I have been slowly reducing my quetiapine dose over the last 4 months and am now down to 50mg slow release at night. As yet I have had few withdrawal symptoms apart from since going on the slow release tablets my left eye has a continuous twitch, I decided  to try taking the normal 2x25 mg and it seems to have helped with the twitching. Should I be alarmed at this or carry on taking the slow release tablets and put up with the twitching. I can’t get an appointment with my Dr due to COVID restrictions. Any help would be most appreciated. Thank you.

When you were tapering from 150mg down to 75mg, were you using the immediate release or sustained release tablets?

 

We recommend tapering by no more than 10% of current dose every four weeks.

 

Why taper by 10% of my dosage?

 

To get the smaller doses needed for a 10% taper, you'd be better off using the immediate release version, since splitting an extended-release compromises the extended-release quality, possibly resulting in immediate-release quetiapine. If you are used to the extended-release form, you may be sensitive to the "dosage dumping" resulting from immediate release of the drug.  The twitch that started when you took the sustained release is another argument for using the immediate release.

Tips for tapering off quetiapine (Seroquel)

 

We don't recommend a lot of supplements on SA, as many members report being sensitive to them due to our over-reactive nervous systems, but two supplements that we do recommend are magnesium and omega 3 (fish oil). Many people find these to be calming to the nervous system. 

 

Magnesium, nature's calcium channel blocker 

 

Omega-3 fatty acids (fish oil) 

 

Add in one at a time and at a low dose in case you do experience problems.

Hi, thank you for your reply. When the Dr put me on 75 mg it was a 50mg slow release and a 25mg immediate release tablet. The eye twitching only started when I take the 50mg slow release on its own.

Update

managed to change prescription for 2x25mg immediate release, eye twitching now a thing of the past. Have now started on 25mg at night for the last week with no side effects.

Hi

i am currently tapering off 150mg Seroquel and am now down to 25mg. My Dr has reduced my dose by 25 mg per month. As yet I have had few side effects apart from when he put me on 50mg slow release and 25mg instant release, I developed a constant eye twitch when using 50mg slow release on its own. I have managed to get the prescription changed to 2x25mg which resolved the eye twitching last month. For the last week or so I am now down to 25mg at night. The only side effect I seem to have is I’m constantly eating at night which I don’t like but am putting up with it at the moment.

Update, as of a few weeks ago I’m down to 6.25mg at night with very few issues, however had to change Dr as have moved house and new Dr wants me off quetiapine in two weeks, don’t know if I feel comfortable doing this as can’t afford time off work. I’m thinking of skipping a dose every other day to see what happens. Wish me luck

We strongly recommend against skipping doses.  Slipping doses makes the amount of the drug in your blood stream go up and down, battering your nervous system and making withdrawal worse.

Thank you for the insightful information, don’t think skipping a dose will be a good idea. My Dr at present has never met up with me and is just following my previous drs advice, however there is no guidance to use that is reliable in the UK in successfully coming off quetiapine, I have read numerous books and educational studies in this area, but when I bring any off it up with Drs they say it’s not reliable information as it’s not UK based. So I’m going it pretty much on my own. My Dr thinks I’m still on 25mg but I have been reducing it over the last few months to 6.25 mg. my last appointment with the Dr was last week and wants to review agin in two weeks so in other words she wants me to go cold turkey after next appointment which I’m not comfortable with.

In 2019, the U.K. revised its guidelines regarding withdrawal from its previous position that withdrawal ws mild and transitory.  While the following guidelines don't address taper rates for quetiapine, I would hope the following would be helpful.

Thank you for the information, I am aware of these guidelines as well as Mind-UK coming off guides, however when I bring up these studies the Drs are not interested and I’m once again left with coping on my own. I am very grateful for websites like these as I feel people on here know what they are dealing with unlike some medical professionals, I say some as I have only had bad experiences to date with my own mental health issues. I find once a person is Labelled the so called professionals don’t listen to the person anymore they only see the label which is sad but true for myself, however I intend to be and stay positive as I feel that is best for my own physical as well as mental health.

Update, have now been quetiapine free for three days and am feeling very positive, as yet have had no WD and am sleeping well 7/8 hours a night with no issues.

I was just looking at some of the previous posts here too. 

What dose of quetiapine did you jump off of?

And do you have any still around of the quetiapine?  For just in case you need a "crumb" in the days to come.  I'm just slightly worried that as you had been on the slow release, that there might still be some WD to come.  Hoping NOT!

 

You may be able to use non-drug coping if so.  Many great topics in the Symptoms and Self Care forum

Please post too, if we can help you find practices for any symptoms that might occur or re-occur.

Rounding up to 30 years of medications(30 medication trials, poly-pharmacy maximum was 3 at one time).

5/28/2015-off Adderal salts 2.5mg. (I had been on that since hospital 10/2014)

12/2015---just holding, holding, holding, with trileptal/oxcarb at 75 mg. 1/2 tab at hs.  My last psycho med ever!  Tapered @ 10% every 4 weeks, sometimes 2 weeks to

2016 Dec 16 medication free!!

Thank you for the reply, I am still only on day 4 medication free.

my taper was very slow. Was prescribed quetiapine after having a breakdown due to life circumstances at the time and wasn’t sleeping. I was diagnosed Bipolar but have never had Bipolar symptoms at all apart from lack of sleep due to stress. I have updated my signature to reflect my tapering schedule. I only had a few issues along the way. I had always been prescribed instant release quetiapine which worked well, however on my reduction dosages a new Dr prescribed 50mg slow release and 25 mg instant release. As soon as I started on the slow release I had really bad left eye twitching so I made the decision to stop the slow release tablets and continue with instant release at the same dose (50mg). This resolved the eye twitching completely. I did have a few issues with eating after taking the medication but I took my medication about 30 min before bed so wasn’t munching away after lol.

i did have a concern with not being able to sleep once I was on such a low dose and discussed this with my prescriber. My Dr suggested I take Nytol original 25mg and this way I could increase to 50mg if needed. ( this is an over the counter sleep aid) it is working for me very well other than the Nytol I am not on any other medications. I plan to use the Nytol for a week or so then see how it goes. I do have a good stock of Quetiapine left over and the moment so do have it to fall back on if needed.

Studysue, quetiapine is a drug that's really two drugs in one - at doses over about 150 mg, it affects dopamine. Under about 150 mg, it affects histamine. 

 

This is why it's prescribed in low doses as a sleep aid. 

 

So you're basically substituting Nytol for quetiapine. Please note you will need to taper off the Nytol, just like any other drug.

 

It would be better to have done a much slower taper off quetiapine than to come off too abruptly and substitute with another drug. Since you've only been off quietapine for a few days, you could reinstate at a low dose, but it also depends on when you went on the Nytol.

In November, you reduced from 25 mg down to 6.5 mg, which is a 74% reduction. And then from 6.5 mg in late November, down to 3 mg in the beginning of December, which was a 54% reduction. And then you stopped altogether. 

 

The recommended taper rate is no faster than 10% a month of the prior month's dose (not the original dose). Please see:

I appreciate your feedback on my reduction, however as I have had so few side effects, I listened to my own body on making the decision to taper the way I did. I am also aware that everbody is different and should reduce accordingly. At no point in my taper have I put myself at risk and have followed my prescribers advice. For me this has been ideal but do understand the risks involved.

 

To answer your question on my dosage before needing Nytol, the answer is 0 as my dr said to go on the Nytol as soon as I was no longer on quetiapine for a week which will be tomorrow then stop.

Thank you Lucy1983, I am very aware of any changes no matter how small. I also journal all feelings, thoughts and behaviour as I have previously worked in a mental health field for a number of years so am very aware of the risks involved. Thank you for the reminder though most appreciated. 

For the last two nights I have stopped the use of the Nytol and am sleeping peacefully for around 7/8 hours. No other symptoms as yet but I am keeping a close eye on how I’m feeling.

quetiapine 100mg- August 2020, quetiapine. 75mg- September 2020,quetiapine  50mg slow release October, October 50mg instant release 

November 2020 25mg instant release for 2 weeks November 12.5mg for 2 weeks, late November 6.5 mg for 2 and a half weeks, December 2020 3 mg for 1 week, Mid December 0 mg for 4 days so far 😀🥳

mid-December 2020 - Nytol 25mg

End December 2020 - 0 medications

Thank you Shep, so far so good have stopped Nytol medication as advised by Dr. At the moment I am not showing any WD symptoms however I have started jogging for short periods on a daily basis. I have also started a new hobby over the last few months which I enjoy. I am also eating healthy and doing everything I possibly can for my own mental health. Taking each day one step at a time but in the right direction.

Sunday update,

so today has marked 17 days quetiapine free for me with very few side effects. As yet I have had only one side effect eg. The eye twitching which I experienced on the 50mg slow release tablets which I resolved by switching back to instant release. 
I am feeling very well at the moment and am looking forward to returning to work tomorrow. 
i have taken up regular exercise, a new hobby and am eating healthy once again. I also meditate which I have always done in the past as I find it works well for me.

I hope others get as few side effects as I have done, I find what helped me most is reading others stories and listening to my own body and what it needs. I am aware this isn’t an easy path but it is doable.
