LostInCanada
Thank you for this website and lifeline. After 20 years on Paxil, this July I went to a psychiatrist to get off this drug. (I had tried in the past to reduce the doses to see if I could live without it but always was unsuccessful. I realize I was in withdrawal not relapse those times) The psychiatrist had me go from 30mg Paroxetine to 0 in 4 weeks. She wanted me to start Prozac at 10mg and go up if necessary for the withdrawal. I took Prozac for 5 days and had side effects so I stopped it. In Canada you get a one time visit with a psychiatrist and your GP manages the damage. I had insane withdrawal that I kept reading should end at 2 weeks, then I read up to 8 weeks but something changed at about 3 months and it got even worse. I was already dealing with nausea, insomnia, burning feet, sweats, chills, but it progressed to desperation, thoughts of suicide, hopelessness, everything overwhelmed me, I couldn’t watch tv, I struggled to read, couldn’t follow a conversation. I found this website and read a bit about reinstatement though I knew 3 months off the drug wasn’t ideal for this. So I started back on Paroxetine at 5 mg which was probably too high. I later read probably should have went with 1mg but I am here now. It’s been 2 months back on the 5 mg. Some improvement- no more suicidal thoughts, hopelessness or desperation. The nausea though still present has settled a lot. My thinking is getting better. Still can’t do TV, still have dizziness, can’t sleep more than 1 1/2 to 2 hours at a time, reading is better. So my question is what is stable enough to start to taper? Should I hold longer at 5mg? What size of the Paroxetine tablet is better to use for cutting? At present I cut 10mg in half but I noticed the higher the dosage the more filler is in the tablet. ie 10mg is 120 mg tablet, 30 mg is a 560 mg tablet. Is it better to grind the pills to powder or is the drug evenly distributed in the tablet making it safe enough to cut into small pieces for tapering? What is the usual jump off dosage for Paroxetine? Can jump off be done using the tablet/ powder or do you have to make liquid at some point? I’ve read Paroxetine self metabolizes-so basically the body thinks you are tapering faster than you are-so is the slide method more effect for Paroxetine or only necessary say after 3mg? Any and all help is so welcome. Moderator Emeritus getofflex Posted January 27, 2023 Moderator Emeritus Share Posted January 27, 2023 (edited) Hello, and welcome to SA. We are a volunteer-run community of people who have been or are getting off of psychiatric drugs. I'm sorry that your doctor had you do such a fast taper of Paxil - it was way too fast, and this is what destabilized your nervous system, so that when you reinstated to the 10 mg, it was too much. Please add the date to your signature that you did the attempted 10 mg reinstatement. I'm very glad to hear that the 5 mg reinstatement is helping. I would strongly suggest that you do a nice, long hold period on the 5 mg Paxil, before doing any more tapering. I would not increase the dose, either. Be sure to take it each and every day, at the same time of day, no missing a dose. Your nervous system really needs stability right now. I would suggest holding on the 5 mg Paxil for at least 6 to 9 months, maybe even 12 months. I did that myself, after I did a couple of fast taper attempts with my Lexapro. Here is some important information about how these drugs actually work. This explains why we get symptoms from going off of these medications, and why it's so important to taper slowly and carefully, and be very cautious about changing our doses. If you don't read any other links, please read this one: How Psychiatric Drugs Remodel Your Brain This helps you understand what withdrawal syndrome is: Video on Recovery from Psych Drugs Windows and Waves Pattern of Stabilization Tapering is best done extremely slowly, and we generally taper by 10% of the current dose no more than once every 4 weeks, so that the reduction becomes exponentially smaller. The other option is microtapering - you reduce by 2.5% each week for 4 weeks, then give yourself a 2 week hold period. It is a gentler method. You may want to consider this. Why Taper by 10% of my Dosage Tips for Tapering Paxil Brass Monkey Slide Method of Microtapering Here is a link with checklists of common WD symptoms: Dr Joseph Glenmullen Withdrawal Symptom Checklists Here are some techniques to cope with symptoms: Non Drug Ways to Cope with Withdrawal Symptoms Stability is really important when we are tapering off psych meds. Please read the link about stability: Keep It Simple, Slow, and Stable We don't suggest many supplements, but 2 that many of us find helpful are magnesium and omega-3 fish oil. Here are the links for info about those. It is suggested to add one at a time, and start with a low dose to see how it affects you. Magnesium Omega 3 Fish Oil If it were me, after I did the long hold period, I would gradually switch to the liquid Paxil. This will make it much easier to measure non standard doses, and reduce by small amounts. And, you don't want to taper while you are switching forms of the drug. This link will explain how to cross over from tablet to liquid. Crossing Over Tablet to Liquid or Vice Versa I wouldn't worry yet about the jumping off point. Let's take things one step at a time. The keys to getting off these drugs are time, patience, and perseverance. It takes a long time to taper off according to our protocol, but is is a much safer and effective way, and in the long run, actually takes less time and incurs much less suffering. Take care, and keep us posted. Edited January 27, 2023 by getofflex Please do not private message me. LostInCanada
Thank you for your response and those references, most of which I have read. I purchased an analytical balance that measures .0001g to be able to do accurate cuts as I found even cutting 10mg in half varies a lot. I am concerned about changing to liquid due to the fact a lot of the filler doesn't dissolve and how will that affect the measurement in a syringe due to displacement. Also I still would like to know the jumping off point for Paroxetine even though I know I am at least 2.5 years away. I need that goal. I do take 200mg magnesium bisglycinate/ 1000mg Vitamin D/ probiotics/ algae oil (2000mg DHA/40mg EPA/2480mg Omega 3) alternating days with 1000mg flaxseed oil (I am allergic to fish) I am not a medical professional. Moderator Emeritus getofflex Posted January 28, 2023 Moderator Emeritus Share Posted January 28, 2023 (edited) There is no hard and fast jumping off point. We recommend people get as low as they possibly can before jumping off. I personally went all the way down to 0.005 mg of Lexapro before jumping off. I'm glad I did, because I still felt some effects from jumping off. You could possibly switch to liquid when you get to a very low dose, because it's much easier to do the low doses with the liquid. Perhaps a jumping off point of 0.01 mg of Paxil would be good. Managing the Endgame Taper I know this sounds ridiculous, to go to this low of a dose, but at the lower doses, SSRI drugs have a disproportionately large effect on our brains. Please read this link that explains it well. Pay attention to the graph in there. You can see that on the left side, at the very low doses, the line is almost vertical. This is why as we taper, the amount we reduce b gets smaller and smaller. SERT Transporter Occupancy Studies Edited January 28, 2023 by getofflex Please do not private message me. LostInCanada
I had a really bad experience using a compounding pharmacy and was curious about your thoughts. I got them to make capsules with 5mg of paroxetine. Of course they have to use filler and grind paroxetine tablets. I only took the capsules for 3 days. Right from day one I was vomiting, chills, shakes, anxiety...just like withdrawal. Then I went back to cutting my own pills in half and was immediately fine. So I called them to find out what machine they used and found out that the filling part is quite manual. So I weighed the capsules and they were not consistent. So either I was underdosing or overdosing in my mind. So I am terrified to use a compounding pharmacy after this experience. Any thoughts? It's why I bought a better scale and am afraid to do the liquid. I am not a medical professional. Every morning from waking till about noon, I am very emotional/weepy. Is it is due to the short half life and it taking a couple hours for the dosage to absorb? Thanks again I am not a medical professional. I would tend to suspect it is more from the sudden fast tapering, and then reinstating too high. This has probably caused nervous system destabilization, which is why I previously suggested a long hold period on your current dose. Please do not private message me. souim
I, Lost in Canada, I'm so sorry that you had such a bad experience with a compounding pharmacy. Personally, I had no adverse reaction to the compound liquid version I'm taking now. I just wanted to wish you all the best, after reading your story... It sucks that psychiatrists give such bad advice. December 2020 : got prescribed 20 mg paroxetine (paxil) for an anxiety disorder Tried tapering in July 2021 by taking 20 mg every other day and failed. Started tapering with the 10% method in January 2022 and switched to an oral suspension. 20 mg = 10 ml Turned out to be a bit hard. Continuing with 5% drops approx. 25 January 2022 : 9 ml / 24 February 2022 : 8,1 ml / 06 April 2022 : 7,3 ml / 2 June 2022 : 7 ml / 11 July 2022 : 6,5 ml / 09 August 2022 : 6,3 ml / 26 August 2022 : 6 ml / 18 Septembre 2022 : 5,8 ml / 17 December 2022 : 5,7 ml / 04 January 2023 : 5,6 / 17 January 2023 : 5,5 Xanax from december 2020 to January 2021 / Omeprazol on and off for GERD since december 2020 : 20 mg / Birth control pill since february 2021. Do you make it fresh each day? Any other tips are welcome. Thanks so much. Wishing you all the best. 👍❤️ I am not a medical professional. Since I'm just beginning, I only use 8 ml of the Paxil juice with 1/2 of a 20 mg pill, giving me the 18 mg I need for my first cut. I know there are folks here on very low doses who are either using the pharmacy-bought Paroxetine liquid or making their own, as I do. I use a small brown bottle and a syringe and according to folks here on SA, it lasts in the refrigerator for 3 days. If you need help figuring out the ratio for these very small doses, I'm sure if you ask, you will receive. BC, by the way, is beautiful. By commercial liquid do you mean something like Ora Plus. My understanding is that GSK no longer makes the oral suspension but are other pharmaceutical companies making it? Canada drug laws are restrictive and don't let us bring in a drug unless we have a shortage. Since we have paroxetine in tablets there is no shortage so therefore we can't bring in a liquid form. CloudyToday
When I crashed after my fast taper I was shaky too, but that was the first symptom to go away when I stabilized at 10mg. I'm sure it will subside for you as well. I'm still dealing with constant nausea that started after I dropped from 6mg to 5.4mg. I do see here and there little improvements, like less nausea in the night, but as you mentioned it is a very slow process. 2017 - 2018 Lexapro (CT, no withdrawals) 2018 - 2019 Zoloft (CT, no withdrawals) 2/15/21 - paxil 20mg 5/1/22 - started fast tapter, 10mg 3 weeks, 5mg 3 weeks, 0mg 2 days. 7/1/22 - start of withdrawals, dizziness, nausea, loss of appetite, shaky. LostInCanada
@CloudyTodayI am glad the nausea is getting a bit better. I lost 30 pounds from mine. I had worse nausea when my psychiatrist wanted me to try to bridge with Prozac...I tried for 5 days and knew I couldn't do it. I have to have faith we can succeed getting off this drug because others have. 👍🙏❤️ 2 I am not a medical professional. LostInCanada
Still feeling small steps in improvement and it has been just over 3 months since reinstatement of 5 mg. Still holding. A few shakes some mornings when waking. Neuropathy in my feet is better. Have watched a couple shows on Netflix and Disney using the laptop. Have got to a couple larger stores with a bit of anxiety. Staying very conscious of trying to be stress free when possible. Still struggling with social interaction. It is just a fact of the process that we try to control and live with. It is very rare for a reduction to be symptom free. The Brassmonkey Slide is designed to try and keep those symptoms to a minimum while tapering as quickly as possible. LostInCanada
Is this a wave? I don't get it. Shakes are getting worse starting 3-4am. They wake me every hour or so. I might get one 3 hour stretch of sleep at night and was waking every 90 minutes but now due to the shakes it is every hour till I get out of bed at 8am. Nausea is back with dizziness. Nothing has changed in dosage since reinstatement November 28. Time change in Canada moved the clocks ahead an hour...I took my drug 15 minutes earlier each day for 4 days to adjust. Crying has increased again. Just want stability. I am not a medical professional.
