Bluma: Paxil and mirtazapine

Hi All

i am finally down to Paxil and Mirtazapine.  My doctor plans to taper me off 15 mg of Mirtazapi me.   I’ve been taking it for about 8 months.  I’ve been on 60 Paxil for 3 months.  That’s the next to go.  

50 mg Fluvoxamine started 1994

increased to 150 mg in 1997

tapered off 50 mg in 2017 to 2018 to 0 mg March 15/18. 

.25 mg clonazepam twice a day for 6 months.  Tapered off clonazepam starting April 16, 2018 reducing each dosage by .125 mg for 5 weeks.  Last dose May 17/18.  

 Paxil 60 mg at night for last 3 months

15 mg Mirtazapine for last 8 months

Hello, Bluma, and welcome to SA.  Before you do any tapering (including based on your doctor's suggestions), please read the following links.  Please be aware that doctors have little, if any, idea about psychiatric drug withdrawal or slow tapering and will, almost invariably, set you on a too-fast taper that will not work.

I'm going to give you several links to read.  Please take your time and don't be overwhelmed.  First off, at Surviving Antidepressants, it is recommended that a person taper by no more than 10% of their current dose with at least a four week hold in-between decreases.  The 10% taper recommendation is a harm reduction approach to going off psychiatric drugs.  Some people may have to taper at a more conservative rate as they are sensitive to even the smallest drops.
  
 
 
 
Here are some tips for talking to your doctor, who, as I've said, may not be up-to-steam on what's best for you in going off these drugs.

Gridley Introduction

 

Lexapro 20 mg since 2004.  Begin Brassmonkey Slide Taper Jan. 2017.   

End 2017 year 1 of taper at 9.25mg 

End 2018 year 2 of taper at 4.1mg

End 2019 year 3 of taper at 1.0mg  

Oct. 30, 2020  Jump to zero from 0.025mg.  Current dose: 0.000mg

3 year, 10 month taper is 100% complete.

 

Ativan 1 mg to 1.875mg 1986-2020, two CT's and reinstatements

Nov. 2020, 7-week Ativan-Valium crossover to 18.75mg Valium

Feb. 2021, begin 10%/4 week taper of 18.75mg Valium 

End 2021  year 1 of Valium taper at 6mg

End 2022 year 2 of Valium taper at 2.75mg 

End 2023 year 3 of Valium taper at 1mg

Jan. 24, 2024: Hold at 1mg and shift to Imipramine taper.

Taper is 95% complete.

 

Imipramine 75 mg daily since 1986.  Jan.-Sept. 2016 tapered to 14.4mg  

March 22, 2022: Begin 10%/4 week taper

Aug. 5, 2022: hold at 9.5mg and shift to Valium taper

Jan. 24, 2024: Resume Imipramine taper.  Current dose as of April 1: 6.8mg

Taper is 91% complete.  

  

Supplements: multiple, quercetin, omega-3, vitamins C, E and D3, magnesium glycinate, probiotics, zinc, melatonin .3mg, iron, serrapeptase, nattokinase

Are you off everything?

Hi Again

 

I had been on Luvox for about 23 years.  It stopped working about 15 months ago.  The doctor tried to put me on different medications without success.  She realized that I was going through withdrawal from Luvox or fluvoxatine.  I was taking 150 mg per day.   When she realized I was going through withdrawal she reinstated 50 mg.  In December 2017.  I tapered off and took my last dose March 15th 2018.  I was also taking .50 mg of clonazepam two times per day for the past 6 months.  I started weaning off of that doseage on April 16, 2018 and completed the taper on May 17,2018.  

 

Im currently on 60 mg of Paxil and 15 mg of Remeron.  I’m having stomach pain, light headed, bloated stomach and frequent bowel movements per day.  Now the doctor wants me to introduce 50 mg of Pristiq and reduce Paxil to 50 mg.  She wants to cross taper Pristiq for Paxil.  Paxil not working.  I’ve been on Paxil for 3 months and Remeron for about 8 months.  

 

I dont know what what to do.  Please help.

It's a good idea to check Drug Interactions Checker for current drugs and also proposed drugs changes.  You could end up going from the frying pan into the fire.  The following is the interaction Remeron & Pristiq.

 

If you check your current drug combo you will see that there is a different major interaction.

Applies to: Remeron (mirtazapine), Pristiq (desvenlafaxine)

Using mirtazapine together with desvenlafaxine can increase the risk of a rare but serious condition called the serotonin syndrome, which may include symptoms such as confusion, hallucination, seizure, extreme changes in blood pressure, increased heart rate, fever, excessive sweating, shivering or shaking, blurred vision, muscle spasm or stiffness, tremor, incoordination, stomach cramp, nausea, vomiting, and diarrhea. Severe cases may result in coma and even death.

These symptoms may be withdrawal symptoms from having gone off the Luvox so quickly.
