Hi All:

I have been taking 3.75 mg. of generic remeron daily for the past 4.5 years.  About a month ago, I thought I should taper off by reducing to 1/8 of the 15 mg. pill that I had been cutting into quarters for the 3.75 mg. dosing.  Around day 26 of the taper, I started getting GERD, heartburn, insomnia, dizziness, all the usual suspects. 

 

I upped my dose back to 3.75 about 3 nites ago.  I thought I was on my way to feeling stable again, but last nite came no sleep, with sweating, racing thoughts, and diarrhea.  This a.m., I feel so terrible.

 

My question is:  how long to expect the withdrawal symptoms of the taper on the way back up to my pre-taper dose.  I felt fine while taking this amt and was prescribed it for insomnia and post menopause symptoms.  Now I realize I may be a life member of the remeron suckers club.

 

Hope to hear from those who have had similar experiences.

Hi Shebon, welcome to SA,  In order to taper a small dose of remeron you will need either a liquid solution from the pharmacy or you can make a liquid from tablets. Then you will need to taper slowly to avoid withdrawal. Remeron is an awful drug and needs tapering carefully but it isn't easy when cutting tablets. It is likely that the pieces were not equal when cutting into 8, which would have meant the doses were different as well as cutting the dose by 50%  You did your best and I am not in any way criticising, just expleining why you feel so bad. I thought a low dose of effexor would be easy to taper but I was wrong, the lower doses are harder! 

It is 3 days since reinstating 3.75, it takes 4 days to reach a steady state in the blood so hopefully you will see an improvement. Did you feel any better after updosing?   Hold that dose until you have been stable for a few months. This is to let your nervous system settle down again before restarting a taper. 

 

 You can dissolve a tablet in water, then use an oral syringe to get the right dose. You can get 1ml oral syringes from the pharmacy, and can use as much water as you need to get the correct dose. You can keep the solution in the fridge. It will need a good shake so the particles are fully mixed. Withdrawal insomnia is terrible but should settle down as you stabilise.  I will post some links for you.

1993 - 2002 in and out of hospital. every type of drug + ECT. Staring with seroxat

2002  effexor. 

Tapered  March 2012 to March 2013, ending with 5 beads.

Withdrawal April 2013 . Reinstated 5 beads reduced to 4 beads May 2013

Restarted taper  Nov 2013  

OFF EFFEXOR Feb 2015    :D 

Tapered atenolol and omeprazole Dec 2013 - May 2014

 

Tapering tramadol, Feb 2015 100mg , March 2015 50mg  

 July 2017 30mg.  May 15 2018 25mg

Taking fish oil, magnesium, B12, folic acid, bilberry eyebright for eye pressure.

Lesson learned, slow down taper at lower doses. Taper no more than 10% of CURRENT dose if possible

Thank you MamaP and Chessie for your replies.  I never expected this reaction from reducing a small dose.

I had been on 3.75 mg. remeron for 3 1/2 years.  In July, I decided to taper off the remeron (cut 1/4 in half -- 1/87 mg. ) as I was feeling pretty good after a long benzo recovery.

 

Day 26 of the 50% reduction brought on heartburn, GERD, hot flushes, and insomnia.  I went back up to my pre-taper dose of 3.75 mg. on day 3- of the taper, and just had nite 9 of 3.75 mg.  While I have had (5) good days/nites since updosing, last nite was another sleepless nite (have had (3) of these).  With burning, tingly body feelings, nausea, diarrhea, intrusive thoughts.  I am wondering how long to expect before I stabilize on my previous dose again.  I could shoot myself for doing this taper and wish I had never done this.  Seems like last nite/this a.m. was worse than any others.

 

Should I go up higher on my dose or wait it out?

How are you getting your dose?  Are you still cutting your tablets?  Or have you got/are you making a liquid?

5 out of 9 good days is a good sign that updosing is working.  It may take a few more weeks for your symptoms to settle.  Please see the topics I posting about especially windows and waves.

 

Sleep problems - that awful withdrawal insomnia

 

It's important not to get caught up being anxious about the symptoms (this adds stress) and to use non drug techniques to keep yourself as calm as possible.

I use a pill cutter to split the 15 mg. tab into quarters -- I try to make them as equal in size as I possibly can.  So, you're thinking I will stabilize again with just the 3.75 mg. dose that I was taking pre-taper.  Don't go up in dosing?

 

On a few of the good days I have had, I think I overexerted myself with 4 mile hikes, zumba, weights, etc.  I am just going to take it easy until I reach the pre-taper place.

 

I know I have PTSD from having lived through benzo WDS when I went off in 4/2011.  It's scary because the symptoms are so scary.  I am quite familiar with the windows and waves of WDs that I thought I would never experience them again.  But voila, here I go doing a stupid thing too fast and back to the WDs, which are frightening.

DRUG HISTORY:

 

November 2013- Zoloft, ( Bad reaction).

January 2014 - March 2014 Seroquel.( Quit Cold Turkey).

January2014- Mirtazapine, I was taking 15mg at one stage, reduced to 7.5mg, Pgad reactions to Mirtazapine. Doctor kept increasing it to 37.5mg, until July 2014. No improvement, experiencing panic attacks, on 37.5 mg. I had enough by October 2014. Began tapering.

October 2014- Started tapering Mirtazapine from 37.5mg.

September 2015- Down to 4mg of Mirtazapine. Crashed.

September 16th- Up dosed to 5mg. Held this dose for almost 5 months. Stabilised.

February 2016- Began tapering again. From 5mg to 4.5mg of Mirtazapine. (Rocking the boat, again)! Lol. :(

I will be traveling the first few weeks of June so I decided to get down to my taper after the travel.  But I did something stupid; took magnesium/calcium one hour before remeron (10 p.m.) for four days and it threw me back into the withdrawals.  Go figure....I should know calcium will interfere with remeron absorption.  Live and learn right?

 

How is your taper going?  Are you using the regular mirt and not the soluable tabs?  I think I will be sticking with the regular mirt and not switching over.  Your thoughts??

Hi Everyone:

 

Just an update to my current situation with tapering down from 3.75 mg.  I have now reduced by 50% to 1.87 mg. mirt.  I started the taper 1/1/20.  So 6.5 months later, I have reached my first big milestone.  

 

Symptoms come and go but were worse at the start of the taper.  I will hold at 1.87 for 1-2 months and then resume again with my modified CITA taper.  The 1/8 piece of the 15 mg. tablet is quite small but I figure I can still cut a piece of that in half and start over with the 6 nites at 1.87 mg. mirt and 1 nite of .93 mg. mirt.  Hold this for a month and then add 2 nites of .93 mg. mirt in, and so on down for another 6.5 months until I am at .93 mg. mirt.  It is a SLOW process, but I think healing is happening as one tapers and that is a huge benefit.

Hi Shebon. Congrats on getting to your halfway milestone. You're getting close! I remember when I was that low on trazodone I couldn't weight it anymore as it was not registering.

What I did was crushing a small piece on a mirror and dividing it by 2, then dividing it again and again until I had a bunch of very tiny equal amounts. I did this on a big flat mirror with a razor blade. I had lots tiny doses at a time set to take every night. I would make new ones when I ran out. I put a dab of peanut butter on my finger and touched the dose. It stuck on it and I licked it off. Like a druggie lol. That is how I tapered off the tiny bits of trazodone. Marie.

10/13--10/14 Ambien. Started tapering 1/14  Jumped 10/14.  Done.                                                                              

3/14        7.5 Remeron  still taking this.                              

2/14         75 Trazodone   -    Tapered by dry cutting all the way down.

1/16        4 mg Trazodone  -  Jumped. Bad mistake. Got hit with late withdrawal 6 weeks later. Reinstated.

4/16        Reinstated 1 mg, updose to 2 mg Trazodone

2/19        .04 Trazodone. Walked off.  Done.

10/3/19  Started 7.5 Mirtazapine taper cut to .073 gram weight, pill weighs .076

To get the effect of magnesium (for calming/anxiety) it should be taken 2 hours apart from calcium, otherwise the calcium cancels out the magnesium.

Hi Shebon. I was wondering how thyroid medications can affect your Mirt taper. Are you tapering your thyroid med at the same time as the Mirt? Marie.

10/13--10/14 Ambien. Started tapering 1/14  Jumped 10/14.  Done.                                                                              

3/14        7.5 Remeron  still taking this.                              

2/14         75 Trazodone   -    Tapered by dry cutting all the way down.

1/16        4 mg Trazodone  -  Jumped. Bad mistake. Got hit with late withdrawal 6 weeks later. Reinstated.

I have gone hyper on my thyroid meds at least 5 times.  The symptoms are much like benzo and AD withdrawals.   I did find an article that showed one taking Prozac needed to take more thyroid med.  So I deduced that if we are tapering lower on the  mirt, we do not need as much thyroid med.  

 

I had gone off my thyroid med for almost a year (2017-2018) and I did great.  My mirt dose was 3.75 mg.  I fell asleep fine and why I went back on is still a puzzle to me today.  Interesting thing about levothyroxine - the half life is 7 days.  It will build up in the system.  Just trying to even out now (0.125 mcg Synthroid every other day) which can take about 2 months and then I plan to reduce mirt again.  I have been doing well with my reducing mirt and I know the symptoms of overdosed thyroid meds very well.  It comes on suddenly and gets worse for about 2 months.

Started 3.75 mg. remeron 4/2013.  Had tapered off benzo 4/2011.  Had tapered down to 1.87 mg remeron but mistakenly thought I was in bad withdrawals, when it was Synthroid again.  Stopped  Synthroid 3/31/3021-side effects are the same as hyperthyroid symptoms, but doctors have no knowledge of this side effect.  Started taper end of 7/2021, now at 3.48 mg. daily.

Shebon I like your first posting...a member of the Remeron Suckers Club. I had a good laugh on that one. I hope I never have to mess with thyroid meds. I've never had it checked, so no news is good news. It seems you got a good handle on how to taper it while cutting the Mirt too. We'll finish up in time. Be well. Marie.

10/13--10/14 Ambien. Started tapering 1/14  Jumped 10/14.  Done.                                                                              

3/14        7.5 Remeron  still taking this.                              

2/14         75 Trazodone   -    Tapered by dry cutting all the way down.

1/16        4 mg Trazodone  -  Jumped. Bad mistake. Got hit with late withdrawal 6 weeks later. Reinstated.

4/16        Reinstated 1 mg, updose to 2 mg Trazodone

2/19        .04 Trazodone. Walked off.  Done.

10/3/19  Started 7.5 Mirtazapine taper cut to .073 gram weight, pill weighs .076

4/5/20    New Mirtazapine Taper - Compound Liquid 7.35 mg April '20, 7.25 mg May, 7.05 mg June, 6.99 mg June, 6.78 mg July, 6.57 mg Aug, Sept 6.35 mg, Sept 6.24 mg, Sept 6.21 mg, Oct 5.99 mg, Oct 5.90 mg, Oct 5.70 mg.

1/11/21 6.05 mg Messed up taper due to syringe change. Must remember the 1 ml syringe contains 1.5mg! 1/16/21 5.99 mg

2/21 5.75 mg, 3/21 5.6 mg, 4/7 5.45, 4/14 5.30, 5/12 5.15, 5/25/21 4.99 mg, 6/29 4.87 mg, 7/14/21 4.74 mg, 8/5 4.62 mg 8/17 4.5 mg, 8/30 4.38 mg,9/16 4.26 mg,10/9 4.14 mg, 10/23 4.05 mg, 11/6 3.96 mg,11/17 3.87mg.***Jan 22 Liquid was changed/couldn't tolerate***Changed back to pills.
