hello

I have been on Paxil 20 mg and4 mg of diazepam for 15 years. They were not working great but I could manage. One day I woke up and had a terrible anxiety attack. It got worse, my Doctor tried me on every AD you can think of and upped my diazepam to 30 mg and put me on a sleeping tablet too. Now, 3 years later, I am on the couch constantly and when I sleep I am having very vivid dreams. I have no joy or any comfort from anywhere. I am down to 20 mg of diazepam and 20 mg of paxil. My problem is I don't know which med caused the problems in the first place and I am finding it impossible to taper. What should do? get off diazepam and then try tapering paxil? If anyone can please help I will worship you forever, I am at the end of my tether and can't find any reason to live. \\please someone help me please

Paxil 2005 at 30 mg Diazepam in 2005 at 30 mg quickly reduced to 4 mg of diazepam, Slowly tapered to 20 mg of Paxil and start at the dose until June 2019

March 2020 Paxil 20 MG Stopped taking 120 mg of propranolol in 2021

I am currently taking 20 mg of Paxil, 20 mg of Diazepam and 10 mg of Ambien

I am start a slow taper of Paxil when my scales arrive next week, I will update with each drop in dose

4 th November 2022, 20 mg of Paxil, 10 mg of Zolpidem and tapered down to 15 mg of Diazepam. 5th December dropped to 13.75 mg of Diazepam, 5/1/23 dropped to 12.5 mg of diazepam, Feb 24th 2023 cut to 10 mg of Diazepam. 31/3/23 Dropped from 10 mg of Zolpidem to 7.5.14/4/23 up dosed to 10 mg of Zolpidem. June 10th 2023 crossed over from 10 mg of Zolpidem to 5 mg of Diazepam. Now on 15 mg of Diazepam and 20 mg of Paxil. 29/6/23 dropped to 14 mg of Diazepam

Hello thanks for your reply

the name of my sleeping tablet is Ambien 10 mg

I take 5 mg of Ambien at night about 1.30 am

I wake up at 8 am and take 10 mg of Diazepam

that makes me sleep till about 11.30 am

I eat cereal at that time and immediately take 10 mg of Diazepam and 5 mg of Ambien

that put's me back to sleep till about 3.30 pm

as you can probably see I don't want to be awake, but I have vivid dreams and it's nothing like natural sleep.

 

History of medication

1994 30 mg Paxil (worked great for a while)

2000 stopped cold turkey

never really recovered from that

2005 I had a breakdown and went back on 30 mg of Paxil

I was on 30 mg of diazepam but reduced it quickly but could not get below 4 mg

so since I have been on 4 mg of diazepam and 30 mg of Paxil I managed to reduce the Paxil to 20 mg

in 2019 something happened chemically with the medications

I was in hell as you can imagine

my Doctor tried me on other AD's for 6 week periods all of which made me worse he also upped my diazepam to 30 mg and added Ambien 10 mg and added 40 mg x 3 a day of propranolol 

Meds tried:

Mirtazapine 30mg

Citalopram 20 mg

Zoloft 20 mg

Effexor up to 112 mg

 Prozac 10 mg

I stopped Propranolol 2 years ago

while on the the med's that I was tried on mentioned above there where days I wanted to die, some days I would take up to 120 to 160 of diazepam just to knocked myself out

for the last 4 months I have not gone above 20 mg

So this is my med regime

Ambien 10 mg

I take 5 mg of Ambien at night about 1.30 am

I wake up at 8 am and take 10 mg of Diazepam

that makes me sleep till about 11.30 am

I eat cereal at that time and immediately take 10 mg of Diazepam and 5 mg of Ambien

that put's me back to sleep till about 3.30 pm

I have no motivation, nothing excites me, I don't move all day, I have not bathed for 2.5 years or cleaned the house. I want to die 24/7 but also want to be myself again. All my hobbies do not interest me. I live alone. I have no libido. I have agoraphobia, Social phobia, My CNS is destroyed and my depression and anxiety are overwhelming. Tinnitus 24/7. Just one final note is that I had a traumatic childhood, I drank alcoholically from 12 years old, I never missed a night. At 19 years it stopped working which was like hell on earth. At 25 years old I went into a Psyche ward and was detoxed. I joined AA and now I am 34 years Sober, I am 59. If you can help me you must be an Angel because I have tried and tried but I have run out of steam. Thanks for reading and thank you for your concern I really appreciate it.

Please let us know the details of your drugs for 2021 and 2022. In your opening post, you mention being on 20 Paxil and 20 mg diazepam currently and in your last post, also being on 10 mg Ambien.  Please list any dose changes you've had in the past year for all of these drugs. 

 

Knowing how you've been taking your drugs over the past year will help us guide you in what to do now. 

 

Also, what time of the day do you take the Paxil?

In the last year

Paxil 20 mg taken at 8 am

Ambien 10 mg

Diazepam has fluctuated  a lot. I am prescribed 30 mg a day however in the last year I have took up to 160 mg in one day, I did this about 3 times, the last time was about a year ago. Since then I have tried to stay at 30 mg but about once or twice a week I have taken up to 60 mg in one day, I have done this often. In the the last 4 months I have not exceeded more than 20mg in one day. I don't take any supplements. Thank you for your reply, if you need any more information please let me know. Thank you

@Kingy Before beginning your taper, you'll need to be on the same dose of each drug, taken at the same time each day. Diazepam has a half-life of up to 200 hours (8 days), so it's a drug that stays in your system a very long time. Taking more than your regular dose even once a week increases the level in your system for a long time afterwards. 

 

Also, Ambien and diazepam work on the same neurotransmitters as alcohol - they all effect GABA. It sounds like you may have replaced alcohol with benzos and a sleeping tablet in order to get the same numbing effect. You'll need to work on non-drug coping skills to be able to transition off of these drugs and learn to handle life without being numb, especially to the point of taking enough to sleep 12 hours a day. Learning mindfulness can be very helpful because a lot of people who drink to excess and / or take drugs like benzos in excess are dealing with acute levels of anxiety. Learning how to manage your anxiety levels without drugs is something you'll need to do in order to get off these drugs. Many people with this history are also dealing with trauma, especially trauma from childhood. At some point, you may want to work with a trauma-informed therapist (one who can't prescribe drugs), but you may want to hold off until you're in the healing path from psychiatric drug withdrawal. This type of therapy can be too stressful for someone going through withdrawal. Just something to think about for the future. 

 

Keep in mind, benzos and sleeping pills are central nervous system depressants, so many people feel depressed while taking them. As you come off, this will improve. So there a LOT of benefits to getting off these drugs. 

 

Before you begin your taper, you may want to set up a daily routine for yourself. Have a chore or two that you do every day for just a few minutes at a time so it's not overwhelming. Work on improving your diet so you're getting lots of protein, veggies, and fruits, and avoid sugar and caffeine. Also, when people are sleeping most of the day away, many times they're not staying hydrated. Dehydration can increase withdrawal symptoms such as anxiety and fatigue, so make sure you're drinking plenty of clean water throughout the day. 

 

None of this needs to be done in one day. Over time in the coming weeks, please work on setting up a routine and work on personal hygiene, diet, and getting your home straightened up. A little bit at a time. 

 

You mentioned that you started Paxil in 1984. When did you start taking Ambien and Diazepam? Please list the month and / or year. Also, when were you taking the other drugs you mentioned (Mirtazapine, Citalopram, Zoloft, Effexor, Prozac, and Propranolol)? Please list the month and year for all of these. If you don't remember the month because it was a long time ago, just list the year as best as you can remember.

Thank you for replying { I started Paxil 30 mg in 1994 and stopped cold turkey in 2000} I started taking it again in 2005 at 30 mg.

I started Diazepam in 2005 at 30 mg a day but quickly reduce to 4 mg a day, my doctor said it was ok to continue taking 4 mg so I have been taking 4 mg since 2005 to 2019 July then my dose was upped to 30 mg in August 2019 because I was in a suicidal state and added Ambien 10 mg on the same date. Also I was put on 120 mg of propranolol in August 2019. I stopped taking that in 2021 I am not sure of the month.

September 2019 I was put on Mirtazapine 30 mg {6 weeks}

November 2019 Zoloft 20 mg {6 weeks}

January 2020 Citalopram 20 mg {4 weeks} this made me feel suicidal so my doctor took me off it.

February 2020 Effexor 112 {6 weeks}

all the above AD's made me feel worse so I was put back on Paxil 20 mg in March 2020 and have been on that up to present day. I have been on Paxil 20 mg since March 2020. I have been taking 20 mg of diazepam and 10 mg of Ambien from Late June 2022. 

I live in the UK and the mental facilities here are appalling. My Doctor referred me to a psychiatrist and counsellor whom I spoke to 3 times by telephone in the space of a year. I was discharged from the mental services by my psychiatrist because I refused to up my dose of Paxil to 30 mg. 

I know what I should be doing, I should move, do tasks and rest eat and drink plenty of water however when morning comes I feel so bad I just can not contemplate doing anything but I know I have to try. Anyhow thank you for taking time to talk with me I really appreciate it. Will give me a taper plan or should taper benzos first and then Paxil?. I have no idea what to do. Hope you can help thanks

This is great advice from Shep. Most physicians just prescribe a drug as if it will cure anxiety by itself, when at best it masks the symptoms for a while. I depended on Paxil to help with my anxiety and OCD, but all it did was make me lethargic and numb.  Once I found the right therapist and  started practicing CBT and ERP, I found I could manage it without medication.  Right now is the best time to start working on CBT, since it can support your thought habits during WD and recover.  You can check out Dr. David Burns' "Feeling Good Handbook" for some great coping methods.

We advise tapering most antidepressants, including Paxil, before tapering benzos and sleeping pills like Ambien. Please read the first post in this thread, as it gives a very good explanation as to why:

Because you may be a bit over-sedated with the benzo and Ambien, you could reduce the Ambien a bit first. However, if you're more comfortable being sedated, you may want to simply hold the benzo and Ambien where they're at and work on getting off Paxil first. Here's information on that:

It's very important to keep the benzo and Ambien doses at the same time of the day every day and at the same dose. So no upping either of these to go numb. Please work on non-drug coping skills. Distraction can be helpful, such as watching a favorite TV or internet show, listening to favorite music, watching cat videos on YouTube, etc. Whatever takes your mind off your struggles until it's time to take the next dose. 

 

And remember to eat several healthy meals a day and drink plenty of water.

It looks like you're taking Paxil and Diazepam at 8 AM. You'll notice in that drug interaction link, those two drugs may cause problems when taken together. So you may want to space them out by 2 hours.

Thank you for the info, I will space them out. I hate the feeling of sedation because it takes away my will to do anything. I have dropped my diazepam to 17.5 mg from 20 mg. I will hold that for awhile 2 or 3 weeks. I changed my routine by going to bed earlier and getting up around 9 am. I try and stay awake until 3 pm and have a 1.5 hour nap. It's very difficult to motivate myself but I am trying to do a least one significant thing a day. I will keep you updated on my progress. Thank you for your help.

Kingy, please updose to at least 18 mg. We recommend tapering no faster than 10% a month based on the prior month's dose (not the original dose). Going from 20 mg to 17.5 mg is a 12.5% reduction. 

 

Also, you may want to come off the Ambien first, since it has a shorter half life. Let's discuss this before you make any more changes. Let's set up a game plan first.

How should I reduce Ambien? The medication is a 10 mg tablet. What percentage should I reduce it by and how? I sent off for some Scales maybe I can do it by weight? Thanks

For all psychiatric drugs, we recommend the 10% or less reduction per month (based on the prior month's dose, not the original dose). 

 

Which drug helps you the most with sleep - Ambien or diazepam? 

 

You'll need to come off the Paxil before you come off the drug that is the most helpful for sleep. The goal will be to decrease the sedating drugs so you're not overly sedated during the day but still have enough to take at night to help with sleep as you come off the Paxil.

ok thanks. Ambien helps me sleep more than diazepam. I think you are right of course. I will do a 10% taper with diazepam first and stay on Ambien while I attempt a 10% taper of Paxil. I know it will take some time but I am sick of living like this. Thanks for your help and advice.

Just a quick reminder about the half-life of Ambien - it's a lot shorter than diazepam. This means if you come all of the way off of diazepam, you may need to split the Ambien out several times a day to prevent inter-dose withdrawal (going into withdrawal in between doses). Some people will crossover from short-acting drugs like Ambien to longer-acting drugs like diazepam in order to taper. Just a thought. You may want to only taper off part of the diazepam, hold the Ambien, and then work on getting off of Paxil. 

 

You can always move the diazepam to the evening later on to help with sleep as you work your way off of Ambien (after coming off Paxil).

 

Here is a possible game plan: 

  • Reduce the diazepam 25% (10 mg down to 7.5 mg) at a rate of no more than 10% per month. This will help reduce daytime sedation
  • Taper off Paxil
  • Taper off Ambien
  • Move the diazepam to the evening to help with sleep and then taper of it

I was thinking that too. I thought about tapering to 10 mg of Diazepam and hold the Ambien a 10 mg and then concentrate on Paxil tapering at 10% a month or 6 weeks. These are my thoughts, any suggestions you have are very much appreciated and thanks you again

Sounds good, Kingy. As you reduce the diazepam, if you start to feel more alert during the day, then switch over to the Paxil taper. 

 

Please continue working on your non-drug coping skills so as you taper off these drugs, you'll be able to handle not only the withdrawal symptoms, but whatever stress life throws your way.

Hello I am trying to get a routine to each day. I am finding that just before I wake up I have a dreadful feeling and image of doom. I wake up feeling very tired and disturbed. The disturbance fades fairly quickly. I have not up dosed and at the end of the month I am planning a 10% drop of diazepam. I feel so tired mentally all the time. I am just wondering if you had feelings of doom and vivid dreams. Also I have no interest in anything. Anyhow hope you are well and thanks for your help, I appreciate it

Hi Kingy

 

Yes, I've experienced early morning anxiety and dread.  Its also common to have these feelings in the early evening (sundowner's syndrome).  I found positive affirmations to be very helpful.  Turn the negative thoughts inside out.  Instead of asking why you feel so dreadful in the morning, try to be thankful for being  another day closer to recovery.  Be thankful you have SA for support.  I know it sounds trite and simple, but saying at least three positive things to yourself every morning really is quite powerful.

Hello

I am cutting 10% of diazepam every 3 to 4 weeks. I am due another cut around the 5th of November which will drop me down to 15 mg. I am still taking 10 mg of Zolpidem and 20 mg of Paxil. I think the Paxil was the drug that most damaged me. I am finding it very difficult to get motivated and just want to sleep. I am have very vivid dreams (I have been for over a year or so) My nervous system has been destroyed and I find extremely difficult to go outside but at the same time I am so restless. Sometimes my thoughts are not my own and can be very negative, they are terrifying. I am just wondering if this is normal and if anyone has any advice or encouraging messages I would be very grateful. The feeling of loneliness and emptiness are equally freighting. I have been in this sate for over 3 and a half years and as you can imagine I find difficult to see an end to this. Anyhow I hope someone can reach out to me, I just want a moment of peace even a second and more importantly a sense of hope even if it only lasts for a second would be enough to keep me going. Thanks in advance

I hesitated to respond as I don't have any advice per se but wanted to let you know that I am experiencing the exact same symptoms and have been for a long time as well.  I really get feeling like there isn't an end to this.  I did have an amazing break for one year during this most recent taper that I started in 2017.  At that time, all of the symptoms went away so that was reassuring that they weren't me as one starts to wonder after a while. Unfortunately I had some sort of setback due to stress (stress always seems to bring on these symptoms as my nervous system seems so very sensitive, and well, messed up) and I am unfortunately back to feeling awful.  Even though I had that break all of my doubts are also back.  I actually wonder if these thoughts/feelings are part of this process and perhaps withdrawal symptoms in themselves....at least for me. Anyway, I hope it helps in some small way to know you aren't alone and the break I did get was encouraging that change can happen at any moment (it really came out of the blue).  I am thinking of you and hope you get some relief soon.

-1/06 - 3/07 Cymbalta. Fast taper (essentially CT); withdrawal symptoms after 4 mos (didn't realize was WD)

-10/07: 100 mg Zoloft; 1 mg Klonopin - tapered off Klonopin after 4 mos. Several unsuccessful slow tapers of Zoloft; went up and down in dose a lot

-Spring 2013 back on 1 mg Klonopin to counter WD symptoms; switched over 5-6 mos from Zoloft to 35 mg citalopram
-Two attempts at slow tapering citalopram, always increased dose due to WD; also increased Klonopin to 1.25 mg in 2014, then to 1.5 mg in 2015

-8/17-9/17: After holding one year at 20 mg, feeling withdrawal symptoms due to stress - slowly increased to 25 mg. No change in symptoms after 6 months (? tolerance ?)  - decided to start citalopram taper February 2018 (still on Klonopin 1.5 mg).

Supplements: fish oil; magnesium; vitamin D3; curcumin

Thank you very much for responding. It does help to know that it's not just me suffering this demonic mental and physical state. I will just  continue tapering however I feel like my life has no meaning and so what's the point of it. I hope you get a another break that lasts. I think all we can do is do our best each day and not scold ourselves for no longer achieving or having any goals or motivation. Again thank you for response, you will be in my thoughts

Paxil 30 mg in 1994 and stopped cold turkey in 2000 Took nothing from 2000 to 2005

Paxil 2005 at 30 mg Diazepam in 2005 at 30 mg quickly reduced to 4 mg of diazepam, Slowly tapered to 20 mg of Paxil and start at the dose until June 2019

August 2019 added Ambien 10 mg and 120 mg of propranolol also 30 mg of Diazepam, November 2019 Zoloft 20 mg {6 weeks}, January 2020 Citalopram 20 mg {4 weeks} February 2020 Effexor 112 {6 weeks}

March 2020 Paxil 20 MG Stopped taking 120 mg of propranolol in 2021

I am currently taking 20 mg of Paxil, 20 mg of Diazepam and 10 mg of Ambien

I am start a slow taper of Paxil when my scales arrive next week, I will update with each drop in dose

4 th November 2022, 20 mg of Paxil, 10 mg of Zolpidem and tapered down to 15 mg of Diazepam. 5th December dropped to 13.75 mg of Diazepam, 5/1/23 dropped to 12.5 mg of diazepam, Feb 24th 2023 cut to 10 mg of Diazepam. 31/3/23 Dropped from 10 mg of Zolpidem to 7.5.14/4/23 up dosed to 10 mg of Zolpidem. June 10th 2023 crossed over from 10 mg of Zolpidem to 5 mg of Diazepam. Now on 15 mg of Diazepam and 20 mg of Paxil. 29/6/23 dropped to 14 mg of Diazepam

Recovery is very much a rollercoaster. and the highs and lows aren't consistent.  The thoughts of your life having no meaning are a common WD symptom.  When I was n WD, I felt like I was a burden to my wife and family, but looking back it wasn't all doom and gloom--WD affects our perception of reality.  Its totally normal to have negative thoughts.  Don't judge yourself or your worth as a human by them.  Your WD thoughts aren't who you are.  And who you are can be whatever you choose.  Once I came through WD and recovery, I realized I wasn't the "old me" anymore.  I was more resilient and more empathetic! WD was the hardest thing I ever did, but it was worth it. keep coming here for support!

Thank you for your reply and words of encouragement, I feel like I am going insane sometimes but I know they are not my thoughts or my true self. I have no choice expect to battle through the best way I can't and above all I must remain patient and not try and taper too fast. You are right it's the hardest thing ever but hopefully I will get there eventually. I will keep you updated if you don't mind and let you know how I am doing. Your words help me to put things in perspective, I really appreciate it. Thanks again

I made another 10% cut yesterday I am now down to 15 mg of diazepam, 10 mg Zolpidem and 20 mg of Paxil. No real difference in the way I feel, however I still have zero motivation and would rather be asleep then awake. Unfortunately when I sleep I have the strangest dreams. I find very difficult to go outside and feel an intense disconnect from reality. I hope this will improve and my energy levels improve, I am in desperate need of motivation? Anyhow thanks for your help

Diazepam reductions may not show any symptoms for a few days or even a week due to the long half life. Please let us know how you're doing in a few days.
