Hello!  I was put on medications during a weeklong hospital stay for major depressive disorder with psychotic features. I've experienced severe withdrawal symptoms, akathisia, insomnia, loss of appetite, body aches, depression 2 different times since the hospitalization.  Once because I tried to come off the drugs on my own and the other because I was weaned off of Zyprexa too quickly.

 

Now I'm on escitalopram and aripiprazol.  I'm tapering the aripiprazol first and am doing well with that so far.  I found out I was pregnant again and am 8 weeks along now.  I have some anxiety about being on these medications while pregnant, but I should at least be off the aripiprazol soon if everything goes well.  I plan on waiting 2-3 months after my last dose of aripiprazol before tapering the Lexapro.  I'm hoping everything will go okay.  It's difficult to know what symptoms are withdrawal related and which are pregnancy related.  Right now I've got stomach cramps with loose stools every day for the past 2 weeks.  Sleep is okay, but I feel so tired.  Emotions are a little difficult to handle when they come. 

 

Trying to stay calm and positive about all these changes.

@LotusRising So far everything seems to be going well with the Abilify taper.  I'm here to get on the right course and get some good advice and support during my taper.  My psychiatrist told me to quit the Abilify after I told him I was pregnant at 2.5 mg. He said it wasn't a big dose and I'd be fine.  After getting withdrawal twice that caused me to reinstate or start new medications, I didn't want to go through it again if I didn't have to.  So I have been tapering the Abilify at 10% and a few times reducing by 20% and holding anywhere from 7 days to 14 days.  I plan on jumping off somewhere around .3 mg and waiting a few months before tapering the Lexapro.  

 

The biggest struggle right now is figuring out if it's pregnancy symptoms or withdrawal.  I sometimes feel overly fatigued, nauseous, overly sensitive and unable to deal with emotions that feel very strong to me, and ruminating thoughts at night that at this point are easy to put aside if I do a coping technique.

It sounds like you have a good handle on what you're doing. Please just continue to listen to what your body is telling you and ensure you are feeling stable prior to starting the next taper.

Yes, I can understand how this could be challenging. Since you've been pregnant before, this may help you differentiate between the two, but coping skills will be the same nevertheless.

It's been 2 and a half weeks since my last dose of Abilify and the past two mornings I've had bad anxiety and crying spells in the morning.  By lunchtime I feel much  better.  I could use some encouragement to stay the course and maybe some advice on reinstating if my symptoms get worse.

Hi @doublehawk

 I’m not a mod but I do have experience with antipsychotic withdrawal, and that definitely sounds like withdrawal.

your brain is trying to acclimate to not having the drug. I don’t know how long you were on it but it seems like maybe your taper may have been a little fast. 
you have to go slow enough so the Neural adaptations that occurred because of the drug have time to start dialing back. Think about how long you’ve been on it and how long it took you to get where you are now. 
Our dopamine receptors up regulate and that takes time to reverse.. going slow enough for that to happen is really important. 
Any Dr who tells you that you can get off at a low dose has no idea what they’re talking about. The lowest doses are where you get the most disengagement from D2 receptors.. hence the highest chance of having difficulty. It’s called the hyperbolic curve. At the low doses you have to Proceed with caution and go as slow as necessary. 
As far as Reinstatement , perhaps the mods can help with that.

It’s kind of a personal decision and you need to listen to your body.

Are you sleeping? 
are you eating?

 If not ,those are some indications of dopamine super sensitivity and that’s a problem, It can snowball if you’re not sleeping. Anyone who doesn’t sleep long enough will eventually go psychotic… not to mention those of us who have been on antipsychotics.

Depakote once, ALL AWFUL 

and most recently Zyprexa.

All of which I discontinued shortly after leaving the hospital except Zyprexa. It proved to difficult. So now, a year and a half later I am trying to taper off the zyprexa. I am here for support, Help with my taper, and to hear the success stories of those who have come out on top of this and are getting on with their lives.

Thank you for the response.  Yes I did taper more quickly than recommended.  I did 20% decreases with a hold of 2 weeks each time.  As for now I'm sleeping and able to eat from lunchtime onward.  My waves are in the morning and have gotten shorter over a period of 3 days.  I even had a small 5 minute window today around 8:30 AM.  

 

I was on Zyprexa before Abilify and had a bad withdrawal following my psychiatrist's taper.  I wish you well on your tapering journey.  You're doing it right.

I wish you luck as well.. getting off this stuff is hellish.  If I was pregnant I wouldn’t want to be on antipsychotics either.  I’m glad you are sleeping and eating that’s a great sign. If you can handle the waves of emotions hopefully you won’t need to reinstate. 
Withdrawal can last a while and be scary at times. “Windows and waves”

so please don’t be afraid to reach out again if things get weird, we’re all supposed to be here to help each other right? 
 I’m going to send you the article anyway, because when I’m going through it… I read it to remind myself that I’m not going crazy, that I’m healing. Knowledge is power .

2016-present: 100mg Seroquel for sleep -> May 2023: 90mg -> June 2023: 81mg -> September 2023: 72mg -> switched to brand name, much too strong, down to 60mg -> October 2023: 54mg -> November 2023: 50mg -> January 2024: 45mg -> April 2024: 40.5mg

2016-Present: 100mg Wellbutrin SR -> January 2023: 75mg IR (37.5mg 2x a day) -> February 2023 (33.75mg 2x a day) -> July 2023 (30.37mg 2x a day) -> August 2023: 27.33mg 2x a day

2015-present: 600mg Gabapentin (200mg 3x a day) -> December 2022: 300mg Gabapentin (100mg 3x a day) per GP's recommendation after side effects -> March 2023: 90mg 3x a day (switched to liquid suspension) -> April 2023: 81mg 3x a day -> September 2023: bad generic, switched back to homemade liquid; too strong after bad generic, down to 70mg 3x a day, still bad. Long hold till -> December 2023: 54mg, still feels too high after November Seroquel switch from brand name to generic, doc recommended 50mg which feels better -> January 2024: When Wellbutrin went down, Gabapentin started putting me to sleep, went down to 45mg, then 41mg to stay awake, so far so good -> February 2024: 36mg, still too high, 34mg -> March 2024: 31mg, STILL too high, 30mg

Supplements: Multivitamin w/magnesium, probiotics, digestive enzymes, anti-viral nitric oxide nose spray as needed

Thank you. It was very difficult when I was losing the baby and shortly afterwards, but my family and I are doing better.

 

my sleep took a hit last night. I kept getting increased heart rate while lying down even when I was trying my best to relax. I got somewhere between 2-4 hours. 
 

My very short window in the morning just feels like I can interact with the world normally again. It’s a brief moment where I don’t feel like I’m riding a rollercoaster while not strapped in. Then I have a big window from about 12 PM until I wake up except for last night.  In the windows I am able to push fears aside easier and I can eat.

 

The intensity of my morning waves has been going down over the past few days. Even with the little sleep last night, I think I did alright.

 

If I’m unable to sleep for a few nights in a row, I might reinstate. Insomnia is a huge trigger and the effect will be cumulative.

Are you able to check on hormone/nutrient/blood levels just to make sure nothing is off? The heart-rate while lying down makes me wonder about electrolytes. I am not a doctor, just a thought based on things I've read while looking into my own heartrate issues.

Insomnia is absolutely brutal. What have you tried so far to help? There might be some alternatives that could help worth trying before going for reinstatement (especially if the meds had side effects you'd like to avoid).

 

Hope you're able to get some relief and rest, there's nothing like lack of sleep to make everything seem more difficult. If you're a hug person, sending a big ole internet hug and hope that you get restful sleep soon.

I think the heart rate thing was anxiety related.  I have been drinking some electrolyte powders because I noticed that it helps my anxiety along with taking iron.  

 

For the insomnia I've been doing what I have been for the past 4 years which is taking magnesium.  Right now I'm taking magnesium glycinate gummies.

 

I've notice more pockets of windows in my big morning wave.  It's making me feel hopeful.  I did get about 5 hours of sleep last night which I think is pretty dang good right now.

Thank you so much!  I'm doing alright.  I have 3 living children and have had 3 miscarriages.  It can be devastating, but this time I had 3 children to take care of and bring the joy back into my life, so it helps to get outside yourself when something difficult is happening.

4-28-23 Zoloft 100mg, fast taper to 0 in May 2023 reinstated 100 mg, 7-25-23 50 mg, 8-1-23 0 mg

4-28-23 Zyprexa 10mg, fast taper to 0 in May 2023 reinstated to 5 mg, 7-25-23  2.5 mg, 8-29-23 0 mg. Severe withdrawal was put on other medications because the side effects of Zoloft and Zyprexa.

9-14-23 Lexapro 10 mg

9-14-23 Abilify 5 mg, 9-17-23 2.5 mg, 11-10-23 2.25 mg, 11-18-23 2.02 mg, 11-29-23 1.81 mg, 12-7-23 1.62 mg, 12-13-23 1.25 mg, 12-30-23 1 mg, 1-7-24 .8 mg, 1-14-24 .64 mg, 1-27-24 .51 mg, 2-10-24 .44mg, 2-22-24 .3 mg, 3-8-24 0 mg

Magnesium glycinate 200 mg, Fish oil, Nettle tea

I’m happy you’re doing well, and yes it can be devastating. But you’re absolutely right about getting outside yourself.. it really helps. 
 Kids also bring you a lot of comfort in difficult times. I don’t know what I would do without my boys. They are the love of my life.

I have been on and off psychiatric medications for about 10- 11 years…

since 2013-

I have been on

Risperidone once,

 palliperidone once,

 Abilify

Depakote once, ALL AWFUL 

and most recently Zyprexa.

All of which I discontinued shortly after leaving the hospital except Zyprexa. It proved to difficult. So now, a year and a half later I am trying to taper off the zyprexa. I am here for support, Help with my taper, and to hear the success stories of those who have come out on top of this and are getting on with their lives.
