5 mg (.026 pw) - 10 pm

I would suggest that you do not make any other changes at the same time as moving the doses.

My suggestion for moving Luvox / fluvoxamine to 8am and 8pm would be to move BOTH doses earlier at the same time.

It's up to you whether you move by 1/2 hour each day or 1 hour each day.

Examples:

9.30am and 9.30pm etc

OR

9am and 9pm etc

When Alto had said to start tomorrow morning, I figured she meant just go right to 8am.   I actually started last night at  8:30 pm.   This morning 8:00 am.   Tonight I will do 8:00 pm.    Did I rush?  I did move both at the same time.

How long after splitting will I maybe level out?   I'm feeling pretty rough right now.  My symptoms don't seem let up all day.  Does it take a bit?   and yes NO other changes.

My sleep though is really really suffering.  Tinnitus is severe.  Noting my symptoms best I can.  Sometimes doing so feels too obsessive.

You've been taking 25mg Seroquel at night since February, 2019? When did these jerks start? Do you get them during the day? Have they gotten worse since you split the Luvox dose?

What times o'clock do you take each of your drugs, with their dosages? What times o'clock are the jerks at their worst?

Just wanted to note as far as the jerks during the day, although I did say I notice them here and there, today I was trying to rest at about 12:30 and they started up in my legs.   This has happened before when trying to rest in afternoon.   I dont rest much during day so wouldn't notice it often.

I have been keeping track of my symptoms.  Which I need to post but cognitively I'm in rough shape.   These last couple of days have been awful.   I dont seem to get any relief where as before I had periods of feeling normal.   I have lost the nice window i used to have every night. And I have much more restlessness and agitation where in before was just anxiety.   Its a whole different kind of feeling.

The lack of sleep is getting unbearable.   Melatonin does not work for me.   I try to meditate, use an app for my tinnitus, but tinnitus is so bad now it is heard over everything else.   Hanging on by a thread and not sleeping makes everything worse.

I'm not coping with the tinnitus well where in I had adapted to my "baseline" for some time.

What are these leg jerks like?

What times of day are you taking your drugs now, with their dosages? The Luvox change should take full effect in a few days.

Same as what I get at night.  Just a sudden jerky movement.

8am - 5 mg luvox

8 pm - 5 mg luvox

10-10:30 pm - 25 mg seroquel

Could splitting the dose have worsened sleep?  I am sleeping worse than ever.  Maybe the luvox full dose was helping with sleep?  I hope it settles its been a few days.   If I can't get sleep I dont know how ill even know if the split was beneficial.     I've tried melatonin, magnesium.  I dont know what else to do.   Sleep is so important.  Any suggestions?

How strong are these leg jerks, how long do they last? Do you get a few or many? Do you get them every day? Do they happen when you are about to nap or at other times?

It will take a few days for you to feel the full effect of the change in your Luvox schedule. You've been on the new schedule for one day, correct?

Please note any changes in your symptom pattern after you take Luvox in the evening.

Since Sunday.   Well, Sunday was 10 am and 10 pm.   One day of 8 am and 8 pm.    Im just feeling bad all day now.

The jerks can be quite strong.  And I do get them when I try and nap.   I say try because I cant due to my tinnitus. They are quick but can happen more than once.  They seem to vary whether its a few or many.  Like last night not so many.

My symptom pattern has changed.   I pretty much feel bad all day.  I used to have times where I felt ok.  Even whole days.   And im sleeping much worse.   I will post a day shortly.

Could splitting my dose have increased the times I have side effects because its twice a day. .  Taking it all at night maybe I slept through it?

Please read my questions carefully and answer them directly.

We were talking about the leg jerks you reported when you were trying to take a nap at midday, correct?

Do you get one or 2 leg jerks when you're about to fall asleep? Is this common?

How do you feel after you take Luvox at night?

What do you mean by "feel bad"? How is this different from after you started Luvox again in May?

You do not report neck jerks at night. Has that stopped?

I do get a few jerks before I fall asleep.  Its common   More than two.   Again, this varies.  The one night my head did it all night as I previously told you.  Yesterday trying to rest at 5 pm it was my legs and torso repeatedly.  This morning once I woke up, laying in bed I had them.

I'm not sure if I feel anything  after I take the luvox.  If anything,  im more anxious at night now where this used to be my better time of the day.  And worsened sleep.

By feeling bad I mean the symptoms I have.   Worsened sleep, dizziness/disconnected from surroundings,  more body jerks, severe tinnitus.  In may, I didnt start taking the luvox again, i have been on luvox all along since 2019.  I started to taper it again earlier this year.

I hope I've answered your questions as best I could.

9/8- symptom log

1.5 hours of sleep.  A few body jerks

7 am- calmer tinnitus

8 am - 5 mg luvox

9:30 am- tinnitus spikes up and worsens. Agitation , anxiety, restlessness,

10:30 - ear pain

11:30 - tried to rest but had leg jerks

1:00 - calmer for a bit but tinnitus spikes and continues to worsen

2:30- ear fullness, Agitation,  anxiety

4:00 - tried to nap.  Can't bc of tinnitus and restlessness especially in legs.

5:00- bad wave of depression  crying,  anxiety all night.

7:30- 80 mg magnesium

8- 5 mg luvox

8:30-  nodding off on couch had body jerks

9:30- 25 mg seroquel

I am wondering if I simply cannot tolerate the luvox?    Maybe when I took it all at night I slept through the symptoms?   Tomorrow will be a week since the split.  How long does it generally take to settle after?   The tinnitus is just unbearable and just getting worse.     I can deal with anything else that comes my way.  And its worsening.   I had a window yesterday and it was great, tinnitus wise    I get more sleep last night and feel worse today.  Don't get it.  🤨

Yes I had other factors - the antibiotic 2 doses over two weeks ago, one vistiral 10 days ago, mri two weeks ago.

I apologize for venting or complaining.   Its a rough day.

Is your symptom pattern better or worse since you moved Luvox to 8 a.m. and 8 p.m.? How long have you been taking it on that schedule?

8 am and 8 pm versus 10 am and 10 pm?  Or just splitting my dose in general? The dose split was Sunday and 8 am and 8 pm Tuesday.   body jerks are still there but have not kept me up in a few nights if I can recall .    My main big problem right now is the tinnitus which is very severe.   Its the worst I have ever had.  I've adapted in the past and dealt with spikes but this is beyond that.   My symptom pattern in general has changed.   I get less relief, less times of feeling good.  No more nice window in the evening I had every night.   Without the tinnitus I maybe could deal .  More waves during the day of agitation, restlessness, anxiety.

Okay.  my tinnitus started about a month after i had stopped the gabapentin previously, back in december when i was on 300 mg per day. and at the time, i had other factors at play - dental work, antibiotics, an upper endoscopy with anestesia, stress, etc. so who knows what started it.  i actually have always had it, but so so mild i forgot it was ever even there, it was irrelevant.    as far as the gabapentin aggravating it now, i cannot say for sure, since i have three meds.  so annoying.  :( ....

What approximate date did the tinnitus start?

2/20/19 is when you started Luvox, correct?

Please include in your daily notes the times when the tinnitus is more severe.

It appears to me you are having an adverse reaction to Luvox. When you reinstated 10mg in June, that was too high. Suggest you decrease the morning dose by 1mg, to 4mg. Observe to see if symptoms change.

So an adverse reaction can happen even after being on the med a while?  Am I just noticing because its first time I've taken it during the day?  The 10 mg was not really a reinstatement. It was  a reduction back to what i was at after a huge updose inpatient.    Maybe it was the wrong wording in my signature, but im sure you could see what I meant.   I have been on it consistently, just tapered too fast and had that updose.   I just wanted to clarify that.

Please add the accidental double dose of the Seroquel on Sept.2 in your signature line

As Brassmonkey said these 2 meds can cause serious repercussions and they happened 2 days in a row.

You may continue to have some symptoms from this interaction.

Thanks!

And in reality, adding any med PRN with an accidental double dose of another can cause an uptick in the symptoms you may already be having. You did get worse for a bit right after this happened.

Seems like forever.  🙄  its so crazy that in the past I've had variables like that and did fine with it. I'm just shot now I guess.

And on top of it, doctor says my hormones are now post menopausal.   I'm sure it is also affecting things.

9/2018- lexapro low dose few days.11/3/2018- zoloft 2 days

11/7/18  - 11/15/18 - Prozac 9 days, from 10 mg for week, to 20

11/16  inpatient put on Lexapro for a few days, Cymbalta, 2 days

11/24-12/8 - gabapentin 100 mg 3xs per day - a very fast taper

1/7 - tbuspar for  three days- blurry vision, jerky eye

1/17/19 - 2/15/19- mirtazapine 15 mg - started taper on 1/30

2/20/19 gabapentin 600 mg. .  12/20-  taper finished

2/20/19 - seroquel 25 mg current - taken  10 pm

2/20/19- luvox (generic) 25 mg.  4/6/19  to 18.75 mg .held  . Started taper again  1/7/21- 15 mg, 2/7/21- 12.5 mg, 3/7/21 -10 mg, 4/1/21- 9 mg, 5/1/21- 8.1 mg, 5/27/21- updose to 10 mg,  6/21-  in patient updose to 50 mg,  6/25/21- reduce to 10 mg (current) .  9/5- split dose 5mg am/5 mg pm.  9/20- 4 mg am/5 mg pm .

9/1/21-  took one dose of vistril 50 mg.

9/1/21-accidental double dose of seroquel- 50 mg

The 10 mg was not really a reinstatement. It was  a reduction back to what i was at after a huge updose inpatient.    Maybe it was the wrong wording in my signature, but im sure you could see what I meant.

It's not fair to assume others should surely be able to understand what you meant when your signature isn't listed as requested. To be honest it took me quite a bit of concentration to understand what happened the way it's listed.

"Reinstate" is generally only used when going back on a med after going off it. The problem can be we know what we mean by what we write but others don't have the full knowledge of what is going on in our head when we write something.

Your Luvox signature area going forward is a bit hard to comprehend because there's a lot of excess info that makes seeing the dosing a bit harder, as well as it's small print.  You have a lot of "explanations" in it as to why you did things which make it harder to get "just the facts" so to speak. .

(Held long time to taper gaba) - updose to 10 mg due to anxiety (too fast taper),  6/21-  in patient updose to 50 mg, 6/25/21- reinstate to 10 mg (holding here).  . Had 2 pill cases out  took one in other container.

With extra info, it's easy to either miss what is important, or mis understand the info that is there.

Also, the months should be listed either spelled out or abbreviated.

2/10/21 could mean either Feb. 10 or October 2nd.

Also, the meds are not listed chronologically, so it's easy to miss that your still taking things such as the Seroquel as it's only mentioned once so far up.

This: 2/20/19 - seroquel 25 mg current - taken at bedtime around 10 pm

To this: Feb. 20/19: Seroquel: 25mg, 10pm

It appears to me you are having an adverse reaction to Luvox. When you reinstated 10mg in June, that was too high. Suggest you decrease the morning dose by 1mg, to 4mg. Observe to see if symptoms change.

Hi alto.   I'm not sure if you saw my response about the 10 mg not being a reinstatement.   I was incorrect in my verbiage .  It was a reduction back down to that dose.  I apologize.   Colonial did tell me that was wrong for me to assume everyone would know what I meant.   I apologize if I wasn't clear.   Not sure if that changes your suggestion.

I know I put some variables in the mix during the dose splitting and I will take longer to stabilize,  but I think I may have done better at once per day.   But I have to stick it out now.

One good thing the jerks seem to have lessened.   But the tinnitus is getting hard to get through the day with.

9/15-

- slept ok.

Up at 5:30- some anxiety and panic

- shakey feeling in head (for a few days now) layer in bed till 7

- up.  Felt ok.

8:00 am- 5 mg luvox

9:00 - tinnitus spikes (but lower), head thick

- pgad flare (on and off thru day)

- vibration/internal tremor feeling up left leg, groin, chest (all day)

10 am- dizzyness, feeling disconnected (this is daily)

12- Tinnitus spikes more (better today as it has been)

2- whole body feels, electrified,  activated,  sense of unease, anxiety

5pm -  lots of anxiety

8pm- 5 mg luvox -

- 10:30 - 25 mg seroquel

11- bed.  Tinnitus is bad ( very bad at night)

- dizzyness .

Summary of daily symptoms-

Tinnitus spikes up in the morning after luvox.   Usually its good upon waking up.  Some days worse than others.

very intrusive upon going to sleep.

Dizzyness and disconnection after luvox dose.

dizzy at night again when I go to sleep.   Cannot sleep completely flat because of dizzyness.

Feelings of anxiety,  whole body activated, restless , fear in afternoon.

This now seems to be the daily pattern, aside from the pgad flare.  And the tinnitus is better some days than others but always spikes up.

A positive-  the jerks have gotten better, still get them but not as bad.

I have kept my luvox morning dose still at 5 mg because as I clarified previously my 10 mg reinstatement was not a reinstatement , it was a reduction back down  from an inpatient large updose .  So I wasn't sure if reducing still was the way to go, since it wasn't a matter of a reinstatement being too high?

@Altostrata

Really struggling with sleep now.  2 hours a night and it doesn't help my tinnitus either.   Melatonin is no good and neither is magnesium for me.    I've been searching on here about chamomile tea and I see some mixed results.  I've been ok in the past when not on meds or so sensitized.   Is there any interaction with it with the meds im on?  I couldn't find it on Google.  Thanks.

I have been thinking this member may have been having a reaction to the Luvox for a week now, seeing as how the ear issue becomes worse after both dosages.

Looking back I see this was already suggested to this member in 2019.

At that time, the member was only taking it once a day in the morning.

At some point they ended up switching it to the night.

After the recommendation to do split dosing to see if interdose wd may be the issue, the pattern has returned.

Thanks, BfromNJ.

Your journal is very helpful.

11:00- 25 mg luvox
11:30- ringing is bad, sounds like whole room is filled up with it.  causing me distraction.
feeling again of being far away from my surroundings, out of it, not part of what is going on.   pretty much same  as yesterday at this time.

12- lunch -luvox 25 mg
1:00- starting to feel worse.  ears ringing way more, buzzing and foggyness, feeling disconnected from surroundings, same as previous days.   bad head pain, feels like someone is squeezing it, and the vibrating sensation.  increase in throat sensation.

You are dealing with mirtazapine withdrawal, which is what landed your in the hospital last month and you're now on this cocktail of drugs. But it looks like you're having an adverse reaction to Luvox - you take it around 11 or 12 every day and about an hour later, you're feeling disconnected, among other symptoms.

The feeling of being "disconnected from surroundings" is depersonalizaton / derealization:

Derealization or depersonalization (DP/DR)

This is a very common side effect and withdrawal effect from these drugs.

From what you've posted (including what is in your signature), it sounds like you've had adverse reactions to not only Luvox, but also Prozac, Lexapro, and Cymbalta.

Do you feel this is true?

I'm going to re-post Alto's earlier post:

Yes, Luvox would go first, as you want to maintain sleep with the other drugs (if they help sleep). Since you've been on it for more than a month, you are at risk for withdrawal symptoms. As it's been only 35 days, you might try a 25% reduction to start, but you'll have to put it up right away if you get withdrawal symptoms.

It's now been 51 days, but if you feel this is an adverse reaction, you might want to go ahead with the 25% reduction. With adverse reactions, you may be better off with a more rapid reduction.
