Barb and Petu had posted for me that Nefazadone with Imipramine and Celexa are a no-no because it can cause serotonin syndrome.

 

When I first went on Nefazadone to taper the other two I did have shivers, what felt like a fever and a few other minor symptoms.

 

I dug a little deeper on google and found that there are cognitive side effects which I am having now.

Extreme anxiety, depression, humumgus fear, confusion, irritability.

 

I get that from WD but this time around these issues are worse then they have ever been....there is something different than the usual WD nonsense.

2009 Mar.: lexapro 10mg for headache for 2 weeks.

2009-2012: on and off 1/4 to 1/3 of 10mg

2012 June--2013 Jan,: 1/4-1/3 of 10mg generic, bad jaw pain

2013 Jan-Mar: 10 mg generic. severe jaw and head pain;

2013 Mar--Aug. started tapering (liquid ever since) from 10 to 5 (one step) then gradually down to 2.25 mg by July. first ever panic attack, severe head/jaw pain

2013 Aug.: back to 2.75 mg; Nov: back to Brand Lex. 2.75mg -- 3mg,

2014 June: stopped PPI, head pressure/numbness. up-dosed 4.5mg, severe reaction mental symptoms added on

2014 Aug--2015 Aug: Micro taper down to 3.2mg, .025mg (<1%) cut holding 2-3 weeks.

its not one article.

I probably should have provided more details there.  

 

from the linked page, click 'drug' on the menu bar on top then you can select particular SSRI of interest, it will open up a new page for informations about other drugs intereact with SSRI of your interest so you know what you should NOT to take together with.

 

thinking more now, it is more for 'medications interact with SSRIs' in general not only pain med related. it will be nice if there is a way to change/correct it.

"Food-drug interactions can result in two main clinical effects: the decreased bioavailability of a drug, which predisposes to treatment failure, or an increased bioavailability, which increases the risk of adverse events and may even precipitate toxicities."

 

I find this very interesting study since I have become extremely sensitized to any updosing (which caused me an adverse reaction more than once).

 

Since citalopram is metabolized by CYP3A4 and CYP2C19 I pay attention to the foods that inhibit especially those drug-metabolizing enzymes. 

 

"The CYP3A4-related interaction based on food component is the best known; it might be related to the high level of expression of CYP3A4 in the small intestine, as well as its broad substrate specificity. If we consider that CYP3A4 is responsible for the metabolism of more than 50% of clinical pharmaceuticals, all nutrientdrug interactions should be considered clinically relevant, in which case all clinical studies of drugs should include a food-drug interaction screening [43]."

 

Here is a list of fruits and vegetables that can inhibit CYP3A4:

 

Grapefruit(I guess grapefruit is the worst),

Sevilla orange,

Cranberry,

Pomegranate,

Papaya,

Cauliflower and

Broccoli.

 

Maybe fruit/vegetable-drug interaction is one of the reasons why we find that we become sensitized to some foods during taper?

Citalopram 40mg from 2003-2015

Jan 2015 started tapering first dropped to 35mgFeb 30mg, March 25mgApril 20mg, May 17,5mg, June 15mgJuly 12,5mg, Aug 12,5mg,

Sep 0mg for 5 days because of stomac flu and after I raised to 7,5mg. All the symptoms of acute WD shaking, diarrhea, vomiting, barely could walk ect. Still didn't realize that it wasn't only stomac flu but I was also going through WD.

Oct 2,5mg and crashed again badly and quickly raised to 4mg. It was then when I knew my symptoms were due to WD.

Then in November after a month holding on 4mg raised to 5mg due to muscle weakness and had a VERY BAD reaction to reinstatement: akathisia(lasted for one or two weeks), insomnia, anhedonia... Drop quicly back to 4mg, Dec 3mg

Jan 2016 2,6mg( in the middle of Jan after I had been on 2,6mg for a week I tried to updose to 2,8mg and immediately had bad reaction to it: akathisia for a day, andehonia got worse. The next day dropped back to 2,6mg), Feb 2,4mg( a new symptom PGAD lasted 24/7 for 2 months after that on and off), March 2,4mg, April 2,3mg, May 2,2mg, June 2,1mg, July 2,0mg( Pgad almost nonexisting, sleeping pretty good, still some anhedonia but there has been a lot of gradual progress), Aug 1,97mg-1,89mg, Sep 1,88mg-1,49mg, Oct 1,48mg- 1,70mg,

Nov 0,65mg- current dose 0,5mg

Ops, I missed one fruit from the list that also inhibits CYP3A4. That is grapes. I also noticed that Pomegranate doesn't inhibit CYP3A4 but it does inhibit CYP3A... Well, you can see for yourselves from the link in case have made more mistakes.

Hi everyone

 

I've just used the rxisk.org drug interaction checker and am curious about interpreting this:

 

  • The metabolism of Acetaminophen can be decreased when combined with Sertraline.
 
I think it suggests that it make take longer for acetaminophen to clear from the system when taken with sertraline. Am I reading this correctly?

January 2013 started Sertraline, over time worked up to 100mg

July 2014 Sertraline dropped from 100mg to 75mg, held for six months, slower tapering until 2019 22 Dec 3.2mg

2020 Sertraline 19 Jan 3.1mg, 26 Jan 3.0mg; 1 Mar 2.9, 7 Mar 2.8, May (some drops here) 24 May 2.5, May 29 2.4, June 21 2.3, June 28 2.2mg,  July 4 2.1mg, July 24 (or maybe a bit before) 2mg, early Nov switched to home made suspension; 29 Nov 1.8mg; approx 25 Dec 1.6mg)

2021 Some time in about Jan/Feb realised probably on more like 1.8mg and poss mixing error in making suspension; doses after 10 Feb accurate; 10 Feb 1.6mg; 7 Mar 1.4, continued monthly

Oh, very interesting. I just noticed that this thread starts with a different interaction checker so I did the same list of meds on that checker. It didn't flag the acetaminophen/sertraline interaction. I will ask a pharmacist next time I go in.

Why do doctors prescribe multiple medications that clearly interact with each other?    Do they not check or care? 

 

Now being stuck on three medications that are clearly causing me increased side effects until i can at least taper one is disheartening to say the least.

 9/2018- lexapro low dose few days.11/3/2018- zoloft 2 days

11/7/18  - 11/15/18 - Prozac 9 days, from 10 mg for week, to 20

11/16  inpatient put on Lexapro for a few days, Cymbalta, 2 days

11/24-12/8 - gabapentin 100 mg 3xs per day - a very fast taper

1/7 - tbuspar for  three days- blurry vision, jerky eye

1/17/19 - 2/15/19- mirtazapine 15 mg - started taper on 1/30 

2/20/19 gabapentin 600 mg. .  12/20-  taper finished

2/20/19 - seroquel 25 mg current - taken  10 pm

2/20/19- luvox (generic) 25 mg.  4/6/19  to 18.75 mg .held  . Started taper again  1/7/21- 15 mg, 2/7/21- 12.5 mg, 3/7/21 -10 mg, 4/1/21- 9 mg, 5/1/21- 8.1 mg, 5/27/21- updose to 10 mg,  6/21-  in patient updose to 50 mg,  6/25/21- reduce to 10 mg (current) .  9/5- split dose 5mg am/5 mg pm.  9/20- 4 mg am/5 mg pm . 

9/1/21-  took one dose of vistril 50 mg.   

9/1/21-accidental double dose of seroquel- 50 mg

I am kicking myself that I did not bring this up at my last appointment with my psych nurse.  But he had me so upset because I wanted to taper and he would not agree to give me a compound script.  Or even want to hear about it.  And he pretty much ignored everything I said.  Im debating calling him back to ask him why the interations weren't checked.  But what can I do about it now?  I cant just stop them.

i began my celexa and geodon in 2008. my doctor said that the pharmacy might give me a problem filling them but to ignore their warning and fill them anyway. they did have warnings the first few times but not again and i was too naive and followed doctors orders. i find out now the risks for heart problems is listed as major risk and the benefits do not outweigh the risks. i keep finding out so many things my doctor did or didnt do that are dangerous and ignorant

 

prozac 2008. do not know the dose. after a few weeks i had suicidal idealization and went to the hospital 4 days.

xanax .5 mg only took a few times

geodon started 2008. highest dose 180 mg

celexa 2008 40 mg

i tried so many different meds i cannot rrecall the name or dosage but all of them had severe side effects within a week and all were stopped

2008-present geodon. current dose 80mg

2008-present  celexa. current dose 40 mg
