Bp1961: Lexapro and Vyvanse withdrawal

Hey @manymoretodays,

 

I do plan to take 2.5mg in liquid (I'm getting it tomorrow, the pharmacy had to special order it) and 10mg pill. Since I went down from 15mg to 12.5 mg I feel better and my symptoms are milder. I still think that it is high for me due to the reasons I mentioned before. However, I will wait to hear from you and Alto as to when I should reduce to 10mg. Hopefully, you will agree that 10mg might be appropriate for me soon.

 

Yes, still seeing 2 psychiatrists (about to drop 1 of them). They both know that I take my instructions from SA and not them, and I need them only to write me the prescriptions. Unfortunately, I can't find any good psychiatrists in Atlanta and the list of providers on SA is outdated (A couple of them from ATL are no longer with us). I can tell you right now that my experience with doctors, psychiatrists and GP, is that for them you are just an insurance code or $$. They are not well versed in many areas, they can't see the bigger picture, they are not detailed oriented (They don't care at what time you take the drugs, they don't pay attention to details like smoking and caffein, etc.). Basically Dr. House is just a TV show. One psychiatrist told me that he just took 1 patient off Lexapro 3 months ago (From 5mg to 0mg) and he's doing just fine. What he and other psychiatrists don't understand is that every time they do that, they are playing Russian roulette with their patients well being and life. With 40%+ probability that the patient will experience protracted and painful withdrawal. 

 

I mentioned to the psychiatrist yesterday that Alto told me that prescribing Vyvanse and escitalopram was stupid. He asked me, why? I didn't have a good answer for him.

You might have showed him the drug interaction report and tell him it appears the combination has caused your tinnitus.

@Altostrata I did tell him about the drug interaction and because of that you told me to take them at different times. As for the tinnitus I didn't know that it's due to the interaction, I thought it's the lexapro.

Well, your psychiatrist did not think of the interaction. Further, he paid no attention to your tinnitus, a sign of neurological activation very well known to be caused by many drugs. The symptoms of serotonin toxicity, which can lead to serotonin syndrome, are activation. 

 

Now, there's a slight chance he's learned something about drug interactions.

 

I think you may do better with less Lexapro. The Vyvanse probably was unnecessary, but there's a fad for it right now so now you're on it.

@Altostrata Should I go down on the lexapro? To what 12,11 or 10mg? 

Vyvanse is not a recent fad, I've been taking for 10+ years.

 

P.s. I started feeling the tinnitus when I CT'ed not when I reinstated.

@Altostrata The tinnitus didn't appear while I was taking Finasteride. It was around the 3rd week in May when the Tinnitus started.

04:00 Woke up -> bathroom, couldn't fall back asleep. Tinnitus at 1/10 low sound of insects and crickets at night. Congested left nostril.

06:00 Went back to sleep.

08:15 woke up. Took Vyvanse 50mg. Tinnitus at 4/10. Went back to bed, laying in bed.

08:30 Tinnitus down to 1/10, turned from a high frequency/pitch sound to crickets & insects sounds at night. Congested.

09:45 got out of bed. Reacted magnesium + fish oil. Overall feeling good.

10:00 Tinnitus at 0-1/10

10:30 breakfast.

12:00 Took escitalopram 12.5mg. crickets & insects sounds at night.
14:00 Tinnitus is up to 3-4/10.

20:00 dinner + reacted magnesium.

21:30 shower
22:00 My tinnitus was going up and down in volume throughout the day, mostly higher than yesterday.

Not feeling good today. Probably the lousy sleep I had last night didn't help. Feeling depressed and down, heavy feeling in the chest. The Tinnitus is getting to me. It's not the loudness, it's the high frequency. It really sucks. I hope it gets better soon. Might be just a wave due to the dose reduction...I don't know and not knowing makes you feel hopeless. Common window.

06:45 Got out of bed. Tinnitus is at 4-5/10.
07:30 Drink a cup of coffee to see its affect + reacted magnesium.

08:00 Took Vyvanse 50mg.

08:35 Tinnitus at 1-3/10.

09:30 went to the office. Tinnitus at 3-5/10.

August 19, 2022 - Friday:

 

01:00 Woke up -> bathroom. tinnitus at 4-5/10.

07:15 Woke up. Tinnitus 3-4/10. Had a good sleep, total sleep: 7 hours & 19 minutes (Deep sleep: 1hr & 1min; REM: 2hrs & 11 min; light sleep: 4hrs & 7 min; awake: 47min).

08:00 Took Vyvanse 50mg. A bit of morning anxiety.

08:15 feeling some ear congestion. Tinnitus at 2-3/10.

10:00 a bit jittery and mild headache. Tinnitus at LE: 2-3/10 RE: 4-5/10.

10:30 going to the office.

12:00 took escitalopram 12.5mg (10mg pill + 2.5mg liquid).

Tinnitus LE: 2-3/10 RE: 3-4/10.

13:00 Lunch - Chicken sandwich.

13:30 Tinnitus elevated LE: 3-4/10 RE: 5-6/10.

Feeling anxious. The Tinnitus, which I believe is caused by my unstable CNS.

14:30 Feeling my CNS activated Tinnitus high. Tingling in the brain in the usual spot. 

15:15 Starting to feel a little better, tinnitus more like violet noise now at 3-4/10.

17:30 Mild headache. Tinnitus LE: 2/10 RE: 4-6/10.

19:45 Feeling pretty good despite the tinnitus of LE: 2-3/10 RE: 5-6/10. No anxiety and mood is good. Mild headache. Watching TV, which I couldn't do for the last few months.

20:00 dinner.

21:30 Reacted magnesium+ashwagandha.

21:30 meditation ~20min. Tingling sensation in the head, same spot. Tinnitus 4-6/10.

22:00 shower.

23:00 going to bed.

 

The day started off OK after a good night sleep. Then became a little bit rough, and started getting better late afternoon. The tinnitus was relatively high today.

August 20, 2022 - Saturday:

 

01:45 Woke up -> bathroom -> back to sleep.

03:3p Woke up -> bathroom -> Congested sneezing runny nose (might be the effect from taking ashwagandha) -> back to sleep.

07:30 Woke up -> bathroom. Congested. Tinnitus at 3-4/10. Mild headache. Total sleep: 6hrs & 31min (Deep sleep: 57min; REM: 1hr & 13min; light sleep: 4hrs & 21min; awake: 1hr & 6 min).

08:00 Took Vyvanse 50mg.

08:15 ear congestion and spike in tinnitus 5-6/10.

08:43 I feel activation of the CNS that causes spike in tinnitus activity.

09:10 meditation 30 minutes. It was difficult, started with some anxiety and jitteriness. Brain activity was high with a bit more than usual tingling sensation in the same spot as usual, the sensation expanded through the left side area of the brain. tinnitus was high at 6-7/10.

I think I'm noticing a pattern that about 10 minutes after taking the Vyvanse the Tinnitus starts spiking and I become jittery and anxious with low mood and it levels off throughout the day, and improvement in the afternoon around 4 p.m.

10:16 Breakfast + Reacted magnesium + fish oil.

10:30 Tinnitus sounds like muffled violet noise. LE: 2-3/10 RE: 4-6/10.

12:00 took escitalopram 12.5mg (10mg pill + 2.5mg liquid)Tinnitus LE: 1-3/10 RE: 3-4/10. Feeling a little anxious.

12:45 feeling better, tinnitus a tad lower. BP: 130/76

15:20 Just sitting on the couch, surfing the net as if I'm goingto find an answer. Tinnitus is high at about 6-7/10. 

16:00 Helped my son move to his college apartment. Tinnitus lower at 1-3/10.

feeling better today.

21:30 back home, took reacted magnesium.

22:00 Hot shower.

August 21, 2022 - Sunday:

 

12:50 Woke up -> bathroom -> back to sleep. Tinnitus 5-6/10.

03:00 Woke up -> bathroom -> tinnitus 2-3/10.

06:43 Woke up with light sweat. Tinnitus 2-3/10. Congested. 

07:14 Got out of bed. Total sleep 7hrs & 3min (Deep sleep: 57min; REM: 1hr & 5min; light sleep: 5hrs 1mi; awake: 55min).

08:00 took 50mg Vyvanse. Feeling my brain becoming active and electric current sounds spiking my tinnitus, feeling a tingling sensation in the ears, chilling. 

Is it possible that I kinddled on 1 drug and not the other?

No doubt in my mind that the Vyvanse has a direct effect on my tinnitus (strange that it's not on the ototoxic drug list).

10:30 Breakfast+Reacted magnesium.

Feeling LESS jitteriness and anxiety. Tinnitus calming down, but still high. Maybe the side effects are getting shorter 🤞

11:30 Tinnitus high again at 6-7/10.

11:57 Took fish oil.

12:00 took escitalopram 12.5mg. tinnitus at 6-7/10.

12:30 Feeling tingling sensation in the brain, same spot as usual.

13:20 Shower therapy.

13:50 Tinnitus LE: 1-2/10 RE:4-5/10.

14:00 Feeling fair.

14:30 Tinnitus is LE: 2-3/10 RE: 3-5/10.

Feeling alright, going to run some errands.

16:00 A little spike in tinnitus LE: 3-4/10 RE: 5-6/10.

17:30 Tinnitus is still at high level LE: 4-5/10 RE: 5-6/10.

18:30 dinner

19:00 watched the documentary Cause of death Unknown. I'm feeling Ok emotionally, but the tinnitus is loud 6-7/10 both ears. Feeling tingling sensation, this time on the left side of the head just above the left ear.

20:00 took reacted magnesium. 

From time to time during the day I experience air hunger (I previously referred to it as shortness of breath). Which I read on SA is a common side effect of WD that should resolve over time. It reminds me that as a child I used to experience this.

21:00 Hot shower.

22:00 Journaling.

22:30 go to bed early.

 

I think over the last 3 weeks since I started going down in escitalopram dosage I am getting better slowly, except the Tinnitus. There are times when it's more tolerable, when it's at lower volume. August 22, 2022 - Monday:

 

01:10 Woke up -> bathroom. Tinnitus very high 7-8/10. The highest it's been the last several days. Tingling sensation top of the head.

01:45 Got out of bed, couldn't fall asleep with the tinnitus volume. Moved to the living room couch. Put violet noise in the background. Have an important meeting in the AM.

03:58 awake little sweaty, shortness of breath and headache. Turn off the violet noise. Tinnitus LE: 1/10 RE: 3-4/10.

05:15 Awake again, light sweat. Tinnitus at 3-4/10.

07:00 Get up. Total sleep: 5hrs & 4 min. (First 1hr & 22min no breakdown available. Next 3hrs & 42min, Deep Sleep: 16min; REM: 51min; Light Sleep: 2hrs & 35 min; Awake: 43min) Tinnitus LE: 0-1/10 RE: 4/10.

19:00 Meditation (30 minutes). Was quit calm and not a bad session, although the tinnitus was loud and tingling sensation in the head. got quieter to wards the end of meditation.

Tinnitus LE: muffled sound 1-2/10 RE: insects sound 1-2/10. But the stereo effect makes it feel louder.

07:50 took reacted magnesium + fish oil. Tinnitus LE: muffled 4-5/10 RE: insects sounds 2/10. Feeling some morning anxiety.

09:00 breakfast (oatmeal+banana+strawberry+little Honey). Feeling fair. Tinnitus same as before taking Vyvanse, maybe a tiny bit lower.

20:00 tried group meditation via zoom. After 15 minutes started getting air hunger.

After improperly reinstating with too quickly and higher dose then I was on before CT, and then 3 subsequent reduction in doses and change in time between the drugs, here are my thoughts & observations:

1. It appears that escitalopram activates my tinnitus. It's usually lowest early morning until I take escitalopram at noon (with some exceptions).

2. After taking escitalopram the Tinnitus is activated with tingling sensation in the head. It's often followed by headache and brain fog. I have low energy and very little motivation to do anything, I'm having trouble concentrating at work (I might be in a wave).

3. Overall I'm feeling better now than I did a month or 2 ago. I don't feel debilitating panic and anxiety as I did before and during the first month of reinstatement.

3. I don't think that the liquid escitalopram affects me any different than the pill, but can't be certain.

4. I wonder if it could be beneficial to split the escitalopram time (i.e. 10mg pill at 12pm & 2mg liquid form 1hr later)?

Went to a social event with lots of people. It was loud. As I entered the house my tinnitus immediately flared up with a hissing sound to a level of 8/10. It was so loud that I felt a vibrating sensation in the ears. At first I felt sad and depressed, I felt like crying. I had to leave and go outside, put ear plugs and go back in but it was difficult to engage with anyone, I wasn't in the mood and couldn't hear with ear plugs. Eventually I told myself "the heck with it" and  took the ear plugs out and started engaging with people. I smiled and pretend to be happy. It was like an out of body experience. Seeing everyone celebrating and happy while my heart was sinking in. I was sweating. I thought I'm in WDNormal. If that's the case, I just realized how far I am from "Normal", and how much I will have to go through until things get better. It will take a long time and I just have to get through it. Hopefully, a year or 2 and things will get better. I'll try to move on and not be hard on myself for getting myself into this mess. It's not going to be easy but I will give it all I've got for the sake of my wife and children.

-1/06 - 3/07 Cymbalta. Fast taper; withdrawal symptoms after 4 mos (didn't realize was WD)

-10/07: 100 mg Zoloft; 1 mg Klonopin - tapered off Klonopin
-Tried several times to slowly taper Zoloft by 10%, then 5% every 4-6 weeks; could never get below approx. 40 mg - spring 2012 experienced major WD symptoms due to stress; tried to updose but no relief, back on Klonopin 1 mg.
-Switched over 5-6 mos from Zoloft to Citalopram. Finished Zoloft 1/13; Citalopram 35 mg and 1 mg Klonopin.
-8/13: 27 mg Citalopram; 1 mg Klonopin

-11/14: 12.6 Citalopram - began to have bad withdrawal symptoms; out of desperation increased to 1.25 mg Klonopin at the beginning of December.  12/13/14 16 mg Citalopram - going to stay here to try to stabilize; stabilized on 16 mg Citalopram after 4-5 months

-7/15 - 3/16: reduced to 15 mg; ~ 2 months later w/d hit hard (probably r/t stress); 6/16 updosed to 20 mg Citalopram and trying to stabilize. Updosed to 1.5 Klonopin as well. Stabilized on 20 mg Citalopram after 4-5 months

8/17-9/17: feeling withdrawal symptoms at 20 mg Citalopram (due to stress) - slowly increased to 25 mg. No change in symptoms after 6 months (? tolerance ?)  - decided to start citalopram taper February 2018 (still on Klonopin 1.5 mg).

August 28, 2022 - Sunday:

 

01:30 Go to sleep.

02:40 Wake up bathroom.

04:00 Wake up bathroom. Tinnitus LE: 2/10 RE: 4/10.

06:30 Wake up -> bathroom. Total sleep ~5 hours. Couldn't fall back asleep. Shortness of breath, tight chest, mild headache, congested. Tinnitus LE: 2/10 RE: 4/10.

08:05 Took Vyvanse 50mg. Tinnitus LE: 2/10 RE: 2/10.

08:10 Was very tired, so I vegged on the couch for 2 hours catching some sleep.

11:30 moved from the couch in the living room to the couch in the family room 🙄.

BP: 129/76 tinnitus LE: 5/10 RE: 2/10.

11:56 Took reacted magnesium+fish oil. Tinnitus LE: 4/10 RE: 2/10.

12:00 took escitalopram 12mg.

13:30 lunch. Tinnitus 3/10 both ears.

17:00 Tinnitus LE: 2/10 RE: 1/10.

18:00 did a 10 minute walk.

18:30 Dinner. Tinnitus 4/10 both ears.

22:30 shower. Tinnitus LE: 4/10 RE: 3/10.

23:00 Journaling and going to bed.

 

I spent today mostly at home on the couch. I need to calm my tinnitus and CNS from yesterday. I had tightness in the chest and shortness of breath throughout the day. Watched YouTube videos on various subjects from nutrition to anxiety. I would categorize it as WDNormal.

August 29, 2022 - Monday:

 

23:30 go to sleep.

02:20 woke up -> bathroom -> back to sleep. Tinnitus LE: 2/10 RE: 3/10.

06:00 woke up -> bathroom -> can't go back to sleep. Tinnitus LE: 2/10 RE: 4/10. Congested right nostril. Tightness in the chest.

07:05 Got out of bed. Total sleep ~6 hours. Tinnitus LE: 3/10 RE: 3/10. Took reacted magnesium+fish oil.

07:15 went for a 2 miles, 44 minutes walk. During the walk my chest was tight, but kept going. Tinnitus at 8:00am LE: 1/10 RE: 3/10. The walk made me feel good, thanks Alto for suggesting the light exercise.

08:02 took Vyvanse 50mg.

08:45 Shower. Tinnitus LE: 2/10 RE: 4/10.

09:15 meditation ~30 minutes. BP: 125/72. Tinnitus LE:2/10 RE:4/10.

10:00 breakfast (Oatmeal/oat milk, banana, strawberry, blueberry).

10:50 Go to the office. Tinnitus LE:1/10 RE:3/10.

11:55 Tinnitus LE: 2/10 RE: 6/10. It happens often in my office for some reason, and I'm not anxious or stressed due to work.

12:00 took escitalopram 12mg.

Tinnitus LE: 2/10 RE: 4/10.

12:35 Tinnitus LE: 2/10 RE: 4/10.

15:00 3 hours after taking escitalopram, I feel fine but have a headache and the Tinnitus is on the high side, LE: 3/10 RE: 6/10.

16:00 headache is lower and Tinnitus LE: 2/10 RE: 4/10.

17:00 headache intensifies Tinnitus LE: 3/10 RE: 6/10. Other than the headache, Tinnitus and shortness of breath I feel fine. These are all physical in nature. I'm wonderingif this means that my escitalopram dosage is higher than it needs to be!?

18:30 My headache is gone. Tinnitus LE:1/10 RE:2/10.

20:00 tinnitus LE:3/10 RE:6/10.

20:00 Dinner.

21:00 shortness of breath. tinnitus LE:3/10 RE:6/10.

21:30 shower.

22:00 Journaling. Tinnitus LE:2/10 RE:4/10.

22:30 going to bed.

Please note I have been suggesting you reduce your escitalopram dosage for weeks.

Alto, I've been following your suggestions to a T and I've been reducing the dosage accordingly. All I want is to overcome my WD stabilize and taper without losing my sanity or life.

@Bp1961 I m going through what you are experiencing right now. It is hell. You are not alone. Stay strong. I have child on the way soon myself. 

Celexa 10 mg started (for panic attack) and quickly stopped. From Jan 2001 to September 2001 stopped cold turkey.

Celexa 10mg taken for short period of time in 2010; could not take for a long period of time due to side effects, thus only taken for about 2 months. Went through withdrawal symptoms, and protracted symptoms for at least 1.5 years.

Restarted Celexa 10 mg in 2013 to 2016 off and on. (never could tolerate anything above 10mg) to due work related stress, and not because of anxiety or depression. Never could consistently take a pill daily for weeks at a time, due to side effects. 

Restarted in 2019 off and on again for a year, than in October 2020, cold turkey stopped. Did not know about this website, never was told how to stop or to taper since "I was not taking a high dose", suffered insomnia, elevated blood pressure, anxiety, stress, but not any real devastating symptoms. 

Have not swallowed a micro dust of Citalopram since October 2020, and it's October 2021 now.

Started trazodone 25mg in April 5th, 2021 for insomnia, horrible choice, recommended by doctor, couldn't tolerate side effects, tapered, every other day for a month starting on April 24th, than every 2 days for 1 months in May as recommended by doctor, until last 25 mg pill swallowed on June 07,2021. What a mistake! 

No SSRI's or SSNI's or any psycho medications have been ingested by me since June 07,2021. And life has been hell, especially after the Trazodone was introduced. 

March 17th, 18th, 2022. I took a cough suppressant that contained Dextromethrophan. Took a total of 20 mg over 2 days, to suppress cough. Caused a major exacerbation of previous symptoms. Have suffered 2 Panic attacks, difficulty sleeping, anxiety which had been well controlled for at least 3 months. And also worsen my Withdrawal induced Tinnitus.
