for now? Either that, or just grit my teeth (even more)!

When I took celecoxib it gave me an upset stomach, even taking it with food.  The doctor then prescribed meloxicam and gave me 7.5mg to take twice a day (with food) instead of 15mg once a day.

 

Of course do a drug interaction check like you did for the celecoxib.

I saw my family doctor on Thursday and he wound up prescribing arthrotec for the low back pain. It's a combination of diclofenac and misoprostol (intended to protect the stomach lining). Hoping for a reduction in the pain level soon; it's a solid 7-8/10 for most of the day and I'm now very limited in what I can do. Walking is out; standing or sitting for any length of time is painful. Lying down is the best. Doing my best to cultivate acceptance of this latest affliction.

Hi TreeFrog,

 

Your drug signature is very long and it takes up a lot of room on the screen.  Please condense it by using this method.

 

Start with the year so you don't have to repeat it.

Abbreviate the month to the first 3 letters; eg Apr

You could use a v for decrease and a ^ (above the number 6) for increase so you don't have to use the word

You can omit the word "to"

Omit the space between the dose and the mg, eg 50 mg --> 50mg

You could abbreviate, except the first time it is used, citalopram to cit and trazodone to traz

Instead of using a line for every entry separate them with a semi colon

 

This:

March 31, 2022: Increased trazodone to 37.5 mg

April 3, 2022: Increased trazodone to 50 mg

April 6, 2022: Increased citalopram to 10 mg

April 13, 2022: Increased citalopram to 15 mg

April 15, 2022: Decreased trazodone to 37.5 mg
