You may be experiencing interdose withdrawal from gabapentin, as you take it only once a day and it wears off overnight.

 

Suggest you move 100mg of the 200mg gabapentin earlier by an hour each day, until you are taking it at 9 a.m. Please let us know how you're doing as you make this change.

@AltostrataShould I just discontinue the diphenhydramine? I’m only taking it for sleep purposes. I’ve always thought it helped with sleep but if you think I should stop I will. I take a Zyrtec in the morning for allergy purposes so I wouldn’t need the diphenhydramine as well. I will start to move the gabapentin tomorrow. Thank you

Never properly tapered by doctor before switching medicines* 2020  Started on Buspirone 15mg three times a day    2020-2021   Antidepressant was changed three times: Trintellix, Pristiq, Fluvoxamine.  Underwent 6 months of ketamine infusions. Advised to discontinue all meds due to inefficacy in November 2021.  Weaned off Fluvoxamine by psychiatrist over 6 weeks-- last dose ~December 10, 2021.  Severe ongoing withdrawal.   February 2021  Started on 300mg Gabapentin for anxiety, restless legs, insomnia, and wide spread body pain from withdrawal.  Began Gabapentin taper to 200mg on 8/5/2022. Worsened withdrawal symptoms.  Also began 2000mg of L-Tryptophan for depression/AD withdrawal, 2400mg of N Acetyl Cysteine for OCD and 300mg of Magnesium for depression.  Began L-Tryptophan taper from 2000mg to 1500mg on 8/14/2022. Completed l-tryptophan and NAC taper on 8/29/2022. 

Current Meds: 15mg of Buspirone twice a day (morning and night since 2020), 5 mg Buspirone at midday (Started taper of midday dose from 15mg to 5 mg in September 2023) , 200mg of gabapentin at night (Since August 2022), 1mg of alprazolam at night (Started at 0.25mg in Jan 2023 and gradually upped to 1-2mg three times a week until end of 11/2023--reinstated 1mg daily on 12/1/2023), 80mg propranolol in the morning, 10mg Zyrtec in the morning, birth control pills Current Supplements: 300 mg of magnesium citrate midday, 1000mg of Fish Oil in the morning, 1mg melatonin at night

Zyrtec has not been mentioned before. Please specify what times o'clock you take each of your drugs, with their dosages. Please put ALL your drugs in this Interactions Checker and post the report or a link to it in this topic.

 

Suggest you cut down diphenhydramine by half by splitting the tablet or emptying half out of the capsule for a week, then stop.

@Altostrata It’s 2:45am and I haven’t slept in three days. I’m so tired, it feels like my body is dying. Every time i begin to fall asleep, I jerk awake in complete terror. I start the full body shaking and turn ice cold. My heart rate averages about 130bpm during this time with horrible palpitations. I am very nauseous and have near constant diarrhea. It’s almost as if I go into shock. I slept for 7 hours two days in a row before this and before that went two and a half days with no sleep. I don’t know what to do. I cut the diphenhydramine in half during the previous two days with no sleep as you suggested but went back up to a full pill thinking it would help me to relax. It did help me to sleep the two nights I got seven hours. My anxiety is so severe that I feel stuck in a constant fight or flight mode. I have tried deep breathing, guided meditation, light yoga, distracting myself with activities around the house, and talking to supportive friends but I am making no progress. My windows are blacked out and I stop using electronics for two hours before bed. I don’t know how much longer I can go without sleeping. Please can anyone help? If I don’t sleep I won’t heal and I’m so desperate to heal.

@AltostrataAm I experiencing tardive akathisia?  I feel like I am being tortured.  I just need it to end.  If you think going back on meds will end it, I will do it.  I am so sick.

I will pray for you. I know exactly what akathisia feels like.

 

From what you are describing, with the pacing, rocking and insurmountable extreme anxiety, it sounds like that is the case. Though we can't know whether it is acute, chronic or tardive at this point. Specially, fluoxetine withdrawal and/or gabapentin withdrawal being involved.

 

Is the akathisia worse before your gabapentin dose is due? Or is it worse after you take it? It could also be interdose akathisia but from your daily notes of symptom timing it doesn't seem like it!

Anyhow, your best bet is to stabilize right now and improve your sleep. I would implement the changes altostrata suggested.

December 2021 - Metoclopramide started. Akathisia symptoms start; Metoclopramide gets changed to PRN.

March 2022 - Akathisia diagnosed; Metoclopramide stopped; Propranolol 10mg x twice a day. Biperiden PRN (0.5mg to 1mg).

April 2022 - Tandospirone 30mg (10mg 3x day), Quetiapine 25mg (only taken once, immediate adr). Mirtazapine 7.5mg. . Discontinued Propranolol.

May 2022 - Mirtazapine upped to 15mg. Tandospirone cut to 2x 10mg. Low dose Depakote for the month; 100 to 200 to 100 to 0. Mirtazapine cut back to 11.75mg (3/4 of a 15mg pill).
June 2022 - Mirtazapine updose to 15mg. Tandospirone, Biperiden discontinued. Klonopin started PRN (0.5mg). 
September 2022 - Akathisia slowly starts improving, WD/ADR normal sets in in mid September. Hold for 4 months.
March 2023 - Off mirtazapine; no Klonopin for 5 months either! Started quercetin (250mg x 2) to soften the histamine rebound.

May 2023 - Stopped quercetin and changed from magnesium carbonate to oxide - reacted badly. Reverted back to carbonate. 
June 2023 - Added fish oil.
Current regimen: CALM Magnesium (Carbonate into Citrate) 175mg x2; Vitamin E 268mg x2; Fish oil (100mg Omega3; EPA 30mg; DHA 37mg)x2

@PortugueseSeathank you so much for reading and responding. I took 200mg of gabapentin last night with my 9pm instead of breaking the dose up through the day and was finally able to sleep some.  I think I will go back to that for now and see if it helps. The no sleep is killing me along with trying to keep my job while experiencing all of this. I feel so afraid all the time.
 

Are you in an akithesia support  group? I have two friends in other states right now who have been my only support but I know they are exhausted and frustrated with me. I desperately need support if I am going to make it. 
 

I will be praying for you too. How long have you had akithesia and has it gotten any better?

You gotta be consistent and stick to it. I am glad you have slept but doing what Alto said will likely even out your plasma concentration. Move in the direction he recommends.

I know it's scary, all of this is terrifying but we will see it through, I promise. Keep in touch. We will do just fine, it just takes time.

 

Yes, I'm in the akathisia support group but, to be honest, there is too much godless despair for me to visit often. I will support you as much as I can through SA. Altostrata and all the others will do their best too. Let's do this!

 

I've had akathisia for 9 months, last dose of the causative agent on March the 1st, last dose of a dopamine antagonist in mid-April. Yes, it has gotten better. I am still on Mirtazapine and that might be helping too. Basically my inner distress/tension and dysphoria are still out of control but, that said, panic attacks have faded, pacing, rocking and fidgeting are 90% gone. The compulsion to move is less intense too, it still happens though.

 

It gets better, it's a marathon though, not a sprint.

Have you noticed the akathisia getting worse or better in relation to your gabapentin dosing? Like, before or after? Anything like that? Or is there a daily pattern to it that you have noticed?

The akathisia has seemed to increase dramatically as I moved 100mg of my gabapentin dose an hour each day as suggested.  I used to sleep at least 4-5 hours a night but have been going three days at a time on absolutely zero sleep and in a constant state of terror.  I know my medicine needs consistency but after having slept again once returning to the 200mg at 9pm last night I'm afraid to split the dose.  It just seemed to make me so much worse.  Before moving the dose I honestly thought I just had extremely bad anxiety, panic attacks, and terror upon waking.  While moving it, I felt terror 24/7 to the point of just being unable to function whatsoever.  The only thing I could think about was how to make this end.

 

I appreciate your support so much and will do my best to support you on here as well.  It makes me so happy to hear that your akathisia has gotten better.  It gives me hope that maybe mine will as well.

Worsened withdrawal symptoms.  Also began 2000mg of L-Tryptophan for depression/AD withdrawal, 2400mg of N Acetyl Cysteine for OCD and 300mg of Magnesium for depression.  Began L-Tryptophan taper from 2000mg to 1500mg on 8/14/2022. Completed l-tryptophan and NAC taper on 8/29/2022.

I see, and so you decided to stick with the evening dosing. The panic attacks, bad anxiety and terror upon waking could be part of the akathisia or secondary to the akathisia. One way or the other, I completely understand the fear, despair and the drive to make it stop. I live with it everything. The thing about akathisia is that is that any amount of it is maddening and drives one to despair. Right now I am having a morning wave, which is unusual, I am fidgety and have that unmistakable nervousness but it is not bad enough to make me pace. That said, it instantly makes me suicidal, not being able to relax even for a second is a prison I do not wish upon no one.

It gets better but it is not linear, some days you will feel like it is gone or largely gone, other days it will hit you like a pile of bricks. When that happens practice radical acceptance, affirmations are great, it is temporary, I am healing; and do reach out. This is not something one should go through alone.

Is aerobic exercise an option for you? Like jogging or even brisk walking outside. Anything that can reduce and rebalance noradrenaline might help!

March 2022 - Akathisia diagnosed; Metoclopramide stopped; Propranolol 10mg x twice a day. Biperiden PRN (0.5mg to 1mg).

April 2022 - Tandospirone 30mg (10mg 3x day), Quetiapine 25mg (only taken once, immediate adr). Mirtazapine 7.5mg. . Discontinued Propranolol.

May 2022 - Mirtazapine upped to 15mg. Tandospirone cut to 2x 10mg. Low dose Depakote for the month; 100 to 200 to 100 to 0. Mirtazapine cut back to 11.75mg (3/4 of a 15mg pill).
June 2022 - Mirtazapine updose to 15mg. Tandospirone, Biperiden discontinued. Klonopin started PRN (0.5mg). 
September 2022 - Akathisia slowly starts improving, WD/ADR normal sets in in mid September. Hold for 4 months.
March 2023 - Off mirtazapine; no Klonopin for 5 months either! Started quercetin (250mg x 2) to soften the histamine rebound.

May 2023 - Stopped quercetin and changed from magnesium carbonate to oxide - reacted badly. Reverted back to carbonate. 
June 2023 - Added fish oil.
Current regimen: CALM Magnesium (Carbonate into Citrate) 175mg x2; Vitamin E 268mg x2; Fish oil (100mg Omega3; EPA 30mg; DHA 37mg)x2

I am so sorry you are experiencing a wave this morning. I completely understand the despair you are talking about. Akathisia is so overwhelming and all encompassing that I would do almost anything to get it to stop. It feels unlivable. I saw in a video online that said the Gulag used to give large doses of antidepressants to political prisoners and then stop them suddenly to induce akathisia as a form of torture. I mean if that doesn’t just sum up the situation right there then I don’t know what else could. No one deserves this pain. 
 

I try to walk for at least twenty to thirty minutes a day and I find that helps to burn off some of the anxiety. I also try to do some light circuit training 3-4 days a week but some weeks I’m too sick to do it so I don’t push. I appreciate talking to you about this so much. I truly cannot even express how grateful I am. I pray that your wave remits soon and you are able to find some peace. Will you please update me on how you’re feeling later in the day or tomorrow? I will be praying for you.

Antipsychotics, yes. 

Which is exactly what some anti-emetics are, namely the one that got me into this mess, metoclopramide.

No, this pain is inhuman. But we will make it through.  Your aerobic exercise habit is great, yeah, never push it.
I also walk around 10.000 steps a day and I feel it helps. It is also good for your brain.

The anhedonia and depression are also quite bad but I know it is "just the drugs".

Every month I get further away from dopamine antagonists I am closer to healing, though I'll have to taper the mirtazapine eventually...

@PortugueseSeaI’m glad to hear you got a little relief. It’s so scary to think anti nausea meds can do this. My doctor kept trying to give me anti nausea meds for my withdrawal nausea and I’m so glad that I read on here about them affecting the nervous system because I never would’ve known not to take them otherwise. I’m so paranoid about every medicine now. I always thought I was doing “the right thing” by taking the antidepressants and anti anxiety meds.  Now I am in this mess and would do anything to go back and never touch them but wishing to change the past never helps. 
 

I am very thankful I slept again last night. Not sleeping makes my akathisia so much worse. Going three to four days with no rest will break anyone. 
 

I am still on gabapentin and buspirone. I will eventually need to taper both of these but right now my focus is on stabilization. Do you feel stable even though you are still experiencing waves and windows?

@Altostrata I typically have diarrhea or a lose stool at least once a day since withdrawal started, with it ramping up to several times a day or more when in a bad wave.

When did you start metaprolol? Have you ever reviewed adverse effects of this drug? It's your responsibility as a drug consumer to look up adverse effects of your drug, and recommendations about how to take it. Were you aware of this:

I have been taking metoprolol for close to a decade. I will make sure I eat something with it and will move the fish oil to lunch.

Before I took that poison, my health problems were a little occasional nausea and work related stress. That is it. Now I suffer daily and am moving back to my parents', leaving my job and family behind so I can recover from all the damage. If you had told me a year ago what was about to happen I wouldn't believe you. How could a doctor prescribe such a medication so haphazardly? Specially without a diagnosis. 

I didn't do my due diligence, true, but neither did the doctor or any of the pharmacists. When I got worse I talked with several doctors and no one did even consider akathisia as a possibility, despite my rx at the time.

It took months for them to realize, and then I got a prescription cascade that would have berried my if it wasn't for this website and Alto's advice.

I was just withdrawing, I am just withdrawing, now after being kindled so it is worse, should have left it alone. Like you with gabapentin and buspirone, I am taking Mirtazapine, still have crises and wouldn't be surprised if it took me a year to stabilize fully. We have to both be patient and suffer through this. We will make it. I promise.

It’s truly heartbreaking. We trust doctors to act in our best interests not to harm us with medication. I was 15 when I was put on medication. A minor. I had no idea what these pills could do to me. By the time I was legal age I was brainwashed into thinking the meds were the only thing keeping my abnormally functioning brain going. I was so desperate to just feel better and lead a normal life like others my age that I would’ve taken anything handed to me.

 

Im so sorry you have to leave your family so you can recover with your parents, but it’s such a blessing that your parents are willing and able to care for you. I am terrified because I don’t have anyone to help me. I am so scared to think about what will happen to me if I lose my job and cannot support myself. I have nowhere to go really.  Im afraid all the time and I never feel safe. I have a good therapist but she doesn’t take insurance and the cost is high.  
 

Is constant terror and unrelenting fear your main akathisia symptom? Do you have waves and windows or is the akathisia constant? Have you found anything that helps make it better?

Restlessness is by far my worst symptom, it's the most relentless and "painful" to bear. It's almost constant. I get terror too at times, but it's not all the time.

The akathisia is constant in that I barely get any windows from the restlessness, it does vary in intensity though. Terror and other symptoms (head pressure, chest pressure, tinnitus, despair, anxiety, depresson) fluctuate, often wildly.

 

I have found that autogenic training helps me a bit, the mirtazapine and klonopin SEEM to help too. Though I'm hardly comfortable with taking anything but supplements right now. I'm very afraid of making things worse as getting worse means more time away from my family and regular work.

December 2021 - Metoclopramide started. Akathisia symptoms start; Metoclopramide gets changed to PRN.

March 2022 - Akathisia diagnosed; Metoclopramide stopped; Propranolol 10mg x twice a day. Biperiden PRN (0.5mg to 1mg).

April 2022 - Tandospirone 30mg (10mg 3x day), Quetiapine 25mg (only taken once, immediate adr). Mirtazapine 7.5mg. . Discontinued Propranolol.

May 2022 - Mirtazapine upped to 15mg. Tandospirone cut to 2x 10mg. Low dose Depakote for the month; 100 to 200 to 100 to 0. Mirtazapine cut back to 11.75mg (3/4 of a 15mg pill).
June 2022 - Mirtazapine updose to 15mg. Tandospirone, Biperiden discontinued. Klonopin started PRN (0.5mg). 
September 2022 - Akathisia slowly starts improving, WD/ADR normal sets in in mid September. Hold for 4 months.
March 2023 - Off mirtazapine; no Klonopin for 5 months either! Started quercetin (250mg x 2) to soften the histamine rebound.

May 2023 - Stopped quercetin and changed from magnesium carbonate to oxide - reacted badly. Reverted back to carbonate. 
June 2023 - Added fish oil.
Current regimen: CALM Magnesium (Carbonate into Citrate) 175mg x2; Vitamin E 268mg x2; Fish oil (100mg Omega3; EPA 30mg; DHA 37mg)x2

@Altostrata is there any correlation between length of time on meds and length of withdrawal? I was on SSRIs for 27 years. Does that mean I am stuck in this hell for decades? I know everyone is different but what has been your experience with this?

2018-2019  Bupropion    *Have been polydrugged since 1995 with a variety of benzos, sleeping pills, anxioltyics, and ADs.  Too many to list or remember. Never properly tapered by doctor before switching medicines* 2020  Started on Buspirone 15mg three times a day    2020-2021   Antidepressant was changed three times: Trintellix, Pristiq, Fluvoxamine.  Underwent 6 months of ketamine infusions. Advised to discontinue all meds due to inefficacy in November 2021.  Weaned off Fluvoxamine by psychiatrist over 6 weeks-- last dose ~December 10, 2021.  Severe ongoing withdrawal.   February 2021  Started on 300mg Gabapentin for anxiety, restless legs, insomnia, and wide spread body pain from withdrawal.  Began Gabapentin taper to 200mg on 8/5/2022. Worsened withdrawal symptoms.  Also began 2000mg of L-Tryptophan for depression/AD withdrawal, 2400mg of N Acetyl Cysteine for OCD and 300mg of Magnesium for depression.  Began L-Tryptophan taper from 2000mg to 1500mg on 8/14/2022. My dear Etta, we can't know how long it will take to get 100% better. That said, you will stabilize much earlier than that. Take heart.

December 2021 - Metoclopramide started. Akathisia symptoms start; Metoclopramide gets changed to PRN.

6:00am  Wake up, take meds--slept better than usual

6:10am  Loose stool

6:15am  Breakfast

6:30am  Walk dog

7:50am  Diarrhea, anxiety building

8:30am  Body turns ice cold, full body shaking

9:00am  Diarrhea, extreme fatigue, panic attack

9:30-10:30am  Receive support from family

10:30am-12:30pm  Low level anxiety

12:30pm  Meds/lunch

12:45pm-3:00pm Low level anxiety

3:00pm  Panic attack

3:30pm  Diarrhea

3:35pm Full body shaking, panic attack

3:40pm-5:00pm Sobbing uncontrollably, sheer terror

5:00pm  Try to eat

5:15pm-10:30pm  Severe akathisia, shaking in terror, sobbing, rocking back and forth while hiding in closet, on phone constantly for support, can only repeat the phrase "I'm so scared"

10:45pm  Startle awake in absolute terror after dozing for fifteen minutes, diarrhea, try to self soothe

11:00pm-2:00am  Sleep but mind is still in constant terror

2:00am-5:45am  In and out of sleep, filled with terror, shaking

 

Family has pulled together and vowed to support me.  Mom wants me to move home so she can help take care of me.  Sister promises to help in any way she can.  I'm in constant terror but my family believes I have akathisia and promises not to force medicate me and help me through.  Talking to boss about relocating out of state.

Lately I have been having a glucerna shake because i haven’t been tolerating food and thought I wasn’t getting enough nutrients. Sometimes I eat a piece of Ezekiel bread too and/or a banana.

At 6am I take 50mg metoprolol, 10mg Zyrtec, 15mg buspirone, 100mg magnesium, and a probiotic.

Please review adverse effects of metoprolol. To be an informed consumer, you should look up adverse effects of every drug you take.

 

It is possible the diarrhea is from metoprolol. Is there some reason you are taking it in the morning?

That was how I was directed to take it. It’s never given me diarrhea before and I have been on it for over a decade but that doesn’t mean it’s not giving it to me now. I personally feel like the diarrhea is related to my extreme anxiety though. The panic attacks seem to bring it on. 
 

The Glucerna shakes have helped me I think. I have seen an increase in appetite and ability to stomach food since starting them.

Your symptom pattern, which is consistent, indicates something you're ingesting in the morning is causing gut problems shortly afterward.
