TreeFrog: trying to stabilize from Cymbalta withdrawal

I took my last dose of duloxetine on February 11, so almost 5 weeks ago. Things don't seem to be getting any easier. For the first couple of weeks, I had some physical symptoms like nausea and brain zaps, but those have passed. I've been feeling a succession of intense and unpleasant emotions--irritability to the point of rage, anxiety, grief--throughout, but now I seem to be descending into a real pit of feeling what I've always called "depression"; a heavy, dark, nauseating ache.

 

Some history:

  • Started citalopram in 2008; prescribed by family doctor; by 2010 I was up to 50 mg.
  • Started seeing a psychiatrist in December 2010. Over the course of 2011, I tapered from 50 mg down to 20 mg, then settled in at 20 mg (with occasional increases to 25 or 30 mg) until around 2018.
  • Tapered down from 20 mg to 5 mg over at least a year(?) starting in 2018, until fall 2019.
  • Experienced a major, unprecedented episode of intense anxiety and insomnia in fall 2019 that lasted at least two months; went from 5 mg back up to 20 mg in six weeks.
  • Stayed on citalopram until November 23, 2021. On recommendation of family doctor and psychiatrist, I switched to duloxetine. I tapered off of citalopram, from 20 mg to 15 mg (over one week), from 15 mg to 10 mg (over two weeks); then started duloxetine 30 mg. Increased to 60 mg after two weeks. It was recommended to me as a possible treatment for chronic musculoskeletal pain, as well as depression.
  • Stayed on 60 mg duloxetine until January 16, 2022 (for almost five weeks). Started getting intense night sweats about three weeks in, and no chronic pain relief, so told my doctor(s) I wanted to get off it.
  • Went down to 30 mg duloxetine until January 31 (for two weeks), then started skipping doses (I know, but I had never heard of bead counting; I just followed the advice I was given). Two days on, one day off for five days; then two days off, one day on for six days; end date February 11.

 

I would never consider going back on duloxetine, but I am thinking seriously about going back on citalopram. When I talked to my psychiatrist last week about this, and about the feelings I've been having since I stopped taking duloxetine, she said I should do whatever I think I need to. But she also made it clear that she doesn't think I currently have a "disorder" that warrants an antidepressant, and that I am experiencing such strong feelings because I am no longer subject to the antidepressant "side effect" of "emotional muffling." She also doesn't think that I am in withdrawal anymore, after four weeks plus.

I'm sorry you are still suffering after stopping duloxetine. The advice you were given was completely inappropriate. Very few doctors or psychiatrists have any idea how to taper off duloxetine. It changes your brain within 2 - 3 weeks of starting to take it. Sudden drops, alternate days: these all make it harder for your brain to stabilise. Bridging to another drug does not help - it just means you now have another dangerous psychotropic drug to taper at some point, along with the withdrawals you have now.

There is, even at this late stage, the option to reinstate the duloxetine, as there is some chance it would work. But it often does not help, sometimes makes things worse.

Withdrawals from duloxetine can last months: your specialist is wrong in thinking they have finished. They can make your life intolerable, prevent you functioning and caring for yourself and others. This is not to alarm you, but to warn you to be prepared. In some cases, what some call Protracted Acute Withdrawal Syndrome (PAWS / PWS) may set in, which is as bad as it sounds. 

Your best approach now seems to be to manage the withdrawals. You cannot cure this with more drugs. the more you add to the mix, the harder it all becomes.

It sounds like you don't think there would be any benefit at all to starting a very low dose of citalopram and then very very gradually tapering off of that? I haven't taken duloxetine for almost 5 weeks and I haven't taken citalopram for almost four months.

 

Is there anyone reading this who has had experience with switching from citalopram to duloxetine and back, or even just generally with switching ADs and then discontinuing the new AD, and  reinstating the original AD?

My experience tells me it would be risky, not just have no benefit. You need to deal with the withdrawals you are suffering, through a clean and healthy diet and lifestyle, not adding more drugs.

Your psychiatrist doesn't know what she's talking about, but that's not unusual. Few grasp what withdrawal syndrome is.

 

Since you went off duloxetine, have your symptoms lessened or increased in intensity? How has your sleep pattern changed?

The past couple of years on citalopram, I had some occasional bouts of pretty mild depression and anxiety, but generally I felt what I think of as "ok", even with the pandemic etc. I felt much the same the short time I was on duloxetine; the main difference I noticed, as mentioned, was that sleeping "hot" suddenly scaled up to real night sweats, virtually every night. I told my psychiatrist that even though I wouldn't be on it long enough to find out whether it would relieve chronic musculoskeletal pain, the night sweats were a deal breaker and I wanted to get off it.

I tapered duloxetine exactly as advised and outlined in my signature. When I saw my psychiatrist last week, I'd been off duloxetine for 3.5 weeks. I told her what I was experiencing and how intense and painful my emotions were and that I thought I was in withdrawal; as mentioned, she said I was not, after that much time.

I'm back to sleeping hot, and maybe hotter than I was, but no night sweats. The kind and intensity of emotions I'm experiencing fluctuate from day to day; earlier this week, when I first posted, I felt as depressed as I ever have in my life for most of several days, but that's receded for now. The extreme irritability I felt a couple of weeks ago seems to have moderated too.  It still takes very little for me to start crying, and I'm having some pretty intense jolts of anxiety, but those may be a bit better too. (I realize now that it would have been good to keep detailed notes over the past few weeks, but I haven't been). I don't feel confident of my mental state though; I feel like the emotions could become overwhelming at any time.

 

I'm very grateful to say that my sleep hasn't changed. I sleep quite heavily for at least 8 hours.

 

There's something else: I've been doing quite a lot of reading in some of the topics, and I'm wondering now if the two months of intense anxiety and insomnia I had in starting in late August 2019 was at least partially caused by the citalopram taper I was on at the time. Since I first posted, I've looked back at my prescription records, and it looks like this:

 

December 2013-August 2018: 20 mg (with a couple of brief forays up to 25 mg, and one down to 10 mg)

August 2018-February 2019: 10 mg

February 2019-April? 2019: 7.5 mg

April 2019?-September 2019: 5 mg

 

So I was on 5 mg for around 5 months before I crashed, and boy did I ever crash. It started with acute insomnia, then I became extremely anxious about the insomnia and then so anxious in general that I basically stopped eating, lost a good 10 pounds I couldn't afford to lose, and then developed GERD or something like it. It really, really scared me; I learned exactly what SA members mean when they say, "I didn't feel like myself." And I was terrified of where my mind would take me next. I even thought about trying to admit myself to the psych ward. But oddly, the one thing I was not was depressed.

 

After four weeks of this, I went back up to 10 mg citalopram, then up to 15 mg a week later, then back to 20 mg two days after that. And within 2 weeks or so, I was feeling MUCH better (except for the appetite loss and GERD; legacies of the freak out).

 

Thinking back on that experience, I'm in some distress now, but qualitatively it's nothing like that (yet?) I've been pondering reinstatement of citalopram, which I haven't taken now for four months (not duloxetine, rightly or wrongly). Hence the research on compounding pharmacies and liquid formulations. I called my psychiatrist today and left a message asking for a call back; I want to get a prescription for liquid citalopram. But now I'm thinking things aren't really so bad, maybe I should tough it out a while? Keep the prescription in my back pocket, in case the *^%/ really hits the fan again?

Yes, you might have had withdrawal symptoms while you were tapering citalopram in 2019. Intense anxiety and insomnia are characteristic of antidepressant withdrawal syndrome and ,as you described, your "crash".

It is common that reinstatement of the drug resolves withdrawal symptoms.

Overall, it sounds like you are recovering from antidepressant withdrawal syndrome. This occurs very slowly and fitfully over months. If your symptoms are not getting worse, and nothing happens to upset your system more (such as drinking alcohol, taking antibiotics, severe stress), this probably will continue.

I can't think of anything I felt on citalopram that went away on duloxetine.

Thanks..I felt a noticeable increase in anxiety this morning, and a bit more trouble getting to sleep last night, so I'm not sure how this is trending. I'm trying not to read too much into it, but it's hard.

I've been taking magnesium citrate for years, about 500 mg after supper, but I may need to cut back on it because my digestion is now (for the first time in years) tending toward loose. I forgot to mention that I started 250 mg of l-theanine in the morning, almost two weeks ago, thinking it might help take the edge off the anxiety. I'm not sure what, if anything, it's doing...maybe just muddying the waters.

So when you made the switch from citalopram to duloxetine, you got no new symptoms that might have been withdrawal from citalopram, is that correct? If that is the case, you are suffering duloxetine withdrawal.

 

After taking duloxetine for a while, you had adverse effects from duloxetine at night? What time o'clock did you take duloxetine every day?

 

Magnesium is more effective when taken in small doses throughout the day.

I've thought about it some more, and I'm pretty sure that there were no new symptoms after I switched to duloxetine, and no dramatic change in my mental state. What you say makes sense, that I am in withdrawal from duloxetine, not citalopram...I guess that also suggests that if I want to reinstate, it should be duloxetine? In one way, that simplifies things, since I don't have to try to get a prescription for compounded citalopram so I can get a low enough dose; guess it also means I'd need to figure out what dose to start at and I'd be bead counting.

I always took it in the mid- to late-morning (and citalopram, when I was on it).

 

My anxiety today has become so intense that I am thinking more about reinstatement, though I understand that there's no guarantee it will improve my situation. The way I feel reminds me more and more of that pit I fell into in 2019. It's scary.

If you wish to try it, I might see if 10 beads of Cymbalta relieves your symptoms. It's possible that will be enough, or you might need more.

 

It's important to take the same dosage at the same time each day, no slippage of an hour or two. Please let us know how you're doing.

So I can have significant withdrawal symptoms from the duloxetine, even after just 11 weeks on it? Not that I disagree, but it just seems so incredible that it could have such a profound effect...and the situation is definitely getting worse, not better.

 

My anxiety level has gone way up since yesterday morning, and last night I had some trouble getting to sleep and woke up too early, for the first time in years. Just like the last go-round; when I start to fall asleep, I get a jolt of anxiety and I'm awake again. I'm also having stomach upset and diarrhea, which is the opposite of the norm. Haven't lost my appetite yet, but based on past experience, that would be next. And weirdly, this morning I had an orgasm during my sleep again, which has happened now five times since I quit taking duloxetine (and citalopram). it has happened in the past, but very rarely.

 

So I am follow your suggestion and try 10 beads. I've counted out three 30 mg capsules, and there are 156-157 beads in each, which works out to ~1.9 mg in 10 beads...does that sound about right? I have to get some gelatin capsules, but I can start it either this evening or tomorrow morning.

 

Thinking I should stop the l-theanine too, if I'm going to reinstate duloxetine. It doesn't seem to be helping with the anxiety anyway, though I suppose it could be even worse without it...hard to tell, with no control group 😞

 

I'd like to say again how much I appreciate what you are doing for me and so many others. I don't feel so alone with this, which really helps.

Withdrawal risk is established in about one month of use.

 

You may wish to make a note of the average number of beads in a 30mg capsule and milligram of active ingredient equivalent. You can take a random 10 beads as a trial run. Make sure to always use 30mg capsules, from the same brand if possible.

I restarted duloxetine Sunday morning at 10 beads (about 1.9 mg), so I've taken two doses so far. On Sunday, my anxiety level was very intense, with no appetite, stomach in a knot, gagging, total loss of appetite, diarrhea, and chills. Symptoms eased very slightly over the day, and I had a pretty normal sleep of at least 8 hours, despite the anxiety level when I first got into bed.

 

Yesterday, Monday, I was still quite anxious when I woke up, but after I got up I noticed a kind of sedated feeling overlaying it, and the anxiety and other symptoms steadily decreased over the day. By lunch time my appetite was much better and by supper time I felt almost "normal".  

 

I slept at least for 8 hours again last night, but since I woke up I've had a real spike in anxiety, along with that strange sedated feeling overlaying it, and immediate diarrhea. 

 

My question is this: should I stick with the 10 beads, or go up by a bead or two? As mentioned above I've only taken two doses so far. But I guess I expected that symptoms would continue to ease, if the reinstatement was working.

I just started to reread the topic About reinstating and stabilizing to reduce withdrawal symptoms, and I think the answer to my question is found there; i.e. wait a week before I make any changes. And it was helpful to read that I may still experience waves of symptoms, as I am this morning.

I'm starting to wonder if the 10 beads is working for me. I guess things could be worse, but it's hard to imagine how right now. My anxiety increased throughout the day and into the evening, to an extreme level. Not sure I would even call it anxiety anymore; it feels more like pure fear.

 

I went to bed around 10:00 and fell asleep in spite of it for maybe an hour and a bit,  but now it's 12:30 and I'm not sure how I'll get back to sleep. It just seems so unbelievable to me that on Monday, such a short time ago, I felt almost normal, and now I'm in such a painful state that is so much worse than anything that I've experienced up till now. I have been trying my utmost to accept what I'm experiencing, but it's extremely frightening.

 

I guess bottom line, things are such that I am not sure I'm on the right track.

Just wondering if you have any further advice, @Altostrata. I have now taken five doses at 10 beads. The day after I started, Monday, my anxiety level decreased significantly during the day, until Tuesday morning. I also had significantly less anxiety from late Wednesday afternoon until Thursday morning. Otherwise, my anxiety level has been very high, along with difficulty eating, diarrhea, etc. Last night I also had trouble with sleep, waking with high anxiety around 4:00 and unable to return to sleep. 

 

I'm wondering if a change in dosage is warranted. Also, I had a call with my family doctor on Wednesday and he suggested I restart citalopram at 5 mg. Based on what I have learned, that seems awfully high, but the lowest dose I can get from pill splitting is 2.5 mg.

 

Not sure where to go from here, but I am wondering if enough time has gone by on this dosage to consider a change. It seems like any improvement in my symptoms has been very modest. I'm trying my best but it's very wearing.

You saw benefit of reinstatement on Monday, Tuesday, and Wednesday, but your symptom pattern changed on Thursday? How do you feel today?

 

Withdrawal has ups and downs. We would never recommend a drug changed based on ONE DAY that's rough. You need to see the pattern before making changes, or you'll be bouncing around with drug changes.

I was unclear, Altostrata...I saw improvement in my anxiety levels and the associated symptoms from mid-afternoon Monday until I went to sleep, and from mid-afternoon Wednesday till I went to sleep. Otherwise, anxiety level has been at least 6-7/10, which is much as it was before I began the 10 beads, except for yesterday evening--it really spiked before bedtime, and has been more like 9-10/10 ever since.  Last night was also the first night my sleep was seriously disturbed; I woke around 4:00 and was too anxious to get back to sleep. Today I feel so anxious it's very hard to function.

 

So there's been about 8 hours on each of Monday and Wednesday where I saw an improvement over the way I had been feeling before I started 10 beads, otherwise I've felt about the same as I felt before. But today is significantly worse.

As it takes several days for a duloxetine increase to ramp up to full effect, appears that 10 beads is too much. Suggest you try 6 beads. Yes, people are that sensitive -- skipping doses can do that.

I'll give it a try starting today. Yesterday as mentioned I had anxiety at 9-10/10 for the entire day, except for a slight lessening for a couple of hours in the evening. When I went to bed I found that whenever I started to doze off, I would get a jolt of anxiety that would wake me up again. At around 12:30 am I took 25 mg trazodone and that put me out until after 7 am.

 

During my last "meltdown" in 2019, my doctor prescribed trazodone for sleep and I found it very effective for that purpose. I suspect that adding in another drug is not ideal, but I also need to sleep and guided meditations sure weren't working.

I took it from September 16, 2019 to July 31, 2020. I started at 50 mg and ended at 12.5 mg.

As mentioned, I slept until 7:00 a.m. yesterday. When I woke I had somewhat less anxiety but more of a kind of dull numb somewhat nauseated feeling, which dissipated as the morning went on. I didn't take any trazodone last night, but I woke up feeling the same way today and it has persisted right up till now. It has a different feel than the previous feeling of intense anxiety.

I awoke with it today as well, although I didn't take any trazodone last night. The anxiety is still there but feels less intense, kind of numbed. By this time of day, it's changing back to the more intense anxiety I had been generally feeling before yesterday. I don't want to read too much into it, though there is a certain difference in these two "feelings."

The only thing I can recall about trazodone is that I had some concern about my ability to sleep without it, but it turned out not to be an issue. I was also on 20 mg citalopram at that time. In general, I recall that I was feeling pretty happy about being able to sleep a good 8 hours and no longer having any GERD symptoms.

I have been keeping some notes, so here is what I have from the past few days. I can post every day from here on. Some information (e.g., mealtimes) is missing:

 

3/25:

4 am: woke; anxiety 9-10; didn't get back to sleep

7 am: got up; no appetite; diarrhea several times

11 am: went out; walked; anxiety slightly less (8-9)

11:35 am: took 1.9 mg duloxetine (10 beads)

3 pm: laid down; slept a bit. Anxiety ~8 when I got up

6 pm: stomach ache (lasted til bedtime)

10:45 pm: went to bed; couldn't sleep; sweating, then chills

 

3/26:

12:30 am: took 25 mg trazodone; went to sleep

7:30 am: woke; "blank" numb feeling; anxiety ~2

8:45 am: got up; anxiety at 7; diarrhea several times

11:35 am: took 1.15 mg duloxetine (6 beads)

2:30 pm: waves of anxiety 9

7 pm: anxiety down to 5-6; stomach ache (lasted until bedtime)

11 pm: went to bed; sweating a lot; got up 5 times to pee

 

3/27:

8:30 am: woke; "blank" numb feeling; anxiety ~4

9 am: diarrhea several times

Here is the rest of 3/28 and yesterday, 3/29. As you can see, I made a decision to stop duloxetine and restart citalopram. And last night, I took my doctor's advice to take trazodone before bedtime, rather than waiting tuntil I wake up in the middle of the night and can't go back to sleep.

 

Remainder of 3/28:

11 pm: went to bed; anxiety ~8; woke at midnight

 

3/29:

1 am: still awake; up at least 4 times to pee between midnight and 1 am; took 25 mg trazodone; slept

6:45 am: awake; anxiety 6

7:30 am: got up; loose stool x2

9:35 am: took 5 mg citalopram

10:35 am: felt some tightness around temples; anxiety ~7

11 am: felt a bit "shaky", weak; photosensitive

1:30 pm: had short acupuncture treatment. Tightness in temples increased.

3:30 pm: napped for 1 hour; awoke with anxiety ~7

7 pm: dinner; slight stomach ache for ~1 hour

10 pm: took 25 mg trazodone

11 pm: went to bed; anxiety ~3
