Mjau: Starting to taper Wellbutrin and Sertraline

It's been a while since I visited the forum. But now I wanted to share an update. A very interesting thing happened last week. I was beginning to feel more and more exhausted. Heart started pounding for no reason. Eventually I was in a condition similar to what I experienced 1.5 years ago when I had a severe burn-out. I couldn't find any reason for all this. And the symptoms worsened very quickly. I had done a small increase in my amount of work but I couldn't see that was the cause.

 

I was on Wellbutrin 92 mg, last taper 4 months ago I think. And Sertraline 62 mg, last taper 3 weeks earlier from 68 mg. The reason why I halted my Wellbutrin taper was because I had a lot of restless legs (RLS) at the time - I side effect from Sertraline. That's why I've tapered Sertraline lately - about 10% every month. Some wd symptoms and almost regularly occuring waves but it's been manageable.

 

I got really scared of how things were turning out - I was so exhausted I couldn't read, work out, make dinner - I just wanted to sleep. Because Wellbutrin increases the activity of the sympathetic system I chose to decrease Wellbutrin to 75 mg. And - it was like MAGIC! The exhaustion diminished! Less heart-pounding! My thinking became clearer, my memory a bit better! And this only 2-3 days after the dose-reduction! Now I'm almost in the same condition as before the exhaustion. 

 

This just reinforces my theory that my meds - especially Wellbutrin - have caused the burn-out 1.5 years ago along with a whole bunch of other symptoms!! It's scary! 

 

Now I have a bit more RLS again (Wellbutrin alleviates RLS-symptoms). But that's so much better than being in a state of total exhaustion!

 

Anyone else with similar experiences with Wellbutrin/bupropion?

1997-1999 Citalopram 20 mg

1999-2014 Sertraline 50 mg

2012 Sertraline very quick taper due to side effects. Switched to Wellbutrin 150 mg-300 mg. Reinstated Sertraline 25 mg-50 mg.

2013 Exhaustion. Wellbutrin 150 mg. Sertraline 75 mg-100 mg.

Sept 2014 Found this site. Started tapering. Sertraline 87,5 mg + Wellbutrin 150 mg 

Aug 2015 No more Wellbutrin!! Sertraline 50 mg

2016 Sertraline 35 mg (January) - 33 mg (March 21st) - 32,5 mg (July 11) - 32 mg (July 27)

I more had exhaustion with cutting, but I think it is wonderful that your body is becoming more balanced! Epsom salts really helped else for me, but only if I waited till the "restless" started.

1st round Prozac 1989/90, clear depression symptoms. 2nd round Prozac started 1999 when admitted to dr. I was tired. Prozac pooped out, switch to Cymbalta 3/2006. Diagnosed with bipolar disorder due to mania 6/2006--then I was taken abruptly off Cymbalta and didn't know I had SSRI withdrawal. Lots of meds for my intractable "bipolar" symptoms.

Zyprexa started about 9/06, mostly 5mg. Tapered 4/12 through12/29/12

Wellbutrin. XL 300 mg started 1/07, tapered 1/18/13 through 7/8/13

Oxazepam mostly continuously since 6/06, 30mg since 12/12, tapered 1.17.14 through 8.26.15

11/06 Lithium 600mg twice daily, 2.2.14 400mg TID DIY liquid, 2.12.14 1150mg, 3.2.14 1100mg, 3.18.14 1075mg, 4/14 updose to 1100mg, 6.1.14 900 mg capsules 7.8.14 810mg, 8.17.14 725mg, 8.24.24 700mg...10.22.14 487.5mg, 3.9.15 475mg, 4.1.15 462.5mg 4.21.15 450mg 8.11.15 375mg, 11.28.15 362.5mg, back to 375mg four days later, 3.4.16 updose to 475 (too much going on to risk trouble)

9/4/13 Toprol-XL 25mg daily for sudden hypertension, tapered 11.12.13 through 5.3.14, last 10 days or so switched to atenolol

7.4.14 Started Walsh Protocol

The strange thing is that I was feeling rather stable until maybe two weeks ago when the very stressful symptoms appeared. And the symptoms worsened really quickly during a week. First I thought "maybe it is withdrawal". But as I eventually became quite burned out I realized it might have to do with Wellbutrin. I would call it Wellbutrin-intoxication. And with a moderate dose-reduction symptoms alleviated. It feels very strange to one day be a total wreck and next day so much better and the third pretty fine (well, 'fine' is a relative word in my case......)!!

We'll take "relatively fine" however we can get it! I did have the drop-withdrawal-then too much thing with Wellbutrin.

Yes, "fine" for me in my present situation is perhaps "wasted" for someone else...

Do you mean that you experienced something similar when you say too much thing? Or do you mean the opposite? Which is what happened to me last year when reducing from 300 to 150 mg and 3 months later had suicidal ideation :-o

I had been on an antipsychotic which was clearly the only reason I was able to tolerate Wellbutrin. Once I stopped the antipsychotic, I was way over-activated by Wellbutrin. I had WD symptoms with dropping, then would get over-stimulated again....basically the opposite. But hang in there, I never, ever think "Maybe I need an antidepressant." But I did have depression in WD and at times afterwards.

It's been a while... I think I've managed very well without the Wellbutrin, I've been without it for 5 months now!

 

A lot of stress lately. Because of that I made a break in my tapering plan but today I reduced the dose from 38 to 35 mg. 

 

Any suggestions about tapering when life is difficult and stressful? Going slower? Staying on the dose?

There are a couple of big stressors in my life right now and I've been feeling more depressed lately. Before this happened I was getting better, had more energy. 

It's hard to write, the words don't come easily right now.

Wow! Congratulations on getting off Wellbutrin Mjau :)  Its good to hear you've been doing well.

 

If life is particularly stressful, then going slower or even holding until things settle down is an option. If you wanted to keep tapering, perhaps do 5% cuts rather than 10% and try and find ways to reduce stress. We have some good ideas in the  Symptoms and self-care   forum.

 

There is nothing wrong with holding at your current dose until your life becomes less stressful. Are you taking any supplements?

 

Many people find  fish oil and magnesium helpful, see King of Supplements: Omega 3 Fatty Acids (Fish Oil) and Magnesium, Nature's Calcium Channel Blocker

It's extremely nice not having Wellbutrin in my system! I was very tired the first two months after quitting it but slowly things have become better.

Now things are more difficult due to external factors - stressors. At the same time it is a learning experience. Learning about my own patterns of reaction. And I see there is some work to do. And I do need to get off Sertraline. I think it clouds my thinking. It is as though my thought are only halfway thought. I have a feeling, a beginning of a thought, and it is hard to find the words to end the thought.... hard to explain. A lot of these half thought thoughts pop up in my head one after another, like loose threads, each thread that I pull slip away.

 

Now it is 5 or 6 days since I made a decrease in my Sertraline dose. From 38 to 35 mg. I think it's status quo so far. Depending on life in general and withdrawal in particular I might hold for a longer time now or maybe the next step will be just 1 mg. I'd like to have the process going. So that the cloud in my brain might lift.....

Now down on 33 mg, last reduction 3 weeks ago. Last two weeks have been really tough. I am exhausted. In the mornings I feel stressed out with feeling of pressure in my chest and heart palpitations. I find it hard planning things. Reading is more difficult. I haven't been able to work the last two weeks because I am so tired. Now and then feeling depressed. Usually I feel better in the evening, worse in the morning.

The external stressors in my life are somewhat lesser now compared to maybe a month ago.

What to do? I'm getting quite sick of this endless situation. Sorry just have to let out some steam. I am feeling so lonely and isolated ! I feel like I have lost my social capabilities, I don't know how to get to know people.

Hi Mjau , could you add the date you dropped from 35 to 33mg to your signature please?

 

That's less than 10% , you're clearly sensitive to even small drops.

Next time round you might consider an even smaller drop , just 1mg. The more we can retain our

quality of life while tapering , the better all around.

1987-1997 pertofran , prothiaden , Prozac 1997-2002 Zoloft 2002-2004 effexor 2004-2010 Lexapro 40mg

2010-2012Cymbalta 120mg

Sept. 2012 -decreased 90mg in 6months. Care taken over by Dr Lucire in March 2013 , decreased last 30mg at 2mg per week over 3 months. July 21 , 2013- last dose of Cymbalta

Protracted withdrawal syndrome kicked in badly Jan.2014 Unrelenting akathisia until May 2014. Voluntary hosp. admission. Cocktail of Seroquel, Ativan and mirtazapine and I was well enough to go home after 14 days. Stopped all hosp. meds in next few months.

July 2014 felt v.depressed - couldn't stop crying. Started pristiq 50mg. Felt improvement within days and continued to improve, so stayed on 50mg for 8 months.

Began taper 28 Feb. 2015. Pristiq 50mg down to 45mg. Had one month of w/d symptoms. Started CES therapy in March. No w/d symptoms down to 30mg.

October 2015 , taking 25mg Pristiq. Capsules compounded with slow-release additive.

March 2016 , 21mg

Hi Mjau. I just want to cheer you on, fellow zoloft (Sertraline) taperer. What you wrote about Wellbutrin - feeling better after a drop- I have experienced on zoloft. I feel stoned, exhausted and have headache, and then I make a tiny drop and feel so much better. I am glad you are off that medication.

As Fresh said, maybe you should try smaller drops.

Current dose: 0! Free!  Quit June 2017.

Started on 25 mg Zoloft in March 2011 due to stressrelated tinnitus that gave me panicattacks. Had a terrible reaction to Zoloft from start, but was told to "hold on". After four months I was stuck. Therefore the long taper. Crazy, I know... Super sensitive to drops and have dropped by 4-6 % from the previous dose.

You're probably right - I should go slower. Maybe I have reached a point where the reductions are adding up and my nervous system has become more sensitive.

I took a look at the post about micro tapering. I guess that's what I will have to do from now on!

 

Amy - it's encouraging to hear that you've felt better after the tiny reductions!

The last two days have been better - what a relief!! Still a lot of heart palpitations and fatigue, trouble concentrating and so on but my mood is better. It is so wonderful to feel a bit better. I - like so many others here as I've read - get the feeling that one will never recover when the symptoms are really bad. Now it feels like the sun is shining a bit brighter again :-)

 

I sure will hold for longer now. I'm still far from "stable". Next time I'lI go for that 1 mg drop. Maybe that'll be in June...

Once again - this forum is a truly great place to turn to in the midst of WD. Thank you all!

Now it's been 8 weeks since my last drop.

 

The last two days I've started to have a creepy feeling in my body. Restless body. Now and then I have restless legs and this feels somewhat similar although in the whole body. I want to move. It feels like a pressure inside my body, like my chest wall is too tight, I want to explode! It starts building up in the afternoon. It's hard to sleep. I feel restless.  I've had this symptom before when tapering Wellbutrin. 

 

I use magnesium lotion, it helps somewhat as do stretching. If I drink coffee the symptoms worsen dramatically, I haven't had any coffee for maybe two weeks because of this. Initially quitting coffee made the symptoms disappear but now they appear although I don't drink any coffee nor tea. (Ok I ate some chocolate today...)

 

The question is: do you think I should keep holding the dose? Or could this be a sign that I should cut the dose? 

I've had restless legs on and off for the last 8 years and I've regarded it as a side effect of Sertraline. That's why I wonder if I should cut the dose.

Hi Mjau! Thanks for stopping by my thread.

I would try a small drop, but that's me. You recognize this from the Wellbutrin taper, how? What would you do then?

Current dose: 0! Free!  Quit June 2017.

Hi Amy! Thank you for your response!

 

With Wellbutrin this symptom came maybe a week after dropping the dose. And then I hold the dose and the symptom would get better in another week, ten days. The thing is - and now I am theorizing - I was prescribed Wellbutrin because of restless legs syndrome because of it's dopamine effect which alleviates RLS. And Wellbutrin did alleviate my RLS. So the theory is that when I had less Wellbutrin in my body but the same amount of Sertraline the restlessness increased. And now with a "stable" dose os Sertraline the past 8 weeks - could it be a side effect building up? Because of my nervous system becoming more sensitive??? 

And yes I know the drugs make our nervous systems go totally haywire and it is probably hard to tell up from down..... so what do you think? Would it be wise to stay on the dose or micro taper?

In your sig it says that you tapered Sertraline quick due to side effects in 2012. But then you started it again? Have you tolerated it better since?

How are you today, Mjau? I dont know how to respond to your question whether to hold or to drop. Maybe try a tiny drop? 0.5-1 mg? It might lead you in the right direction.

Thank you Amy for asking! :-) It's been both ups and downs today. Terribly tired and foggy in the morning and afternoon. Didn't feel very restless earlier today. But so tired and I made the big mistake of having coffee. Now I've got the creeping feeling all over my body although I had the coffee 9 hours ago. Totally my own fault and stupid to give in to the craving.

 

I think I will wait and see how the restlessness evolves before I do any tapering since today it actually felt better to begin with.

 

I've been so very tired lately, I hope things will stabilize soon.

 

When I CTd Sertralin 2012 I also CTd Mirapex, a dopamine agonist, at the same time. I also had a lot of side effects of Mirapex. It's hard to say if I tolerate Sertraline better now compared to then. Everything is pretty messed up. I would say that I do not tolerate it at all!

The last couple of weeks have been quite tough with a lot of restlessness in my body. Today and yesterday has been a little bit better! I'm so happy for that. Now I've been sitting in front of the computer for half an hour maybe, and I feel the akathisia building up a bit. Seems like screens are not the best things to expose oneself to when having akathisia.... I'm on the same dose as two months ago so it seems the akathisia has been a withdrawal symptom - not a side effect this time. I'm so grateful to feel a bit better! It is horrible to have the feeling that ants are crawling around on ones body! I wouldn't say I'm completely stabilized yet. Maybe I will hold for another month.

 Hi Mjau.    I would perhaps hold for a while, but keep notes on your symptoms at the same time. It's hard to tell what's going on as you were put on Wellbutrin for RLS , but it's also a withdrawal symptom. I have had horrible RLS, and I've been off all drugs for 2 years. I didn't have it prior to the drugs, so for me it's a withdrawal symptom - a not very pleasant one !  I can empathize with the " ant crawling sensation"  but so can many others here. It is a common occurrence in withdrawal . It doesn't make it any easier at the time , though . I use Magnesium oil ( even though it stings) , and Magnesium baths. Nothing really gets rid of it in the moment , but certain strategies can help .  

I think, for now if you hold , you can't go too wrong .

Yes, I will hold. This day has been quite good, I'm so grateful for entering this window!! Looking back I can appreciate what I've achieved so far, quitting Wellbutrin and dropping Sertraline from 100 to 33 mg. And despite of that I'm having a window!! When I was feeling miserable with all the ants and brain fog I was afraid that maybe I won't be able to drop. But this window confirms that I can heal!! I wish I'll remember this in the next wave, because I'm pretty sure there will be more waves. 

 

I'll write it here so I can go back and read: yes your nervous system is able to heal. You've had windows. Just go slow!! 

 

I have a magnesium lotion. It helps a little.

 

This last wave episode makes me afraid though. Att 33 mg I've still got a long way ahead. A lot more withdrawal ahead.... :huh:​ 5% next time?

Mjau.  Certainly , there is a way to go , but it's a worthwhile goal , and maybe 5% is the best for you at this point. Don't let the length of time  do your head in . Just try , if you can to take it moment by moment .

You know now , that there are " windows" !   :)   Just remember to breathe.

Oh yes, it's been rough and still is but not at all as much as it used to be. I'm so thankful for that. The last week has been pretty stable as in "withdrawal-stable" and I'm considering taking my next step to reduce the dose, but very cautiously this time, just 1 mg. I've had some mild akathisia the last couple of nights which I hope is because of my monthly cycle and not a sign of destabilization.

What's positive is that my thinking has been a little clearer during the last week! 

I'll keep holding at least this week through to see what happens with the nightly akathisia.

I'm so excited to hear that your thinking has been clearer. I think that is such a big thing. It is so frustrating when we can't think straight, or our memory isn't working. 

 

I hope the akathisia passes with your cycle. Those monthly cycles really are a bear to get through. It makes you question all of your symptoms, is this worse because of my cycle or is it something else?

 

I think it's a great idea for you to hold until you know what is going on with the akathisia. I hope your next cut goes really well and you don't have withdrawal symptoms from it. That's always the hope :)  staying withdrawal stable. I'm glad you have been doing pretty well as far as that goes. I'm sorry you are having to deal with the nightly akathisia though. Do you stay in bed with it or is it bad enough that you have to get up and move?

 

I hope the akathisia goes away and you are able to sleep well. Hugs.

The cognitive symptoms are really tough. It's such a relief to notice a slight improvement. Even if the symptom worsens again - which a lot of symptoms have done - they wax and wane in intensity - you know it gets better! 

 

I usually have this restlessness a couple of days before the cycle. And this should be the case now, I hope! I stretch and it often helps, but sometimes if I'm in bed I have to get up and move. I go to the bathroom and then I cuddle with my cats. And then I stretch a little more. After that I usually can catch sleep. And now I've also found a magnesium lotion with menthol. The menthol is cooling and somehow this cooling sensation intermingles with and mitigates the creepy crawling akathisia sensation.

I'm so glad you have your cats to cuddle with. Animals have such a calming affect.

 

I'm glad you were able to find something to help with the akathisia. My anxiety and depression usually get me pretty badly right before my cycle. I've never experienced mood swings so bad. I hope that with each cycle it gets better for you despite the fact that you are still tapering.

Tntd, thank you for your kind words! Last night was a little better akathisia-wise than the night before. I went to sleep right after sending my last post yesterday, and I was filled with a positive energy from our little conversation. I actually think that was beneficial to the restlessness, I felt more relaxed about the akathisia.  

 

And today I had a moment when I felt confident in a way that is not usual for me. I've managed to do some more work without being more fatigued than usual. Wow, seems like things are finally going in the right direction! My gut feeling tells me to keep holding, to see how things evolve now that I've had these positive experiences. Maybe reduce with 0,5 mg next week?
