Does anyone have tinnitus constant hissing in the ears?

 

Also I get a metallic taste on my lip.

 

In January 2021 I had lip reconstruction surgery because 12 years ago I had some fillers injected  that made my bottom lip drop after few years.

The substance did not dissolved and I had to be on the table for two hours to get the stuff out and carful be stitched.

 

I am thinking if this surgery affected my nervous system because after this long time I still feel my lip tingling and when anxiety rises,

the lip feels like has been punched.

 

 

2008-2020 Prozac  60-40mg 

2020 - Prozac 20mg - 0mg 

2021 April -  Prozac 40mg  and  Klonopin  0.75mg - stoped CT after one month and ended up in emergency for three days and was added Lexapro 5mg - for couple weeks and quit. 
2021 NovemberPaxil 40mg  and Quetapine 50mg for 12 days then switched to Sertraline 100mg and Mirtazepine 15mg  for sleep plus 0.5mg Ativan TID 

2021 November Sertraline 100mg rapid taper to 0mg  till February•remained on Mirtazepine 7.5mg  and 0.5 Ativan  when needed

2022 June  - Reinstated Sertraline 25mg increased to 100mg weekly  for one month and ended up in ***hospital again*** because the side effects.  Tapered after couple weeks to Sertraline 50mg, Ativan 0.5mg  TID, Mirtazepine 15mg.

July - when I reached to SD forum and was stabilized on Sert 50mg • Ati 0.25mg BID  • Mirt 15mg  - August - Sert 35mg •Mirt 15mg • Ati 0.5mg >> Sept & Oct >> Tapering - November - Sert 29mg • Mirt 11mg •Ati 0.3mg - December- Sert 25mg • Mirt 10mg •Ati 0.15mg

I just wanted to ask anyone in here about my case with Prozac.

 

Would had been better if I had never stoped Prozac and stayed on 20mg which seemed to work good for me? 
 

Will I ever get rid of these intrusive thoughts (they are bad only when I am under stress) without medication?

Starting  Zoloft was hell on earth for me the whole month of June 2022 and definitely I don’t want to continue with this medication so I am slowly taper it.

@annatino  Welcome to SA.  Unfortunately that means you are hurting and looking for help.  These drugs are strong and do change our brains and bodies.

This might be bumpy for a little bit but you can do it.  I know how easy it is to panic and just run for the quickest fix, I did it myself.  Try  to stay as calm and focused as you can. When do you get your scale?  It will be helpful to get accurate dosing of your Zoloft. 

Get your dosing of the Ativan and Zoloft set up and then you can make your next plan. 

 

I want to reassure you that you are not alone.  Learn as much as you can about these drugs, learn coping skills,  trust that you can heal from this.

Yes I am hurt and I would love to get of these meds quicker but unfortunately doesn’t work that way.

I want to order the scale soon and with the Ativan I want to do water titration.

 

I used to take Ativan every few days and I was feeling rough after the second day maybe, probably because the inter-dose withdrawals.

 

Now I take 0.5mg split twice a day at the exact time.
 

I find that my ears are hissing from Ativan and I get some weird guilt feelings toward my son. 
 

I don’t know if I want to stay long daily on this medication.

 

I will probably wean myself off this one first and stay on Sertraline 25mg until I completely stop the Ativan and after that I will slowly decrease the antidepressant.

 

Any suggestions are really appreciated.

Yes, this is extremely common during withdrawal. Please see:

Does tinnitus keep you awake at night? If so, running a fan or using a white noise app or white noise machine at night can help.

Yes, taste changes can definitely be caused by withdrawal (and as side effects, too):

The long-term outcome of SSRIs are not good. They cause bone loss, a chronification of sadness and anxiety, they can interfere with other medications you may need later in life, etc. 

 

Intrusive thoughts are not caused by a lack of serotonin. How do you know you would have intrusive thoughts at this point in your life without medication? If you look at the history, it tells a story of people who, before the antidepressant era, went through a rough patch in their lives and felt better after a few weeks or months. Please see:

You may want to find guided meditations to help with intrusive thoughts. Here is an example:

Worrying and obsessive thinking are coping mechanisms that can turn into a bad habit. Once it becomes habit, it can feel familiar and familiar can make us feel safe, even though this coping habit is not healthy. The goal is to find a replacement for this coping mechanism, such as learning mindfulness, and then replace the negative coping mechanism of intrusive thoughts with the positive coping mechanism of mindful thoughts. When you feel intrusive thoughts come on, you may want to ask yourself, what do I hear? See? Touch? Smell? Taste? By tapping into your current surroundings - the tangible things around you - you are less susceptible to the abstract ramblings of your thought-stream.

 

Over time - and yes, this does take time and practice - the habit of rumination and intrusive thinking fades out. 

 

And don't let your withdrawal symptoms fool you into thinking that your withdrawal symptoms are who and what you really are. These are just temporary. It does get better.

I read somewhere that 5mg Prozac helps with many disorders. 
Now it makes sense.

Thank you so much Shep for taking time to respond to me.

 

I tapered myself after total one month on 25mg 1st week; 50mg 2nd week, 75mg 3rd week and 10mg 4th week to 35mg in two months.

 

In few days I want to get to one capsule of Sertraline fine powder which is 25mg and start my tapper with the Gemini scale.

I will take 10% each month.

I would like to know about SERT occupancy regarding Sertraline, knowing that this medication can go up to 200mg instead other antidepressants that will stop at 40mg like Paxil or Celexa where SERT occupancy is very important when you get to 1mg, it meets to continue with 10% monthly cut to 0.0mg.

 

I my case, when I will reach, let’s say 2,5mg would it me ok to stop to 0mg or will I have to continue same as the other antidepressants like Paxil, Celexa etc.

When I asked the psychiatrist what did people do before when there were no meds, instead of saying herbs or whatever; she said they suffered till the end or worst...they committed suicide.

Gosh! How could you tell this to a patient that is in distress already?

It depends.  Sometimes a person will see their medical professional and complain that they feel like their drug isn't working any more; this is more likely to happen if the person has been taking that drug for a long time.  If a person finds that their AD stops giving the effects that they want and they gradually start feeling worse it might be because the drug has reached tolerance, technical term tachyphylaxis or common term poop out.  When this happens if the person takes more of the same drug then they may start to get the wanted effects happening again but gradually they will start to feel worse as the drug reaches tolerance again.  And if the dose is again increased then the same thing happens.

However, if a person tells their doctor that their drug doesn't seem to be working any more (ie giving the effects that they want) the doctor might add a drug (if they are both serotonergic drugs then there is a risk of serotonin toxicity) or switch to a different drug.  This does not always work and can make things worse.  If switching drugs, you can get withdrawal from the first drug and/or immediate adverse effect/side effects from the new drug.  If this happens because there have been 2 variables it can be difficult to know what is causing any issues.

I didn’t know that AD’s could reaches tolerance.

I thought just benzodiazepines to that.

They sure can.  Brassmonkey was taking Paxil and had to taper out of poop out.  And it happened to me after I had been taking citalopram for about 20 years.  In 2011/2012 I had several life stressors occur in a short period of time and it felt like the drug wasn't work at all.  Because I felt so bad on it, I decided to stop taking it (this was before I found SA).  I felt great for a few months, better than I had felt in a very long time.  But then I got very sick and was bedridden for 2.5 weeks, lost 8kgs because I couldn't eat.  If felt like the flu but I could tell it wasn't.  I was then told that this was proof that I needed to be on an AD for life (chemical imbalance which has only every been a theory and was debunked several decades ago; thankfully they have recently started publicising in the media that the chemical imbalance is NOT a fact) and ended up on Pristiq.  It wasn't until I found SA that I made the connection and realised that the citalopram had reached tolerance which is why I wasn't able to cope with the life stressors.

 

After a 6 years taper off 100mg Pristiq I have now been off successfully for 9 months.

I was taking Prozac for 12 years for OCD Pure-O and I don’t think that this medication ever made me feel normal.

Ever since it’s been one year now, I am surviving.

One year was hell back and forth.

There is also such a thing as delayed withdrawal.

 

You were taking a high dose at 60mg and 40mg so the dose occupancy might have been at play here.  The reason for the delayed withdrawal may have been due to the very long half life of Prozac AND when the stress happened your system wasn't able to cope both the changes it was making from getting off Prozac and the stress itself.

Congratulations! Wow! 6 years taper that is a long way to go and to have the patience.

 

I was on Sertraline for short period of time 3 months and came to 35mg from 100mg in two months.

 

I would like to know if I could go a bit faster till I reach 12.5 let’s say.

 

Where the SERT occupancy would fit at Sertraline dose? 
 

I already asked this question but nobody answered yet back.

It seems like you are very keen to get off your drug as quickly as possible.

 

I think you need to change the way you think about getting off.  The idea is to get off with the least discomfort as possible and be able to live your life as normally as possible during, and after, that time.

 

I have seen members of SA try to go too quickly and it has ended up taking them longer than if they had done a nice steady taper.

 

It takes as little as 1 month for the brain to fully adapt to getting a drug.  Also the effect on the nervous system of taking various drugs/doses is cumulative.  Some members have chopped and changed drugs and doses with (what they say) no problems and then the next time they try to do the same thing it doesn't work and things get really bad.  You have already added Ativan back in which you probably did because you were experiencing withdrawal symptoms after reducing sertraline.

 

You made a 30% decrease when you reduced your sertraline from 50mg to 35mg in July 2022 (you don't state if it was at the beginning, middle or end of the month - please add this information to your drug signature; please also add the date that you started 35mg).

 

Instead of trying to hurry to make another reduction, think about the fact that you are now taking a lower dose.

 

Personally I think you should hold on 35mg sertraline for at least another 3 months before even considering making a reduction.  And when you do reduce again only reduce by no more then 10% of the current dose of 35mg.

 

PATIENCE and non drug coping skills are key to getting off successfully.

 

And the learning of patience as well as non drug techniques to cope helps once you are off the drug too and trying the cope with life in general.

 

It is important to listen to your body/symptoms and not reduce again until you are stable.

The suggested final dose is generally around the 0.025mg dose.  I went lower (but was reducing weekly) because I was dealing with other issues at the same time but still wanted to get off.

When I arrived at SA I was hoping to be off my Pristiq in 2 years, when I turned 60 years old.  It ended up taking me 4 years longer for a total of 6 years.  But it was worth it.

The reason I wanted to get off this meds faster is because I was told to reinstate Ativan 0.5mg a day, which I was taking sparkly.
 

Now I can imagine going daily on Ativan and build tolerance until I am off Sertraline 1-2 years. 
 

I should go back on Ativan only when needed? But again I was told that I was having Ativan withdrawals.

 

I am really confused. I don’t want to mess myself up more.

 

I am feeling 80% better.

SA recommends tapering only one drug at a time and generally the sedating drug continued to be taken until after the taper of the activating drug has been competed.  See:

Yesterday I took the whole 0.5mg pill

at 9am instead dividing it, an today in the morning at 9am I woke up with anxiety and took half of the pill.

Still having anxiety all day. 

I tapered from 50mg to 35mg two weeks ago and I am starting the 14th day to feel bad.

 

Should I go back to 50mg? 😭

 

I don’t know what to do.

 

Taking Ativan daily 0.5mg split twice a day is not helping me. 

 

***Update 

Two hours later since I took the second half 0.25mg Ativan, I am feeling better.

Still shocked of how quickly I can change in mood. Sadly.

 

At this point I really don’t know if it’s Ativan inter-dose withdrawal or is from Sertraline change 14 days ago and is catching up now.

I understand the frustration regarding the benzo. Periodic dosing is problematic on many levels, but unless you're able to stop it without symptoms,  you're dependent. Please note that withdrawal from benzos and from SSRIs are almost identical in the types of symptoms. 

 

In a "Guardian" article, Dr. Peter Gøtzsche writes:

It's really important to be consistent with this drug and take it at the same times of the day and at the same dose. The non-drug coping skills can help you out in between doses. 

 

The fact that you feel like you "need" it is a sign of dependency. So this answers the question you asked, "What should I do about the Ativan?" 

 

Please take it at the same times of day every day and at the same dose. In time, your nervous system will settle down from all of the changes, but it's important that you stop making changes in order for that to happen. 

 

Questions:

 

  • How do you feel before you take your sertraline? How do you feel 30 minutes later?
  • How do you feel before you take Ativan? How do you feel 30 minutes later?

Hi Shep,

 

Thank you for responding back.

 

I believe I am hooked on such a small dose 0.5mg. It’s making my tinnitus hiss worst and left ear sounds like popping popcorn sometimes.

I know it’s from Ativan.

 

I will take this at the same time but what worries me is building tolerance.

 

I don’t want to increase it at all. 
 

I am thinking to take it until I get to 25mg (one capsule) Sertraline and then staying on Sertraline for 6 months at this dose while I am out benzodiazepine.

 

Then slowly cutting down Sertraline in time up to two years.

 

I cannot think of me staying daily on benzodiazepine.

 

Both are hard to come off but with Ativan there is a risk of becoming tolerant. Although with Sertraline there is no way of increasing it, it will always decrease the dose.

 

And after those two are done to 0mg, I will still have one heck of a nasty drug,

15mg Mirtazepine wow!

Yes, the risk is there for Ativan, but it's also there for sertraline. Please see:

You'll have to weigh the risk of tolerance against the risk of developing withdrawal symptoms if you come off it too quickly or too soon. But that's true for both drugs.

I will ad my first day bellow:

 

August 14

 

8:30 a.m. Woke up a bit anxious 

9 a.m. Took 0.25mg Ativan - did not have tinnitus after taking it.

11 a.m. Feeling better 

12 p.m. Took 35mg Sertraline

  1 p.m. -  Tinnitus started and is a bit stronger, left ear clicks sometimes but both are hissing but not too loud.

12 p.m. - 1 p.m. Reading other people’s stories about benzodiazepine withdrawal got me scared and had a bit of anxiety.

1:30 p.m. Feeling blunt. Feeling afraid what if I will have hard time tapering.

Trying to eat because I skipped breakfast.

2 pm - 200mg magnesium glycine 

3 pm - 0.25mg Ativan - feeling anxiety

4:30 pm - no anxiety 

10:20 pm - 15mg Mirtazepine for sleep

Had no anxiety, tinnitus is vey low, no intrusive thoughts, no any withdrawal symptoms since 4:30 pm

2008-2020 Prozac  60-40mg 

2020 - Prozac 20mg - 0mg 

2021 April -  Prozac 40mg  and  Klonopin  0.75mg - stoped CT after one month and ended up in emergency for three days and was added Lexapro 5mg - for couple weeks and quit. 
2021 NovemberPaxil 40mg  and Quetapine 50mg for 12 days then switched to Sertraline 100mg and Mirtazepine 15mg  for sleep plus 0.5mg Ativan TID 

2021 November Sertraline 100mg rapid taper to 0mg  till February•remained on Mirtazepine 7.5mg  and 0.5 Ativan  when needed

2022 June  - Reinstated Sertraline 25mg increased to 100mg weekly  for one month and ended up in ***hospital again*** because the side effects.  Tapered after couple weeks to Sertraline 50mg, Ativan 0.5mg  TID, Mirtazepine 15mg.

July - when I reached to SD forum and was stabilized on Sert 50mg • Ati 0.25mg BID  • Mirt 15mg  - August - Sert 35mg •Mirt 15mg • Ati 0.5mg >> Sept & Oct >> Tapering - November - Sert 29mg • Mirt 11mg •Ati 0.3mg - December- Sert 25mg • Mirt 10mg •Ati 0.15mg
