Hello

I suffered some anxiety from my surgery complication that occurred on July 7, 2022.

I was taking Ativan as needed for the developed anxiety.

In September 2022 I experienced three panic attacks and was prescribed more frequent dosages of Ativan. I was trying to return to work but too sleepy due to the Ativan and anxiety was still very high.

my surgeon suggested I have my general practitioner prescribe some type of anti-anxiety SSRI to take the place of the Ativan so that I was not so sleepy during the day and could return to work.

I was prescribed gabapentin for nerve pain in August after my surgery. At one point I was up to 1200 mg a day. I reduced that dosage slowly down to 300 for several weeks until my doctor recently told me to try to get off that as quickly as possible so I have been reducing that over the past two weeks and taking 100 mg a day as of today .


 I was Prescribed Celexa 20mg for anxiety in Nov 2022.
I have repeatedly Stopped and started Celexa back to 10 mg trying to taper off ever since then.

 

The more I read about celexa the more I didn’t want to be on it right after it was prescribed. It was actually a girlfriend who suggested I ask my doctor for it as she had been taking it.

(Also Sometime in January I noticed mild rash on stomach and Doctor said he could barely see it and didn’t think anything about about it).
Every taper and episode of reduction has been sanctioned by my general practitioner.
I was Told by my doctor a few weeks ago I could take 10 mg every few days and developed debilitating nausea and headaches and insomnia.

The last dosage I was taking consistently for a few weeks was 10 mg before I started taking every four days.

Seven days ago I reinstated at 5 mg every day per my doctor upon my request. My doctor says that it’s not the Celexa that is causing the side effects it is the re-introduction of anxiety side effects. I explain to him that I never experienced nausea this entire time I’ve had anxiety.
Anxiety and nausea and insomnia have improved over the days, however, still experiencing some nausea, some insomnia and periods of just not feeling good. Slight itching here and there still and also tinnitus returned.

I was wondering if it’s too late to reinstate at a lower dose as advised on this website?

I was not aware to try to return to very small doses,  so I just decided to dose down to 5 mg after asking my Dr, he said I could try that dose. No real leader ship from him.

 

I just would like to know if it’s too late to try even a smaller dose as a reinstatement or is seven days into this reinstatement too late?

Hello. As you can see from my update in my signature. Ive increased to 3 mg celexa after feeling relatively stable for a few weeks in the spring and summer but when yo-yo trying to go off, go down to 1 mg back up to 2.5, trying to break pills accurately. Since I tried to reduce from five to quickly, I have suffered insomnia, louder tinnitus, explosive anxiety at times during the day.

so I increased back up to 3 mg to see if that would help abate withdrawal symptoms. I am going through some health challenges so pain probably affects anxiety.

I increased up to 3 mg 8 days ago with no real improvement in symptoms.

should I return to the 5 mg where I felt I got the best relief from withdrawal and then stay there about two months before trying to do the 10% taper per month ?

I’m not sure if I put this in the right place or if I added to my signature correctly. Thank you so much for your help and I appreciate patience if I have made any mistakes with protocols and posting.

Welcome to SA. Thank you for filing out your signature but a more concise format will make it easier to decipher your drug history. Please see guidance at How to Summarize Your Drug History in Your Signature.

 

I would also advise you to check out Tips for tapering off Celexa (citalopram) as well as Considerations about "Stability". You have been jumping around a lot with dosages so I would advise you to hold at 3mg a little longer to see if you can stabilize at that dose. If you continue to struggle, I would not jump directly to 5mg but instead try a smaller increase to say 3.5mg to see if it can provide relief.

 

Once stable, holding for a month or two before tapering is a solid plan.

Get a load of this. So because I was having so much trouble breaking apart 10 mg pills down to 3 1/2 mg I asked for a liquid.
Specifically told the NP the milligram dosage I needed. 

Instead of prescribing by mg she prescribed by ML.

Liquid Citolapram is 2 mg per ML.

So instead of continuing to hold my taper and work on reducing I was doubling it!

Yes, @102820, I agree. You need to check what the doctor and pharmacy give you before you consume it.

 

I know another member who paid a compounding pharmacy to prepare something for them. The member decided to weigh the pills prepared for them and found that the pharmacy was not particularly accurate!

 

It makes tapering rather difficult when you don't have consistency of dose.

Greetings 

I was advised by pharmacist it would be safe to go back down to 4 mg. I was back up to 7!!

I feel much better, the raging anxiety is gone, nausea and headaches gone, but the tinnitus is still loud. Experiencing constipation still. 
But much better. I’d like to try to go back down a little more to the 3.5 where I was before the fiasco, but I’m reminded that I was unstable and having a lot of difficulties after tapering down too fast from 5mg but I’m not sure if I should. 
In another note, I love Peter Breggin and decided to order one of his books where he details tapering.

This is positive, @102820...other than the tinnitus. To this day, I experience tinnitus, which I put down to ADs. I do find that the intensity can vary. It has recently been very loud with each weekly AD reduction I make...mostly it is just background noise. Sometimes, it can be bad for weeks, but it calms down for me. I hope it calms down for you, too, once you've been stable at the same dose for some time.

Any reason to hold a little longer is a good reason..."But I'm not sure if I should". That sounds like a good reason to wait a little longer. There is no rush.  Wait until you feel comfortable for a month or so, then consider tapering.

 

When you are ready to taper, remember the maximum of 10% per month of the most recent dose. The size of the monthly reduction should get smaller each time. I think Peter Breggin recommends a slightly different approach of 10% per month of the initial dose....so the reduction is the same each month. I've read on this site that people tend to find they have to go slower and by smaller reductions as their dose gets smaller. 

 

Some of us find tapering in smaller weekly drops more manageable than one larger monthly drop. Have a read of the following information to find out more about the BrassMonkey slide.

 

Be sure to take your AD every day at the same time of day. It’s also essential during the tapering process.

Hi Emonda and Firefly

Thank you so much for you valuable help and links!!

I have been holding at 4 mg liquid Celexa (reduction from fiasco doubling 9 days ago) tinnitus is raging today and anxiety worse. Enter headache, slight nausea, palpitations. Not tolerable really.

 

Tinnitus hasn’t been this bad in months.

How much longer should i try to hold at this dose? How do I know what’s causing this?

Is it from not going down far enough or do I need more?

Also, I am working on a phone to type this so I will try to get back on here tomorrow on my PC and figure out how to summarize my history correctly.

Hi @102820,

Thanks for adding the AD to your drug signature. I can't quite see the detailed changes you made in your signature, as described below. The signature should be: Date, Drug, and Dose so people can see what you took, when and for how long. Have a look at FireflyFyte or mine to get the idea. I am with you re trying to update the details on your phone...I have to do it via a computer, too.

There seems to have been a fair bit of changing to your dose, or yo-yo-ing, as you mentioned above. This is going to be destabilising and lead to WD symptoms. Have a read of the WD checklist to reassure yourself that what you are experiencing sounds like WD symptoms. WD symptom checklist

 

In my experience, and the experience of others on this site, you need time and stability for the symptoms to calm down. Have a read of this post. It's the way to succeed on this journey. The link refers to waiting ~4 weeks after any dose change before adjusting again.

 

 

I hope this information is helpful/reassuring. Time and patience, and stability comes.

I’m so appreciative of these messages and links.

It’s been four weeks since I started holding out for milligram.

thanks to these messages, I have been able to determine whether or not to reduce 2.5 a week.

I decided not to go down any further and wait another two weeks due to a lot of stomach issues, tinnitus, anxiety. If anybody has any comments about reducing gas it would be helpful.

also I am going to check through the website because I discovered that SSRIs can cause hemorrhoids and I’ve  constipation, gas, and and hemorrhoid flareups. Trying to establish regular bowel movements.

Could it be a dairy or other food intolerance? May not be the ADs, although I know they can cause gastro issues.

Hi thank you for your reply and assistance. Yes I read the number 1 side effect of celexa is nausea.
I did a word search on here and found other people were experiencing stomach issues. I think I had read somewhere that as one starts to reduce to the lower doses when tapering is when withdrawal symptoms may become more pronounced.

Greetings friends

Please help.

Ive believe I've tried to taper way too fast. I am trying to taper Celexa and Ativan prescribed due to anxiety after a surgical complication.


I have tapered down presently to .5-1 mg of Ativan at night only for many months. I rarely take Ativan during the day anymore, But for months from July 2022 to Oct 2023 I was taking 1-2 mg in the day time and 1-2 mg at night.

 

The healing from surgery complications are long gone, now I’m left w drug tapering.

 

I have developed gastro problem which started about the time I started trying to taper celexa. I’ve also developed acute Tinnitus.


I now understand how toxic these drugs are and now I’m over 60 and really concerned about the health ramifications.

 

My original plan was 10% Citolapram reductions but then I read Dr Breggin’s book and he stated some ppl can taper at 25% so I tried to increase taper percentages when I was feeling better and confident.

 

Ive been so confused trying to calculate exact doses on a one ml syringe for liquid Citalopram, preventing air bubbles, tapering etc.

 

My withdrawal symptoms have exacerbated again and I’m experiencing some Insomnia, much louder Tinnitus, flu like symptoms, night hot flashes, vivid dreams, some anxiety, today it was chills. The last few days it’s been side ache, burning on bottoms of feet, last couple weeks headaches, BP spikes, Runny nose, sinus congestion etc etc

I also have allergies so I have a hard time determining whats withdrawal and what’s pollen.

They Ruled out kidney pain last year as only v small stone.


It mind blowing the innumerable health problems / WD symptoms these drugs can create. Because of the withdrawal symptoms in Jan, I decided to reinstate to my previous 
dose. Since increasing dosage this last time after several weeks of feeling like crap, i feel great one day and increasingly horrible the next.

I feel the expediency to be off these drugs due to treating constipation and its complications, tinnitus, memory issues and brain fog. Just general instability and risks like dementia etc.

 

Questions: Should I be trying to taper Ativan AND Celexa at the same time?

Can someone please advise How I know if it’s Ativan or Celexa causing withdrawal symptoms?

Should I try to calculate my Citolapram dose on a syringe in milliliters or milligrams?

When I do start tapering Ativan do I need to be as precise and meticulous as Citolapram?

Math is not my strong point.

I’ve read celexa and lorazepam both can cause delayed withdrawal symptoms.

Any help is appreciated.

Hello. I don’t know why I didn’t respond to this. When using my phone sometimes I’ve noticed because I’m unfamiliar with this site, I miss things. I haven’t been back in a while, just want to thank you for replying.

with regard to a post I made last night, I will add here that I take Ativan now only at night around nine or 10 o’clock. Then I’m taking the Celexa around eight in the morning 7 to 8 am.

Thanks for updating us. It looks like you're at .9mg celexa and holding ativan currently? You've been taking ativan nightly since May 2023?

 

Are you currently feeling WD symptoms?

 

 

2003-2009 on and off various SSRI's for short periods, Ativan prn

2010-2011 Ativan, up to 1.5mg/day - tapered off without issue

2013-2021 ativan 1-1.5mg 10-12x/month, daily starting Oct 21 to help with buspar WD

2016 - Effexor 75mg, short-term

2021 Mar -Jun Buspar ADR at high dose, tapered 3 months

2021 Aug Wellbutrin 150mg for 5 days (ADR), then MIrtazapine 7.5mg for 7 days (ADR)

Oct 22/21 - Direct switch ativan to clonazepam (don't do this)

Tapered clonaz Oct/21 - Apr/23  - 0mg!

 

Supplements: omega-3, mag-glycinate

Yes

I was taking Ativan .5 mg as needed to sleep from 2015 to July 2022. Dr placed me on high daily and nightly doses due to surgical injuries and complication induced anxiety. I tapered off Ativan over the months for a yr back to pm only .5 to 1 mg. I was wondering if Lorazepam was causing WD too so lately I’m back to a full mg at night to see if it was contributing to WDs. I am also Holding Ativan bc I figured it wasn’t a good idea to keep tapering Celexa and Ativan at the same time.

Yea WD which are due to blundering thru tapering Citolapram,I believe.

I actually posted a new message last night asking questions about it all.

I see you switched to Clonazepam to 0.25 mg.

How is that going?

Yesterday I felt awful. Today v good.

It's best to taper one medication at a time. That way, if you have unpleasant WD symptoms, you know which medication is causing the problem.

 

Taking multiple psych drugs? Which drug to taper first?

Yeah, refer to my answer above. If you just taper one at a time, you'll know the cause of your symptoms.

If you are using a liquid, I'd be using ml. I use a 0.5ml syringe for my liquid.

100%! Consistency and accuracy are so important.

I use a spreadsheet to outline my plan and cross off each day as I go to try an avoid errors.

 

today: 0.9mg, next month: 0.9mg * 90% = 0.81mg, the following month: 0.81mg * 90% = 0.73mg etc

Thank you SO  much for your help. I have been unable to find a .5 ML syringe that does not have a needle attached.

I was reading where other people just started diluting the citalopram which would be best and it would cost a whole lot less money, however, I don’t have faith in my abilities to do that accurately. 

I guess I need to learn how to use Excel because I don’t know how to plug-in those percentages, etc.

I just asked chat AI and received detailed instructions.

Also, I just called my pharmacist and he said I could meet w him about my 1 ml syringe so I can know how to document my reductions in milligrams and milliliters. 
Reading from Mark Horowitz book though, he suggests for instance, for a moderate taper up to five percentage points of Receptor occupancy between each step with reductions made every 2 to 4 weeks. But in his table in the book he is referring to a 0.4 mg/mL dilution. So I don’t think I can go by that Table because of the dilution rate. No need to dilute further and make more liquid to have to deal with. 
I have just started reading his book so I have yet to understand how to taper according to receptor occupancy. I don’t know if I should worry myself with it.
Here’s what I got from AI: 

To taper down a liquid medicine that is 1 mg per 0.5 ml with a reduction schedule of 10% of each last dose every 6 weeks until you are not taking any, starting at 1 mg, and showing each dose in a 1 ml syringe, you can follow the following reduction schedule in milliliters and milligrams: 1. Start with an initial dose of 1 mg (0.5 ml). 2. Calculate 10% of the previous dose (1 mg) to determine the reduction amount for each step (10% of 1 mg is 0.1 mg, which is equivalent to 0.05 ml). 3. Follow the reduction schedule in milliliters and milligrams: - Week 1: Start with 0.5 ml (1 mg) - Week 6: Reduce by 10% of 0.5 ml (0.05 ml), so take 0.45 ml (0.9 mg) - Week 12: Reduce by 10% of 0.45 ml (0.045 ml), so take 0.405 ml (0.81 mg) - Week 18: Reduce by 10% of 0.405 ml (0.0405 ml), so take 0.3645 ml (0.729 mg) - Continue this pattern every 6 weeks until you reach zero. I purchase 0.5ml syringes with a needle, which I simply pull out with pliers. 

 

That's good that your pharmacist will help you with the process. Once you see it / do it out once, you'll be fine.

Hello friends, I need help.

 

I reduced my dose of citalopram during my taper in January and quickly increased three days later due to physical withdrawal symptoms. I have been holding now since then, and things have improved a lot, but still having sporadic side effects, such as headaches, a little brain, zap here and there every few days, louder Tinnitus, some headaches, insomnia.

 

I have two questions. One, what are your thoughts as a community regarding Mark Horowitz Receptor Occupancy taper strategy?

Does anybody have any advice if that strategy is better than the other protocols regarding percentage?

 

I still have not seen pharmacist to under how to read the mg reductions on a syringe 😞

I do not have a psychiatrist who is helping me with this. I can’t find a Doctor Who knows zilch about the tapering and withdrawal issues.

Second question, is it OK to continue tapering after over 6 weeks even though I’m still having some mild withdrawal symptoms?

 

I want to add that I wanted to get off this medication as soon as possible because I do believe it is the reason I am having some medical problems.

Are you taking a consistent dose of both citalopram and ativan each day? In your signature it says ativan 0.75-1mg/night. You need to take the same dose every day if you are trying to taper citalopram.

We recommend a maximum of 10% of your CURRENT dose each month. Many people have to do smaller reductions, however. I can't imagine Mark Horowitz's taper strategies are much different.

In order to help we would need to know the concentration of the liquid you are taking, and the capacity of the syringe you are using.

I wouldn't start until things have settled down.

We only recommend two supplements. Omega 3 Fish Oil and Magnesium. Both should be introduced separately and increased slowly.

Hello and thank you for your comments.

I am using a one ML syringe. There are 2 mg per ML. 
take the Celexa in the morning. I’ve been taking the Ativan at night.

I understand what you are saying so I am going to just continue with my regular dose of Ativan at night so that it doesn’t cause additional withdrawal symptoms.

I have taken a picture of the syringe I use and pointed my finger nail to the dosage, which is two lines above the large line above four. I am deducing. This is 0.47 ML. The five number on the syringe means 1 mg. I hope this makes sense and I certainly, appreciate your kind and helpful response .

I have been feeling really good every day! except for the extremely loud tinnitus and these small withdrawal symptoms here or there. I don’t mind toughing  it out if I can get off this quicker.

withdrawals do we ever really get to the place where we feel fine before stepping down again?

But if you think it’s better to wait, I can wait. What a couple more weeks?

Please don't start another introduction topic when you do not receive responses. The moderators who initially responded will likely get back to you in time.

 

Ensure you take the same dose of ativan every night, and hold for a couple more weeks at 0.47ml citalopram. I suspect if you have been varying your benzo dose between 0.75mg and 1mg this is creating additional withdrawal problems. Once you're ready to taper again keep following the 10% reduction plan, so your first drop could be from 0.47ml to 0.42ml.

Hi dear friends,

Do I need to start a new topic if I have ongoing questions?

The tinnitus I’ve been experiencing since around the same I had my surgical complication and was placed on celexa has become much louder over the past few weeks. 
Can this increase be a withdrawal symptom from tapering as well?

If so, has it been known to decrease  as the brain heals?

Also being at below 1 mg, how low does one go before just jumping off? 
I feel so very alone trying to handle this.  All the medical professionals with whom I  have discussed Celexa withdrawal and help managing  tapering are not educated about the need for long term tapering and withdrawal symptoms.

Yesterday I guess all this became too much and I went into tears, as I try to make plans to set a static schedule to work part time. I have been able to work setting my own appointments for my patients for 2 years and now I’m facing having to make a commitment. 
It’s an fearful thing to try to make commitments when one doesn’t know how they are going to feel w regard to tapering.

I am afraid to let Kaiser doctors see my discouragement or discuss emotional aspects of this celexa situation and withdrawal symptoms lest I am dx with relapse anxiety or another dx.

I'd suggest going down to at least 0.1mg before considering jumping off.

Hi, thank you so very much! Right now I am at 0.92 mg
that is a little less than 1 mg.

I am having some symptoms I would consider withdrawal, as slight headaches here, and there on certain areas of my head, and some insomnia here and there, and the louder tinnitus. Should I stay on Celexa further or discontinue now and just try to stabilize on the Ativan and then start to taper it? Since the withdrawal symptoms could have to do with my unstable dosing of the Ativan as discussed previously. I was, vacillating between .75 and 1 mg to sleep at night and was advised to stabilize at one dose on that and not to reduce any further until I’m off citolapram.

I hope this makes sense. 
i’m sure any doctor is going to tell me I should jump off now since I’m below 1 mg. That’s why wish I knew how to just dilute this medication instead of trying to talk to doctors about eventually having to get a new prescription. I do have a couple bottles of at least 2 oz of the citalopram left. At this point I just don’t need any more discouraging conversations with doctors.

Hi

I have another question. I had a friend who stopped her Celexa cold turkey (she couldn’t afford her medication)  and started BuSpar two weeks later. 
Is Buspar ever used strategically to help taper off citalopram and then easier to taper off BuSpar?
