Yes, TD is related to Seroquel. My GP says clonazepam is the only Med that can cause Positive Histone Antibodies and she wants me to taper when I can.

 

Drug interactions in the drug checker all moderate. 
 

The benzo, Seroquel, and duloxetine all added during psych hospital stay in 2018 Nov. When I asked pdoc about all the drugs, he was unconcerned about polydrugging.

Many drugs can cause Positive Histone Antibodies. I got it from terbinafine. If I were you, I would stop taking statements from my doctors as the final word on anything. GPs know very little about adverse effects of psychiatric drugs.

My increased symptoms lasted 2 months. I appear to have TMJ which appeared a year ago (not confirmed by doc). This also increases symptoms like chewed up inner cheeks and clenching. I held my dose for August through October.

I’m aware of the number of medications and more that can cause PHA. I wrote what my doc says here for your information, not as the “final word”. Please let me know if you need anything else.

The reason I point out that many drugs may cause elevated ANA is that I suggest you taper clonazepam last. It may assist in masking withdrawal symptoms from  going off the other drugs.

 

Any of your drugs may be causing your elevated ANA. It's too bad the ANA did not resolve with Lexapro reduction.

 

The TD is most likely caused by Seroquel, Cymbalta is the second most likely.  

 

All of your drugs are going to be difficult to taper, and after you finish with Lexapro, I might reduce Seroquel next, in the hopes of reducing the ANA and TD.

 

Depending on how you feel physically now, you may wish to accelerate the taper, if your current symptoms are equal or worse than the risk of withdrawal. Have you had liver and kidney panels recently? The results would be important to this decision.

I might notify the hospital of the outcome of the drug treatment you initiated as an inpatient in 2018.

Yes, both negative. My symptoms have subsided since my first post. I am not wedded to tapering my Benzo and not feeling rushed.

I take them at the same time. There is no reason (I used to take clonazepam at bed with the Seroquel, but that made me uncomfortably dead to the world so I gradually moved it up). Just convenience, I guess. Do you suggest otherwise?

I forgot to indicate the PM times. Vitamins and Omegas at dinner Seroquel is at 9 PM.

I'm sorry, I am not being very clear. The oral TD symptoms were aggravated by something, and it seemed to occur after a cut, although it was also during the same period that I had a low grade fever for several weeks that led to the tests that found the positive ANAs. These are the events that may have increased symptoms in my tongue and jaw that have subsided now. In general, my tongue and jaw movements are mildly noticeable to others, mostly when I talk. I was concerned that my tongue was beginning to push against my teeth (rather than side-to-side) and out my mouth. My tongue is no longer pushing against my teeth, but I am not saying that the TD is gone. It has returned to its usual milder state.

Yes. When I was last here in 2016, I was experiencing horrific withdrawal from tapering 50% of the Lexapro. My doctor reinstated back to original dose which made it worse. We tried to add Wellbutrin again, and that was worse. That was when he prescribed the Lexapro in liquid so I could titrate off. I was still desperate and felt the only thing I could do was find a new medication, even if it meant kicking the can down the road and making things worse. I felt I could not let my world fall down around me and I had to survive. So, my doctor and I started trying to find something to take the edge off so I would survive. We didn't find anything that would help. I suffered from suicide ideation and would think of ending my life constantly. Not because I wanted to die, but because it was unbearable to be alive. The 2016 date is correct. I was hospitalized in November 2017, NOT 2018, as I wrote above- my apologies. All medications on the list above, except the Lexapro, were prescribed in hospital.

Yes, I was given a prescription for Clonazepam after having postpartum anxiety. About 3 months after that the psychiatrist added Zoloft and Clonazepam on an as needed basis, which I rarely needed. The, about 6 or 7 years into taking an antidepressants (1997ish), I started having squirrly feelings in my legs. My GP didn't find anything wrong. I noticed that I also felt agitated and that it happened mostly at night, and I would take a low dose to ease it and sleep. 

 

Seroquel was given in the hospital for panic attacks at night. When I got out of the hospital, I was a shell of my former self. I also had ECT which did nothing. My pdoc told me to keep taking everything, and I was in no place to challenge anyone about anything.

 

I hope this is clearer. Thank you, again.

Okay. Please keep daily notes of times o’clock you take your drugs, their dosages, and your symptoms throughout the day. We need to know how you feel before and after taking each drug, and your symptoms in between. Post 24 hours of notes at a time in this topic, in a simple list format with time o’clock on the left and notation (symptom or drug and dosage) on the right.

 

This can show if your symptoms are adverse effects from one of your drugs. With some small changes, such as changing the schedule for the clonazepam, you may be able to go off that last bit of Lexapro and start on Seroquel.

Honestly, I’m not sure. There are times when I come down from my office at the end of the work day and I notice that I chewed my cheeks up while I was working. I’m usually unaware of it until it hurts or is super active. I think I feel it mostly in the evening because I’m tired and hurting. I guess tracking will flesh that out. The guard is a good idea. Thank you.

It looks to me like this very dumb combination of drugs at 8:15 a.m. every morning causes problems some hours later. Since the escitalopram dose is so small, the other 2 are the more likely culprits.

 

How do you feel in the morning before you take this batch at 8:15? We need to know how you feel before and after taking your drugs. Please include this in your daily notes.

 

When did you last reduce escitalopram, from what dosage to what dosage, and what happened after that?

I usually wake up feeling symptom-free, unless I've had a busy jaw and then I might feel a headache. After a cut of the escitalopram, I sometimes experience mild anxiety in the morning, but it usually resolves as I start moving around. I will include in daily notes.

My last reduction was 11/28 from .14 ml/.14 mgs to .13 ml/.13 mgs. Usually within a week I experience mild anxiety that is short lived, and emotional lability that comes and goes and doesn't usually interfere with my functioning. Symptoms used to last longer and be more troublesome but this was relieved when I added the magnesium.

8 am - woke up no headache or anxiety

8:15 am - Duloxetine 60 mgs, Clonazepam 0.5, Escitalopram .13 ml/13 mgs, Probiotic. 

Throughout the day my tongue quivers and I am unaware of it. It is visible when I talk or when I put on lipstick I can see it in the mirror. The times I indicate tongue movements is when I notice it because it pushes against my mouth and teeth. I didn’t notice much tongue movements today or other symptoms.

5:30 pm - tongue movements while making dinner

9 pm - Seroquel 

9:40 pm - jaw feels tired, indigestion

10:30 pm - noticing tongue movements - sleep 9.5 hrs

@Freeby60 since you're taking 4 drugs, yours is a complex situation.

 

The Seroquel is the prime suspect for the tongue movements, which are not a good sign. I would prepare for tapering Seroquel. 

 

See Tips for tapering off quetiapine (Seroquel)

 

An initial test reduction of 10% is a way to see how tapering Seroquel will go. It's possible you might be able to get down to 12.5mg Seroquel without affecting your sleep. Then you can finish up the escitalopram taper.

 

Please continue to post daily notes. Please include the dosage with all your drugs.

I was thinking I would make a suspension- assuming I understood what I read and that’s possible. I stopped Wellbutrin that way and it worked fine.

7:20 am - woke up, no symptoms.

8 am - Duloxetine 60 mgs, Clonazepam 0.5, Escitalopram .13 ml/13 mgs, Probiotic. 

11:30 am - tired and decide to get back into bed. Sleep 3 hours - didn’t set alarm or expect to sleep that long (I attribute the periodic need to go back to bed during the day to being over medicated).

6:30 pm - omegas, magnesium chloride 1000 mgs

7:30 pm jaw clenching 

9 pm - Quetiapine fumarate 22.5 mgs 

9:40 pm - drowsy

10:10 pm - irritable - mild

10:30 pm - sleep 8 hrs. Woke during the night a few times (not usual). Fell back to sleep easily.

5:30 pm - tongue movements. Spasm of upper soft palate in mouth twice.*** this is new. It started 12/9 BEFORE the Seroquel cut. I didn't realise what it was until it happened again today. 

6:30 pm - omegas, magnesium chloride 1000 mgs 

8:30 pm - spasms again inside roof of the mouth

9:40 pm - Quetiapine fumarate 22.5 mgs *hits faster than taking the tablet

10:30 pm- sleep 9 hours

7:30 am - wake, no symptoms but rapid tongue movements while making coffee

8 am - Duloxetine 60 mgs, Clonazepam 0.5, Escitalopram .13 ml/13 mgs, Probiotic. 

9:30 am - oral spasm 2x

12:30 pm - feel tongue against teeth roof of mouth and resting between teeth

5:45 pm - bit tongue on the side while eating - crooked jaw

6:30 pm - omegas, magnesium chloride 1000 mgs

7 pm - mild anxiety passes

8-10 pm - tongue rests between teeth pushing against roof of mouth but behind lips

11:30pm - sleep 9.5 hrs

1:10 pm - roof of mouth spasms

6:30 pm - omegas, magnesium chloride 1000 mgs

7-11 pm - head and jaw ache, eyes sensitive to light

11 pm - Quetiapine fumarate 22.5 mgs* 

12 am - sleep 9 hours

* I try to take meds at the same time each night, but we were out until late.

12:30 pm - anxiety, not mild. Hit hard - did some deep breathing/ mindfulness and it passed.

6:30 pm - omegas, magnesium chloride 1000 mgs

7-10 pm - Jaw clenching, soreness

9:30 pm- Quetiapine fumarate 22.5 mgs *hits faster than taking the tablet

10:30 pm - slept 9 hours
