Yes Alto, tapering Prozac 25% to 15mg since Sun Sept 6.

His yelling starts the moment he's awake (like now) even when the room is still dim and he says the daylight is not bothering him much. He says he yells because of discomfort caused by his tight chest which begins upon waking up and gets better by late afternoon especially after a good nap. Yes, he's fully aware of his behavior and even apologized to others to excuse him.

In the meantime, it is really agonizing for him and everyone here, to hear his loud yelling and witnessed his sufferings, and he's constantly begging for drugs (antihistamines or benzos) to relieve him. Thanks.

Nov 2013 - Insomnia. xanax, slept well 5 days. Insomnia returned. Lexapro, benzos (xanax or lexotan). Agitation starts. Jan 2014 - See Dr A, given Mirtazapine, slept better. Quetiapine, Lorazepam. Agitation, low mood, nervous, short yelling. Jul 14 - See Dr M. Mirtazapine. Sodium Valproate, Valdoxan, Sulpiride, Tianeptine, Effexor, Lyrica, Venlafaxine. Nov 14 - Mirt. Olanzapine 5mg. Dec 14 - Agitation worsens, morning yelling. Addicted to benzos. Warded for ECT. Jan 2015 - almost 90% recovered but only for 1 month, milder agitation and yelling returns. Apr 15 - Agitation returns, stop supplements. consulted TCM, took the wrong advice, stopped all meds. Hysterical yelling all day. May 15 - Mirt15mg (night), Olan 15mg, add Vortioxetine 10mg every am. Chlorpromazine 25mg 3x daily. June 15 - V Bad agitation, loud yelling AM to night. Addicted to benzos. warded for uni ECT. after 1 week, mild agitation and morning yelling returns. 4/8/15 - Mirt, Olan, Vort. Body auto clock, or daylight around 7am. (Sorry I just edited my earlier post abit).

 

Recently, we went to another hospital ER (and he was yelling there for 5hours) to check his chest and heart and the lab tests were ok. So they sent him home instead.

When he wakes up, has he had a bad dream? Has he felt a jolt? Does he feel scared?

 

If I were you, I'd stop taking him to the hospital to have doctors work on him. That's how he's taking all those drugs.

 

You might darken the bedroom as much as possible with blackout shades to lessen the light stimulation that causes him to wake up. He might be experiencing a cortisol spike from the morning light, which can feel like a big jolt of panic.

No, no dreams, it happens almost every morning, but milder yellings. Mornings like these 2 weeks are really bad it's almost like a drug addict's withdrawal trying to tear his skin off.  We had to keep all doors & windows closed.  The recent ER visit was just to ensure he has no other health issues eg heart problem.  Thanks.

What to do, aren't doctors to be trusted when we're not well?

However, this episode of insomnia has totally cast our doubts on their expertise and the dangers of these drugs. It has been a good 20 month of roller coaster rides for us.  I really don't know how to deal with his hysterical yellings that's why we resort to the doctors for help. But in the end, they will pump in more med and stronger benzos to sedate him.  Sigh....

He is ok and complies with the antidepressants. But constantly asking for other drugs to relieve him. 

Should I relent and give him the benzo Valium till he calms down?

He is fully conscious of his loud yellings but he repeatedly says he cant control it. he feels slightly better by yelling.
What does he yell? Sorry, I dont know how to describe. Its like forceful shouting, ARGHHHHH....

Why? He says he feels agitated, chest is tight, uncomfortable (but no pain).

it's almost like a drug addict's withdrawal trying to tear his skin off. 

 

I'm wondering if your husband might be experiencing akathisia, caused by the anti-psychotics, but its presenting as yelling rather than pacing. When most people have akathisia, they have an uncontrollable urge to move or pace and doing it can bring a little relief, but your husband has an uncontrollable urge to yell and doing it makes him feel slightly better.

 

Does it feel like an inner tension or pressure inside the body and a feeling of wanting to get out of the body or get away from it to feel calm again?

 

Did the yelling start only after he had been given an anti-psychotic?

 

Of course, you don't have to be on an anti-psychotic to get akathisia, it can be a symptom of antidepressant withdrawal. But its also a common side effect of anti-psychotic drugs, especially Trifluoperazine

 

Your husband is on too many drugs and needs to taper off all of them, but I'm wondering if perhaps getting off the anti-psychotics first might stop the yelling?

His agitation and some yellings started after the anti-depressants and ironically he was put on "mild dose" of the psychotic drugs to counter this yelling issue. Today, his yellings is one of the worse and till evening, he is still feeling agitated. Thanks.

jncs, have you explained to your husband that the drugs are causing his symptoms and making him sick?

 

What does he think of REDUCING the drugs?

 

Did you tell him that as he reduces the drugs, he may feel odd? What does he think of that?

 

It sounds like you and he do not talk much. What is the problem there?

 

How much Prozac is he taking now?

Alto, as mentioned earlier, he is on 15mg Prozac (25% taper since Sun). We went for our usual evening walk yesterday. He agrees that he is taking too much drugs and is supportive of the withdrawal, and has been cooperating with me. He only commented that it's more work for me.

Lately, yelling is sometime he cant control even in a public place. This week, it is one of the worst! Should I increase Mirt or Prozac slightly? Or should I give him antihistamines or benzos? Is this something new to the forum here, why is he yelling? I'm so lost!

No, do NOT increase any of the drugs, even slightly!! If the drugs are making him sick, why do you think of this as a solution??

 

Are you giving him things you haven't mentioned here?

 

When he yells, ask him what he is feeling. We can't tell you what that is.

 

I'm going to be very honest here, jncs -- it's very difficult to discuss going off drugs when the person is not participating here on the site.

 

You MUST find out what's going on with him. What is his daily symptom pattern? Is he feeling certain things when he wakes up? What causes him to yell when you are taking a walk?

 

If you don't do that, we can't do much to help.

Okay, look carefully at this:

 

 

Using ***** together with ***** can increase the risk of a rare but serious condition called the serotonin syndrome, which may include symptoms such as confusion, hallucination, seizure, extreme changes in blood pressure, increased heart rate, fever, excessive sweating, shivering or shaking, blurred vision, muscle spasm or stiffness, tremor, incoordination, stomach cramp, nausea, vomiting, and diarrhea.

 

 

This applies to at least THREE of the drugs your husband is taking: mirtazapine, fluoxetine, and vortioxetine.

 

These symptoms are similar to what your husband is exhibiting, aren't they? Now do you think your doctors know what they are doing? He is vastly overmedicated and suffering from adverse effects, if not outright serotonin syndrome -- which can kill him.

 

You need to find new doctors, ones that will work on minimizing his drugs. Your husband's drug cocktail is bizarre.

 

As for what to reduce first:

 

24 Aug 15 - as above. Prozac increased to 20mg. Mirt increased to 11.25mg. Bad morning agitation and loud yelling. Usually subsides by later PM .

 

 

Increasing drugs that are causing adverse reactions increases the adverse reactions.

 

If I were you, I would make a fast taper off the Prozac, by 25% per week. I choose Prozac because it has a longer half-life than the others and, if luck holds, is less likely to add withdrawal symptoms to his problems.

 

Then I would go off vortioxetine. These are only my personal, non-doctor opinions.

He may very well be "yelling" because of the interaction of the drugs he is still taking.

 

If I were your husband, I would reduce Prozac by 25% again.

 

When your husband "yells," does he think he is having a heart attack? Can you talk to him about deep slow breathing instead?

 

What does he think about reducing his drugs? Is he afraid?

I think this is drug interactions too although it's strange that the yelling has not been experienced by anyone here, so far.

He is not afraid of the tapering, except he apologizes he cannot control his yellings, due to his tight chest discomfort.

No heart attack, no pain. Yes, have been reminding him to do deep slow breathing but it doesnt seem to help.

I read a lot about atypical depression - it's horrible during the day and much better at night - and with that can be lack of emotional regulation- I know when I increased my Paxil dose this happened to me - almost like the opposite of serotonin syndrome, it got to where I was thinking of harming myself - maybe the 2 drugs together are causing a paradoxical effect and his receptors are completely desensitized to serotonin?

 

I just know reducing my medication helped -

 

*Currently at 8.2-8.5 mg of my 10mg pill of Paxil (they actually weigh 12.5mg) 

january 2023 I began reducing my med again. I was a 9mg weight for years, I went to 8.9 in January, went to 8.6mg in February, and in March 2023 I went down to 8.5-8.2 mg ( my scale varies, so I stick within that .3 range because of that)

Hi Vonne, we totally agree that he should cut down the drugs. At the moment, I'm very confused not knowing how to relieve his yellings and agony. All of us are also very disturbed esp when he yells uncontrollably in public places at times.

He's taking antihistamines which doesnt seem to work much and he will ask for Valium, which I will avoid giving him.

I think this is drug interactions too although it's strange that the yelling has not been experienced by anyone here, so far.

He is not afraid of the tapering, except he apologizes he cannot control his yellings, due to his tight chest discomfort.

No heart attack, no pain. Yes, have been reminding him to do deep slow breathing but it doesnt seem to help.

Alto, just want to add that he is also yelling longer till late afternoon this week. Before, he would quieten down by early PM. So need your advice if we should continue to taper Prozac. Thanks.

Jncs, if he's taking a lot of drugs and the interactions are causing him discomfort, what do you think the answer is?

 

Can he tell you if the symptoms have gotten worse since you reduced Prozac? Why are you only reporting how long he is "yelling"? We need to know how he feels. You cannot make these decisions without knowing how they're affecting him.

 

Did you tell him he may feel odd when the dosages are changed? What does he think of that?

 

What do you and he think of reducing the vortioxetine now by 25%, rather than the Prozac?

He's feeling worse after the tapering, he just confirms it. The agitation and yelling are signs of discomfort.

Sorry don't want to add spanner in works but I think there is a major interaction between antihistamine and srri? This must be very distressing, I'm sorry.

2000 - sertraline for job anxiety low confidence (17 years old) ..which turned the next 16 years into nightmare!

 

On/off sertraline severe withdrawals every time. 2014 - felt better as reduced dose of sertraline no more inner restlessness. Doctor rushed off again. Hit severe withdrawal. Lost the little I had in life. Couldn't get stable again on 12.5mg. Was switched to prozac. Had severe reaction to prozac..came off in November 2015 at 6mg as felt more confused and damaged on it..Even more withdrawal ..rage, depression, dyphoria, near constant suicidal ideation, self harm impulses, doom, concrete block in head, unable to do much of anything with this feeling in head..went back on 6mg of sertraline to see if would alleviate anything. It didn't..reduced from December to June 2016 came off at 2.5mg sertraline as was hospitalised for the severe rage, suicidal impulses, and put on 50mg lofepramine which in 2nd week reduced all symptoms but gave insomnia which still have..psych stopped lofepramine cold turkey..no increased withdrawal symptoms new symptoms from lofepramine except persistant insomnia which has as side effect.

 

Taking Ativan for 8 months for the severe rage self harm impulses 1-3 times a week (mostly 2 times a week) at .5mg. Two months (I'm unsure exactly when the interdose started to happen) ago interdose withdrawal seemed to happen..2 days I think after the Ativan.

 

 

Nightmare that could have been avoided!

I can't make these decisions for you. If you and he don't want to reduce the drugs, he'll have to stay on them.

We want to reduce the drugs, just that he's feeling worse. So should we wait till his body adjusts, or should we taper Prozac or another drug instead. Thanks,

I'm replying to your PM here in your thread so others can weigh in too.

 

I'm so sorry you and your husband are going through this. It must be very difficult for both of you. I wish there was an easy solution.

 

I usually don't provide support via personal message. Its better if you post on your intro thread, that way you get the opinions of a variety of people. None of us are doctors or medical professionals and so we can't be responsible for any decisions you or your husband make.

 

It sounds like your husband is getting worse.

 

Alto has been trying to get you to communicate with your husband, to find out what he is thinking and experiencing. Its very difficult to support someone through a third person. Is it possible he could use the computer to read what has been written and answer some questions?

 

But having said that, I still think your husband might be having an adverse reaction to his anti-psychotic drugs along with all the other drugs and interactions.

 

From what you have written in your signature, the yelling started after the olanzapine was added.

 

The reason I'm concerned about the anti-psychotics is from of my own experience of having been put on them for a very short period when I was in my early 20's. I was put on halidol and Trifluperazine. I'd had an unusual experience, was scared and hadn't been sleeping. I agreed to go into hospital to get some help for sleep. I was in hospital for 9 days and was forced to take these drugs, but right from the start I knew they were making me feel much worse.

 

I was told I would have to take this drug for the rest of my life, but they made me feel so bad I knew that I would rather end my life than have to feel that way. I would take the pill at night and wake up the next morning with a feeling like I couldn't be inside my own skin, it was impossible to relax, I had the feeling I needed to get away from myself. This sensation would last all day until the evening when the effect of the drug would wear off. For about 2 hours in the evening I felt normal and happy, then I would take another pill, go to sleep and wake up the next morning and it would start all over again.

 

I realized it was the drug and so I stopped taking it. Just like that, no tapering. I was young, had never been on any medications before. I had been on these drugs, first haldol then trifluperazine for about 3  weeks in total. The next day I felt normal when I woke up. I never took another one of those pills. There was nothing wrong with me, I had just needed to get some sleep and some help with understanding what had happened to me. It took me a few weeks to get back to feeling completely normal after the shock of what had happened, but then I was fine and got on with my life. Although I never really got over the trauma of what happened to me in hospital, being misdiagnosed, force medicated and almost driven to suicide. I had gone into the hospital voluntarily for help with sleep.

 

It was many years later that I started taking anti-depressants for something completely different. But it was anti-psychotic drugs that caused severe akathisia. I've experienced milder akathisia since being in protracted withdrawal from anti-depressants over the last few years, I recognize the feeling from all those years ago. But the difference between then and now is that when I was taking the anti-psychotic, the symptom was directly related to when I took the drug, the pattern was exactly the same every day. In withdrawal, akathisia has come and gone in a random way when I have been on no medication at all and its been milder and more bearable. Akathisia caused by anti-psychotic drugs is pure, unrelenting, inner torture, there's no other way to describe it.

 

In your husband's case, he is on a lot of harmful drugs, with interactions and in my opinion he is also experiencing akathisia, whether caused directly by the anti-psychotic drugs or interactions or just the sheer number of drugs he's on. Please don't underestimate this symptom, its extremely unpleasant and can lead to violent behaviour including suicide and homicide, and your personal message indicated that this might now be a concern.

 

When someone has been on these drugs for more than a week or two we always advise tapering, to avoid the risk of withdrawal symptoms. But when there is an adverse reaction to a drug, its different and the situation gets more complicated. You have to weigh up the risk between causing withdrawal symptoms from stopping too fast and the ongoing adverse effects.

 

If symptoms are getting worse the longer he is on the drugs, to me it seems clear that he needs to get off them and in my opinion sooner rather than later. But beyond that, I don't know what else to advise. I'm so sorry that it has got to this point, I wish I had an answer.

Hi Petunia,

Thank you for your support, I really appreciate it.  As my hb condition is deteriorating, his sleep is shorter, yelling and agitation hence longer.  I'm terrified of his yellings and when he yells, I will just quiver, sometimes my heart will just stopped. 

 

We are strongly for tapering the drugs. 

We are seeing a psychotherapist psy to get their support to help us through this.  Thanks again.

Hi, I need your advice pls...

On 15 Sept, Our new psychotherapy psy dr has asked my hb to stop Trifluperazine 3mg immediately. We took 2 weeks to taper off and he is now on 0.5mg.

 

Today, our psy has instructed as follows:

1) Stop Trifluoperazine as of now.

2) Prozac to taper off now, 10mg - > 5mg - > 0mg weekly. So in two weeks time, he is off this drug.

3) Reinstate Mirtazapine to 15mg as of now, to keep him stable. (He was on 11.25mg since 24 Aug.)

What do you think? Thank you.

 

In the last two weeks, I gave melatonin (1.5mg - 5mg) and/or atarax to sedate him and after a good nap, his yelling is milder. However, the last two days he was very agitated and yells more as he did not sleep well, this could be due to the milder melatonin (1.5mg) as I was afraid his body clock will be confused.

No one can predict what will happen if you change or reduce any of the drugs. His "yelling" may get worse. Other symptoms may get worse.

 

Yet, your husband is taking so many drugs in such an odd combination, it's likely the drugs are causing him great distress and damaging his health.

 

One way or the other, my guess is he has to go off the drugs. How it is done should be your husband's decision.

This week is better, at least last two days he did not yell. ;)

But today, there's some yellings.

As discussed with doc, Prozac cut to 0.25mg from today and to stop in a week's time.

Remeron up to 13mg since last Friday, has helped in his mood & sleep.

Good to hear. I'd be very, very careful about adding any additional drugs such as propanolol, hydrozyxine, or Valium to calm him down. Adding drugs to treat his "symptoms" is exactly how you got into this mess. They interact and cause problems.

None of the published articles shed light on what ssri's ... actually do or what their hazards might be. Healy 2013. 

This is so true, with anything you get on these drugs, dependance, tapering, withdrawal symptoms, side effects, just silent. And if there is something mentioned then their is a serious disconnect between what is said and reality!

My hb is still yelling 5/7 days a week, sometimes to the point he will roll on the floor. and begged for Valium.

Our psychotherapy doc has asked to reduce his med quicker, but we are doing a gradual reduction (pl refer to Signature).

 

An ex consultant from CITA advised that Olanzapine does have evidence of causing yelling, screaming to their clients.

 

During our recent hols, he only had mild and short yellings 5/12 days. Wish we could take him on hols 365 days!

 

His sleep is also shorter and he is up by 5am, which further aggravates the agitation/yelling.

 

Should we do a quicker taper to Olanzapine, or Mirt?
