Colonial: worsening withdrawal symptoms changing from Paxil CR to liquid suspension

In the 3 months since 2/26 I've dropped the Wellbutrin from 8 mgs to 6.4, and the Paxil from 6mg to 5.2., and the Xanax from .25 mg to .1875

 

I had been sleeping and feeling ok until I decided to do a 10 day percocet fast from 5/8 thru 5/18.  I normally take four 10/325's day for a total of 40 mgs.  I had the normal withdrawal symptoms the first few day but started to have a bad reaction about day 8, so I don't think I will be able to do that anymore.  Ever since I restarted on 5/18, I've been sleeping alot and feeling flu like symptoms and tired.

 

Before I started the paxil and Wellbutrin withdrawals, I used to be able to stop the percocet no big problem.  I did it so that It would work better when I restarted, to keep my body from building up a tolerance over time and it being less effective.

 

Now that my central nervous system is in free fall from the A.D. WD, I have a worse reaction from just lowering the percocet in half than I used to when I would stop it altogether.

 

So, I'm currently recovering from that.  I'm sleeping about 10-11 hours a day right now and feeling flu like over all.

I just found information on what that is, the 'kindling effect", so I am glad to know It's not just all in my head:

 

Kindling due to substance withdrawal refers to the neurological condition which results from repeated withdrawal episodes. Each withdrawal leads to more severe withdrawal symptoms than the previous withdrawal syndrome. Individuals who have had more withdrawal episodes are at an increased risk of very severe withdrawal symptoms, up to and including seizures, and death. Withdrawal from GABAergic-acting sedative–hypnotic drugs causes acute GABA underactivity as well as glutamate overactivity, which can lead to sensitization and hyper-excitability of the central nervous system, excito-neurotoxicity, and increasingly profound neuroadaptions.

 

It's interesting that WD from 2 anti depressants can ruin me from being able to fast from another med.  And the percocet fast also kicked up the "brain zaps" and increased thirst, which was a Wellbutrin symptom. 

 

 

 

So, I suppose "fasting" from that is a bad idea as well.  I was just hoping to not build a tolerance to it so it continued working, I'm not going to continue to take higher dosages.  They are really cracking down on that now, especially people who take both Opioids and Benzo's together, due to the accidental overdose risk.  I know a person who, because of "tolerance" issues is taking close to 180 mgs of oxycodone and 4 whole mgs of xanax EVERY DAY...  I can only imagine what taking the equivalent of 18 percocets and 16 xanax EVERY DAY must do to you over time.  I don't want to go that route, and was hoping the "fasts" would help me stay receptive to lower dosages of the percocets, but because of the "kindling" effect brought on from the Paxil and Wellbutrin, I guess it's making things worse instead of better.

 

Interestingly, I noticed the LOWER my xanax dosages, the HIGHER the pain.  So I did some research and for certain conditions, xanax does boost the percocet's effectiveness.  Xanax and opiods are regularly used together for things like fibromyalgia and back pain.  So that made me feel better to know it wasn't my personal situation getting worse, it was just that the Xanax was boosting the opiod effect.

 

So, I've delayed the next paxil drop, that would have been a few days ago, for now... See how I feel in Mid June..

I asked the other mods about your particular drug cocktail and Alto mentions that you may want to taper the Wellbutrin next, as opposed to the Paxil.

 

Wellbutrin can be a very stimulating AD. It was for me when I was on it. 

 

I would caution against tapering multiple drugs at one time and really work to get off one at a time. Your symptoms are letting you know that your nervous system is sensitive and tapering more than one drug at a time can make that worse. 

 

I'm glad you are going to keep the Percocet dose steady. 

 

The "bell" ring in your head when you were half asleep may be a symptom called "exploding head syndrome":

This is common for many people during withdrawal. I had a bad case of exploding head syndrome when I first came off Klonopin, but it's gone away completely now. 

 

Let us know what you think about tapering the Wellbutrin next.

If I held on the Paxil would I be able to make larger Wellbutrin cuts?  I'm having a hard time finding discontinuation info plan on amounts of Wellbutrin this small.   I did want to work on getting off the Wellbutrin but I thought cutting .08 of a mere 1 milligram of it a month was not that much of a drop to preclude making drops of .4mg of Paxil as well.

Also, no more drops of Xanax for now.  As you know from my other thread, it's providing a certain amount of stability during the A.D. Withdrawal, and every few drops of Xanax is also increasing the physical pain as I was using the xanax and percocet together to manage the pain.  Every certain level of xanax reduction is making the percocet less effective, so I'm not dropping any more of that for now, just the Wellbutrin and Paxil.

As stated in the post above:

So no more cuts on the xanax, and if I held the Paxil, what could I drop the Wellbutrin monthly?  It's being compounded 30 days at a time, so I am committed to dose in 30 day increments.  Could I drop more than the .08 schedule  we had planned for the rest of the year? 

I have 18 more days of Wellbutrin at this level and I have been cutting the paxil .4 mgs monthly for a few months now.  And the xanax hasn't changed in 3 months either.

Hard to know if the trace Wellbutrin cuts are making that much of a difference.  Other than the bell ringing in my head and an episode of "free falling", (feeling like I was falling off a building when I was almost falling asleep) I think most of the recent issues were from the percocet fast.

Hi, Colonial.

When people taper more than one drug at a time, they don't come off their drug cocktails any faster than those who go one drug at a time. Whether you would be able to make larger Wellbutrin cuts isn't just about Paxil, but about how you are feeling. I've read through the past two pages of your thread, and I'm not seeing a really specific symptom pattern (please correct me if I'm wrong about that). 

The people who do the best tapering multiple drugs have a clearer symptom pattern, so I would stick to just one drug taper at a time.  Making changes on multiple drugs can really confuse your already sensitive nervous system. And since antidepressants are notorious for delayed withdrawal, you may not even feel it for several weeks. So it's easy to confuse the "delayed" withdrawal symptom from one drug from the "current" withdrawal symptom from the current drug being tapered.

Please don't make this any harder than it needs to be.  I would stick to tapering Wellbutrin until you're off and then work on tapering Paxil.

The rule is the same for all drugs - we don't advise going any faster than a 10% reduction off your previous dose. Please note the reduction amount is calculated from the previous dose, not your original dosage.

Another visit at the doctors.  Government is really pressuring them to stop having people on both percocet and xanax at the same time.  Trying to explain to him that the Wellbutrin and Paxil withdrawal cause a certain amount of anxiety with it and, while you won't put someone ON xanax for A.D. withdrawal, if your already on it, it makes sense for that to be the last thing you take them off.

 

I thought dropping from 6 a day to  1 was pretty  good.  He said of the 4 meds the percocet and xanax are the 2 dangerous ones.  (Eye rolling).  Still trying to decide what to drop the Wellbutrin to next month if I'm NOT cutting the Paxil.  

But I can't keep ONLY cutting it by 10% of the last dose, or I'll never get off it, and the insurance isn't going to pay for it to be compounded forever.

 

If I'm NOT dropping the paxil at the same time, I can't take another 7 months JUST to drop the last 6.4 mgs of Wellbutrin if He's pressuring me to start cutting the xanax even further.  

I may have to speed up the Wellbutrin taper if he's gonna threaten to cut the amount of my xanax in each refill.

I'd rather be off the Wellbutrin altogether before I have to start dropping the xanax now too.  

Just a real dower when they start threatening to take away meds before you know your body is ready...

Each morning from July 27th to August 10th I woke up "feeling" like I was in mid October.  The withdrawal symptoms for this wave started picking up Sept 10th but were mild but now I've been really bad the past 3 days.

 

Nausea, dizzy, paxil flu muscles, smelling colors and tasting numbers, items seem to have a "sex' assigned to them, ( the toaster is male but the lamp is female) , very thirsty, brain zaps, problems with depth perception, trouble reading ( the letters are upside down at times or missing from the words).

 

Today I woke up very anxious, and if I remember right, I can look forward to being suicidal by Tuesday.

I keep burning-tasting the color brown in the center of my chest now that my cortisol is up..

It's difficult to explain what its like to experiencing tasting thing in your chest, or tasting a color for that matter..

I have been on half sleep for 5 nights.

 

Synesthesia (n.) - a neurological phenomenon in which stimulation of one sensory or cognitive pathway leads to automatic, involuntary experiences in another; Experienced as a "blending" of the senses.

Synesthesia is confirmed when the aforementioned involuntary experiences are subconscious and occur with marked consistency over an extended period of time.

these sound like very serious symptoms.

 

I know that your are under pressure from your doctor but it seems tapering too fast is catching up with you. Shep did her best warning you about it. If you continue pushing things it might get even worse.

It's quite difficult, not to say impossible, for me to understand how you tapered from your signature but obviously it is too fast.

If you were referring to the Wellbutrin, I had both 100 and 75 mg pills that I could "quarter" into 4ths, until we got down to the 12.5 mg dose, since then, it's been being compounded exactly by a pharmacy.

When I say it is difficult to follow your tapering I'm referring to the fact that you seem to be tapering 3 drugs soit's quite difficult to see when you tapered which, especially when you are tired. It just takes too much effort fro me now. 

 

8/29 you reduced Wellbutrin by more than 10 %

 

less than a month from that

 

9/20 you reduced Paxil by 10 %

 

less than 2 weeks after that

 

10/2 reduced Wellbutrin by more than 10 %

Thank you so much.  I will discuss the recommendations with Him.  He did tell me 2 weeks ago, that from His perspective, we can hold at these levels for a few months.  The issue will just be getting the insurance to approve to continue to pay for the compounded Wellbutrin after 12/31/2017.

 

I had been told originally by the insurance that I would only be approved to the end of the calendar year, which is why I was rushing to get off it.  I had to file for a court hearing with the state to get them to pay to compound it for 2017 when they  originally denied the doctors orders. That is why I was being more aggressive with the Wellbutrin cuts along with the moderators recommendation to get off that one first since the fact that Wellbutrin is notoriously know for being a harsh stimulant that would which was likely the drug responsible for raising the anxiety,  and it has the FDA warning for serious neuropsychiatric  side effects.  The hope was that once I was off the Wellbutrin my anxiety would not be as high.  Unfortunately this episode - flare up has completely blown my xanax taper as I'm now I'm needing MORE xanax to compensate, with is, obviously,  counter productive to being on the lowest  daily xanax dose possible.  The doctor also originally told me to use extra xanax to also calm down the pins and needles, burning hands and feet symptom from the withdrawals.

I know that you have these external pressures but regardless of the reasons a rushed taper is a rushed taper and the consequences can be really disastrous. 

 

Can you update your signature with changes to Xanax?

 

Do you need to compound Wellbutrin? A lot of us make our own liquid from the pills. I've been doing it for 4 years now and it is very accurate. Maybe you wouldn't have problems with the insurance if you don't need compounding.

Considering I am down to 3.5 mgs and the smallest pill is 75 mgs, I don't see how I could possibly reliably be dissolving the tablets into distilled water and coming up with a reliable daily dose.  I know that some people have success doing so, but for me I just sense a huge red flag attempting that for myself as I've read through the instructions often and I don't have much confidence I wouldn't be doing myself great harm by attempting it.    When I last spoke to Him in the office He said He thinks the "denial of services" on the compound last time was a technicality, and that He "should" be able to get the insurance to over ride it and get it approved again for 2018.

 

Why I actually had to file for a court date with the state when they denied me last time, I'm not sure.  Medicaid changes so often.

Well, ladies and gentlemen.. the day has arrived... for the FINAL WELLBUTRIN DOSE...

3 YEARS....2 MONTHS...  27 DAYS...

 

Just to be able to get off a med that I had only taken half a year when I started to withdraw in March of 2015..

I stayed at both 1.7 mgs and the 0.08 mgs 50 days each, and I just took the last one...

I'll leave the Wellbutrin in the signature line for  3 months until I'm past any delayed reaction.

I was routinely getting E. Coli bacteria infections in my bladder 10 weeks after a Wellbutrin dose drop...

Think I'll go reward myself to this accomplishment and go plan out the last 2 mgs of the Paxil..

 

I wonder if I should save this RX Bottle of 0.08 mgs of compounded Wellbutrin to be donated to the "Medical Malpractice Museum" that's sure to come in the next 100 years for this debacle we have inflicted on ourselves the past 20 years..

I see I was still on 3.2 of the Wellbutrin and... 4.4mgs of the Paxil...

I'm now 2 days shy of bring off the Wellbutrin 4 months...

So hopefully we are past any delayed reaction on that one...

Although I'd give that 6 months...

Down from 4.4 to 1.2 on the Paxil, 12 days into this drop...

I seem to do "ok" if I leave 8 weeks in between Paxil drops and drop it only 1/10th of a ml on the syringe, 2 times a day....

For a total of a 0.2ml drop.  Which would equal 0.4mgs and a total of 2mgs every 5 drops....

Each of the past 2 springs I tried to speed it up a bit and drop it every month, and the first drop right on the heels of the other was ok, but it was the 2nd drop so close that did me in each time.

 

So if you see my drop pattern from this year:

 

 1/1/18 3.6 mgs,   2/15/18 3.2 mgs.   4/13/18 2.8mgs,   5/11/18 2.4mgs,   6/10/18 2.0 mgs,   8/4/18 1.6mgs,   9/27/18 1.2mgs, 

 

I made a drop each month April through June, and it bit me hard the middle of July... even though I didn't make a drop IN July...

So even the Paxil can sneak up on you like that...

 

It was the same thing that happened to me the spring of 2017 

 4/3/17 5.6mgs,    4/24/17 5.2mg,        6/13/17 4.8mgs,       9/20/17 4.4mgS, 

 

So it seems every 8 weeks at 0.4 it is..

 

I might do an extra 2 month dose when I get down to 0.4 and drop it to, about 0.15 for 2 months...

At this point it will be 4 years in January since I started, what's another 2 months...

And I think the July 2018 issue could have been complicated by finally detoxing off the end of the Wellbutrin as well....

 

I do have physical symptoms each drop, they are just "manageable" if I stick closer to the 0.4 every 8 weeks...

More than that and the physical symptoms are so bad I'm bedridden for 10-14 days again and the neurological symptoms come back as well:  smelling colors, tasting numbers, colors hovering in the center of my chest,  "smelling" burnt meat, etc...

 

The 1 ml syringe does break down into 5 lines per .10%...

So I can make smaller drops if the symptoms get worse as I get closer to 0.

So it has a total of 50 lines on it

Dropping 5 lines (1/10th ml) twice a day is where the total drop of 0.04 mgs is coming from...

But I could drop 3 lines instead of all 5 if it gets worse, whatever that would be equal to....

60% of 0.04 

And just add 2 more cycles if necessary...

Since each 0.04 mg drop is a higher % of what I am currently then taking every 2 months..

 

So I "could" lower the drop from 0.04 mgs to, whatever 60% of that would equal...

I would just like to be off it by June, so my chemistry is stabilized  before "neurological autumn" kicks in for me...

Which seems to be mid August every year...

We will see how I do over the next 6 weeks...

If you can find a 0.5mL oral syringe, it has even finer graduation marks.

 

There's no rush, as you know. Paxil can be very difficult to go off. Where are you with Percocet, Xanax, Skelaxin?

The other 3 meds aren't ones I am currently looking to withdraw off of, considering they are all actively performing a valid function for which I am taking them.  It was the Paxil and the Wellbutrin that were completely useless and causing most of the  "major danger" interactions.

I had been ok until about 11 days ago..

 

I had been stable on the Paxil for 3 months at 1.2  mgs.  Then on 12/24 I made a 1 line drop on the syringe of 0.04 down to 0.08.  Nothing the first 3 days, days 4 through 10 just some brain zaps and some brain fog in the afternoon on January 1st and 2nd.  Nothing to write home about until about January 6th, which would have been 14 days since the drop.  Since then really rough 11 days.  The "Paxil flu" and the cortisol wave must have hit because I'm not sleeping as much, jolting awake in the morning after only a few hours of sleep with the really bad cortisol induced nightmares I remember from when I was so bad at the beginning.  Just feeling of over all sheer terror in the morning and the anxiety has been worse in the afternoon as well.  All just from dropping 0.4 mgs from 1.2 to 0.8 after being stable on that dose for 3 months.  So its been like 11 days of the flu without any sleep, or not that much, and constant waves of panic... 

 

That had been a good drop for me for a long time, as long as I left enough time in between them.  I guess now that it represents a much higher percentage of what's still in my system each time I drop I'm gonna have to make smaller cuts from here on in again.  The 1 mm syringe has 5 smaller lines in between the bigger lines that I've been cutting by all at once, so next time instead of dropping what is equal to 5/50th of a syringe we will go with 3/50th.  That would be only 60% of what I have been cutting.  See how that goes.  Although to be honest I can't even begin to do the Percentage math on that one yet, if 5/50ths is 0.04 what 3/50th would be...  I take it twice a day so I have to multiple by 4 instead of 2 to convert the total dose of mill to mgs or vice versa.

 

So right now, I'm down to 0.2 on the mill syringe twice a day, which works out to 0.08 mgs daily total.

 

I'm just too overwhelmed now to work it out..

So you reduced from 1.2mg to 0.8mg.  That's a 33% reduction.  You might want to consider doing a small updose.

You can also dilute the liquid to make it easier and more accurate to measure your dose.

Well, proportionately, that's a pretty big change, Colonial. But it never ceases to amaze that tiny amount of Paxil has such a big effect.

Yes, your both very correct, which is what makes it seem so hard to figure...

I was going steady at dropping 0.4 mgs and not having any problems as long as I had enough time between drops....

But eventually the closer You get to the end of the syringe, the greater the percentage exponentially the change is..

I just wasn't expecting this bad a reaction since I didn't really have much the last 2 drops.

When percentage wise I had dropped 20%  from 2 to 1.6 mgs, and than 25% when I dropped from 1.6 to 1.2 in October.

I gave myself a nice long 3 month wait so I wasn't dropping the beginning of December and inducing a reaction around the holidays and even the long hold wasn't enough.

This 33% drop was too much I guess...

So much for being off by May...

The symptoms are the same so far for the last 11 days, the flu like symptoms, insomnia, cortisol spikes in the morning with nasty nightmares.

I actually fell asleep 2 hours ago for 30 minutes, and I awoke with a really bad startle and a rush or terror...

 

I reported what happened to the doctor and He said maybe I should have only dropped from 1.2 to 1.0 even.

It really does make one wonder:

Is it the percentage drop at one time, or is as little as 2 mgs of Paxil reeking that much havoc in your brain system?

It's a combination of things.  SA recommends reducing by no more than 10% of the current dose.  Even going from 1.2mg to 1mg is a 16.66% reduction.

 

BrassMonkey tapered Paxil and has commented that he has seen a lot of people have trouble in the 1.2mg range.  He has suggested that you updose back to what would have been a 10% reduction amount.  1.2mg x 0.9 = 1.08mg.  After that you will need to hold for a good amount of time to make sure that things have stabilised before reducing again.

 

I will try and work out the best way to dilute the dose so you can measure 1.08mg.

Colonial, I agree with your doctor. I would updose to 1mg and hold for a while. Let us know how you're doing.

 

A 10% reduction on 1mg would yield a dose of 0.90mg. A 10% reduction on that would result in a dose of 0.81mg. 

 

You have to calculate 10% on the last dosage you've taken. 

 

Paxil can be a bear to quit. I know of one person who was dipping a toothpick into the liquid for her daily dose, at the end.

I will check in the morning if He was actually wants me to uptick it a bit this late in the dosage change or see if a few more days turns the tide...

I took it as He was more theorizing what I "should have" done as opposed to changing it up this late after the drop and this long into the wave....

Obviously if I had known 11 days ago it was going to be this bad I would have done it then but that's what makes the decisions so hard since the worst of my Paxil waves usually subside in 2 weeks.

If I remember clearly, at least for the physical symptoms the worst of the last 2 waves lasting about 2 weeks..

 

ETA: CHESSIE:  If I do up dose I will just go up 1 or 2 mini lines on the syringe, Instead of dropping 5 at a time....

I can't see Him approving Me mixing water into the Liquid suspension...

I think He'd rather see me drop less than 10 % than going off and tampering with the suspension...
